    Duodenal Gastrointestinal Stromal Tumor: A Case Report

    Gastrointestinal Stromal tumors (GIST) are uncommon primary mesenchymal tumors arising from the gastrointestinal tract. The duodenum location is unusual. The authors present a case of duodenal GIST, mimicking a pancreatic tumor. Preoperatively computed tomography (CT) revealed a hypervascular mass of 10 cm in diameter, in the pancreatic head. Endoscopic examination showed erosion in duodenal mucosal with local bleeding. The patient underwent sucessfully pylorus-preserving pancreaticoduodenectomy

    Actitudes de padres y madres sobre la práctica deportiva de hijas basquetbolistas pertenecientes a la zona sur insular de Chile

    The practice of sports influences the acquisition of skills, values and attitudes. The family, especially fathers and mothers, plays a fundamental role. This research seeks to determine the attitudes of parents towards the sports practice of basketball daughters and its relevance in their development. The questionnaires Analysis of the Behavior and Performance of Parents in Sports (ACAPMD) and Questionnaire for Children about the Opinion of Parents in Sports (CHOPMD) were used. On the one hand, the one-factor ANOVA test was used to determine the differences between categories and the Bonferroni test was used to identify which groups they fell into. Then, the t-student test was used for the comparison of the responses according to the sex of the parents, setting the significance level at p<0.05. The results indicate that there are significant differences in communication between parents and coaches between the U-13 and U-15 categories. Significant differences were also found in the valuation and importance that parents give to the sports school between the U-13 and U-18 categories. However, no significant differences were found between fathers and mothers in any of the categories analyzed. Fathers and mothers influence the sports experience of their daughters, showing importance in communication, environment and involvement. Although communication with coaches is low, they value their work and recognize the importance of the club in sports development. These findings highlight the relevance of effective communication for positive sport environments and to support the integral development of young athletes. © 2024 Federacion Espanola de Docentes de Educacion Fisica.     Pauta de pesquisa para Apraxia del Habla Infantil en hispanohablantes: Traducción y validación cultural: Traducción y Validación Cultural

    Childhood apraxia of speech (CAS), is a rare disorder during childhood, and it can trigger serious consequences in the future if not treated promptly. The existing diagnostic methods for this disorder are found in the English language only, which leads to difficulties in finding valid and agreed diagnostic methods by speech therapists to apply to the Spanish-speaking population. Hence, this study aims to provide the translation of an evaluation tool which is suitable for Spanishspeaking countries and that facilitates phonoaudiological work in the diagnosis. The method that will be used is that of translation-retranslation, to which the analysis of an expert opinion will be incorporated to give validity to the instrument. The results expose a wide acceptance of the instrument (94% of positive results) with substantial agreement among experts (IRR inter rater reliability), presented as agreement probability for the entire instrument of 0.57 (57%). In conclusion, this study provides a research pattern for AHI, to facilitate the early phonoaudiological evaluation of children, allowing the patients to access reliable care regarding to diagnosis.La apraxia del habla infantil (AHI) es un trastorno pediátrico poco común, en el que, si el niño o niña no es diagnosticado a tiempo, puede desencadenar graves consecuencias a futuro. Los métodos de diagnóstico existentes para este trastorno se encuentran disponibles en idiomas distintos al español, lo que conlleva una dificultad para encontrar un método de diagnóstico válido y consensuado por los fonoaudiólogos para aplicarse a la población hispanohablantes. Es por esto que este estudio pretende dar un primer aporte, realizando la traducción de una herramienta de pesquisa apta para el idioma español, que permita facilitar la labor fonoaudiológica en el diagnóstico. El método que se utilizará es el de traducción y validación de una herramienta de evaluación en salud, al cual se va a incorporar el análisis de un panel de expertos para brindarle validez al instrumento. Los resultados muestran una amplia aceptación del instrumento (94% resultados positivos) con un buen acuerdo entre jueces (IRR inter rater reliability) presentada como probabilidad de acuerdo para todo el instrumento de 0,57 (57%). En conclusión, este estudio brinda una pauta de pesquisa para la AHI, con el fin de facilitar la evaluación fonoaudiológica temprana de los niños y niñas, permitiendo que los usuarios puedan acceder a una atención especializada y confiable acordes con el diagnóstico

    Evidence-based nursing emerges as a reliable alternative to address the current needs of the profession, with the best practice guidelines (BPGs) of the Registered Nurses´ Association of Ontario (RNAO) serving as a key tool to optimize the healthcare system and both the clinical and academic aspects of nursing. This study aims to reflect on the implementation of RNAO's BPGs at the Universidad Autónoma de Chile, Talca campus.     A systematic review and cluster analysis approach of 103 studies of high-intensity interval training on cardiorespiratory fitness

    Aims This study aims to systematically review the systematic reviews and meta-analyses examining the effect of high-intensity interval training (HIIT) protocols on improving cardiorespiratory fitness (CRF) and to characterize the main patterns of HIIT modalities using clustering statistical procedures to examine their potential differences on improving CRF. Finally, we aimed to develop a comprehensive guideline for reporting HIIT protocols. Methods A systematic review was conducted on PubMed and Web of Science from their inception to 31 October 2022 for systematic reviews and results and meta-analysis aimed at assessing the effect of HIIT on CRF in the entire study population. The Assessment of Multiple Systematic Reviews 2 tool was used to evaluate the risk of bias of each review. Additionally, a principal component analysis testing the data adequacy for the factor solution through the Kaiser–Meyer–Olkin procedure test was conducted. Once the number of factors was identified, in order to identify data patterns according to the main characteristics of the HIIT protocols, a two-step cluster analysis was conducted. Nineteen systematic reviews and/or meta-analyses comprising 103 studies were included. Clustering of systematic reviews and meta-analyses identified three HIIT modalities (‘HIIT-normal mixed’, ‘HIIT-long running’, and ‘HIIT-short cycling’) underlying the interventions across the included studies. Similar effectiveness in increasing CRF among the three HIIT modalities was observed. Subgroup analyses showed no significant differences in CRF by sex, weight status, study design, and baseline physical activity level (P > 0.05), but differences were observed by age group, and exercise intensity indicator was used in the HIIT programmes (P < 0.05) Conclusion All three HIIT modalities produced significant improvements of CRF, although some modalities showed greater changes for some specific age groups or intensity indicators. Law summary • In this review, including data from more than 100 individual studies, 3 patterns of high-intensity interval training (HIIT), ‘HIIT-normal mixed’, ‘HIIT-long running’, and ‘HIIT-short cycling’, were characterized using a cluster analysis approach. • Independently of the sex, weight status, study design, and baseline physical activity level, all three proposed HIIT modalities improved cardiorespiratory fitness (CRF). However, for the age group of 20–44 years, the ‘HIIT-long running’ reported greater improvement in CRF than ‘HIIT-short cycling’. Additionally, the heart rate as an intensity indicator reported higher CRF in ‘HIIT-long running’ compared with ‘HIIT-normal mixed’. • A HIIT reporting guideline is provided to address the lack of information on HIIT protocols and provide transparency among studies. © The Author(s) 2023.     “Who Takes Care of Carers?”: Experiences of Intensive Care Unit Nurses in the Acute Phase of the COVID-19 Pandemic

    Introduction: The COVID-19 pandemic caused an international health emergency situation where nursing took on a fundamental role. The high number of patients in hospital ICUs led to a shift in nurses’ working conditions and workload. Objective: The objective of this study was to describe the experiences of nurses who worked in ICUs during the acute phase of the COVID-19 pandemic. Methodology: A qualitative, descriptive study was carried out, with the participation of 21 nurses who worked in the ICU during the pandemic. Data collection took place between May and July 2021 through 21 in-depth interviews. Results: Three main themes emerged: (1) COVID-19 in ICUs: nurses on the frontline. (2) United against adversity: teamwork. (3) New optics of critical care and the nursing profession. COVID-19 was perceived with harshness, and the lack of knowledge about the virus generated confusion, anxiety and fear due to the risk of transmission to family members and relatives. The pandemic marked a shift in the management of human, material and economic resources. Novice nurses learned critical care at an accelerated pace, with significant physical and psychological strain. Expert nurses carried the burden of training new nurses. Although there were tense situations, experiencing these adverse situations as a team led to feelings of increased belonging, togetherness and professional bonding for nurses. While the participants noted an increase in motivation to continue in their profession, they also had a feeling of not having been cared for as they deserve by healthcare institutions. © 2024 by the authors.Universidad de Almería, UAL, (CTS-451)Universidad Autónoma de Chil

    Segmental Osteotomy with Interpositional Graft in Posterior Maxilla Area

    A variety of techniques have been documented for the recovery of bone loss in areas where rehabilitation with dental implants is planned. The loss of teeth is followed by the loss of height and thickness of the bone too, precluding the installation of dental implants. The premature bone loss in this region can also cause pneumatization of the maxillary sinuses. So, the corrections of these defects are a real challenge for dentists and oral surgeons. The technique of segmental osteotomy with bone interposition graft has proven to be a viable option to solve this problem. This clinical case report describes the successful application of the technique in the treatment of decreased vertical dimension in the posterior area of the maxilla, where 7 months later 4 dental implants were installed. Two dental implants of 13 mm, and two dental implants of 11 mm, demonstrating success with bone height that was achieved in the case, making the use of this technique very sustainable in bone loss of posterior area of the maxilla with pneumatized maxillary sinuses

    Exploring the use of CO2-expanded ionic liquids as solvents in microextraction of 3,3′,4,4′-tetrachlorobiphenyl from aqueous solutions

    This paper explores the effect of adding supercritical carbon dioxide (scCO2) to Ionic Liquids (ILs) for the microextraction of very diluted pollutants from aqueous matrices. The proposed system uses an IL, trihexyl-tetradecyl-phosphonium bis(trifluoromethylsulfonyl)imide ([P6,6,6,14][Tf2N]) and 1-hexyl-3-methyl-imidazolium tris(pentafluoroethyl)trifluorophosphate ([hmim][FAP]), as extractant, and scCO2 as diluent that may change the solvent capacity by changing the solvent polarity. The study was first carried out for the extraction of 3,3′,4,4′-tetrachlorobiphenyl (PCB-77) from aqueous solutions using a designed microextraction cell. Different ILs, sample volumes and kinetics of extraction were investigated. Results show that within 15 min the maximum extraction percentage was achieved employing a scCO2 partial pressure of 80 bar. To investigate the influence of the polarity change by the presence of carbon dioxide more broadly, other pollutants with different water solubilities were used. It was observed that scCO2 (partial pressure) reduced the recoveries for benzophenone and PCB-77 but increased the extraction of triclosan. This is due to the change on solvent's polarity and viscosity of the mixture. This was corroborated through a ternary system simulation including IL-pollutant-scCO2. All these results establish that the combination of CO2 and IL can in some cases enhance the extraction, but the affinity between the pollutant and scCO2 is a key parameter that discerns whether expanding the IL with scCO2 is beneficial for the extraction. © 2023Comisión Nacional de Investigación Científica y Tecnológica, CONICYT; Fondo Nacional de Desarrollo Científico y Tecnológico, FONDECYT, (11150417); Agencia Nacional de Investigación y Desarrollo, ANID, (21170488)Universidad Autónoma de Chil
