Transitioning Patients on Ventilators in Emergency Intensive Care Units from deep to light sedation
Full text link目的：集中治療室に緊急入室した人工呼吸器装着患者への深鎮静から浅鎮静に移行する間の看護実践を明らかにする。
方法：ICU看護の経験豊富な看護師を対象に半構成的面接を行い，質的帰納的に分析した。
結果：対象者8名から【深鎮静を維持する】【循環動態を安定させる】【二次的な弊害を予防する】【苦痛を予測し積極的に緩和を図る】【浅鎮静に移行してくる時のパニックを予防する】【説明したうえで援助を実施する】【心理的安定が図れるように環境を整える】の7つの看護実践が明らかとなった。
結論：深鎮静から浅鎮静に移行する間の看護実践には, 2つの特徴がみられた。深鎮静下では意識がなくても積極的に苦痛を緩和するとともに心理的な安定を図ることでエネルギーの消費を最小限に抑え, 浅鎮静に移行してくる時はパニックを予防してその後の治療や看護につながるように看護実践を行っていた。この間に患者の心理的安定が図れるように関わるケアは、その後の回復を支える実践であることが示唆された。Purpose: We explore nursing procedures for transitioning patients on ventilators emergency intensive care units(ICU) from deep to light sedation.
Methods: Semi-structured interviews were conducted with nurses experienced in ICU nursing care, and a qualitative
inductive analysis was performed.
Results: The following seven nursing objectives were identified by eight study participants: “maintain deep sedation,” “stabilize circulatory dynamics,” “prevent secondary damage,” “anticipate and proactively work to alleviate pain,” “prevent panic when switching to light sedation,” “provide explanation followed by support ” and “create an environment conducive to achieving psychological stability.”
Conclusions: It is necessary to proactively alleviate pain during deep sedation to achieve psychological stability, even if the patient is unconscious. Caution must be exercised during the transition to light sedation to prevent panic, which can negatively impact recovery.departmental bulletin pape
オンデマンド教材と対面を組み合せたブレンド型授業のデザイン
Full text linkコロナ期に実践したオンライン授業の経験を生かしておこなった、資料提示型のオンデマンド教材と対面の講義・ディスカッションを組み合わせたブレンド型授業の実践を振り返ったのち、アクティブラーニングの観点をふまえつつ能動的学習を促す授業デザインについて考察する。受講生アンケートからは、対面のディスカッションへの評価が高い一方で、パワーポイントを用いた講義は対面よりもオンデマンド教材での学習の方が理解が深まるとの意見がみられた。これらの結果から、能動的学習を促すためには、オンデマンドによる丹念に設計された事前学習を対面のディスカッションに繋げるという反転授業の要素を取り入れたブレンド型授業が適していると考えられる。departmental bulletin pape
