地域における減塩活動の展開
No full text塩分過剰摂取と高血圧症あるいは心血管疾患との関連性から、減塩活動が行われてきている。本稿では、減塩指向社会について、減塩活動の現状を、特にポピュレーションアプローチの観点から概説した。減塩活動のポピュレーションアプローチに関する研究と実践は、近年、加速しつつある。 その上で、日本の地域レベルでの減塩活動の事例をいくつか紹介し、その現状をまとめた。地域社会の啓発や食環境の整備を、行政、専門家集団、食品関連業界、そして住民などが協同して進めてきている実態が見てとれる。こうした活動の成果も検証されつつある。さらに、減塩活動の展望や地域医療の発展と類似する点についても見解を加えた。An excessivedietarysalt intakeis associatedwith high blood pressureand cardiovascular and cerebrovasculardisease; thus,salt reductionstrategieshavebeendeveloped. Thepresent review describes the actual status of salt reduction initiatives from the viewpoint of a population-based approach and activities. Such initiatives are currently in progress. In Japan,several community-level initiatives have aimed to reduce the intake ofsalt,and public education and the arrangement of the food-related environment has been conducted with a collaborative network that includes various members, such as local government workers, socio-medical professionals, food developers/providers and community-dwelling individuals. These community-based initiatives appear to improve the salt intake. 看護管理者が管理のために行う夜間勤務の現状と課題
Get PDF医療を取り巻く環境が大きく変化している状況下で、看護管理者が管理のために行う夜間勤務（以下、看護管理者の夜間勤務とする）について調査・検討した。調査方法は、2011年版関東病院情報から抽出した200床以上の一般病院362病院の看護部長を対象に、病院の概要、夜間体制・要員配置状況、看護管理者の夜間勤務についての自記式質問紙調査を郵送法により行った。131部（回収率36.1％）を回収し130部を分析した。その結果、行うべき業務ではないが行っている業務は、救急外来の診療支援、看護業務支援、患者の親族への対応、当直医師の補助だった。火災や災害発生時は、職員の指揮を68.7％が取ると回答し、手当を増額する、職員を増やし夜勤や当直の回数を減らすことを要望していた。夜間勤務に就く前に研修プログラムに基づいた事前教育の実施は11.1％に過ぎず、防犯や防災の体制の研修を81.2％が希望していた。看護管理者の夜間勤務の実態から、勤務体制の整備、業務の見直し、適切な人員補充、役割に見合った評価、研修プログラムの整備と実施の必要性が示唆された。In thecontext ofarapidlychanging medicalenvironment,asurveyandinvestigationofthe night shift work undertaken by nursing managers for the purposes ofmanagement (hereafter abbreviated to nursing managersʼnight shift work)was carried out. For the survey,nursing directors of362general hospitals with ?200beds listed in Japan Hospital Information 2011for the Kanto area were sent bypost a self-administered questionnaire survey,which included an overview of the hospital,night shift system and staff allocation,and nursing managersʼnight shift work. A total of 131 responses (return rate of 36.1％) were received, of which 130 responseswereanalyzed. Theresultsshowed that “work that isdone”ratherthan“work that should be done”included “support for emergency outpatient consultations”, “support for nursing work”, “dealing with patientsʼrelatives”, and “helping the on-duty doctor”. The results also showed that 68.7％ would “direct staff”when fires or disasters occurred and that they needed “to increase allowances”and “to increase staff numbers so that the number of night shifts and frequency of being on duty would decrease”. Only 11.1％ had engaged in “prior learning based on a training program”before being employed in night shift work, and 81.2％ wanted training in “systems ofcrime prevention and disaster prevention”. Theactual situation of nursing managersʼnight shift work suggests the need for organization ofsystems ofwork,revisionofthenatureofwork,appropriatesupplemen arypersonnel,evaluationinline with roles,and the organization and implementation of training programs.原著論文departmental bulletin pape
地域ケアの展開 ―ポピュレーションアプローチ―
Get PDF健康政策として「ポピュレーションアプローチ」の重要性が示されるようになっている。地域ケアにおけるポピュレーションアプローチの展開について概説した。生活習慣や健康課題の改善への取り組みを事例として挙げた。未だ途上ではあるが、地域づくりの大きなビジョン、そして地域の保持・発展に対する明確な意識を持ちつつ、アプローチを練成していくことが肝要である。The importance of‘population approach’has been indicated as a health policy.The current paper herein describes the cases that correspond to population approach in community care context. In particular, the cases that could improve lifestyles and health problems have been described. Although the attempt based on the approach is still developing, it would be pivotal to establish the approach with a grand vision to create a future community and a clear sense
