学位論文要旨 
 
不眠症は、入眠困難や睡眠維持困難を特徴とした一般的な臨床症状である。持続的な不眠症は精神
疾患、特に気分障害の発症の重要なリスク要因であり、また、不眠症患者の 40%に精神疾患が併存
していると言われている。不眠症の有病率が高いことから、睡眠障害の専門家だけでなく、精神科
の臨床医も不眠症の患者を治療している。 
薬物療法、特にベンゾジアゼピン受容体作動薬を用いた治療は、不眠症の有効な治療法であるが、
一部の患者では通常の睡眠薬の量で長期依存や乱用が社会問題となっている。非薬物療法としては、
構造化され、複数の要素からなる認知行動療法(CBTI)が不眠症の治療に有効であるという確かな証
拠がある。さらに、CBTI は、米国睡眠医学会および米国医師会によって、成人慢性不眠症の第一選
択治療法として推奨されている。しかし、CBTI には熟練したセラピストのサービスが必要であり、
日本ではリソースが不足しているため、実際にはほとんど行われていない。そのため、日本では不
眠症治療の第一段階として睡眠衛生教育が推奨されている。日本の厚生労働省が 2014 年に発表した
「健康づくりのための睡眠指針」には睡眠衛生指導に関わる 12 の項目があり、各項目には CBTI の
個々の要素が組み込まれている。このガイドラインは睡眠衛生に有用であるが、外来では睡眠衛生
教育に十分な時間が割くことができず、全ての項目を伝えきることができないため、できるだけ短
時間で効果的な睡眠衛生指導を行う方法を考える必要がある。本研究では、大学病院の精神科外来
に通院する患者を対象に、睡眠衛生指導が不眠の改善に及ぼす影響について調査することを目的と
した。 
愛媛大学医学部附属病院精神科の外来患者を対象として、2021 年 10 月から 4 週間の横断的調査を
行った。患者の自己評価として、アテナ不眠尺度（AIS）、エプワース眠気尺度（ESS）、General Health

Questionnaire-30（GHQ-30）を実施した。患者の睡眠衛生を評価するために、「健康づくりのための
睡眠指針」の各睡眠衛生項目を使用した。また、並行して外来主治医による Global Assessment of 
Functioning（GAF）および Clinical Global Impression（CGI）を実施した。各尺度の信頼性は standardized 
Cronbach’s alpha を用いて検証された。本研究では、AIS で 6 点以上であった患者を不眠症のグルー
プと割り当てた。統計解析には SPSS version27 を用い、Mann-Whitney U 検定を用いて数値の変数を
比較し、χ2 乗検定を用いてカテゴリー変数を分析した。その後、ロジスティック回帰分析を用い
て、不眠症と「健康づくりのための睡眠指針」の各項目の実践状況との関連性を調べた。すべての検
定は両側検定であり、有意水準は 5％とした。なお、本研究は愛媛大学医学部臨床研究倫理委員会に
て承認を得ている（2108015 号）。 
対象者は 241 人（男性 92 人、女性 149 人）で、不眠症群に割り当てられたのは 133 人（男性 51 人、
女性 82 人）であった。不眠症群と非不眠症群を比較すると、GHQ、CGI、GAF において有意差が認
められた（GHQ 不眠 vs 非不眠：77.35 vs 57.03,CGI 不眠 vs 非不眠：3.33 vs 2.73, GAF 不眠 vs 非
不眠：64.59 vs 71.96）。 
健康づくりのための睡眠指針に関して、「良い睡眠で、からだもこころも健康に」(p = 0.002)、「良
い睡眠は、生活習慣病予防につながります」(p = 0.004)、「睡眠による休養感は、こころの健康に重
要です」(p = 0.006)、「年齢や季節に応じて、ひるまの眠気で困らない程度の睡眠を」(p = 0.006)、
「勤労世代の疲労回復・能率アップに、毎日十分な睡眠を」(p = 0.005)、「眠くなってから寝床に入
り、起きる時刻は遅らせない」(p = 0.006)という項目では、非不眠症群の方が不眠症群よりも実践で
きていることが示された。潜在的な交絡因子を制御した上で、多変量ロジスティック回帰モデルを
用いて、不眠症と睡眠に対する意識との関連性を特定した結果、「眠くなってから寝床に入り、起き
る時刻は遅らせない」という習慣を持つ患者は、不眠症である割合が有意に低いことが示された(OR 
= 0.381、95% CI = 0.195–0.743)。 
結論として、大学病院の精神科外来患者の調査では、不眠の頻度が高く、約 2 人に 1 人の外来患者
が不眠に悩まされていた。政府が提案している「健康づくりのための睡眠指針」から、適切な刺激制
御技術に関する項目を指導することが不眠症診療に役立つ可能性がある

Breastfeeding is an unparalleled way to provide ideal nutrition for the healthy growth and 
development of infants and is actively promoted by the World Health Organization. Human breast milk 
contains a number of bioactive substances that modify brain development. Epidemiological evidence 
regarding the relationship between breastfeeding and behavioral problems in children has been limited and 
yielded inconsistent findings. Therefore, the present prebirth cohort study aimed to investigate the 
association between breastfeeding duration and behavioral problems in five-year-old Japanese children 
participating in the Kyushu Okinawa Maternal and Child Health Study (KOMCHS).  
1757 pregnant women between the 5th and 39th weeks of pregnancy provided written informed 
consent to participate in the KOMCHS and completed this self-administered questionnaire at baseline. Of 
the 1757 pregnant women, 1590, 1527, 1430, 1362, 1305, 1264, and 1201 mother–child pairs participated 
in all surveys from the baseline to the second (postpartum), third (approximately 4 months postpartum), 
fourth (approximately 12 months postpartum), fifth (approximately 24 months postpartum), sixth 
(approximately 36 months postpartum), seventh (approximately 48 months postpartum), and eighth 
(approximately 60 months postpartum) surveys, respectively. Study subjects were 1198 mother–child 
pairs. The ethics committees of the Fukuoka University School of Medicine and the Ehime University 
Graduate School of Medicine approved the KOMCHS. 
The baseline survey questionnaire gathered data on maternal age, gestational age, region of residence, 
number of children, parental education, household income, depressive symptoms, alcohol consumption 
during pregnancy, and body weight and height. Body mass index was calculated as weight (kg) divided by 
height squared (m2). A questionnaire in the second survey assessed the baby’s sex, birth weight, date of 
birth, and maternal smoking during pregnancy. The questionnaires in the third and fourth surveys asked 
about household smoking. Information on breastfeeding duration was obtained from the questionnaires in 
the fourth and fifth surveys. In the eighth survey, children’s behavioral problems at the age of 5 years were 
assessed using the Japanese parent-report version of the Strengths and Difficulties Questionnaire (SDQ) 
for 3- to 16-year-old children.

We defined emotional problems, conduct problems, hyperactivity problems, peer problems, or low 
prosocial behavior as present if a child had a borderline or abnormal score on that scale. Multiple logistic 
regression analysis was performed to calculate adjusted odds ratios (ORs) and 95% confidence intervals 
(CIs) for each behavioral problem in relation to breastfeeding duration. 
Among the 1198 children aged 59 to 71 months, the prevalence values of emotional problems, 
conduct problems, hyperactivity, peer problems, and low prosocial behaviors were 12.9%, 19.5%, 13.1%, 
8.6%, and 29.2%, respectively. Compared with < 4 months of exclusive breastfeeding duration, exclusive 
breastfeeding duration for ≥ 4 months was not significantly associated with any of the outcomes under 
study after adjustment for confounding factors. Compared with < 6 months of breastfeeding duration 
regardless of exclusivity, 6 to < 12 months, 12 to < 18 months, and 18 months or more of breastfeeding 
duration regardless of exclusivity were not significantly related to any of the outcomes. When children 
were divided according to whether they had at least one older sibling at baseline, an independent inverse 
association was found between exclusive breastfeeding for ≥ 4 months and the risk of hyperactivity 
problems in children with older siblings (adjusted OR = 0.57, 95% CI: 0.31−0.99), whereas such an 
inverse association was not observed in children without older siblings. On the other hand, in children 
without older siblings, exclusive breastfeeding for ≥ 4 months was independently inversely associated with 
the risk of low prosocial behavior (adjusted OR = 0.54, 95% CI: 0.30−0.93). Nevertheless, no significant 
interactions were observed between exclusive breastfeeding duration and the presence of older siblings at 
baseline with regards to hyperactivity problems or low prosocial behaviors. 
Methodological strengths of our study were its prospective prebirth cohort design, its relatively large 
sample size, its relatively long follow-up period, and extensive information on potential confounding 
factors. However, some limitations of our study include the possible non-differential misclassification of 
the outcomes, selection bias, using self-reported information on breastfeeding, and influence of residual 
confounding. 
Our study showed that neither exclusive breastfeeding duration nor any breastfeeding duration 
regardless of exclusivity was significantly associated with the risk of childhood emotional, conduct, 
hyperactivity, or peer problems or low prosocial behavior in Japanese children aged 5 years. When our 
children were classified according to the presence or absence of older siblings, ≥ 4 months of exclusive 
breastfeeding was significantly inversely associated with hyperactivity problems in children with older 
siblings, while ≥ 4 months of exclusive breastfeeding was significantly inversely related to low prosocial 
behaviors in children without older siblings. Given the specific sociocultural context in which the present 
study was conducted (Japan), the generalizability of the present findings to other populations is limited. 
These new findings warrant further confirmation.

【研究内容のまとめ】背景と目的：N-terminal-pro-brain natriuretic peptide（NT-pro  
BNP）は心不全スクリーニングにおける有用なバイオマーカーであるが、その診断閾値はエビ
デンスの蓄積とともに変化している。2021 年に、日本心不全学会（JHFS）・欧州心臓病学会
（ESC）・米国心臓協会（AHA）が共同で制定した「心不全国際定義」では、心不全の徴候が無
い場合でも、NT-proBNP 値の上昇（NT-proBNP≧125 pg/m1）をもって「前心不全」と定義され
た。これにより、NT-proBNP は心不全診断において一層重要な役割を果たすようになった。さ
らに、NT-proBNP が 125 pg/ml 未満の場合でも、ESC は NT-proBNP 50 pg/m1 を「心ストレス」
の閾値として定め、JHFS は NT-proBNP 55 pg/ml を闇値として心不全に対する経過観察を推奨
している。しかし、NT-proBNP 50 または 55 pg/ml という推奨閾値に関する医学的根拠は十分
ではなく、これらの闇値を満たす人々が将来的に前心不全（NT-proBNP≧125 pg/ml）に進展す
るリスクは明らかになっていない。本研究は、日本に人一般集団において、前心不全への進展
を予測する NT-proBNP の閾値を明らかにすることを目的とした。 
方法：愛媛県東温市で実施している「循環器疾患発症にかかる新たな危険因子の検索を目的と
するコポート研究（東温スタディ）」の一環として、2012 年 6 月から 2019 年 10 月の期間に
東温スタディに参加した 2,119 名のうち、血液検体を取得できた 2,087 名に対して NT-proBNP
測定を行った。5 年後の追跡調査で NT-proBNP を再度測定できた 638 名のうち、初回時に NT- 
proBNP≧125 pg/ml （前心不全診断基準）を満たす 65 名を除外し、573 名を本研究の対象と

した。対象者を 5 年後の NT-proBNP≧125 pg/m1 であるか否かで 2 群に分類し、ROC 解析を用
いて前心不全への進展を予測する NT-proBNP の同値を決定した。また、前心不全進行の予測因
子を特定するために、単変量ロジスティツク回帰分析を行い、さらに年齢、性別、体格指数
（BMI）、推算糸球体濾過量（eGFR）で調整した多変量解析も実施した。統計解析には EZR を
使用した。本研究は、愛媛大学大学院医学系研究科の倫理審査委員会（IRB: 2007004）の承認
を受けている。 
結果：対象者の年齢の中央値は 61 歳であり、70％が女性であった。573 名のうち 53 名（9.2
％）が 5 年後に前心不全診断基準（NT-proBNP≧125 pg/m1）に進展した。5 年後に前心不全と
診断された群は、非前心不全群と比較して高齢（61 vs 68 歳, p＜0.01）であり、eGFR が有
意に低かった（72.0 vs 66.8 m1/min/1.73m2, p＜0.01）。また、ベースラインの NT-proBNP
は有意に高値であった（42.5 vs 75.9 pg/m1, p＜0.01）。ROC 解析により、前心不全予測の 
NT-proBNP 閾値は 52.3 pg/m1（AUC = 0.78）であることが判明した。多変量解析により、ベー
スライン NT-proBNP は前心不全進展の独立した予測因子であることが判明した（オッズ比 
1.03, p＜0.01）。また、本研究によって明らかとなった NT-proBNP 閾値 52.3 pg/m1（オッズ
比 4.30, p＜0.01）も独立した前心不全進展の予測因子であった。 
考察：参加者の 9.2％が 5 年後に前心不全に進展し、ベースライン NT-proBNP 値が高いほどそ
のリスクが増加すること示された。NT-proBNP は年齢、性別、BMI、腎機能の影響をうけるが、
これらで調整後も NT-proBNP は独立した予測因子であり、その診断閾値は 52.3 pg/m1 であっ
た。本研究の結果は JHFS や ESC の診断閾値に近似しており、その意義を支持するものであっ
た。心不全診療において、左室駆出率の保たれた心不全（heart failure with preserved  
ejection fraction: HFpEF）の早期診断は重要な課題である。 HFpEF のリスク因子は NT-pro 
BNP 上昇の要因と重複しているため、本研究で前心不全に進展した人々は、HFpEF を発症する
リスクが高い可能性がある。したがって、本研究で明らかになった NT-proBNP 値 52.3 pg/ml 
を超える症例には、将来の前心不全進展予防に加え、HFpEF 発症予防を視野に入れた介入が必
要と考えられる。本研究の限界として、対象が東温市民に限定され、既往歴・内服歴の全ては
把握できていないこと、心電図の解析や心エコーのデータが得られていないことから、基礎心
疾患の有無を確認できていない点があげられる。 
結語：NT-proBNP は、一般集団における 5 年後の前心不全進展を予測可能であり、その閾値は
52.3 pg/ml であった。この数値は JHFS 推奨値 55 pg/ml および ESC 推奨値 50 pg/m1 と類似し
ており、NT-proBNP が一般集団での前心不全早期発見に役立つ可能性が示唆された。 
【審査会のまとめ】 
公開審査会は、令和 6 年 12 月 23 日に開催された。申請者は、研究発表を英語で行い、本研究の
背景や意義、詳細な方法や結果と考察、対象者の年齢・性別分布の特徴、研究の limitation、
今後の展望などについて述べた。発表後に審査委員から以下のような質疑があった。１）NT-pro
BNP 値が心不全以外にも影響を受ける薬剤や病態等、特に担癌状態、妊娠高血圧症候群、脳卒中、
生活習慣病との関係について、2）NT-proBNP 値上昇（変化量・絶対値）の意義や治療介入の必
要性について、3）追跡 5 年間の治療介入やその後のフォローアップの有無、今後の研究の課題
や計画などについての質問がなされた。申請者はこれらの質問に対して的確に応答した。 
審査委員は、申請者が本論文関連領域に対して学位授与に値する十分な見識と能力を有するこ
とを全員一致で確認し、本論文が学位授与に値すると判定した。
