    A Multi-National Questionnaire-Based Analysis of Dental Students’ Knowledge of the Management of Deep Caries and the Exposed Pulp

    Introduction and aims: To evaluate knowledge regarding the management of deep carious lesions and exposed pulps among undergraduate and postgraduate endodontic students from ten dental institutions across ten countries, and the impact of operator (material, antibiotic prescription) and patient-related (age, symptoms) factors on their treatment protocols. Methods: An online questionnaire was distributed to evaluate student knowledge of the management of deep caries and exposed pulp related to four clinical scenarios. Simple descriptive statistics were used to describe the data and McNemar tests were employed to identify significant differences between the scenarios. The P-value was set at 5%. Results: A total of 435 undergraduates and 139 postgraduates from ten dental schools participated in this survey. The final survey included 401 responses from undergraduates and 127 from postgraduates for statistical analysis. When symptoms were present, the majority of undergraduate and postgraduate students preferred non-selective (complete) caries removal over selective (partial) caries removal in young patients. The majority of postgraduates preferred partial pulpotomy in younger patients and pulpectomy and root canal treatment (RCT) in older patients. The majority of undergraduates preferred pulpectomy and RCT in both young/old patients when symptoms were present. The majority of undergraduates and postgraduates opted for mineral trioxide aggregate and Biodentine, respectively, when treating the exposed pulp. Systemic antibiotics were not recommended by both undergraduates and postgraduates, regardless of the patient\u27s age and symptoms. Conclusion: Among the scenarios surveyed, the majority of undergraduates and postgraduates preferred: a) pulpectomy and RCT for older patients in the presence or absence of symptoms; b) hydraulic calcium silicate cements as pulp capping material; and c) did not recommend systemic antibiotics. Clinical relevance: The majority of students choose non-selective (complete) caries removal in all cases and if the pulp is exposed, the use of hydraulic calcium silicate cements iwas the preferred material. Systemic antibiotics are considered unnecessary, irrespective of the patient\u27s age and symptoms

    Bromelain as a Source of Debridement for Infected Orthopedic Implants

    Introduction: Enzymatic debridement has emerged as an alternative method to combat biofilms. Bromelain is an enzyme derived from pineapple stems that has been used in several studies as a method of biofilm dissolution (BD). As a result, researchers hypothesized that bromelain could be used to debride contaminated orthopedic implants. Methods: In this study, cortical bone screws were incubated in methicillin-resistant Staphylococcus aureus inoculated broth. Treatment groups were exposed to low-dose bromelain solution, high-dose bromelain solution, or bromelain powder with or without manual scrubbing of the implant. The screws were then stained with crystal violet dye and analyzed using optical density (OD). OD means were compared between each treatment group and their respective control group using analysis of variance. Results: Six screws were used for each group. The average OD of the low-dose bromelain solution group (0.104 ± 0.047) was no different compared with the control group (p = 0.345). The average ODs of the low-dose + scrub bromelain solution (0.068 ± 0.020) and high-dose + scrub solution (0.045 ± 0.014) were significantly different from their respective control group (p = 0.012; p = 0.001). The average ODs of screws in the high-dose (0.056 ± 0.012), powder (0.041 ± 0.010), and powder + scrub (0.032 ± 0.005) groups were also significantly different from their respective control group (p = 0.003; p = 0.001; p \u3c 0.0001). Discussion: On the basis of these outcomes, bromelain is a promising alternative for the debridement of biofilm-contaminated orthopedic implants.     Environmentally persistent free radicals lead to selective inhibition of CYP1 monooxygenase activities, and increased production of reactive oxygen species by reaction uncoupling

    This study focuses on the effect of Environmentally Persistent Free Radicals (EPFRs) on the P450 enzymes of the CYP1 family. EPFRs are a component of particulate pollutants, that are stable in the environment, but can generate free radicals, leading to oxidative stress and subsequent toxicity of the respiratory, cardiovascular, and immune systems once they enter an organism. The results show differences in the ability of EPFRs to inhibit CYP1-dependent substrate metabolism, with CYP1B1 being inhibited to the greatest extent. There also were differences in the ability of EPFRs to disrupt the POR•CYP1 complex, with CYP1B1 being the only form where EPFRs disrupted POR•CYP1B1 complex formation. Despite the inhibition of substrate metabolism, each CYP1 enzyme, when reconstituted with NADPH-cytochrome P450 reductase (POR) was able to synergistically stimulate the generation of reactive oxygen (ROS) in the presence of particulate matter. Interestingly, both POR and the CYP1 enzymes were able to stimulate ROS generation, even when in partial reconstituted systems where only one of the proteins was present. However, when both POR and CYP1 were combined in a complete reconstituted system, ROS generation was synergistically stimulated
