    RNF128 expression in lung adenocarcinoma is a favorable prognostic factor associated with decreased tumor-associated macrophages

    香川大学Kagawa University博士(医学)Objectives: Molecular-level research has linked RING finger (RNF) protein family members to carcinogenesis and tumor progression. Among them, RNF128 is related to tumor progression, but reports on its association with lung cancer are few. This study aimed to clarify the unknown association between RNF128 expression and clinical outcomes in patients with lung adenocarcinoma.Methods: Clinical data of 545 patients with therapy-naïve lung adenocarcinoma who underwent lobectomy with systematic lymph node dissection between 1999 and 2016 were retrospectively reviewed. Histological and immunohistochemical analyses were conducted to evaluate the relationship between RNF128 expression and prognosis.Results: Among adenocarcinoma histologic types, acinar, micropapillary, and solid tumors did not express RNF128 compared with other histologic types (p < 0.001). Patients with high RNF128 expression exhibited fewer clusters of differentiated (CD) 68+ tumor-associated macrophages (TAMs) and CD163+ TAMs. Multivariate analysis of relapse-free survival (RFS) and overall survival (OS) revealed that the lack of RNF128 expression was an independent prognostic factor for poor RFS (hazard ratio [HR] 1.60, p = 0.029) and OS (HR 1.83, p = 0.041), suggesting that RNF128 expression is a favorable prognostic factor.Conclusion: RNF128 expression may be an independent predictor of favorable outcomes in Japanese patients with untreated lung adenocarcinoma who undergo surgical resection. Further elucidation of the role of TAM-related E3 ubiquitin ligase in immune function may facilitate the development of effective immunomodulatory therapies for lung adenocarcinoma.doctoral thesi

    Usefulness of the Measurement of Psoas Muscle Volume for Sarcopenia Diagnosis in Patients with Liver Disease

    香川大学Kagawa University博士(医学)Computed tomography (CT) is often used in the diagnosis of sarcopenia. In this study, we validated the assessment of sarcopenia by the psoas muscle volume using versatile software. The study involved a retrospective analysis of data from 190 patients with liver disease who underwent grip-strength testing and abdominal pelvic computed tomography. To assess sarcopenia, SYNAPSE 3D was used to obtain the skeletal muscle index, the psoas muscle index (PMI), and the simple method. We also used the recently proposed PMI cutoff values, for which the usefulness has been evaluated (O-PMI). The cutoff value of the psoas muscle volume index (PMVI) was determined using one of the diagnostic methods as the gold standard. All diagnostic methods showed that patients with sarcopenia had shorter survival, with O-PMI having the highest hazard ratio (HR) (HR, 6.12; 95% confidence interval [CI], 2.6-14.41; p < 0.001). Even when sarcopenia could not be diagnosed by O-PMI, low PMVI was associated with shorter survival (HR, 3.53; 95% CI, 1.34-9.32; p = 0.01). PMVI may be useful in the evaluation of sarcopenia, including the identification of poor overall survival in cases that cannot be diagnosed by O-PMI, which is considered more useful than PMI.doctoral thesi

    極低出生体重児における乳児期の顔認知が幼児期の言語・社会性の発達に与える影響

    本研究では,極低出生体重児12名を対象に,乳児期の顔認知における新奇選好と幼児期の言語・社会性発達との関係を検討した。修正9~10か月時に視線計測を行い,修正3歳および修正6歳時に新版K 式発達検査を実施した。単回帰分析の結果,修正9~10か月時の新奇選好率の低さと修正6歳時の言語・社会領域および全領域の発達指数(DQ)の高さに有意な関係が確認された。少数例の報告ではあるものの,本研究は,発達初期の顔認知に関する新奇選好が極低出生体重児のその後の言語および社会性の発達に影響を与える可能性を示唆した。極低出生体重児の言語や社会性に関する早期支援のために,視覚的注意の新奇選好が重要な視点となりうる可能性がある。departmental bulletin pape

    The Effect of Trabeculectomy on Disc Tissue Blood Flow Across Quadrants in Open Angle Glaucoma

Improvement of tissue blood flow superior and temporal in the optic nerve could be an indicator of the effectiveness of glaucoma treatment.Objective: To investigate changes in tissue blood flow at the ONH before and after trabeculectomy.Materials and methods: This prospective study included 53 eyes who underwent trabeculectomy. The mean blur rate of the tissue area mean blur rate of the tissue area (MT) was determined using laser speckle flowgraphy preoperatively and 1 and 3 months postoperatively. MT, laser speckle flowgraphy waveform parameters [blowout score (BOS) and Resistivity Index (RI)], intraocular pressure (IOP), and ocular perfusion pressure (OPP) were analyzed preoperatively and at 3 months postoperatively.Results: Postoperatively, IOP decreased (17.9±6.0-9.2±3.4 mm Hg) and OPP increased (43.3±9.2-52.1±6.4 mm Hg). As previously described, BOS increased (73.5±8.3-76.7±6.5; P < 0.001) with an inverse correlation to IOP and in proportion with OPP; RI decreased (0.40±0.1-0.35±0.08; P < 0.001) with an inverse correlation to OPP and in proportion to IOP. However, we found that MT increased significantly after surgery (7.9±2.2-8.8±2.2 AU; P = 0.002) without significant association between MT and IOP or OPP. In the 4 quadrants of the ONH, BOS significantly increased, while RI significantly decreased after surgery in all quadrants. In contrast, MT significantly increased in the superior and temporal quadrants only.Conclusions: Blood flow increases in the superior and temporal ONH.doctoral thesi
