    Annual report 2022 on enteric infections and zoonoses

    Enterale infecties worden veroorzaakt door verschillende bacteriële, parasitaire en virale ziekteverwekkers. Meestal krijgen mensen daarvan maag-darmklachten, zoals overgeven, buikpijn en/of (bloederige) diarree. In sommige gevallen kunnen ernstige klachten ontstaan, zoals hepatitis, bloedvergiftiging of hersenvliesontsteking. Mensen raken meestal besmet via voedsel of water dat in contact is gekomen met ontlasting van mensen of dieren. Bij een overdacht via dieren spreken we van zoönosen. Goede hygiëne in toiletten en keukens en bij de bereiding van vlees is heel belangrijk om een besmetting te voorkomen. Het RIVM monitort hoe vaak dit soort infecties in Nederland voorkomen. Ook brengt het RIVM uitbraken in kaart, waarna het de bronnen opspoort. Het doet dat samen met partners als de Nederlandse Voedsel- en Warenautoriteit (NVWA(Nederlandse Voedsel- en Warenautoriteit)) en Wageningen Food Safety Research (WFSR(Wageningen Food Safety Research )). In het algemeen was het aantal enterale infecties in 2022 hoger dan tijdens de COVID19 jaren 2020 en 2021. Het aantal mensen dat ziek werd van Campylobacter en Salmonella was nog wel lager dan vóór de pandemie. Het aantal infecties met Shiga toxine-producerende E. coli(Escherichia coli) (STEC(Shigatoxineproducerende E. coli-stammen)) was in 2022 het hoogst sinds 2016. De oorzaak is niet duidelijk. Het aantal mensen dat ziek werd door de Listeria-bacterie was net iets hoger dan in de jaren voor de COVID19 pandemie. Dit was een van de weinige ziekteverwekkers die tijdens de pandemie niet minder vaak voorkwam dan daarvoor. Dit komt waarschijnlijk doordat besmettingen bijna altijd via (rauw) voedsel verlopen. Het rotavirus- en norovirusseizoen 2021/2022 begon, net als in 2020/2021, eerder in het jaar (oktober) dan in de gebruikelijke wintermaanden. Dit komt waarschijnlijk doordat meer mensen vatbaarder zijn geworden voor deze virussen. In de COVID19 jaren kwamen deze virussen namelijk bijna niet voor en bouwden mensen er geen afweer tegen op. Het totaal aantal mensen met het rotavirus in 2022 was veel hoger dan in 2016-2019. In 2022 werden in totaal evenveel mensen ziek van het norovirus als voor de pandemie. De opvallendste uitbraak in 2022 was dat 72 mensen met buiktyfus (Salmonella Typhi) kregen op een schip in Nederland waar asielzoekers werden opgevangen. Vrij zeker zijn ze via de drinkwatervoorziening van het schip besmet. Deze was verouderd en lag vlak bij de rioolleidingen.Enteric infections’ is an umbrella term for a diverse group of pathogens that infect humans primarily through the faecal-oral route. These infections can be caused by a diverse range of bacterial, parasitic and viral pathogens. They usually lead to gastrointestinal complaints such as vomiting, abdominal pain, diarrhoea and/or bloody diarrhoea. In some cases, these infections can lead to serious symptoms such as hepatitis, blood poisoning or meningitis. Some enteric infections, known as zoonoses, spread from agricultural and other pets to humans. For other infections, humans are the only host and the infection spreads from person to person. In both cases, transmission often occurs via contaminated food, or via the environment (soil/air/surface water) or direct contact with animals or faeces. In order to monitor the spread of enteric infections, RIVM charts for every year how often such infections and the pathogens occur in the Netherlands. Overall, the incidence of enteric infections mainly transmitted through food was higher in 2022 than during the COVID-19 pandemic years of 2020 and 2021, although the incidence of campylobacteriosis and salmonellosis was not yet at pre-pandemic levels. In contrast, the number of infections with Shiga toxin-producing E. coli (STEC) in 2022 reached the highest level since 2016. The reason behind the increase in domestically acquired cases is unclear. The incidence of listeriosis, which remarkably did not decrease during the pandemic, was also higher in 2022 compared to 2016–2019. The 2021/2022 rotavirus and norovirus season started earlier than usual, just like in 2020/2021. This was likely due to an increase in the number of susceptible individuals following the lifting of COVID19 restrictions. The total number of rotavirus cases was significantly higher in 2022 than in 2016-2019, while the total number of norovirus cases in 2022 was at the same level as before the pandemic. The most notable outbreak in 2022 was an outbreak with 72 cases of typhoid fever (Salmonella Typhi) on a ship hosting asylum seekers. The drinking water supply was almost certainly the source of the contamination. In addition, there was a shigellosis outbreak related to travel to Cape Verde that continued into 2023

    Evolution of transient RNA structure-RNA polymerase interactions in respiratory RNA virus genomes.

    RNA viruses are important human pathogens that cause seasonal epidemics and occasional pandemics. Examples are influenza A viruses (IAV) and coronaviruses (CoV). When emerging IAV and CoV spill over to humans, they adapt to evade immune responses and optimize their replication and spread in human cells. In IAV, adaptation occurs in all viral proteins, including the viral ribonucleoprotein (RNP) complex. RNPs consist of a copy of the viral RNA polymerase, a double-helical coil of nucleoprotein, and one of the eight segments of the IAV RNA genome. The RNA segments and their transcripts are partially structured to coordinate the packaging of the viral genome and modulate viral mRNA translation. In addition, RNA structures can affect the efficiency of viral RNA synthesis and the activation of host innate immune response. Here, we investigated if RNA structures that modulate IAV replication processivity, so-called template loops (t-loops), vary during the adaptation of pandemic and emerging IAV to humans. Using cell culture-based replication assays and in silico sequence analyses, we find that the sensitivity of the IAV H3N2 RNA polymerase to t-loops increased between isolates from 1968 and 2017, whereas the total free energy of t-loops in the IAV H3N2 genome was reduced. This reduction is particularly prominent in the PB1 gene. In H1N1 IAV, we find two separate reductions in t-loop free energy, one following the 1918 pandemic and one following the 2009 pandemic. No destabilization of t-loops is observed in the influenza B virus genome, whereas analysis of SARS-CoV-2 isolates reveals destabilization of viral RNA structures. Overall, we propose that a loss of free energy in the RNA genome of emerging respiratory RNA viruses may contribute to the adaption of these viruses to the human population

    Surveillance into zoonoses in veal calfs 2022

    Dit rapport bevat een erratum d.d. 04-12-2023 op pagina 89. Dieren kunnen ziekteverwekkers bij zich dragen waarvan mensen ook ziek kunnen worden. De ziekten die hierdoor worden veroorzaakt, noemen we zoönosen. In 2022 is de mest en neus van vleeskalveren op 180 Nederlandse bedrijven onderzocht. Ook is bij 55 kalverhouders, gezinsleden en medewerkers onderzocht of ze de ziekteverwekkers bij zich dragen. Het RIVM, de NVWA(Nederlandse Voedsel- en Warenautoriteit) en WFSR(Wageningen Food Safety Research ) (Wageningen Food Safety Research) hebben dit onderzoek gedaan. Het onderzoek richt zich op verschillende ziekteverwekkende bacteriën. De belangrijkste zijn Campylobacter, STEC(Shigatoxineproducerende E. coli-stammen), Listeria en Salmonella. Daarnaast is gekeken naar ESBL(Extended spectrum beta-lactamases)-producerende bacteriën, colistineresistente bacteriën en MRSA(Methicilline-resistente Staphylococcus aureus ). Deze laatste zijn belangrijk omdat veel soorten antibiotica daar niet tegen werken. De meeste van deze ziekteverwekkers kunnen bij mensen diarree veroorzaken, maar bij mensen met een kwetsbare gezondheid kunnen infecties ernstiger verlopen. De ziekteverwekkers zitten meestal in de darmen van de dieren en dus ook in de mest. Het vlees kan tijdens de slacht besmet raken. Het is daarom belangrijk om kalfsvlees goed gaar te eten. Campylobacter kwam het meeste voor en is op 96 procent van de bedrijven gevonden. Bij de veehouders en gezinsleden is deze bacterie bij 5 personen gevonden. Het waren wel andere typen Campylobacterbacteriën dan die de dieren op de bijbehorende bedrijven bij zich droegen. Deze mensen kunnen op een andere manier met Campylobacter besmet zijn geraakt, bijvoorbeeld via voedsel of andere dieren. STEC-bacteriën, Listeria en Salmonella kwamen wat minder vaak voor, namelijk op 66 procent (STEC), 20 procent (Listeria) en 15 procent (Salmonella) van de bedrijven. Deze drie bacteriën zijn vaker gevonden bij bedrijven met zogenoemde rosé kalveren dan bij bedrijven met blanke kalveren. Twee personen droegen STEC bij zich en één deelnemer Listeria. Salmonella is niet bij de deelnemers gevonden. ESBL-producerende bacteriën zijn op 27 procent van de bedrijven gevonden en bij 3 deelnemers. Het percentage bij de deelnemers is ongeveer hetzelfde als bij de Nederlandse bevolking. Veegerelateerde MRSA is op 25 procent van de bedrijven gevonden, vaker bij bedrijven met blanke kalveren, en bij 13 procent van de deelnemers; dit is hoger dan MRSA bij de Nederlandse bevolking. Ten slotte zijn op 2 procent van de bedrijven colistine-resistente bacteriën gevonden, maar niet bij de deelnemers. Bij de bevolking is dat 0,8 procent.This report contains an erratum d.d. 04-12-2023 on page 89. Animals can carry pathogens that can also cause disease in humans. Such diseases are known as zoonoses. In 2022 manure and nose swabs of veal calves was investigated at 180 Dutch farms. In addition, 55 livestock farmers, family members and employees were also tested for these pathogens. The study was carried out by RIVM, the Netherlands Food and Consumer Product Safety Authority (Nederlandse Voedsel- en Warenautoriteit, NVWA) and WFSR (Wageningen Food Safety Research). The study focuses on multiple pathogenic bacteria. The most important are Campylobacter, STEC, Listeria and Salmonella. In addition ESBL-producing bacteria, colistin-resistent bacteria and MRSA are investigated. These bacteria are important because they are resistant to multiple groups of antibiotics. Most of these pathogens usually cause diarrhea in humans, but the infections can sometimes be more severe in vulnerable populations. The pathogens are usually present in the animal’s intestines and therefore end up in the manure as well. Meat can become contaminated during slaughter. It is therefore important to only eat veal that has been thoroughly cooked. Of the investigated pathogens, Campylobacter was found most frequently, namely at 96% of the farms. Among livestock farmers and family members, Campylobacter was found in 5 persons. The types of Campylobacter carried by the humans were different from the types found in the calves on their farms. These people could be infected by Campylobacter by other routes, for example via food or other animals. Shigatoxigenic Escherichia coli (STEC) bacteria, Listeria and Salmonella were found less, on 66% (STEC), 20% (Listeria) and 15% (Salmonella) of the farms. These three bacteria were found more often on farms with so called rosé calves then on farms with white calves. Two persons carried STEC and one person Listeria. Salmonella was not found in human participants. ESBL-producing bacteria were found on 27% of the farms and in 3 human participants. The percentage in human participants is comparable to the percentage in the general Dutch population. MRSA was found on 25% of the farms, more often in farms with white calves and in 13% of the human participants. This is higher than the percentage of MRSA in the general Dutch population. Finally, in 2% of the farms colistin resistant bacteria were found, but not in human participants. In the general population this is 0,8%

    Analysis of incidents with hazardous substances at Seveso companies 2023

    Het RIVM analyseert elk jaar de aard, omvang en oorzaak van incidenten bij bedrijven die in Nederland met grote hoeveelheden gevaarlijke stoffen werken. Het gaat om 15 van 24 incidenten waarvan de Nederlandse Arbeidsinspectie de onderzoeken tussen 2019 en 2021 heeft afgerond en waarover genoeg informatie bekend was voor een analyse. Bij veertien incidenten kwamen gevaarlijke stoffen vrij. Bij één incident explodeerde afdichtingmateriaal. Eén persoon heeft aan een chemische verbranding waarschijnlijk blijvend letsel overgehouden. Overige slachtoffers hadden tijdelijk letsel door ademhalingsproblemen, irritaties en brandwonden. Bedrijven zijn ervoor verantwoordelijk dat installaties op orde zijn en werknemers de productieprocessen en werkzaamheden veilig kunnen uitvoeren. Net als uit eerdere analyses bleek dat veel van deze incidenten hadden kunnen worden voorkomen. Het is daarom belangrijk dat bedrijven leren van deze incidenten en maatregelen nemen om ze te voorkomen. Zo waren bij acht incidenten de factoren waarmee processen onder controle worden gehouden, zoals druk, temperatuur of chemische reacties, niet in orde. Hierdoor konden ze afwijken van de veilige eisen die ervoor bestaan. Doordat de afwijkingen vaak niet zijn opgemerkt, konden gevaarlijke stoffen ontsnappen of kon een explosie ontstaan. Soms kunnen noodmaatregelen helpen om een incident te voorkomen. Zo zijn er systemen die een installatie automatisch uitschakelen om te voorkomen dat een gevaarlijke stof wegstroomt. Bij zeven incidenten waren geen noodmaatregelen getroffen of werkten ze niet goed. Bij veertien incidenten schoten plannen en procedures voor de werkzaamheden tekort. Soms waren ze niet gemaakt omdat de gevaren van tevoren niet waren verwacht. Soms zag het bedrijf er niet op toe dat instructies voor werkzaamheden werden nageleefd. Of de instructies waren niet duidelijk genoeg opgesteld. Deze rapportage maakt deel uit van de opdracht van het ministerie van Sociale Zaken en Werkgelegenheid om incidenten te analyseren die de Nederlandse Arbeidsinspectie heeft onderzocht. Het RIVM gaat na wat de overeenkomsten en verschillen tussen deze incidenten zijn. De resultaten kunnen worden gebruikt voor inspecties, of om het veiligheidsbeleid van bedrijven te verbeteren.Each year, the National Institute for Public Health and the Environment (RIVM) analyses the nature, scale and cause of incidents at companies working with large quantities of hazardous substances in the Netherlands. These are 15 of 24 incidents for which the Dutch Labour Inspectorate completed investigations between 2019 and 2021 and for which enough information was known for an analysis. Fourteen incidents involved the release of hazardous substances. In one incident sealing material exploded. One person probably sustained permanent injuries from a chemical burn. Other casualties suffered temporary injuries due to breathing problems, irritation and burns. Companies are responsible for ensuring that installations are in order and that a safe working environment is provided so that employees can work safely. As previous analyses showed, many of the incidents could have been prevented. It is therefore important that companies learn from these incidents and take measures to prevent them. For example, in eight incidents, the factors used to control processes, such as pressure, temperature or chemical reactions, were not in order. This allowed them to deviate beyond the safe design requirements. Because the deviations often went undetected, hazardous substances could escape or an explosion could occur. Sometimes emergency measures can help prevent an incident. For instance, there are systems that automatically shut down an installation to prevent a hazardous substance from escaping. In seven incidents, emergency measures were not in place or did not work properly. In fourteen incidents, plans and procedures for the work were inadequate. Sometimes they were not made because the risks were not anticipated beforehand. Sometimes the company did not ensure that work instructions were followed. Or the instructions were not drafted clearly enough. This report is part of the assignment from the Ministry of Social Affairs and Employment to analyse incidents investigated by the Netherlands Labour Authority. RIVM examines the similarities and differences between these incidents. The results can be used for inspections or to improve companies’ safety policies

    Metabolites in the regulatory risk assessment of pesticides in the EU.

    A large majority of chemicals is converted into metabolites through xenobiotic-metabolising enzymes. Metabolites may present a spectrum of characteristics varying from similar to vastly different compared with the parent compound in terms of both toxicokinetics and toxicodynamics. In the pesticide arena, the role of metabolism and metabolites is increasingly recognised as a significant factor particularly for the design and interpretation of mammalian toxicological studies and in the toxicity assessment of pesticide/metabolite-associated issues for hazard characterization and risk assessment purposes, including the role of metabolites as parts in various residues in ecotoxicological adversities. This is of particular relevance to pesticide metabolites that are unique to humans in comparison with metabolites found in in vitro or in vivo animal studies, but also to disproportionate metabolites (quantitative differences) between humans and mammalian species. Presence of unique or disproportionate metabolites may underlie potential toxicological concerns. This review aims to present the current state-of-the-art of comparative metabolism and metabolites in pesticide research for hazard and risk assessment, including One Health perspectives, and future research needs based on the experiences gained at the European Food Safety Authority
