    A Prospective Study of Outcome of Percutaneous Needle Tenotomy for Tendo-achilles Release in Congenital Talipes Equino Varus

    Aim-To study the outcome of percutaneous tenotomy of tendo-achilles using a wide bore needle in Congenital Talipes Equino Varus. Background-Congenital Talipes Equino Varus is a commonly seen complex congenital foot deformity that can be treated conservative if the Ponseti technique is followed correctly. Percutaneous tenotomy of tendo-achilles forms an integral part of this technique. Material Methods-Forty nine feet were prospectively studied in thirty two patients, treated with the Ponseti technique between July 2011 and June 2013, out of which forty feet (81.6%) required percutaneous needle tenotomy of tendo-achilles. Results-Out of these forty feet, thirty six (90%) feet were managed successfully, without any complication of excessive bleeding, pseudo-aneurysm formation or neuro-vascular compromise, which was reported with the conventional tenotomy by a knife. Conclusion-Percutaneous tenotomy of tendo-achilles with a needle is a easy to learn, safe and effective method of tenotomy in Ponseti technique

    Renal Angiomyolipoma A case Report

    Angiomyolipoma (AML) is a rare benign tumor composed of adipose tissue, smooth muscles and blood vessels with an incidence is 0.3-3% which is predominantly found in females. These tumors have a strong association with tuberous sclerosis but can occur sporadically. Clinical importance lies in their susceptibility to spontaneous haemorrhage. They can be unilateral or bilateral. Interestingly these tumors occur more in right kidney. We present a case of sporadic unilateral angiomyolipoma of left kidney in a 55 yrs old female who had complaints of off and on left lumbar pain. She was subjected to left nephrectomy and diagnosis was confirmed on histopathology

    Proton Pump Inhibitors in Gastroesophageal reflux in Pretems: A Systematic Review

    Background- Gastroesophageal reflux is primarily physiological in newborns, especially in prematures less than 34 weeks of gestation. Recently there has been a surge in the use of anti reflux medications especially proton pump inhibitors to treat significant reflux in newborns the rewards of which are questionable and hence their use needs to be addressed.Material Methods-A PubMed search was done using the key words “gastroesophageal reflux”[all fields] and “preterm”[all fields] and/or “term” [all fields] and/or “newborn”[all fields]. The articles included were randomized trials, prospective studies, retrospective studies, review articles and observational studies.Conclusion- As per current evidence, the risk to reward ratio of proton pump inhibitors does not favour it to be routinely recommended for treatment of gastroesophageal reflux in preterm newborns

    Estimation of stature from head length

    Aim-In this study an attempt has been made to derive a linear regression equation for estimation of stature from the length of head. Background-Estimation of stature from head length is considered as an important parameter in medico-legal and forensic examinations. When highly decomposed and mutilated dead bodies with fragmentary remains are brought for postmortem examination, it becomes difficult to identify the deceased. Material Methods-The present study is conducted on 506 medical students 255 male and 251 female of age group between 18 - 25 years, was conducted at Index medical college hospital and research center Indore. The measurements were taken by using standard anthropometric instruments.. Results-The observed data was subjected to statistical analysis like‘t’ test for correlation coefficient. The value of ‘t’ was found to be statistically significant. Simple linear regression equation derived has been used for estimation of height.. Conclusion-It is concluded that the head length provides good reliability in estimation of stature in forensic examinations & in medicolegal cases and the correlation between present parameters can be helpful in medico-legal cases for identification of individuals

    Recurrent infections in a five year old boy

    Children are prone to develop recurrent illness in the form of upper respiratory tract infections, gastroenteritis, pustulosis and otitis media owing to developing immunity and environmental exposures. Majority of such children improve on symptomatic treatment. However, few of them might develop severe life threatening infections affecting vital organs. Primary immunodeficiency is an entity in which children develop recurrent infection due to defect in the development of immune system. This may affect plasma cells, T lymphocytes, natural killer cells or peripherally circulating leucocytes. We present a 5 year old male child with such diagnosis. A five year old boy was admitted with history of repeated infections since one year of age. His serum IgA, IgG and IgM levels were markedly decreased. Flow cytometry showed absence of B cells. He was started on intravenous immunoglobulin 10 gm every four weeks following which there was marked reduction in intercurrent infections. Primary immunodeficiency should always be suspected in differential diagnosis if history of repeated infections exist

    Role of knowledge in practicing Universal Precautions in Staff Nurses

    Aim-Authors aimed to present the importance and association of knowledge with practices of universal precautions.Background-Staff nurses are at high risk of exposure to infectious agents. Knowledge and practices towards universal precautions are necessary to prevent hospital associated infections.Material Methods-An observational study was conducted at different selected hospitals of Indore city. A total of 130 staff nurses were sought for informed consent considered as subject. The demographic profile had identified and associated with knowledge and practice of universal precautions. The main study tool was self-administered questionnaire.Results-Mean practices (49.6%) of universal precautions were significantly lower than mean knowledge (73.35%). 61.5% staff nurses had good knowledge but 43.8% were practicing at good level while none showed the practice excellence. The knowledge were not found associated (p>0.05) with practices of universal precautions. The practices of universal precautions reported strongly associated (p<0.002) with work experience but significantly (p<0.03) associated with knowledge. The workload of a staff nurse was identified as a distracter because no significant association (p>0.05) was evidenced between nurse patient ratio with knowledge and practice of universal precautions.ConclusionVarious studies have been shown that use of universal precautions lower the risk of hospital associated infection. Staff nurses had good knowledge but low practices of universal precautions. A good practice of universal precaution is a powerful tool to reduce the risk of contracting infections in clinical settings. Age, gender, course taken and clinical experience has their own effect towards knowledge and practice of universal precautions

    Comparative study of various techniques to measure neonatal hypothermia

    Aim-To compare different available modes (forehead infrared thermometer ,axillary temperature by thermister probe and digital thermometer, with axillary temperature by gold standard mercury thermometer) of temperature measurement in neonatal hypothermia in order to come out with most accurate one among them.Background- Hypothermia is very important in essential newborn care as it can lead to mild to severe life threatening complication, so detecting hypothermia takes important role in its management.Material Methods- Study Design: Setting was Neonatal Intensive Care Unit (Level 3), Kamla Raja Hospital ,Gajra Raja Medical College, Gwalior, India. A Prospective study of one year (July 2010 - June 2011 ) duration done with 1690 admissions of neonatal intensive care unit (full fill inclusion criterion ) by applying Fisher test on 2×2 contingency table to get sensitivity , specificity , positive predictive value , negative predictive value and accuracy of above 4 methods by STATA 9.1 (STATA corporation, college station , TX, USA).Results- Digital thermometer is having highest sensitivity (99.1%), specificity (98.1%), positive predictive value (97.4%) and negative predictive value(99.3%).Conclusion- Digital axillary thermometry is the best alternative to mercury thermometer for measuring neonatal temperature compared with axillary temperature by thermister probe and forehead temperature by infrared thermometer. &nbsp

    Worldwide there is an increasing care of academic programs and research in Health Professions Education (HPE). Despite this there is still a lack of the Arabic academic programs and literature related to this field. The Eastern Mediterranean Regional Office of the World Health Organization (EMRO-WHO) is interested in enriching the Arabic literature of HPE in order to promote education and training of health manpower.In 2012, Suez Canal University, with the support of EMRO-WHO, has inaugurated a Diploma of Health Professions Education conducted through distance e-learning in Arabic language. The target group of the Diploma are health professions educators and trainers in the Arab region.To provide convenient, contextual and case sensitive literature for study in the Arabic Diploma of HPE, a set of practical guidelines were developed by a group of experts and consultants. Among these, practical guidelines for curriculum development were developed and published in an Arabic book
