    Study on chloramphenicol resistance pattern in Central India, Indore

    Background- Chloramphenicol (CK) is drug of choice in many life threatening and serious bacterial infections. Due to development of resistance and toxicity associated with this drug, it is not in common use now days. This study was done to find out prevalence of CK resistance pattern. Material Methods- 100 consecutive pus samples from surgical site infections from various specialties of index medical college hospital and research center were studied in microbiology department during period of 1year.  Results- Gram positive as well as Gram negative organisms had shown sensitivity to Chloramphenicol. Also organisms were resistant to commonly used higher antibiotics. Conclusion- CK can be used for common serious IPD patients not having contraindication to this drug. Study also reinforces proper and rational use of antibiotics

    Background- Diabetes as a non-communicable disease is significant public health problem the prevalence rate all the world over is raising. Diabetes mellitus is one of the leading cause of long term complications and a major health hazard in a developing country like India. Indian Diabetes Risk Score (IRDS) developed by Dr. Mohan and his colleagues in 2005 is one of the strongest predictor of incident diabetes in India. Material Methods- The present study entitled was conducted in the department of Physiology Gandhi Medical College and Associated Hamidia Hospital, Bhopal. 120 subjects in the age group range of 30-60 years attending the different medical OPDs. Results- The mean age of the sixty subjects included in the control group was 42.61±8.70 years and sixty Prediabetic subjects were 45.43±8.77 years. Maximum numbers of Prediabetes were in the age group of 40 - 50 years. Conclusion- The results of the study will help to formulate strategies for future preventive efforts and delay the onset of type 2 diabetes

    Cutaneous metastases after radical chemoradiotherapy in carcinoma cervix: An unusual manifestation

    Cervical cancer is one of the most common malignancy affecting women in India, however cutaneous metastasis arising from cervical cancer is particularly rare. Skin metastases are unusual even in the terminal stages of the disease with incidence ranging from 0.1% to 4.4%.Mostly they occur as a sign of recurrent disease and are associated with poor prognosis. We report a case of carcinoma of the uterine cervix with cytology proven unusual extensive metastasis to the vulva, anterior abdominal wall and thigh developing within one month of external beam radiation therapy. Cutaneous metastasis usually presents in the form of nodule, plaque and inflammatory telangiectasia. However in our case presenting symptom was an ulcer which spread to completely destroy the vulva. Adenocarcinomas of the cervix have a higher propensity for cutaneous metastasis. Skin metastases from cervical carcinoma occur predominantly in cases of tumor recurrences, with metastases developing up to 10 years after initial diagnosis. No effective treatment has been identified till date. Palliation is the aim using chemotherapy, radiation and surgery alone or in combination

    Pyoderma gangrenosum is a rare ulcerative cutaneous condition due to dysregulation of immune system with a female predominance. A 20 yr old unmarried female presented with chief complaint of fever and a painful perineal swelling with foul smelling discharge from it and was initially diagnosed as a Bartholin abscess for which drainage with marsupialisation done. But in a time of 3 days it turned into a necrotizing ulcer and was rediagnosed with help of dermatologists as pyoderma gangrenosum which responded well to steroids by rapid healing

    Aims and Objectives: To identify the risk factors and to evaluate measures, to reduce complications, and to improve maternal outcome. Materials and Methods: This was a prospective study from December 2012 to December 2013 which was conducted at M.G.M. Medical College and M.Y. Hospital, Indore, Madhya Pradesh, India, in the Department of Obstetrics & Gynecology. All the patients are having fetal death beyond 20 weeks of gestation and/or birth weight >500 g were included in the study. Maternal and fetal outcome were analyzed. Mode of delivery and associated complications were studied. R esults: Total numbers of deliveries were 13500. Incidence of intrauterine death (IUD) at our center was 38.30 per 1000. 68.83% were antepartum, and 36.17% were intrapartum. Congenital malformations were responsible for 12.77% cases. Among the identifiable causes, placental causes (17.99%), and hypertensive disorders (15.47%) were most common followed by severe anemia (6.58%). The most devastating complication of IUD was disseminated intravascular coagulation (DIC) found in 28 patients (5.42%). Conclusion: The present study has emphasized upon the need for augmentation of health services for the pregnant mother, because the extra attention paid to the mother during the pregnant state goes a long way in improving both maternal and fetal survival. The majority of fetal deaths in our study could have been prevented by appropriate antenatal and intranatal care. Decrease in the incidence of IUD would significantly reduce the perinatal mortality

    Tonsillar papilloma: A rare case

    Benign oropharyngeal tumors are far less common compared to malignant tumors. Squamous papillomas are exophytic masses of the oral cavity. It is an innocuous lesion that is neither transmissible nor threatening. Pedunculated squamous papilloma usually arises from the soft palate, tonsil, or the epiglottis. A 32-year-old female presented with odynophagia on and off for 5 months. History regarding the growth revealed that it was first seen 6 months prior as a slow-growing non-tender papule. The present lesion was exophytic and sessile in nature, pinkish in color with a pebbled surface, soft in consistency, 2 cm × 2 cm in size, and situated on left tonsil. Surgical excision of the lesion was performed. The histopathological diagnosis of squamous papilloma was made. The squamous cell papilloma is a benign tumor with a rare entity arising on tonsil which caused unusual odynophagia (symptomatic) of long duration

    Identify the existence of general and central obesity by anthropometric measurements in the diabetic population

    Introduction: Obesity is usually defined according to body mass index (BMI) cut-offs. However, BMI cannot provide information on the distribution of body fat. Central obesity, as assessed by waist circumference (WC), is more strongly associated with the risk of hypertension, dyslipidaemia, type 2 diabetes, cardiovascular disease, cancer and all-cause mortality. Aim: To identify the existence of general and central obesity by anthropometric measurements and to compare anthropometric parameters in the control and prediabetic subjects. Material and Method: The present study was conducted in the department of Physiology Gandhi Medical College and associated Hamidia Hospital Bhopal, during 2010 - 2011. The Fasting Blood sugar, Lipid Profile and Fasting Insulin Biochemical investigation was done in the department of Biochemistry of the institute. The anthropometric measurement including Weight (kg), Height (cm), Body mass index (kg/m2), Waist circumference (cm), Hip circumference (cm), Waist Hip ratio. was recorded. Result: The 120 subjects aged 30-60, maximum numbers of Prediabetic were in the age group of 40-50 years. In the control group 66% women as compared to the men 32%, In prediabetic group 72% subjects were showed general obesity, central adiposity was identified in 84%. It was observed that body mass index and waist circumference were significantly higher in prediabetic subject. Conclusion: This study has important implications for identification of subjects at higher risk for future type 2 diabetes and suggested that mass screening of prediabetic subjects and aggressive risk modification and close follow-up should be considered for prediabetic subjects with metabolic syndrome

    Cancers of anal canal: A review article

    Malignant tumoursof the anal canal are rare and diverse group of tumours of gastrointestinal tract and comprises of 2.5 percent of all digestive system malignancy in United States. Although incidence rates are still low, there has been a signifi cant increase in squamous cell carcinoma over the last 50 years. HIV infected homosexual men appear particularly at risk. HPV DNA is detectable in most anal squamous cell carcinomas. Despite its short length, the anal canal produces a variety of tumour types refl ecting its complex anatomic and histological structure.Squamous, glandular, transitional, and melanocytic components occur at this site, either alone, or in combination. Due to the paucity of this malignancy it has been diffi cult to establish generally accepted guidelines for treatment. While for some neoplasms, the treatment of choice is clear-cut, for others it is still controversial. This review article makes an attempt to clarify current clinical, pathological and therapeutic options for anal canal tumors in the light of recent information. &nbsp
