Intraoperative pancreatoscopy in pancreaticoduodenectomy for intraductal papillary mucinous neoplasms of the pancreas: Application to the laparoscopic approach
Background: /Purpose: Owing to the characteristics of IPMNs, which have variable skipped lesions along the main pancreatic duct (MPD), determining the surgical margins is very difficult. This study aimed to investigate the efficacy and potential oncologic impact of intraoperative pancreatoscopy (IOP) compared to frozen section biopsy (FSB) in pancreaticoduodenectomy (PD) for pancreatic head IPMNs.
Methods: Data of patients who underwent PD for IPMNs of the pancreas between October 2007 and May 2020 were retrospectively reviewed. IOP was performed in selected patients with IPMNs with inconclusive MPD involvement based on preoperative evaluations. Patients were divided into two groups, IOP group, FSB group. Clinicopathologic features and oncologic outcomes were compared between two groups.
Results: 60 patients underwent PD (laparoscopic or robotic, 42; open, 18) for pancreatic head IPMNs. IOP was safely performed in 28 patients, including minimally invasive approach used in 21 patients (35%). IOP group had a significantly larger MPD size (9.15 ± 4.79 mm vs 6.43 ± 4.11 mm, p = 0.021). Based on IOP, the initial surgical plan could be changed in 5 patients (17.8%) for complete resection. Recurrence occurred in 2 patients in FSB group and 3 patients in IOP group during the follow-up period (33.2 months, [range, 3.5-131.4 months]). Overall disease-free survival rate did not significantly differ between two groups (p = 0.529).
Conclusions: IOP can be safely performed in patients with pancreatic head IPMNs with MPD dilatation, even in the laparoscopic approach. Further studies evaluating the long-term oncologic effect of IOP for the management of IPMNs are required.ope
Alzheimer’s disease-like cortical atrophy mediates the effect of air pollution on global cognitive function
Little is known about the effect of air pollution on Alzheimer's disease (AD)-specific brain structural pathologies. There is also a lack of evidence on whether this effect leads to poorer cognitive function. We investigated whether, and the extent to which, AD-like cortical atrophy mediated the association between air pollution exposures and cognitive function in dementia-free adults. We used cross-sectional data from 640 participants who underwent brain magnetic resonance imaging and the Montreal Cognitive Assessment (MoCA). Mean cortical thickness (as the measure of global cortical atrophy) and machine learning-based AD-like cortical atrophy score were estimated from brain images. Concentrations of particulate matter with diameters ≤ 10 μm (PM10) and ≤ 2.5 μm (PM2.5) and nitrogen dioxide (NO2) were estimated based on each participant's residential address. Following the product method, a mediation effect was tested by conducting a series of three regression analyses (exposure to outcome; exposure to mediator; and exposure and mediator to outcome). A 10 μg/m3 increase in PM10 (β = -1.13; 95 % CI, -1.73 to -0.53) and a 10 ppb increase in NO2 (β = -1.09; 95 % CI, -1.40 to -0.78) were significantly associated with a lower MoCA score. PM10 (β = 0.27; 95 % CI, 0.06 to 0.48) and NO2 (β = 0.35; 95 % CI, 0.25 to 0.45) were significantly associated with an increased AD-like cortical atrophy score. Effects of PM10 and NO2 on MoCA scores were significantly mediated by mean cortical thickness (proportions mediated: 25 %-28 %) and AD-like cortical atrophy scores (13 %-16 %). The findings suggest that air pollution exposures may induce AD-like cortical atrophy, and that this effect may lead to poorer cognitive function in dementia-free adults.ope
Psychometric properties of the Korean adaptation of the communities advancing resilience toolkit (CART) assessment survey
Aims: This study aimed to establish a cross-cultural adaptation of the Communities Advancing Resilience Toolkit (CART) assessment survey for Korean communities (K-CART) and evaluated its psychometric properties of K-CART.
Design: A cross-sectional study design was used.
Methods: A forward and backward translation of the CART was conducted. The psychometric properties of K-CART were evaluated with test-retest reliability, internal consistency, content validity and construct validity. A field study was conducted involving 315 older adults in Pohang city, South Korea from January to March, 2019. Two versions of K-CART were analysed, based on the four-factor and the expanded five-factor K-CART.
Results: The test-retest reliability and internal consistency were acceptable (intraclass consistency coefficient = .90). The consistency reliability was satisfactory (Cronbach's α = 0.91). The content validity index (CVI) was found to be acceptable; wherein the item-CVI and scale-CVI were both 0.95. Confirmatory factor analysis confirmed that the construct validity of the expanded K-CART with five factors reached a more acceptable model fit (χ2 /df = 2.27, CFI = 0.89, TLI = 0.86, RMESA = 0.05).ope
Cancer-associated vasculitides: a single-centre case series
Objectives: This study investigated the frequency of cancer-associated vasculitis, the types of associated cancers and vasculitides, and the temporal relationship in Korean patients who were diagnosed with both cancers and vasculitides.
Methods: This study performed a digital search of the clinical data repository using selected diagnostic terms of vasculitides among patients diagnosed with cancers from May 2001 to May 2021. The time gap between the time of diagnosis of 'cancers' and that of 'vasculitides' was limited to 3 years. The types of cancers and vasculitides were reviewed.
Results: The mean age of 73 patients with both cancers and vasculitides with a time gap of fewer than 3 years was 53.0 years and 42.5% were men. Of the 215,897 patients with cancers, 73 patients were also diagnosed with vasculitides (0.034%). The most common type of cancer was thyroid cancer (28.8%), followed by lymphoma (13.7%), whereas the most frequent type of vasculitis associated with cancer was Behcet disease (52.1%), followed by granulomatosis with polyangiitis (12.3%). The median time gap between cancer and vasculitis was - 17.0 days. Among vasculitides, Behcet disease was closely associated with various cancers compared to other types. Twenty-one patients exhibited both cancers and vasculitides between 0 and 90 days after the diagnosis of the corresponding cancer.
Conclusion: The frequency of cancer-associated vasculitis was 0.034% in Korean patients. The types of cancers and vasculitides in cancer-associated vasculitis and the distributions of sex and age may be dependent on ethnic and geographic differences. Activity Evaluation Methodology for the Disposed Medical Linear Accelerators
For the medical linear accelerators (linac) that utilize more than 10 MV of photon energy, components inside the linac head become radioactivate during the 10–15-year operating cycle. Prior to disposal, radioactive waste must be evaluated for activity, and the same procedure should be followed for medical linacs. In the Republic of Korea, regulation and methodology for the radioactivity evaluation for the medical linac is not established yet. In this study, we employed gamma spectroscopy and a survey meter for evaluating the radioactivity of medical linac components. The components of the Siemens linac considered in this study were classified after decommissioning, and dose rates were measured to up to a 5 cm distance from the component surfaces by using a survey meter. Radionuclides from components were detected using an in situ HPGe detector. Based on the type of radionuclides and dose rate, we estimated the radioactivity of the components. We studied the feasibility of the methodology for disposing of radioactive components by using the in situ HPGe detector.ope
Discrepancy between the Status Quo and Adjusted Risk of First-Onset Suicidal Ideation in Older Adults: A Longitudinal Study Based on the Korean Welfare Panel Study (2011-2021)
Whether older adults can more likely commit suicide than those in other age groups, after adjusting for other possible causes, remains unknown. We aimed to examine why elderly individuals are more likely to develop first-onset suicidal ideation than individuals in other age groups. We identified 2018 young, 3329 middle-aged, and 2714 elderly individuals without a history of suicidal ideation, from the Korean Welfare Panel Study 2011-2021. To determine key stressors that can induce suicidal ideation, selected groups of variables were adjusted progressively in a generalized estimating equation (GEE) model. Incidence rates of the elderly, middle-aged, and young individuals were 15.9, 22.0, and 11.3 per 1000 person-years, respectively. In GEE analysis, a positive association was not noted between age group and suicidal ideation after adjusting for stressor variables. Furthermore, the overly adjusted model (Full model) showed a strong negative association with aging; young [odds ratio (OR): 1.68, 95% confidence interval (CI): 1.35-2.11] and middle-aged (OR: 1.94, 95% CI: 1.38-2.73) individuals were more likely to develop first-onset suicidal ideation than the elderly. We found that full models, particularly assessing wealth rather than income, can explain why the elderly have higher suicide rates than those in other age groups.ope
Comprehensive Prediction Model, Including Genetic Testing, for the Outcomes of Cochlear Implantation
Objectives: Despite growing interest in the genetic contribution to cochlear implant (CI) outcomes, only a few studies with limited samples have examined the association of CI outcomes with genetic etiologies. We analyzed CI outcomes using known predictors and genetic testing results to obtain a comprehensive understanding of the impact of genetic etiologies.
Design: We retrospectively reviewed the medical records and images of patients who underwent cochlear implantation and genetic testing at a single tertiary medical institution, between May 2008 and December 2020. After excluding those whose speech test results were unavailable, and those in whom the implant was removed due to complications, such as infection or device failure, 203 patients were included in this study. The participants were categorized into adult (≥19 years), child (2-18 years), and infant (<24 months) groups. Outcomes were measured based on categories of auditory perception, monosyllable, disyllable, and sentence scores. For the infant group, the Infant-Toddler Meaningful Auditory Integration Scale score was used.
Results: Among the 203 participants, a causative genetic variant was identified in 117 (57.6%) individuals. The presence of a causative variant was significantly associated with better CI outcomes in the infant group (β = 0.23; 95% confidence interval, 0.01 to 0.47; p = 0.044), but not in the child and adult groups. In the genetically confirmed patients without cochlear malformation, genetic variants involving the spiral ganglion was a poor prognostic factor in the child group (β = -57.24; 95% confidence interval, -90.63 to -23.75; p = 0.004).
Conclusions: The presence of known genetic etiology of hearing loss was associated with better CI outcomes in the infant group, but not in the child and adult groups. A neural-type genetic variant was a poor prognostic factor in the genetically diagnosed child subgroup without cochlear malformation. Careful genetic counseling should be performed before cochlear implantation.restrictio
Association Between Proton Pump Inhibitor Exposure and Acute Kidney Injury After Cardiac Surgery
Objective: To evaluate the association of preoperative proton pump inhibitor (PPI) exposure with incident acute kidney injury (AKI) after cardiac surgery.
Patients and methods: The Severance cardiac surgery cohort included 9860 cardiac surgery patients aged 18 years or older. The National Health Insurance Service-senior cohort included 2933 patients aged 60 years or older who underwent cardiac surgery. Preoperative PPI exposure was defined as a PPI prescription within 3 weeks prior to cardiac surgery. Primary outcomes were postoperative AKI and AKI requiring dialysis (AKI-dialysis).
Results: In the Severance cardiac surgery cohort after propensity score matching for PPI exposure, incident AKI (44.0% [472 of 1073] vs 40.5% [1304 of 3219]) and AKI-dialysis (5.8% [62 of 1073] vs 3.7% [119 of 3219]) were more common in patients exposed to PPI than in those who were not. Hospital and intensive care unit stay durations were longer among PPI-exposed than PPI-nonexposed patients. Multivariable conditional logistic analyses revealed that PPI exposure was significantly associated with incident AKI (adjusted odds ratio [AOR], 1.21; 95% CI, 1.03 to 1.42; P=.02) and AKI-dialysis (AOR, 1.74; 95% CI, 1.15 to 2.63; P=.009). The National Health Insurance Service-Senior cohort had similar results, revealing a significant association between PPI exposure and incident AKI-dialysis (AOR, 1.87; 95% CI, 1.25 to 2.81; P=.003). Discontinuation of PPI prior to operation was associated with a lower odds of AKI development in both cohorts.
Conclusion: Preoperative PPI exposure may be a modifiable risk factor for AKI among patients undergoing cardiac surgery.restrictio
Radiomics approach to the condylar head for legal age classification using cone-beam computed tomography: A pilot study
Legal age estimation of living individuals is a critically important issue, and radiomics is an emerging research field that extracts quantitative data from medical images. However, no reports have proposed age-related radiomics features of the condylar head or an age classification model using those features. This study aimed to introduce a radiomics approach for various classifications of legal age (18, 19, 20, and 21 years old) based on cone-beam computed tomography (CBCT) images of the mandibular condylar head, and to evaluate the usefulness of the radiomics features selected by machine learning models as imaging biomarkers. CBCT images from 85 subjects were divided into eight age groups for four legal age classifications: ≤17 and ≥18 years old groups (18-year age classification), ≤18 and ≥19 years old groups (19-year age classification), ≤19 and ≥20 years old groups (20-year age classification) and ≤20 and ≥21 years old groups (21-year age classification). The condylar heads were manually segmented by an expert. In total, 127 radiomics features were extracted from the segmented area of each condylar head. The random forest (RF) method was utilized to select features and develop the age classification model for four legal ages. After sorting features in descending order of importance, the top 10 extracted features were used. The 21-year age classification model showed the best performance, with an accuracy of 91.18%, sensitivity of 80%, and specificity of 95.83%. Radiomics features of the condylar head using CBCT showed the possibility of age estimation, and the selected features were useful as imaging biomarkers.ope
The Effect of Cognitive Impairment on the Association Between Social Network Properties and Mortality Among Older Korean Adult
Objectives: This study investigated the effect of cognitive impairment on the association between social network properties and mortality among older Korean adults.
Methods: This study used data from the Korean Social Life, Health, and Aging Project. It obtained 814 older adults' complete network maps across an entire village in 2011-2012. Participants' deaths until December 31, 2020 were confirmed by cause-of-death statistics. A Cox proportional hazards model was used to assess the risks of poor social network properties (low degree centrality, perceived loneliness, social non-participation, group-level segregation, and lack of support) on mortality according to cognitive impairment.
Results: In total, 675 participants (5510.4 person-years) were analyzed, excluding those with missing data and those whose deaths could not be verified. Along with cognitive impairment, all social network properties except loneliness were independently associated with mortality. When stratified by cognitive function, some variables indicating poor social relations had higher risks among older adults with cognitive impairment, with adjusted hazard ratios (HRs) of 2.12 (95% confidence interval [CI], 1.34 to 3.35) for social nonparticipation, 1.58 (95% CI, 0.94 to 2.65) for group-level segregation, and 3.44 (95% CI, 1.55 to 7.60) for lack of support. On the contrary, these effects were not observed among those with normal cognition, with adjusted HRs of 0.73 (95% CI, 0.31 to 1.71), 0.96 (95% CI, 0.42 to 2.21), and 0.95 (95% CI, 0.23 to 3.96), respectively.
Conclusions: The effect of social network properties was more critical among the elderly with cognitive impairment. Older adults with poor cognitive function are particularly encouraged to participate in social activities to reduce the risk of mortality.ope
