    The effect of different bed head angles on the hemodynamic parameters of intensive care patients lying in the supine position: a quasi-experimental study

    Aims: The aim of this study was to research the effect of different bed head angles on the hemodynamic parameters of intensive care patients lying in the supine position. Methods: This study was a non-randomized and non-controlled, quasiexperimental repeated measures study. The study was conducted with 50 intensive care patients aged 18 and over in a general surgery intensive care unit in Turkey. With each patient in the supine position, the bed head was raised to an angle of 0°, 20°, 30°, and 45° without a pillow, and the hemodynamic parameters of central venous pressure, systolic and diastolic blood pressure, heart rate, breathing rate, and peripheral oxygen saturation were recorded after 0 and 10 min. Results: It was found that the mean central venous pressure value measured at min 0 and 10 was higher when the intensive care patients' bed head angle was raised to 45° than when the bed head was at an angle of 0° or 20° (p < .05). It was found that the patients' other hemodynamic parameters were not affected by different bed head angles. Conclusions: It was concluded as a result of this research that in intensive care patients in the supine position, only central venous pressure was affected by bed head angle, and that central venous pressure measurement can be reliably made at a bed head angle of 30°

    Hope and psychological resilience in primary caregivers of patients with a chronic mental illness followed in a community mental health center

    Purpose: The aim of this study is to examine hope and psychological resilience in primary caregivers of patients with a chronic mental illness. Design and methods: The descriptive study was conducted on 297 caregivers in community mental health centers in Turkey. Data were collected using the Introductory Information Form, Dispositional Hope Scale and the Resilience Scale for Adults. Findings: Hope and psychological resilience of primary caregivers of patients with a chronic mental illness were moderate. To sociodemographic and caregiver characteristics; caregivers who are over 40 years old, lost his/her spouse, low education level, housewife or retired, unemployed, who evaluated their incomes low, mother, living in the same house with the patient, caring for ten years or more, caring for another patient and not getting help in care had lower hope and resilience levels. Compared to patients with a diagnosis of bipolar disorder, caregivers of patients with schizophrenia had lower hope and psychological resilience levels. Conclusions: Primary caregivers of patients with a chronic mental illness should be supported in terms of hope and psychological resilience

    Investigation of the suitability of TRT children's songs for MEB preschool education program gains

    Müzikler, çocukların gelişim alanlarının tamamını destekleyen, estetik algıyı geliştiren, özsaygıyı artıran ve yaratıcılığı ortaya çıkartan son derece önemli kaynaklardır. Okul öncesi dönemde müzik etkinlikleri içerisinde, şarkı söyleme, ses dinleme, sesleri ayırt etme, ritim, müzikli öykü ve dans gibi etkinlikler yer almaktadır. Bu çalışma, TRT çocuk şarkılarının MEB 2013 Okul Öncesi Eğitim Programı kazanımlarına uygunluğunun değerlendirilmesi amacıyla yapılmıştır. Araştırmada, TRT Çocuk resmî web sayfasında yer alan 68 şarkı doküman analizi yöntemi ile incelenmiş ve içerik analizi yöntemiyle analiz edilmiştir. Çalışma sonucunda incelenen şarkılarda kazanımları destekleyen toplamda 1207 örneğe rastlanmıştır. Dil gelişimi alanında yer alan kazanımları destekleyen 593 örneğe rastlanırken bilişsel gelişim alanını destekleyen 253, sosyal ve duygusal gelişim alanını destekleyen 173, motor gelişim alanını destekleyen 149 ve öz bakım becerisi alanını destekleyen 39 örneğe rastlanmıştır Şarkılarda en çok desteklenen kazanımın, 68 şarkının tamamını desteklemesiyle motor gelişim kazanım 5, olduğu görülmüştür. Bu kazanımı 68 şarkının 67’sini desteklemesiyle dil gelişimi kazanım 1’in izlediği tespit edilmiştir. TRT çocuk şarkılarında, bilişsel gelişim kazanım 14, 20 ve 21, sosyal- duygusal gelişim alanında kazanım 11 ve 14’ü destekleyen örnek ifadelere rastlanamamıştır. İncelenen 68 şarkıda bu 5 kazanım haricinde, tüm kazanımları destekleyen ifade örneklerine rastlanmıştır. Yapılan araştırmanın sonucunda, TRT çocuk şarkılarında, MEB 2013 Okul öncesi eğitim programını destekleyen çok sayıda ifadenin yer aldığı sonucuna ulaşılmıştır. Ailelere ve alanda çalışan eğitimcilere çocuklarla uygulayacakları etkinliklerde TRT Çocuk şarkılarına başvurması tavsiye edilebilir.Music is an extremely important resource that supports all areas of the development of children, improves aesthetic perception, increases self-respect, and reveals creativity. Preschool music activities include singing, listening to sounds, differentiating sounds, rhythm, musical storytelling, and dancing. This study was carried out in order to evaluate the suitability of the songs of TRT Çocuk for the MEB 2013 Pre-School Education Program gains. In the research, 68 songs on the official website of TRT Çocuk were analyzed by the document analysis method and the content analysis method. As a result of the research, 1207 examples supporting the gains were found in the songs examined. 593 samples supporting the gains in the field of language development, 253 supporting the cognitive development area, 173 supporting the social and emotional development area, 149 supporting the motor development area, and 39 supporting the self-care skill area were determined. The motor development 5th gain was found to be the most supported gain in the songs, as it was supported by all 68 of them. It was determined that language development gain 1 followed this outcome by supporting 67 out of 68 songs. In the songs of TRT Çocuk, no sample expressions supporting cognitive development gains 14, 20, and 21, and social-emotional development gains 11 and 14 were found. In the 68 songs analyzed, expression examples supporting all gains were determined, except for these five gains. As a result of the research, it was concluded that there are many expressions in the songs of TRT Çocuk that support the MEB 2013 pre-school education program. The songs of TRT Çocuk can be recommended to families and educators working in the field for their activities with children

    Short-term prognostic value of the culprit-SYNTAX score in patients with acute myocardial infarction

    Background: The SYNergy between Percutaneous Coronary Intervention with TAXus and Cardiac Surgery (SYNTAX) score is a scoring system that helps to decide on surgery or percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (MI), and studies are showing the prognostic value of this scoring system in both MI and coronary artery disease patients undergoing PCI. In acute coronary syndrome (ACS) patients, the infarct-related artery and the complexity of the lesions are also important in terms of mortality and morbidity. Our study aimed to determine the prognostic value of the culprit vessel’s SYNTAX score (cul-SS) in patients presenting with MI. Methods: In our study, 1284 patients presenting with MI were analyzed retrospectively. The SYNTAX scores and cul-SS of the patients were calculated. In-hospital and 30-day deaths and major complications were accepted as primary outcomes. The SYNTAX scores and cul-SS were compared in terms of predicting primary outcomes. Conclusions: Major complications were observed in 36 (2.8%) patients, death in 42 (3.3%) patients, and stent thrombosis in 24 (1.9%) patients. The area under the curves for SYNTAX and cul-SS for predicting primary outcomes is 0.64 and 0.68 (p = 0.026), respectively. Cul-SS was as successful as the SYNTAX score in predicting stent thrombosis and was superior in predicting short-term death and major complications

    An innovative approach to order picking and positioning in logistics warehouses

    Amaç: Uluslararası bir lojistik deponun verimliliğini artırmaya katkı sağlamak için depo dolaşım mesafesini kısaltacak yenilikçi ve etkin bir konumlandırma yönteminin geliştirilmesi amaçlanmıştır. Yöntem: 1.239.545 adet ham depo verisi uzman ekiplerin desteği ile analiz edilmiş ve yorumlanmıştır. Makine öğrenimi algoritmalarının depo konumlarının belirlenmesinde kullanılabilirliği araştırılmıştır. Dolaşım hızı ve anahtar değer hesaplamasına dayalı olarak alternatif bir konumlandırma çözümü geliştirilmiştir. Statik olarak beş farklı bölgeye ayrılan sahada uygulamalı testler yapılmıştır. Önerilen yöntemin etkinliği, bilinen konumlandırma yöntemleriyle karşılaştırılmıştır. Bulgular: Öğrenme algoritmalarının başarı oranları (%54-%64) uzman ekipler tarafından yeterli bulunmamıştır. Geliştirilen çözümde ürünleri doğru yere yerleştirme başarı oranı %90,93 olmuştur. Bir aylık gözlem sonucunda depo giriş çıkış işlemlerinde kat edilen mesafe yaklaşık 880 km kısalmış, depo doluluk oranı %54,07'den %55,68'e yükselmiştir. Elde edilen sonuçlar önerilen yöntemin etkinliğini göstermektedir. Özgünlük: Depo yerleşim yüzdeleri ve dolaşım mesafelerinde önemli kazanımlar elde edilmiştir. Bilinen diğer yöntemlere göre daha etkili ve yenilikçi bir yaklaşım sunmaktadır. Bölge sınırı olmayan dinamik, verimli ve başarılı yapısıyla farklı depolarda uygulanabilir özgünlüktedir. Gerçek depo verilerine ve uzman görüşlerine dayalı olarak oluşturulması literatüre eşsiz bir katkı sağlamaktadır.Purpose: In order to contribute to increasing the efficiency of an international logistics warehouse, it is aimed to develop an innovative and effective positioning method that will shorten the warehouse circulation distance. Methodology: 1,239,545 raw warehouse data have been analyzed and interpreted with the support of expert teams. The usability of machine learning algorithms in determining warehouse locations has been investigated. An alternative positioning solution has been developed, based on circulation rate and key value calculation. Practical tests were carried out in the field, which is statically divided into five different regions. The effectiveness of the proposed method is compared with known positioning methods. Findings: The success rates (54%-64%) of the learning algorithms were not found sufficient by the expert teams. In the solution developed, the success rate of placing the products in the right place was 90.93%. As a result of a one-month observation, the distance covered in warehouse entry and exit operations has been shortened by approximately 880 km, and the warehouse occupancy rate has increased from 54.07% to 55.68%. Obtained results show the efficiency of the proposed method. Originality: Significant gains have been achieved in warehouse settlement percentages and circulation distances. It offers a more effective and innovative approach than other known methods. It is uniquely applicable in different warehouses with its dynamic and efficient structure that has no regional borders. Its creation based on real warehouse data and expert opinions makes a unique contribution to the literature

    Bilateral locked posterior shoulder dislocation overlooked for 15 months treated with the modified McLaughlin procedure: A case report

    Science Citation Index Expanded (SCI-E)Neglected bilateral posterior shoulder dislocation is a rare injury caused primarily by an epileptic seizure. The injury is usually associated with a reverse Hill-Sachs lesion in the anteromedial aspect of the humeral head. The modified McLaughlin technique may avoid instability and osteoarthritis when 20 to 40% of the articular surface is affected by reverse Hill-Sachs. In this article, we present the clinical results of a case overlooked in the literature for the longest time, i.e., for 15 months. A 46-year-old male patient was receiving treatment for epilepsy for five years. There was no fall or trauma in the four seizures he had during this time. The last seizure was 15 months ago. When the patient presented to our clinic, both shoulders were symmetrically deformed, the anterior shoulder contour disappeared (empty socket sign), and there was a loss of upper contour. The computed tomography (CT) scan revealed a posteriorly locked dislocation with a reverse Hill-Sachs lesion in 32% of the left shoulder and 36% of the right shoulder. We applied the modified McLaughlin procedure to the dominant right shoulder and, two months later, we used it to the left shoulder (with a graft taken from the anterior superior iliac spine). At one-year of follow-up, both shoulders were moderately functional: forward elevation left 70° and right 50°, abduction left 40° and right 60°, and internal rotation: the back of the hand could touch the fifth lumbar vertebra. Meanwhile, the patient did not suffer from recurrent dislocation. The pre- and postoperative Constant-Murley Scores for the right and left shoulder were 30/52 and 11/48, respectively. Although the operational outcomes using the modified McLaughlin technique were not ideal, with no recurrence, the patient seemed to be satisfied with this outcome. In conclusion, in neglected locked shoulder fracture-dislocations, the modified McLaughlin technique is a method that can respond to the pathophysiology by eliminating reverse Hill–Sachs lesion

    Ülkemizdeki yükseköğretim kurumlarının performansını değerlendirmek üzere 2012 yılından itibaren Türkiye Bilimsel ve Teknolojik Araştırma Kurumu (TÜBİTAK), Türkiye Bilimler Akademisi (TÜBA), Yükseköğretim Kurulu (YÖK), Bilim Sanayi ve Teknoloji Bakanlığı’na bağlı bazı birimlerden temin edilen veriler esasında 23 gösterge dikkate alınarak bir “Girişimci ve Yenilikçi Üniversite Endeksi (GYÜE)” tanımlanmıştır. Bu endekste; üniversitelerin bilimsel ve teknolojik araştırma yetkinliğine, işbirliği ve etkileşim aktivitesine, fikri mülkiyet etkinliğine, girişimcilik ve yenilikçilik kültürüne ve ekonomik katkı ile ticarileşme potansiyeli alanlarına bağlı olarak değerlendirme yapılmaktadır. Bu yıl 2022 yılı verileri kullanılarak yapılan değerlendirme sonuçları açıklanmıştır. Bu sonuçlara göre; daha önceki yıllarda dereceye giren üniversitelerin yine listede olduğu ve sıralama da küçük değişiklikler bulunduğu görülmektedir. Ancak bazı Vakıf üniversiteleri (İstanbul Medipol, Bahçeşehir, Yeditepe, Yaşar, Atılım ve Acıbadem Mehmet Ali Aydınlar gibi) önemli atılımlar yaparak ilk 50 üniversite içinde yer almışlardır. Bu makalede, ilgili endeksin parametreleri tanımlanarak, alan ve ölçüt esasında ilk elli sıra içinde yer alan üniversitelerin değerlendirmesi yapılmakta ve bazı üniversiteler için öneriler getirilmektedir.Since 2012, an “Entrepreneurial and Innovative University Index (GYÜE)” has been defined by taking into account 23 indicators with the data obtained from the Scientific Technological Research Council of Turkey (TÜBITAK), the Turkish Academy of Sciences (TÜBA), the Higher Education Council (YÖK) and some units of the Ministry of Science and Industry and Technology. This index evaluates universities based on their scientific and technological research competence, cooperation and interaction activity, intellectual property activity, culture of entrepreneurship and innovation, and potential for economic cont ribution and trade. Currently, the results of the evaluation made using the data of 2022 are announced. According to these results, it is seen that the universities that ranked in the previous years are still on the list and the ranking is also found with minor changes. Howev er, some foundation universities (such as Istanbul Medipol, Bahçeşehir, Yeditepe, Yaşar, Atılım and Acıbadem Mehmet Ali Aydınlar) have made significant strides and have been among the top 50 universities.     A practical study on automatizing the software deployment process

    Yazılım dağıtımı, geliştirilen bir uygulamanın çalıştırılacak dijital ortamlara yüklenmesidir ve yazılım geliştirme yaşam döngüsünde önemli bir aşamadır. Herhangi bir hataya veya kesintiye yol açmayacak şekilde titizlikle uygulanmalıdır. Küçük organizasyon ve sistemlerde bu sürecin işleyişinin manuel olarak işletilebilmesi mümkündür. Ancak orta ve büyük organizasyonlarda dağıtım yapılacak ortamların çeşitliliği, büyüklüğü, karmaşıklığı buna engeldir. Ayrıca iş sürekliliği, güvenlik ve uyum gibi gereksinimler dikkate alındığında manuel dağıtımdan kaynaklı birçok risk söz konusudur. Bu tür risk ve sorunların aşımında otomatik yazılım dağıtım araçlarına ihtiyaç vardır. Bu çalışmada yazılım yaşam ve dağıtım süreci, manuel dağıtımdan kaynaklı sorunlar, sorunların çözümünde kullanılan otomatik dağıtım araçları incelenmiştir. Yazılım dağıtım sürecinin otomatikleştirilmesi uygulamalı olarak çalışılmıştır. Bu çalışma daha önce yapılan çalışmalardan farklı olarak, konusu ve uygulamalı bir sunum olması sebebiyle ilgili kurum çalışanları, öğrenci ve araştırmacılar için teknik bir rehber niteliğindedir. Yazılım dağıtım süreci ve bu sürecin otomatikleştirilmesine yönelik uygulamalı çalışma literatüre kazandırılmıştır.Software deployment is the installation of a developed application on digital media to be run and it is an important stage in the software development life cycle. It should be applied meticulously so as not to cause any errors or interruptions. In small organizations and systems, it is possible to operate this process manually. However, the diversity, size, and complexity of the environments to be distributed in medium and large organizations prevents this. In addition, there are many risks arising from manual deployment when considering requirements such as business continuity, security and compliance. Automatic software deployment tools are needed to overcome such risks and problems. In this study, software lifetime and deployment process, problems arising from manual deployment and automatic deployment tools used to solve problems are examined. Automation of the software deployment process has been practiced. This study, unlike previous studies, is a technical guide for the relevant institution staff, students and researchers due to its subject and practical presentation.     Analysis of the zezé series in the context of ten core values

    Değerler, toplumsal yaşamın sürdürülmesinde önemli olan yargılardır. Toplumlar birlik ve beraberliklerini sürdürmek için bu değerleri gelecek kuşaklara aktarırlar. Bu değerler çocuklara aktarılırken kullanılan kaynaklardan biri de edebi eserlerdir. Bu çalışmanın amacı, Millî Eğitim Bakanlığının on kök değer ve bu değerlere ilişkin tutum ve davranışlar sınıflandırmasına göre Jose Mauro De Vasconcelos’un Şeker Portakalı serisini oluşturan Şeker Portakalı, Güneşi Uyandıralım ve Delifişek romanlarını değerlendirmektir. Araştırmada nitel araştırma desenlerinden temel nitel desen kullanılmış, verilerin analizinde ise betimsel analiz yönteminden yararlanılmıştır. Çalışma sonucunda Şeker Portakalı serisinde, sevgi, dostluk ve sabır kök değerlerinin en fazla kullanılan kök değerler olduğu, en az yer alan kök değerlerin ise öz denetim, saygı, sorumluluk ve adalet olduğu görülmektedir. Çalışmanın sonucunda serinin kök değerlerin aktarımında etkili ve değerlere ilişkili tutum ve davranışları kazanımda fayda sağlayacak nitelikte olduğu sonucuna ulaşılmıştır.Values are judgments that are crucial to preserving social life. Societies pass on these values to future generations in order to maintain their unity and solidarity. Literary works are one of the resources used to teach values to children. The aim of this study is to examine Jose Mauro De Vasconcelos's the Zezé Series which consists of Şeker Portakalı, Güneşi Uyandıralım, and Delifişek based on the ten core values of the Ministry of National Education and the classification of attitudes and behaviors consistent with these values. Basic qualitative pattern one of the qualitative research methods is used in the research, and the data is analyzed using the descriptive analysis method. According to the findings of the study, love, friendship, and patience are the most frequently used core values in the Zezé Series, while the least common core values are self-control, respect, responsibility and justice. The study concluded that is effective in transferring fundamental values and will help readers develop value-related attitudes and behaviours
