Present Status and Problems on Provision of Information and Decision-Making Support for Fertility Preservation in Young Female Patients with Breast Cancer
Get PDF本研究の目的は，乳がん医療と生殖医療に携わる医師および看護師による若年女性乳がん患者の妊孕性温存への情報提供・意思決定支援に関する現状と課題を関連文献レビューにより明らかにし，当該患者への適切な看護支援を考察することである。医学中央雑誌 Web 版，PubMed，CINAHL を用い，医師および看護師による若年女性乳がん患者への妊孕性温存の情報提供および意思決定支援に関する 16 文献を抽出し，分析対象とした。これらの文献レビューにより，妊孕性温存に関する情報提供はすべての当該患者には行われておらず，当該患者の意思決定支援には乳がん医療と生殖医療に携わる医師および看護師の連携を構築することが課題であることが示された。また，看護師は当該患者に対して，乳がん治療と妊孕性温存治療に関する情報を提示する，それらの情報についての理解度と意思を確認する，理解を促すなど患者に寄り添った支援を行っていたが，乳がん治療の開始までに当該患者本人が納得できるライフプランを選択できるよう支援することの難しさおよび限界を認識する中で, 当該患者に不確かなライフプラン形成を支援しているのが実情であった。したがって，看護師による若年女性乳がん患者へのより良い意思決定支援のためには，診療科を超えたチームとしての連携・協働とともに意思決定支援教材の開発と評価が必要であり，パートナーや家族を含めた意思決定支援への取り組みの重要性が示唆された。
The objective of the present study is to discuss plausible nursing support procedure to young female patients with breast cancer according to the present status and problems on the provision of information and decision-making support for the patients, which has been defined by review of the relevant literature. Sixteen articles regarding rovision of information and decision-making support for fertility preservation were selected from Web version of the Central Journal of Medicine, PubMed and CINAHL as the relevant literature.
As a result, it was indicated that the information regarding cancer treatment and fertility preservation was not provided to all the patients and mismatched collaboration between oncology and reproductive health specialists should be concerned. Although oncology and reproductive medicine nurses supported the patients by providing the information about breast cancer treatment and fertility preservation, confirming their intelligibility and inclination, and encouraging their understanding, it was the actual situation that the nurses provided uncertain decision of life plans for the patients with recognizing the difficulties and limitations of supporting to the patients in making acceptable choices before initiating breast cancer treatment. It was suggested that in order to achieve better decision-making supports for the patients, further inter-departmental cooperation of oncology and reproductive health nurses and development and evaluation of materials for the support would be needed, and decision-making support including patient’s partners and family would be also important.departmental bulletin pape
