    Color Doppler evaluation of carotid vessels in patients with stroke

    Aim- To analyze the structure of carotid plaque morphology and quantify the percentage of carotid cerebrovascular disease is an important formidable health problem globally. The stroke syndrome consists of the rapid development of a focal neurological deficit that is usually localized to an area of the brain stenosis using high resolution color Doppler ultrasound machine in stroke patients. Background: Stroke or supplied by a specific artery. Materials and Methods- This study was carried out on all clinically suspected cerebrovascular insufficiency cases referred for color Doppler of carotid arteries to the Radiology Department of Katuri Medical College and Hospital, Guntur from 2011 to 2013. Various Doppler parameters such as characteristics of plaque, degree of stenosis, peak systolic velocity ratio, end diastolic velocity (EDV) of internal carotid artery, EDV ratios were studied. Results- Totally 90 cases of cerebrovascular accident studied, 58 (64%) were males and 32 (36%) were females. The majority of male patients belonged to age group of 51-60 years (37.8%) and females 61-70 years (37.5%). The risk factors included hypertension, smoking, diabetes mellitus, and ischemic heart disease. The patient’s clinical symptoms included hemiparesis, hemiplegia, transient ischemic attack and altered sensorium. Conclusion- Color Doppler of carotid artery plays an important role in the identification of plaques and quantification of degree of stenosis and identification of “at risk” patients who may develop further stroke

    Neoplasms of the kidney are most commonly adenocarcinomas. Carcinomas of the renal pelvis and ureter are rare, accounting for only 4% of all urothelial cancers. Squamous cell carcinoma of renal pelvis and ureter are very rare and account for 6-15% of all renal tumours of which transitional carcinomas constitute the majority followed by adenocarcinoma and squamous cell carcinoma. Renal squamous cell carcinoma (RSCC) is a rare malignancy of the upper urinary tract characteristically presenting with advanced stage, the reported incidence being 1.4% of all renal malignancies. Very few cases have been reported in literature. Most patients have a history of chronic urolithiasis, renal infection or abuse of analgesics. This tumor is aggressive in nature and usually has a poor prognosis. We report two cases of renal squamous cell carcinoma one with characteristic history of chronic pyelonephritis and one without characteristic etiologic factors. These cases have been reported due to their extreme rarity and also highlight the silent presentation of these tumors and the need to keep it as a differential diagnosis while evaluating cases of nephrolithiasis and chronic inflammations

    Aim: To study the dermatoglyphic patterns and their role in prediction of diabetes mellitus Type 2 in Maharashtrian population. Background: Dermatoglyphic patterns of the individual are genetically determined and once formed they remains constant throughout the life, prevalence of diabetes mellitus is increasing drastically in developing countries particularly in India, in the present study an attempt has been made to find an association of the dermatoglyphic patterns of the individual and diabetes mellitus Type 2. Materials and Methods: This study was carried out on 50 patients of Type 2 diabetes mellitus confirmed by clinical and laboratory assessment obtained from outpatient department of Department of medicine from command hospital, Pune. The control group for study consists of 50 subjects and was obtained from local residents of Maharashtra, teaching and non-teaching staff from our hospital. Palmar and finger prints of both hands were taken using ink and pad method as described by Cummins and Midlow. Result: In the present study analysis of loops, arches and whorls revealed that there is no significant difference in the no of radial and ulnar loops, number of arches, true and composite whorls between diabetes Type-2 patient and non-diabetic (normal) subjects. On the other hand, analysis of “a-t-d” angle revealed that angle was significantly increased in diabetic patients than control subjects. Conclusion: It can be concluded that qualitative fingertip parameters such as arches, radial loops, ulnar loops show fluctuating asymmetry and hence are not useful for prediction of diabetes. The only parameter, which does not show fluctuating asymmetry with previous studies is “a-t-d” angle. Hence, it can be concluded that this parameter is useful for pre detection of diabetes by dermatoglyphic studies

    Aim: The study was intended to determine the level of use of holy water as antiretroviral (ARV) treatment among people living with human immune virus (HIV)/AIDS (PLWHA) at Debrebrihan Hospital and Health Centre, Northeast, Ethiopia. Background: Holy water use in the form of a drink and a shower is the main treatment used by the PLWHAs together with other spiritual practices. A growing number of PLWHA in Ethiopia today are relocating to holy water sites. Materials and Methods: A cross-sectional study design with systematic random sampling through a structured questionnaire by way of interview among 422 respondents in Debrebrihan Hospital and Health Centre from December 26 2011 to January 26, 2012 were conducted. Single proportion formula with P = 50% used to determine the sample size. Frequency and percentage were computed by the SPSS statically software. Results: A total of 422 respondents was included in the study with a median age of 35 years; most of them were females 262 (62.1%) and 168 (39.8%) were married. Among the respondents 282 (66.8%) had experience of using holy water. The result also shown that 73 (25.9%) of respondents reported that it is not convenient to take medication while using holy water with spiritual fear of using ARV drugs and holy water together 51 (69.9%). Conclusion: The study concludes that the use of holy water as treatment of HIV/AIDS by patients and more than half of the study participants visit the holy water site b/c of most of them believe which is a result of sanity and punishment from God. The findings have also important implication in the use of mixed treatment for drug adherence through appropriate advocacy, education and cultural appropriate nearby treatment site depends on the study finding

    Comparison of effect of yoga on fasting blood sugar level, lipid profile and blood pressure in diabetes patient with addiction and without addiction

    Aim: In the present study, an attempt was made to evaluate the beneficial effects of pranayama on glycemic control, hypertension and dyslipidemia between addicted diabetic and non-addicted diabetic patients. Background: Diabetes is a global epidemic and it has been posing a biggest threat ever witnessed with devastating human, social and economic consequences. Materials and Methods: The study was conducted on 28 male patients suffering from diabetes mellitus Type-II between the age group of 30-60 years, for the period of 6 month at Index Medical College Hospital and Research Centre, Indore. Subject will be practicing yoga or simple pranayama (breathing exercise) for approximately 5 min for a period of 6 months. Results: The result of this study suggests that after 6 months of yogic exercises there is a significant decline in fasting blood sugar level of subjects who have no addiction, but lipid profile and blood pressure do not show any significant change either on addicted or non-addicted diabetes patients. The effect of yoga practice on various parameters were recorded and statistically analyzed by paired t-test for evaluation. Conclusion: Diabetes mellitus silently progress it compromises the function of many of the system so, people should not delay in achieving the effective control.     Bony suprascapular foramen, a potential site for suprascapular nerve compression: A case report

    Suprascapular notch is the site where the suprascapular nerve traverses the upper border of the scapulae under the superior transverse scapular ligament (STSL). The anterior coracoscapular ligament runs in the suprascapular notch, below the STSL. Narrowed bony foramen by complete ossification of these ligaments may be one of the predisposing factors for suprascapular nerve compression. We report the two cases of the bony suprascapular foramen, scapula with complete suprascapular foramen and scapula with suprascapular foramen with the notch. The aim of our case report is to provide knowledge on the variations along the course of suprascapular nerve which is essential to understanding the source of the entrapment syndrome. A thorough knowledge of anatomical variations in the suprascapular region and ossification of suprascapular ligament is important for anatomists, orthopedicians, radiologists, and neurosurgeons to obtain a safe zone which would be useful to avoid iatrogenic nerve lesion and for better diagnosis and management of the nerve entrapment syndrom

    Comparing Problem Based Learning Curriculum and the Traditional Curriculum based on outcome

    It has been argued and shown by various outcome studies that traditional knowledge based assessments of curriculum outcomes have minimal or no difference in learners graduating from either PBLC or the traditional curriculums. A literature search on problem based learning has shown several comparison studies between graduates from the PBLC with those from the traditional method of teaching, these include: the academic process; program evaluation; academic achievement; graduates’ performance, specialty choices and practice characteristics; and faculty members’ satisfaction. This review article will compare PBLC graduates with the traditional or conventional style of learning, and based on the above; conclude weather PBLC needs to be implemented in medical schools

    Thanatophoric dysplasia Type 1: A rare case of recurrence
