2023-2024 American Board of Orthodontics practice analysis study: Part 1
As a specialty board, the American Board of Orthodontics (ABO) serves to protect the public and the orthodontic specialty by certifying orthodontists. The demonstration of commitment to lifelong learning and self-improvement is critical to achieving the highest level of patient care. The ABO completed a practice analysis study in 2023 to ensure all examinations represent current assessments of proficiency in orthodontics at a level of quality that satisfies professional expectations. The practice analysis is essential to providing a demonstrable relationship between the examination content and orthodontic practice and provides a critical foundation for ABO’s examination programs
The Use of Tissue Grafts Associated with Immediate Implant Placement to Achieve Better Peri-Implant Stability and Efficacy: A Systematic Review and Meta-Analysis
Background: The goal of this systematic study and meta-analysis was to evaluate the efficacy of hard and/or soft tissue grafts associated with type-1 implants on healing and treatment outcomes. The primary outcomes studied were implant survival rate, pocket depth, marginal periimplant recession, bone loss, bone thickness (volumetric change), interproximal bone level, mesial and distal papilla migration, and radiographic evaluation; and the secondary parameters were Pink Esthetic Score (PES), vertical distance from implant shoulder and bone, Visual Analogue Score (VAS), Implant Stability Quotient (ISQ), and biological complications (fistulas, pain, mucositis, and peri-implantitis). Methods: The PICO strategy was used to formulate the hypothesis under study: “For patients who underwent extraction and immediate implant placement, what is the efficacy of using any type of graft (bone or soft tissue) compared to non-grafting regarding the peri-implant parameters?” The electronic search process was performed on the MedLine/PubMed and Cochrane databases. It included randomized controlled trials (RCTs) from the last 11 years (from 2012 to November 2023), which were identified and analyzed. Results: Nine RCTs (κ = 0.98) were selected (403 patients and 425 implants); they were divided into three groups: bone graft (75 patients and 75 implants inserted), bone graft and membrane (213 patients and 235 implants inserted), and without bone graft (115 patients and 115 implants inserted). Three studies calculated the mid-facial mucosa level and two reported better results when a connective tissue graft was combined with the xenograft, whereas another study found better results in the combination of a dual-zone technique with a xenograft. Three studies evaluated the total Pink Esthetic Score (PES) at 12 months, where the authors found no significant difference in using a xenogeneic graft with or without a membrane. In the same period, the facial bone thickness was assessed in two articles; the authors reported better results in graft-treated and flapless groups. The risk-of-bias assessment found four studies with low risk, four with moderate risk, and one with a high risk of bias. The meta-analysis showed a medium level of heterogeneity for the mid-facial mucosa level analysis (I2 = 46%) and an overall effect size of 0.79 (95% CI [0.18; 1.40]), a statistically significant results (p = 0.01), with a tendency to favor the experimental group. Also, there was a medium level of heterogeneity among studies regarding total PES (I2 = 45%), with no significant differences between studies (p = 0.91). Homogeneous results (I2 = 0%) were found among studies analyzing facial bone thickness, favoring the experimental group; the forest plot showed an effect of 0.37 (95% CI [0.25; 0.50]), which was statistically significant (p \u3c 0.00001) for this parameter. Conclusions: Then, it was possible to conclude that using bone and soft tissue grafting techniques associated with immediate implant placement (IIP), even though they are not fundamental, was a valuable resource to prevent significant tissue reduction, reaching greater bone stability and higher levels in the Pink Esthetic Score (PES) and Visual Analogue Score (VAS)
Sensitivity of Collagenolytic Periopathogenic Microorganisms to Chlorhexidine Solution: A Comprehensive Review of In Vitro Studies
Objective: This review evaluated chlorhexidine (CHX)’s beneficial effect as an antibacterial substance on periodontopathogenic bacteria, which can influence the preservation of periodontal tissues and biomaterials. Methods: A search was performed in the PubMed/MedLine, B-On, ScienceDirect, and Cochrane Library databases. In vitro studies published between 1 January 2000 and 31 December 2023 in the English language were included; studies that did not correlate the sensitivity of collagenolytic microorganisms to CHX and observational and in vivo studies were excluded. The Quality Assessment Tool for In Vitro Studies (QUIN) evaluated the risk of bias. Results: Eight studies were included; six assessed the inhibitory effect of CHX on the activity of various bacteria associated with periodontitis and collagen degradation; two studies evaluated the same effect only for P. gingivalis. All the studies had an evaluation percentage above 70%, representing a low bias risk. Conclusions: There is a relationship between collagen degradation and the microorganisms in periodontal diseases. CHX showed efficacy against various microorganisms, inhibiting their growth and cell viability. CHX demonstrated significant implications for preventing and treating infections associated with collagen degradation

Technology Readiness Level of Robotic Technology and Artificial Intelligence in Dentistry: A Comprehensive Review
This comprehensive review assessed the integration of robotics and artificial intelligence (AI) in dentistry, offering a transparent overview of developments across dental fields. Covering articles in prosthodontics, orthodontics, implantology, surgery, and radiology, the review included 39 articles on robotics and 16 on AI. Screening adhered to PRISMA guidelines, with searches conducted on Medline, Google Scholar, and IEEE. Incorporating the search strategy, the review used keywords related to dentistry, robotics, and AI. For robotics, 296 articles were screened, resulting in 39 qualifying for qualitative synthesis. A separate AI search on PubMed identified 142 studies within the last decade, with 16 studies selected for a detailed full-text analysis, offering a consolidated overview of the current state-of-the-art knowledge in the AI domain. Geographic distribution highlighted East Asia as a major research contributor. The findings indicate an increasing trend in dentistry robotics since 2000 and, particularly since 2016, in AI dentistry. The majority of the literature fell under the category of basic research. The technology readiness level did not cross “three” (proof of concept) in 41% of all articles. Therefore, the overall literature quality remains low, particularly regarding clinical validation
Internal adaptation assessment of implant infrastructures manufactured through five different techniques (heat-press, milling, lost wax, calcinable cylinder, and CAD/Waxx®): an in vitro pilot study
Introduction: The aim of this study was to evaluate the fit performance of implant infrastructures manufactured by five different techniques: heat-press (IPS), milling (ZIR), lost wax (CER), calcinable cylinder (CAL), and CAD/Waxx® (CAD). Methods: The methodology was based on the Replica Technique, which can simulate and evaluate the fit of the infrastructure on the implant component. Thus, each infrastructure was internally filled with low-viscosity silicone addition and seated on the component until its final setting, obtaining the replica of the cementation space. After removing the coping containing the silicone film, light-density silicone was inserted addition in place of the components, and in its surroundings, condensation silicone was applied, establishing support for the assembly. The joint was sectioned mesiodistally, photographed, and analyzed in image processing software in order to measure the thickness of the interface infrastructure/implant at five different areas: marginal opening (M), gingival-axial angle (G-A), axial region (A), axial-occlusal angle (A-O) and occlusal surface (O). Results: The lowest and the highest average thickness between groups was, respectively, IPS: 187.5 μm and CAD: 221.6 μm, with statistically significant differences (p \u3c 0.01) among all five groups; the lowest and the highest average of all groups in each point was, respectively, A: 99.86 μm and O: 279.78 μm. The IPS group exhibited the lowest value of the internal space of the infrastructure on the implant. The marginal region of all groups demonstrated a correlation with the findings in the literature, except the CAL group; otherwise, the occlusal region and the angles A-O and G-A exhibited values beyond that expected. Discussion: It was possible to conclude that the five infrastructure groups presented different adaptations, suggesting possible interference in the internal spaces due to the manufacturing infrastructure processes
The stepping test, and infrared depth sensor, provide reliable measures of balance in community-dwelling older adults
Abstract
[Purpose] The purpose was to examine relationships between variables characterizing the 20-second stepping test movement pattern, assessed with an infrared depth sensor (KinectTM), and measurements of dynamic and static balance. [Participants and Methods] A total of 27 independent-living, older adults (7 males and 20 females) participated in this study. For each participant, the stepping test total movement distance, movement displacement, knee movement distance, and step number were calculated from Kinect closed joint-point coordinate data. Dynamic and static balance were assessed using a NeuroCom Balance Master Platform system. [Results] The stepping test total movement distance had a moderate negative correlation with endpoint excursion (r=-0.469) and a moderate positive correlation with total movement distance, corrected for knee movement distance (r=0.557). Step numbers had a moderate negative correlation with stepping test total movement distance (r=-0.667) and total movement distance, corrected for knee movement distance (r=-0.531). Division into high and low-balance sub-groups (based on endpoint excursion or sway velocity scores) revealed that stepping test total movement distances and movement displacements were greater, and step numbers were fewer, in low balance groups. [Conclusion] The stepping test, combined with a KinectTM-assessed movement pattern provides a simple, objective, reliable means for assessing balance in community-dwelling, independent-living older adults
