A Case of Contribution to Correcting Polypharmacy through Pharmacist Involvement during Introduction of Hemodialysis
Introduction of hemodialysis in patients with chronic kidney disease bring about changes in their pharmacokinetics. Patients with chronic kidney disease often use combinations with multiple medications to treat complications; however, adverse drug events may occur if the same medication as that used before introducing hemodialysis is continued. Polypharmacy is a condition in which multiple medications lead to increased risk of adverse drug reactions, medication errors, and decreased medication adherence. Therefore, correcting polypharmacy is essential for appropriate pharmaceutical management to avoid risk. Here, we report a case of a pharmacist contributing to correction of polypharmacy during the introduction of hemodialysis. In this case, we were involved in blood pressure management and other aspects, which led to appropriate prescription from the perspective of avoiding adverse drug events and discontinuing unnecessary medications. Appropriate pharmaceutical management such as that provided in this case can contribute to maintain or improve quality of life; accumulation of such reports will contribute to appropriate medication in the elderly.departmental bulletin pape
Ergothioneine and its effects in vivo
Ergothioneine(EGT)is a hydrophilic sulfur-containing amino acid and can directly remove reactive oxygen species. More than 100 years have passed since its discovery. Although EGT has a high antioxidant capacity, its role in vivo has not been clarified yet. EGT exists in the bodies of animals and plants, including humans, but animals cannot biosynthesize EGT themselves. Animals have an EGT transporter, which is expressed in organs that are exposed to strong oxidative stress. EGT transporter expresses not only plasma membrane but also mitochondria membrane and protects from oxidative stress. In recent years, it has been reported that EGT is effective against diseases such as neurodegenerative diseases, diabetes, and cancer. EGT is thought to be one of the most important small molecules that may be useful in the diagnosis and treatment of various diseases in the future.departmental bulletin pape
Comparison of the progression of cognitive impairment among different disease stages in patients with multiple sclerosis
Background: Although cognitive impairment(CI)is increasingly recognized as a core feature of multiple sclerosis (MS), the exact mechanisms that underlie CI are poorly understood. However, a recent study suggested that cortical atrophy and cognitive decline accelerate together during the progressive phase of MS. In this study, we aimed to clarify at what stage the progression of CI increases in Japanese patients with MS.
Methods: In this longitudinal retrospective analysis, fifty-five consecutive Japanese MS patients — relapsing-remitting MS(RRMS)(76%)and secondary progressive MS(SPMS)(24%)—were included. We evaluated the information processing speed(IPS), calculated the raw score and Z score, and investigated how much the values declined from 2021 to 2022. Brain volumes were evaluated with FreeSurfer software.
Results: Patients with SPMS had more severe physical disability, lower IPS, and more prominent brain atrophy than patients with RRMS, but there was no significant difference in the annual change in IPS. However, since three of the 13 SPMS patients transitioned from RRMS to SPMS between 2019 and 2021, we divided SPMS patients into the RRMS-to-SPMS transition group and the SPMS group and repeated the analysis. Compared with the RRMS group and RRMS-to-SPMS transition group, the SPMS group showed a significant increase in its annual IPS decline. The SPMS group had a lower Z score in 2021 than the other two groups, and the score tended to decrease further in 2022. Conclusion: Impairment in IPS begins to progress rapidly 2 years or more after the onset of the SPMS phenotype.departmental bulletin pape
