Get PDF症例は90歳，女性。腹部膨満感，食欲不振を主訴に受診し，閉塞性S状結腸癌の診断で入院して大腸内視鏡検査を施行した。S状結腸に完全閉塞した全周性２型病変があり，大腸ステント治療を行った。大腸ステント治療６時間後に下腹部に皮下気腫が出現し，12時間後には頸部から大腿近位部まで進展，さらにCTで後腹膜気腫，縦隔気腫，腹腔内遊離ガスを認め，緊急ハルトマン手術を施行した。腫瘍縁から約１cm肛門側の大腸ステント端部がS状結腸間膜内に穿通していた。術後は全身管理に難渋したが術後100日目に軽快退院した。閉塞性大腸癌に対する大腸ステント治療後の皮下気腫の出現は，大腸穿通の重要かつ唯一の所見になりうることを認識すべきである。departmental bulletin pape
