    Building robust finances : how to assess and drive financial resilience at the Swiss local government level?

    In recent years, the world has faced an increasing array of global and regional crises. These events have significant repercussions on public finances, often resulting in reduced revenues and increased expenditures for national and local governments. Research on the financial resilience of local governments (LGs) shows that financial resilience is in fact a multidimensional concept (Barbera et al., 2017). It is not merely a matter of building financial buffers, but also includes non-financial dimensions. Despite increasing research interest, financial resilience is still modelled abstractly and is therefore not widely used in public financial management practice. This paper therefore examines the factors that contribute to financial resilience of LGs, focusing on Switzerland, whose institutional context provides a perfect opportunity for such an examination due to its LGs' financial self-determination. The study collected perspectives from Swiss LG CFOs, oversight authorities, and public sector consultants through semi-structured interviews. A total of 11 CFOs, 6 external consultants, and 2 cantonal oversight authorities were interviewed. This qualitative approach allows identify the strategies, instruments and practices employed by LGs to manage or develop their financial resilience

    Distribution and specialties of broadly versus narrowly defined public health nurses working in government settings in the United States, 2022

    Objectives: To estimate the size of the US governmental public health nurse (PHN) workforce by setting and specialty, including those working outside of health departments. Methods: We used 2022 data from the National Council of State Boards of Nursing, representing 2846 registered nurses (RNs) in almost all states, with weights applied. We examined “setting” and “specialty” of PHNs according to broad and more restricted definitions. Results: Using a broad definition, PHNs in government settings made up 3.7% of the RN workforce, and 1.8% when specialty areas were narrowly defined. These percentages varied by state. PHNs working in public health, school health, and correctional health settings largely indicated their specialty practice areas as public health, school health, and “unspecified” rather than more narrow specialty areas. Conclusions: PHNs are a small proportion of the RN workforce. They consider themselves generalists and can be identified by specialty and when working in governmental settings outside of health departments. Public Health Implications: States with few PHNs may be underserving their communities. Better data are needed to understand specific functions and activities of the PHN workforce

    Im Zuge des gesellschaftlichen Wandels gewinnt Inklusion zunehmend an Bedeutung – eine Entwicklung, die unter anderem die Lebensrealität von Menschen mit Behinderungen massgeblich prägt. Trotzdem sehen sich Menschen mit Behinderungen nach wie vor mit vielfältigen Diskriminierungen konfrontiert, was im Kontext tabubehafteter Lebensbereiche wie Sexualität oder Tod besonders problematisch erscheint. Vor diesem Hintergrund beschäftigt sich die vorliegende Arbeit auf der theoretischen Basis des Capability- und Intersektionalitätsansatzes mit der Frage, inwiefern für Menschen mit körperlichen Behinderungen in der Schweiz Zugangshindernisse zu sexuellen Dienstleistungen und Suizidhilfe bestehen und welcher Handlungsbedarf sich daraus für die Soziale Arbeit ergibt. Der Ist-Zustand wird dabei anhand der kulturellen, rechtlichen und strukturellen Rahmenbedingungen im Sinne eines beschreibenden wie auch erklärenden Zugangs dargestellt. Die Untersuchung der genannten Rahmenbedingungen bestätigt die Hypothese, dass Menschen mit körperlichen Behinderungen in der Schweiz ungenügende Zugangsmöglichkeiten zu sexuellen Dienstleistungen und Suizidhilfe haben. Die Ausschlüsse ergeben sich insbesondere aus einer offenen und unscharfen Gesetzgebung sowie der daraus resultierenden institutionellen Praxis. Für die Soziale Arbeit manifestiert sich somit einerseits ein Handlungsbedarf im Sinne des politischen Mandats, andererseits muss aber auch bestehenden Diskriminierungen im Praxisalltag gezielt entgegengewirkt werden

    Toward ground truth : evaluating therapists’ reliability in video-based assessment of upper limb compensatory movements post-stroke

    Background: Upper limb compensatory movement patterns, possibly leading to negative long-term consequences, often occur during motor recovery post stroke [1]. Deep learning methods have shown promise in assessing movement quality [2]. Ensuring reliability in assessing compensatory movements is crucial for developing an accurate measurement tool and valid ground truth. This study aimed to evaluate the reliability of therapists’ visual assessments in evaluating compensatory movement patterns using video analysis. Methods: 22 OTs and PTs labeled video recordings showing a standardized drinking task performed by 7 patients after stroke using an online system (scale from 0 = no compensation to 100 = max. compensation). The post-stroke patients had mild to moderate upper limb motor impairments. We analyzed data using a generalized-linear mixed effects model with zero-inflated beta regression [3]. Results: Our cohort demonstrated good interrater reliability for the reaching (ICC: 0.76, CI: 0.44-1), drinking (ICC: 0.88, CI: 0.60-1) and returning (ICC: 0.75, CI: 0.43-1) phases. The intrarater reliability was good to excellent for the forward transporting (ICC: 0.84, CI: 0.20-1), returning (ICC: 0.76, CI: 0.21-1) and drinking (ICC: 1, CI: 0.24-1) phases. However, the global domains of smoothness and interjoint coordination showed poor interrater (ICC Smoothness: 0.49, ICC Interjoint coordination: 0.40) and intrarater reliability (ICC Smoothness: 0.13, ICC Interjoint coordination: 0.19), with CI’s ranging from 0.03-1. The wide CIs for the ICCs across all measured domains suggest that our results may not be replicable [3]. Conclusion: Based on our results we cannot recommend establishing a ground truth for the automated assessment of compensatory movement patterns based solely on therapists’ video-based assessments. These findings underscore the need for further research to determine how therapists’ ratings can be effectively integrated in the process of building a ground truth [3]

    Das smarte Spital der Zukunft – eine lohnenswerte Transformationsreise

    Nicht nur die Pandemie hat in der Schweiz die Notwendigkeit eines digitalen Wandels im Gesundheitswesen offengelegt, doch war es eben diese Zeit, die das Innosuisse Flagship SHIFT ins Leben rief, um sich dem Thema Smart Hospital zu widmen – also konkret der digitalen Transformation der Schweizer Spitallandschaft. Dabei steht SHIFT für «Smart Hospital: Integrated Framework, Tools & Solutions» und entwickelt im Rahmen eines gross angelegten Projektes (3.5 Jahre Laufzeit, 5.7 Mio. Schweizer Franken, 5 Forschungs- und 24 Anwendungspartner*innen) eine Plattform, die demonstriert 1) wie neue Technologien die nahtlose, «liquid» Behandlung ermöglichen und Qualität steigern (Säule Seamless Patient Path), 2) wie intelligente Lösungen ausgestaltet werden müssen, um Personalkompetenzen zu erhöhen und Patient*innen zu ermächtigen (Patient & Staff Empowerment) und 3) wie ein Smart Hospital klinische und unterstützende Prozesse effektiver und effizienter managen kann (Management of Hospital Systems). Damit wird eine Blaupause für die Digitalisierung im Gesundheitswesen geschaffen, die den Menschen (Patient*innen, Angehörige, Spitalpersonal) in den Fokus stellt und neben digitalen Technologien, auch die Vernetzung von Abläufen und Daten sowie moderne Organisationsformen ermöglicht. SHIFT greift dabei die vielfältigen Gründe auf, warum Spitäler den Weg hin zum Smart Hospital gehen sollten und zeigt konkret auf, wie eine digitale Transformation erfolgen kann. Übergeordnet werden eine Tech-Foundation, eine Wissensdatenbank und ein Transformationsmasterplan erarbeitet und vorgestellt, die der Spitallandschaft nachhaltig zur Verfügung gestellt werden sollen

    Securing heterogeneous network (HetNet) communications for wildfire management : mitigating the effects of adversarial and environmental threats

    In the face of adverse environmental conditions and cyber threats, robust communication systems for critical applications such as wildfire management and detection demand secure and resilient architectures. This paper presents a novel framework that considers both adversarial factors, building resilience into a heterogeneous network (HetNet) integrating Low Earth Orbit (LEO) satellite constellation with High-Altitude Platform Ground Stations (HAPGS) and Low-Altitude Platforms (LAPS), tailored to support wildfire management operations. Building upon our previous work on secure-by-component approach for link segment security, we extend protection to the communication layer by securing both Radio Frequency (RF)/Free Space Optics (FSO) management and different links. Through a case study, we quantify how environmental stressors impact secrecy capacity and expose the system to passive adversaries. Key findings demonstrate that atmospheric attenuation and beam misalignment can notably degrade secrecy capacity across both short-and long-range communication links, while high-altitude eavesdroppers face less signal degradation, increasing their interception capability. Moreover, increasing transmit power to counter environmental losses can inadvertently improve eavesdropper reception, thereby reducing overall link confidentiality. Our work not only highlights the importance of protecting networks from these dual threats but also aligns with the IEEE P3536 Standard for Space System Cybersecurity Design, ensuring resilience and the prevention of mission failures

    Das Erleben von Eltern und Fachpersonen in der Entscheidungsfindung für den Langzeit Suprapubischen Katheter in der Pädiatrie : eine Embedded Single Case Study

    Die Einlage eines Langzeit Suprapubischen Katheters (LSBK) gilt in der Pädiatrie als nicht standardisierte medizinische Maßnahme und wird nur in seltenen Fällen bei Kindern und Jugendlichen mit komplexen Erkrankungen oder Behinderungen eingelegt. Um die Familien in der Entscheidungsfindung und im Umgang mit dem LSBK unterstützen zu können, sind Kenntnisse seitens der Fachpersonen notwendig. Die vorliegende Embedded Single Case Study hatte zum Ziel, das Erleben von Familien von Kindern und Jugendlichen mit einem LSBK von der Entscheidung bis hin zum Leben mit dem LSBK zu untersuchen. Dazu wurden Interviews mit drei Familien von insgesamt vier betroffenen Kindern sowie drei involvierten medizinischen Fachpersonen durchgeführt. Die Analyse der Interviews erfolgte mittels einer inhaltlich strukturierenden qualitativen Inhaltsanalyse. Es zeigte sich, dass der LSBK für alle Familien die bestmögliche Option einer längerfristigen Harnableitung war und das Wohl der Betroffenen im Vordergrund stand. Familien und Fachpersonen betonten den Bedarf an Unterstützung im Entscheidungsprozess und im Selbstmanagement. Wichtige Aspekte in der Betreuung waren eine patienten- und familienorientierte Entscheidungsfindung, eine ganzheitliche Unterstützung im Selbstmanagement und eine gute Zusammenarbeit seitens der Eltern mit dem medizinischen Fachpersonal.The insertion of a long-term suprapubic catheter (LSBK) is considered a non-standard medical measure in paediatrics and is only inserted in rare cases in children and adolescents with complex illnesses or disabilities. In order to be able to support families in the decision-making process and in dealing with LSBK, knowledge on the part of the specialists is necessary. The aim of this embedded single case study was to investigate the experiences of families of children and adolescents with LSBK, from the decision to living with LSBK. To this end, interviews were conducted with three families of a total of four affected children and three medical professionals involved. The interviews were analyzed using a content-structuring qualitative content analysis. It was found that LSBK was the best possible option for longer-term urinary diversion for all families and that the well-being of those affected was paramount. Families and professionals emphasized the need for support in the decision-making process and in self-management. Important aspects of care were patient- and family-oriented decision-making, holistic support in self-management and good cooperation between parents and healthcare professionals
