ADVANCES IN ENDOSCOPIC OTOLOGIC SURGERY: CLINICAL EFFICACY, INDICATIONS, AND OUTCOMES

    No full text
Endoscopic ear surgery has emerged as a transformative advancement in the field of otorhinolaryngology. Unlike traditional microscopic techniques, endoscopy provides a panoramic, magnified view of the middle ear structures, facilitating minimally invasive interventions. This article presents a comprehensive review of the development, application, clinical outcomes, and limitations of endoscopic otologic surgery. The study includes a retrospective and prospective evaluation of 150 patients undergoing endoscopic tympanoplasty, stapedotomy, and cholesteatoma removal. Analysis of surgical time, postoperative pain, hearing outcomes, and complication rates reveals that endoscopic techniques result in shorter hospital stays, lower morbidity, and equivalent or superior hearing restoration compared to conventional approaches. Chronic maxillary sinusitis (CMS) is a prevalent and debilitating condition in otorhinolaryngology that leads to significant morbidity, diminished quality of life, and frequent healthcare utilization. Traditional medical therapies often fail to provide lasting relief in a substantial number of patients, necessitating surgical intervention. Endoscopic sinus surgery (ESS) has emerged as a standard approach due to its minimally invasive nature, precise anatomical targeting, and reduced complication profile compared to traditional open procedures such as the Caldwell-Luc operation. This prospective study aimed to evaluate the clinical efficacy and safety of ESS in managing chronic maxillary sinusitis by assessing symptom resolution, imaging findings, complication rates, and recurrence over a 12-month period. A total of 120 patients with refractory CMS underwent ESS at the Department of Otorhinolaryngology No. 2, Samarkand State Medical University. Pre- and post-operative evaluations included standardized symptom questionnaires (SNOT-22), endoscopic examination, and radiological assessment using the Lund-Mackay scoring system. The results demonstrated significant improvements in symptom burden, mucosal healing, and sinus ventilation. Complications were rare and mostly self-limiting, and the recurrence rate remained below 5% at one-year follow-up. These findings underscore the pivotal role of ESS in modern sinus surgery and support its use as a primary surgical intervention for chronic maxillary sinusitis resistant to conservative management. Furthermore, the study highlights the need for comprehensive pre-operative assessment and individualized post-operative care to optimize outcomes

PATHOGENESIS, AGE DEPENDENCE AND MODERN CLINICAL METHODS OF TREATMENT OF EUSTACHITIS

Due to this combination, eustachitis is also called secretory (exudative) otitis media. Other synonyms: tubo-otitis, salpingo-otitis, otosalpingitis, catarrh of the eustachian tube and tubotympanitis [11]. Eustachitis is an inflammation of the mucous membrane of the auditory tube. It can be acute and chronic, unilateral and bilateral. Eustachitis occurs in people of any age, but is more common in children, since their auditory tube is shorter and wider. Some researchers believe that damage to the auditory tube alone is almost impossible: the middle ear, that is, the auditory tube, the tympanic cavity and the mastoid process are usually involved in the inflammatory process in one way or another

POLYCYSTIC OVARY SYNDROME AND METABOLIC SYNDROME: CLINICAL FEATURES AND MANAGEMENT STRATEGIES

    No full text
Polycystic Ovary Syndrome (PCOS) is a prevalent endocrine disorder among women of reproductive age, characterized by hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology. A significant proportion of women with PCOS also exhibit features of Metabolic Syndrome (MetS), including insulin resistance, obesity, dyslipidemia, and hypertension, which elevate the risk for cardiovascular diseases and type 2 diabetes mellitus. This article explores the clinical manifestations, diagnostic challenges, and management strategies for PCOS and its metabolic comorbidities, emphasizing the importance of early intervention and individualized treatment approaches. Polycystic Ovary Syndrome (PCOS) is one of the most prevalent endocrine disorders in reproductive-age women, affecting approximately 10–15% globally. It is often associated with Metabolic Syndrome (MetS), a cluster of metabolic disturbances including insulin resistance, central obesity, dyslipidemia, and hypertension. The coexistence of these two syndromes significantly increases the risk of cardiovascular disease, type 2 diabetes, and infertility. This article aims to explore the pathophysiological overlap between PCOS and MetS, outline the clinical implications of their coexistence, and provide updated management strategies based on current evidence. Understanding this interrelationship is crucial for improving long-term outcomes and quality of life for affected women
