Global Leadership Initiative on Malnutrition (GLIM) plus Patient- and Nutrition-Derived Outcome Risk Assessment Score (PANDORA) predicts intensive care mortality: A 180-day follow-up study
Background and Aim: Malnutrition is strongly related to mortality in intensive care unit (ICU) patients. The Patient- and Nutrition-Derived Outcome Risk Assessment Score (PANDORA) is a novel mortality prediction tool encompassing nutritional assessment. Since there is limited evidence regarding the power of PANDORA in predicting mortality in critically ill patients, we aimed to evaluate the benefit of adding PANDORA to the Global Leadership Initiative on Malnutrition (GLIM) for mortality prediction in the ICU setting by comparing it with the other valid mortality predictors. Methods: A total of 251 ICU patients were enrolled in the study. Malnutrition evaluation tools (i.e., GLIM criteria, The nutritional risk screening 2002, The modified Nutrition Risk in Critically Ill score), and mortality/disease severity tools (PANDORA, The Acute Physiology and Chronic Health Evaluation II score, Sequential Organ Failure Assessment) were performed. Scores >43 were defined as high PANDORA score. Results: The median age (IQR) of participants was 67 (52-78) years, and 47.0% (n = 118) were female. Patients were evaluated according to 30th, 60th, 90th, and 180th-day mortalities. In all groups, patients were older, had a lower body mass index and longer hospitalization time, and were more likely to have malnutrition and higher PANDORA, Acute Physiology and Chronic Health Evaluation II, and Sequential Organ Failure Assessment scores. In the Cox regression analyses, even if adjusted for various confounders, malnutrition according to GLIM criteria and a high PANDORA score had the highest hazard ratio (HR) for mortality (HR: 3.62; 95% confidence interval [CI]: 1.49-8.77; P = 0.004, HR: 3.71; 95% CI: 1.69-8.12; P = 0.001, HR: 3.46; 95% CI: 1.69-7.06; P = 0.001 and HR: 4.00; 95% CI: 1.98-8.09; P = 0.004 for 30th, 60th, 90th, and 180th days, respectively). Conclusions: PANDORA is a valid tool for predicting mortality in ICU patients. Furthermore, to our knowledge this is the first study to reveal that integrating GLIM criteria into PANDORA may enhance its power in this setting. (c) 2024 Elsevier Inc. Reliability and validity of the Turkish version of the test of childhood stuttering (TOCS)
Propose: This study aimed to evaluate the reliability and validity of the Turkish version of the Test of Childhood Stuttering (TOCS). Methods & Procedure: The TOCS was translated into Turkish, then back into English by a linguist, and reviewed by an evaluation committee. The translated version was administered to 50 children who stutter (CWS) and 50 who do not (CWNS). After 7-10 days, 12 randomly selected children were re-evaluated. Reliability was assessed using Cronbach's alpha, test-retest methods, and inter-rater reliability. Content validity was confirmed through a pilot study and expert feedback. Construct validity was examined by analyzing test performance, comparing scores between CWS and CWNS, correlating modality test scores, and performing factor analysis. Criterion validity was assessed by correlating TOCS-TR scores with the Stuttering Severity Instrument-4 Turkish Version (SSI-4-TR). Results: Internal consistency was high, with Cronbach's alpha at .89. The test-retest correlation coefficient for Speech Fluency Measure subtest were .95, the Speech Fluency Rating Scale subtest was .91, and the Disfluency-Related Consequences Rating Scale Index was .91. Inter-rater reliability showed excellent agreement. Content validity was culturally appropriate. Construct validity indicated an 82% positive predictive value, 100% sensitivity, and 78% specificity for the Speech Fluency Rating Scale and Disfluency-Related Consequences Rating Scale. Criterion validity showed 80% agreement with SSI-4-TR. Conclusions & Implications: The TOCS-TR demonstrated strong reliability and validity, making it a valuable tool for assessing stuttering severity in children and distinguishing between CWS and CWN
Evaluation of patient-reported outcome measures (PROMs) of IBD patients who completed upadacitinib induction period
The evaluation of different information methods on behaviour and anxiety levels of children aged 6 to 9 years: a randomised controlled trial
PurposeTo evaluate the alteration of pre-post informing anxiety levels and behavioural scores of the children in the groups between those who were shown the multimedia-application and verbally explained.MethodsIn this randomized controlled study, healthy children, who applied to a paediatric-dental-clinic who never had a dental visit or dental procedure rather than oral examination were included. In the waiting room evaluation of children with a behaviour-control-list and Frankl's-Behaviour-Rating-Scale were performed, Venham-Picture-Test (VPT) was shown to the children. Then, the random information method (with a multimedia-application or verbally) was used for giving information. Finally, the child was observed whilst sitting in the dental chair and meeting with dentist, the behaviour-control-list was filled out again, and child's behaviour were re-scored.ResultsA total of 56 children [mean (+/- sd) age of 7.3 (+/- 1.1)] participated. There was no statistically significant difference between the median values of the change in Frankl and VPT-scores after the information (p = 0.261 and p = 0.651, respectively). The percentage change of pre-post informing in definitively negative children according to Frankl's score was higher in the multimedia application group (25%). Besides, the percentage change in children scored with score 5-6 according to VPT was also higher in multimedia-application group (25%-28.6%, respectively, for scores 5-6).ConclusionAlthough the score changes after the information procedure were more pronounced in the multimedia-application group for both VPT and Frankl-scores, the difference was not statistically significant. Multimedia applications seem to offer a promising modern tool for patient education before dental examination in 6- to 9-year-old children

Medical Students' Tobacco Consumption Status and Experiences with Smoke-free Law Violations in Enclosed Spaces in Turkiye and Northern Cyprus
OBJECTIVE: This study aimed to determine the frequency of tobacco smoking among medical students and assess their exposure to violations of smoke-free laws in enclosed spaces 30 days before the study. It also identifies key locations where such infractions occur and explores associated factors. MATERIAL AND METHODS: A descriptive study was conducted using a questionnaire-based survey among students from two public and two private medical faculties in T & uuml;rkiye and Northern Cyprus in 2023. Of the invited students, 628 responded. Data were analyzed using IBM Statistical Package for the Social Sciences statistics for Windows, version 23.0. Descriptive statistics included frequencies, percentages, means +/- standard deviations, and medians (interquartile ranges). Relationships between categorical variables were assessed using chi-squared and Fisher's exact tests, with significance set at P < 0.05. Binary logistic regression analysis of predictors of exposure to smoke-free violations was conducted. Ethical approval was obtained from each university. RESULTS: Most participants were female (56.4%), with a mean age of 21.5 +/- 2.37 years. Nearly one in five students was a current tobacco user (19.9%). Smokers had higher exposure to passive smoking than non-smokers (92.0% vs. 82.5%, P = 0.009). Encountering tobacco industry-branded vehicles was associated with higher exposure (92.3% vs. 83.7%, P = 0.03). Exposure to violations of the antismoking ban in enclosed spaces was significantly higher among students who smoked with higher frequency (odds ratio: 2.418, 95% confidence interval: 1.172 to 4.990, P = 0.017). CONCLUSION: This study underscores the need for strict tobacco control among medical students, with an emphasis on advocacy and interdisciplinary collaboration to combat the tobacco industry's influence
Active surveillance of cervical intraepithelial neoplasia grade 2: 2025 British Society of Colposcopy and Cervical Pathology and European Society of Gynaecologic Oncology consensus statement
Histological diagnosis of cervical intraepithelial neoplasia grade 2 (CIN2) has traditionally been the cutoff for local surgical treatment, due to a substantial risk of cancer development. However, evidence from the past decade suggests 50-60% of CIN2 lesions spontaneously regress, and active surveillance (or conservative management-ie, leaving the lesion untreated) might be justified in some cases. Active surveillance of CIN2 lesions is now practised widely, although clear recommendations on eligibility, frequency of surveillance, threshold for treatment, and criteria for return to routine recall are insufficient in most countries. In 2023, the cumulative risk of invasive cancer over 20 years was found to be substantially higher in patients under active surveillance when compared with patients who received immediate local treatment, with the greatest difference observed in women older than 30 years. This Policy Review and practice algorithm from the British Society of Colposcopy and Cervical Pathology and the European Society of Gynaecologic Oncology prevention committees aims to review existing evidence and present clear recommendations to assist clinical decision making. Active surveillance, rather than immediate treatment, might be reasonable in a carefully selected cohort of patients. The risk of progression, need for repeat visits, and cumulative risk of future invasion associated with active surveillance should be carefully balanced against the benefits of awaiting regression, including consideration of the woman's age, fertility wishes, additional risk factors, and likelihood of compliance to follow-up. Clinical audit and, ideally, prospective databases are required to monitor long-term outcomes and safety
Thermoeconomic analysis of a geothermal power plant by comparison of different exergetic methods
Exergoeconomics is a vital complementation of thermodynamic performance analysis. In this study, a comprehensive exergoeconomic analysis of a binary geothermal power plant in southwestern Anatolia is conducted to determine improvement potentials in the plant configuration. By utilization of cost allocation rules of three different exergoeconomic methods (Moran, Specific Exergy Costing (SPECO), Exergy Cost Theory) plant is analyzed in terms of exergetic costs and possible optimization areas. Levelized cost of electricity (LCOE) estimated by 3 different methods vary within a 3.6% range, between 7.81 c/kWh. It has also been determined that 51.5% of LCOE is constituted by waste/residual costs. Components especially including a thermal phase change or energy conversion, whose exergoeconomic factors below 0.5 warrant investment and optimization for performance improvement despite their higher individual exergetic efficiencies reported in previous studies. This phenomenon highlights the importance of considering exergetic efficiency and exergoeconomic factors together as plant design parameters. By using advanced materials or by optimizing the temperature gradient between the geothermal brine and the working fluid, heat transfer efficiency can be enhanced in heat exchanger devices. Turbines generally have mechanical losses which can be enhanced by optimizing blade design, reducing friction, and enhancing the thermodynamic cycle (i.e., use re-heat stages or improve steam conditions)
