    Cardiac Resynchronization Therapy (CRT) has been established as a major component of heart failure management, resulting in a significant reduction in patient morbidity and death for patients with increased QRS duration, low left ventricular ejection fraction (LVEF), and high risk of arrhythmias. The ability to synchronize both ventricles, lower heart failure hospitalizations, and optimize clinical outcomes are some of the attractive characteristics of biventricular pacing, or CRT. However, the high rate of CRT non-responders has led to the development of new modalities including leadless CRT pacemakers (CRT-P) and devices focused on conduction system pacing (CSP). This comprehensive review aims to present recent findings from CRT clinical trials and systematic reviews that have been published that will likely guide future directions in patient care

    Use of Calcium Modification in Percutaneous Coronary Intervention: A Comprehensive Review

    Calcified coronary lesions remain a challenge in percutaneous coronary intervention (PCI) in both situations of acute myocardial infarction (MI) and stable coronary syndrome. It significantly increases the risk of procedural complications due to difficulty in equipment delivery, balloon expansion, and stent delivery. Furthermore, stent thrombosis, dissection, perforation, and future in-stent restenosis occur more frequently in calcified coronary lesions, impacting repeat target vessel revascularization and increasing the risk of future MI. With intracoronary imaging (intravascular ultrasound and optical coherence tomography), peri-procedural success for treating calcified lesions has increased significantly. Different modalities of calcium modification techniques have since been introduced. This review will discuss the pathophysiology and phenotypes of calcium deposition in the coronary vessels, including eccentric calcified plaques and calcified nodules. We will also focus on calcium modification techniques and their mechanisms: (1) Balloon escalation technique, (2) intravascular lithotripsy, (3) orbital atherectomy, and (4) rotational atherectomy. We will focus on the strengths and limitations of each technique, based on current recommendations and expert consensus from SCAI. We will also provide contemporary evidence of each modality for treating different phenotypes of calcified lesions. In summary, this article provides a comprehensive review of current guidelines for optimizing the treatment of calcified coronary lesions in PCI

    Exploring Human Centered Design Thinking to Develop Career and Technical Education (CTE) Programs for Incarcerated Individuals: Development and Pilot of a Faculty Survey Instrument at a Utah Technical College

    Postsecondary Career and Technical Education (CTE) institutions are confronted with growing demands to improve efficiency while boosting student outcomes and achieving performance metrics for funding. As enrollment competition grows and funding sources change, technical colleges are looking to develop new programs and identify untapped student populations such as incarcerated individuals. Instructors of postsecondary CTE technical programs often bring industry experience but typically lack formal teaching training unlike their secondary CTE instructors which may create variable teaching success and impacts both teacher confidence and student achievement, especially for at-risk populations such as incarcerated individuals. This research explores faculty perspectives on human-centered design thinking (HCDT) as an approach to student-centered program design and faculty interest in teaching in carceral settings that were captured by a survey instrument validated across two technical college campuses in Utah. The instrument assessed 10 competencies in 4 design thinking areas utilizing a Likert scale ranging from Not Important to Very Important. Phase one of the pilot study included responses from 14 participants. Phase two of the study, collected responses from nine participants. The findings of this study suggest that faculty members understand the value of adaptive teaching methods centered on students but lack sufficient training to apply HCDT principles effectively. The research demonstrates the necessity for faculty professional development programs that match industry requirements while promoting interactive teaching models which center around student needs. Postsecondary institutions can develop more effective learning environments that support student success and workforce preparedness by incorporating HCDT into faculty training

    Testing for Dice Control at Craps

    Dice control involves “setting” the dice and then throwing them carefully, in the hope of influencing the outcomes and gaining an advantage at craps. How does one test for this ability? To specify the alternative hypothesis, we need a statistical model of dice control. Two have been suggested in the gambling literature, namely the Smith–Scott model and the Wong–Shackleford model. Both models are parameterized by θ ∈ [0, 1], which measures the shooter’s level of control. We propose and compare four test statistics: (a) the sample proportion of 7s; (b) the sample proportion of pass-line wins; (c) the sample mean of hand- length observations; and (d) the likelihood ratio statistic for a hand-length sample. We want to test H0 : θ = 0 (no control) versus H1 : θ \u3e 0 (some control). We also want to test H0 : θ ≤ θ0 versus H1 : θ \u3e θ0, where θ0 is the “break-even point.” For the tests considered we estimate the power, either by normal approximation or by simulation

    Advances in Mechanical Circulatory Support (MCS): Literature Review

    Heart failure is a heterogeneous disorder that can lead to cardiogenic shock. Mechanical circulatory support (MCS) devices can replace the cardiac function in an attempt to bridge patients to transplant or they can serve as destination therapy to improve overall patient functionality and in turn quality of life. Currently utilized MCS devices include devices such as Left Ventricular Assist Devices (LVADs), Biventricular Assist Devices (BiVADs), Impella, Total Artificial Hearts (TAHs), and extracorporeal membrane oxygenation (ECMO). This literature review examines a range of studies, case reports, and meta-analyses to present current approaches to mechanical circulatory support (MCS), along with the challenges and limitations of existing devices, common complications, and overall survival and long-term outcomes following MCS therapy

    The Coso Paradox: An Additional Perspective of Indigenous Identity

    The Coso Mountain Range in Inyo County, California is an archaeological site rich with evidence of human interactions throughout prehistory. One of the most striking features is Coso’s abundance of rock art, estimated at over 100,000 etchings, with over half depicting Ovis canadensis (Bighorn Sheep). Coso’s rock art has been long-debated as a site of “shamanistic” prevalence, including vision quests, rain god worship , and interpretations of fertility. While spirituality is an integral part of many Indigenous cultures, it is highly unlikely that such a mass creation spanning hundreds of years and multiple cultural groups would be solely spiritual.https://oasis.library.unlv.edu/durep_posters/1257/thumbnail.jp
