    Desafiando Barreras: Guías de Práctica Clínica de la Asociación de Enfermeras Registradas de Ontario (RNAO) y Cambio Organizacional

    Introduction: over the past four decades, hospitals have faced transformations in funding and management to address growing healthcare demands. The implementation of evidence-based practices, such as the Registered Nurses’ Association of Ontario (RNAO) clinical guidelines and the Best Practice Spotlight Organisations (BPSO®) programme, is crucial to improve the quality of care. The collaboration between the RNAO and the Ministry of Health (MINSAL) in Chile highlights the importance of innovation and excellence in healthcare. Aim: describe the relevance of RNAO guidelines, barriers to their implementation and the role of nursing through a narrative review of the literature. Development: implementation of BPSO® has demonstrated substantial improvements, including significant increases in patient risk identification and management. However, implementation of the RNAO Good Practice Guidelines (GBP) faces challenges, such as political, organisational and professional barriers. Implementation science is crucial to address these by designing strategies that drive evidence-based quality of care. Conclusion: in summary, the implementation of evidence-based practices, such as the RNAO GBP, represents an organisational change supported by programmes such as BPSO® that have improved care. It is essential to identify barriers, especially in nursing, in order to overcome obstacles and ensure the active participation of professionals in the continuous improvement of the quality of health care. © 2024; Los autores.Universidad Andrés Bello, UNAB, (DI-02-21/CBC)Universidad Autónoma de Chil

    Racemose Neurocysticercosis of the Silviano Valley with Inflammatory Aneurysm of the Right Middle Cerebral Artery. Case Report.

    Neurocysticercosis (NCC) is the most common parasitic disease of the brain, has a wide variety of patterns of presentation and can mimic many diseases. We report a patient with inflammatory aneurysm, and found only 5 cases reported in the literature, and only one has histopathological report. This is a case report of a patient with diabetes and arterial hypertension condition which started with sim- plesecondarygeneralizedpartialseizures. Thestudy reported the patient was diagnosed with neurocysticercosis racemosa, resection of lesions was performed with secondary inflammatory rupture intraoperative aneurysm clipping successful NCC, inflammatory changes in the literature described in the aneurysm wall and the dependent artery, secondary to immune process with vasculitis associated with a chronic inflammatory process, these changes in the aneurysm wall increase the possibility of intraoperative rupture. Histopathological report was conclusive with neurocysticercosis

    Using Piezoelectric System in Oral and Maxillofacial Surgery

    Motors and handpieces with the piezoelectric system have recently been used in several surgeries, following the current minimally invasive trend surgery. The system consists of crystals or ceramics undergoing deformation when exposed to electrical current, resulting in an oscillating movement with ultrasound frequency. In the Oral and Maxillofacial Surgery field activity, the piezosurgery may be used in maxillary and mandibular osteotomies in the orthognathic surgery, alveolar corticotomies, cysts tumors, facial trauma surgery, among others. This study aims to present a literary review on the piezoelectric system, which allows discussion to their use and effectiveness in oral surgery procedures

    Use of Self-Expandable Prosthesis for the Treatment of Colon and Rectal Benign Stenosis. Systematic Review

    The main reason for the use of self-expandable prosthesis (SEP) is for the treatment of malignant colonic obstructions. The objective of this study is to determine the understanding with regard to results of applying SEP in benign stenosis of colon and rectum, measured in terms of numbers, etiology, and results. Systematic review. A search on MEDLINE, Cochrane Library CENTRAL, DARE, HTA, NHSEED, EMBASE and CINAHL and TRIP database was conducted. We considered all studies published since 1990 for this report. Search strategies were applied to each database using MesH terms, truncated, free words and Booleans. The results and level of evidence of the studies were summarized in tables summarizing efficacy, safety and complications. We obtained 19 related articles. All of these were case series and case reports (evidence level 4), totaling less than 150 cases treated with SEP. The observed results were adequate in terms of symptom reduction, however, a considerable percentage of complications were observed (intestinal perforation, migration, re-obstruction and pain). Published experience concerning the results of the installation of SEP in benign strictures of colon and rectum is scarce and evidence is limited. Therefore, further studies and articles with improved methodological qualities are required

    Since less than 5000 persons speak the E’ñepa language, it is rare known. Consequently, even a basic knowledge of 113 anatomical terms assembled in this research still with a rough pronunciation of the terms can be useful for communicating with the E’ñepa. The sole purpose of this search was to gather anatomical terms for use in communicating with the E’ñepa.Dado que menos de 5.000 personas hablan la lengua E'ñepa, ésta es poco conocida. Por lo tanto, incluso un conocimiento básico de 113 términos anatómicos reunidos en esta investigación aún con una pronunciación aproximada de los términos puede ser útil para comunicarse con los E'ñepa.     En esta investigación, con  un  enfoque  de  gestión  ambiental  transversal,  se  analizó  el  proceso  de  planeación  local  de  Mexicali,  Baja  California,  a  partir de sus tres instrumentos de planeación vigentes en el año 2012, con el  objetivo  de  identificar  qué  nivel  de  gestión  ambiental  transversal  y  de  articulación  existe  entre  las  instituciones  municipales  responsables  de  la  planeación para la prevención de desastres. Los resultados obtenidos coinciden con los supuestos que guiaron esta investigación, identificando que existe  un  potencial  de  gestión  ambiental  transversal  para  prevención  de  desastres; resultado significativo para el municipio, dada la vulnerabilidad existente; también se identificó un nivel de articulación institucional muy bajo, que afecta negativamente los resultados de la planeación local en materia de prevención de desastres y evidencia la subestimación del potencial de gestión ambiental transversal

    Residents’ perceptions of ecosystem services in an urbanizing basin: A case study in the Guanting Reservoir basin, China

    Understanding stakeholders’ differences in perceptions of ecosystem services (ES) is crucial for guiding ecological conservation and planning. However, the variations of ES perception amongst different types of residents in urbanizing areas along an urban-rural gradient are still poorly understood. Combining a questionnaire-based survey, redundancy analysis, and statistical tests, we delineated the urban-rural gradient according to local residents’ socio-economic characteristics, and investigated the differences in local residents’ perceptions of ES and potential factors affecting them in the Guanting Reservoir basin, a rapidly urbanizing basin in China. The results showed that residents living in urban-rural transitional areas attached great importance to provisioning services of providing food and domestic water, regulating services of carbon sequestration and air purification, and cultural services of providing education and training, which were 0.7%–13.1%, 0.7%–9.1% and 2.5%–21.2% higher than that of residents in other areas, respectively. Age and occupation were major factors affecting residents’ perceptions. In terms of land-use types that deliver ES, the difference in perceptions of ES delivered by grassland was the greatest amongst residents. Our results support recommendations for policymakers to take into account the stakeholders’ diverse perceptions, thus promoting residents’ sense of gain on ES. © 2024 The AuthorsUniversidad Autónoma de Chil

    Prevalence and genotypic associations of epilepsy in Prader-Willi Syndrome: A systematic review and meta-analysis

    Objective: To estimate the prevalence of epilepsy and febrile seizures and their association with genotype, i.e., 15q11-q13 deletions, uniparental chromosome 15 disomy (UPD) and other mutations, in the population with Prader-Willi syndrome (PWS). Methods: A systematic search of Medline, Scopus, Web of Science and the Cochrane Library was conducted. Studies estimating the prevalence of seizures, epilepsy and febrile seizures in the PWS population were included. Meta-analyses of the prevalence of epilepsy and febrile seizures and their association with genotype using the prevalence ratio (PR) were performed. Results: Fifteen studies were included. The prevalence of epilepsy was 0.11 (0.07, 0.15), similar to the prevalence of febrile seizures, with a prevalence of 0.09 (0.05, 0.13). The comparison “deletion vs. UPD” had a PR of 2.03 (0.90, 4.57) and 3.76 (1.54, 9.18) for epilepsy and febrile seizures. Conclusions: The prevalence of seizure disorders in PWS is higher than in the general population. In addition, deletions in 15q11-q13 may be associated with a higher risk of seizure disorders. Therefore, active screening for seizure disorders in PWS should improve the lives of these people.
