    PATHOLOGICAL SIGNALING PATHWAYS IN THE TREATMENT OF DIFFUSE LARGE B-CELL LYMPHOMAS

    This study examines the efficacy of targeted therapies in improving treatment outcomes for Diffuse Large B-Cell Lymphoma (DLBCL) by focusing on the inhibition of CD20, BCL2, and EGFR pathways. A cohort of 21 patients was stratified by age and gender, and molecular analyses were performed using Next-Generation Sequencing (NGS) and Polymerase Chain Reaction (PCR) to assess the expression and mutation status of these key biomarkers. The findings reveal that patients with high CD20 expression benefit significantly from Rituximab-based treatment, resulting in improved overall and progression-free survival. In addition, the use of Venetoclax in BCL2-positive cases showed a marked survival advantage, supporting the role of BCL2 inhibition in overcoming resistance to apoptosis. Although less well-characterized in DLBCL, EGFR mutations identified in a subset of patients were associated with poorer outcomes; however, treatment with Erlotinib led to a moderate improvement in survival metrics. While these results are promising, the study is limited by its small sample size and short follow-up period, which restricts the broader application of the findings. Future research with larger, multicenter trials and longer observation periods is needed to validate these outcomes and to explore combination therapies and additional biomarkers for more refined patient stratification. Overall, the study underscores the potential of integrating molecular diagnostics with targeted therapies to establish personalized treatment approaches in DLBCL, paving the way for more effective management of this aggressive lymphoma

Установлены ультразвуковые диагностические критерии для рака молочной железы на фоне фибрознокистозной мастопатии.     Chronic rhinosinusitis (CRS) is a prevalent inflammatory condition of the paranasal sinuses leading to significant morbidity. Functional Endoscopic Sinus Surgery (FESS) is commonly performed for patients unresponsive to medical treatment. This study aims to assess clinical features, surgical outcomes, and postoperative complications in CRS patients undergoing FESS. Methods: A prospective study including 48 patients diagnosed with CRS refractory to medical therapy was conducted. Patients underwent FESS and were evaluated preoperatively and postoperatively through symptom scoring and endoscopic examination. Results: Postoperative improvement was noted in 85% of patients with significant reduction in nasal obstruction, facial pain, and purulent nasal discharge. Complications were minimal and managed conservatively. Conclusion: FESS is an effective and safe surgical treatment for CRS patients with persistent symptoms despite medical management

    В статье рассматриваются современные принципы лечения гастродуоденальных язвенных кровотечений — одного из самых острых осложнений экстренной хирургии. Исследование проведено на базе Самаркандского филиала РНЦЭМП с участием 62 пациентов, которым была оказана помощь с применением комплексного подхода: консервативного, эндоскопического и хирургического лечения. В работе подробно описаны клинические и диагностические особенности, включая стандартизированное использование эзофагогостродуоденоскопии для выявления источника и характера кровотечения, а также методы эндоскопического гемостаза (электрокоагуляция, инъекционная терапия, аргоноплазменная коагуляция). Результаты исследования показывают высокую эффективность эндоскопических методик и значимость своевременного выбора хирургической тактики в зависимости от степени риска рецидива. Проведённый анализ подчеркивает необходимость активного наблюдения за пациентами с язвенными кровотечениями, что позволяет снизить уровень осложнений и летальных исходов, оптимизировать лечебные алгоритмы и повысить качество оказания медицинской помощи

    THE ROLE OF NEUROPLASTICITY IN FUNCTIONAL RECOVERY FOLLOWING ISCHEMIC STROKE

    Neuroplasticity plays a pivotal role in the brain's ability to adapt and reorganize following ischemic stroke. This study aims to explore the cellular and systemic mechanisms of neuroplasticity and their impact on functional recovery after cerebrovascular events. Through the analysis of both literature and local clinical observations, the study investigates how timely and structured rehabilitative interventions can enhance neural adaptation and optimize patient outcomes. Emphasis is placed on phenomena such as synaptogenesis, dendritic arborization, and cortical remapping, all of which contribute to regaining lost motor and cognitive functions. This work highlights the necessity for personalized neurorehabilitation protocols guided by the principles of neuroplasticity. Neuroplasticity plays a fundamental role in post-stroke rehabilitation by enabling the brain to reorganize and form new neural connections in response to injury. This adaptive capability becomes especially crucial following ischemic stroke, where damaged brain regions need to be functionally compensated by unaffected areas. The present article explores the multifaceted mechanisms of neuroplasticity and its central role in post-stroke recovery, integrating insights from molecular neuroscience, neurorehabilitation protocols, and recent clinical trials. Key biological processes such as synaptogenesis, dendritic remodeling, cortical remapping, and axonal sprouting are discussed in detail, highlighting how these phenomena contribute to the restoration of sensorimotor and cognitive functions. Special attention is given to the "critical period" following stroke, typically within the first two weeks, during which the brain exhibits heightened receptivity to rehabilitative interventions. Leveraging this period effectively can significantly enhance functional outcomes. Furthermore, the article evaluates evidence-based rehabilitation strategies that stimulate neuroplasticity, including early and intensive physical therapy, sensory stimulation techniques, constraint-induced movement therapy (CIMT), and non-invasive brain stimulation methods such as transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS). The application of pharmacological agents targeting neurotrophic factors and synaptic modulation is also examined. The aim of this study is to underscore the potential of individualized, plasticity-based rehabilitation approaches that integrate neurobiological insights into personalized care plans. These strategies have the capacity to optimize recovery trajectories and improve quality of life for stroke survivors
