    Long-term Outcomes of Appendectomy in Acute Appendicitis Patients

    Laparoscopic appendectomy is the preferred operation for acute appendicitis, which is a leading cause of surgical emergent care globally. This is diagnosed more often than not for its short term effectiveness in the treatment of appendicitis but its long term effects have not been well studied. Regarding the appendix, this work aims at evaluating the lengthier consequences of appendectomy, including the postoperative morbidities, the rate of recurrence, QoL, and GI profiles collecting data in five years. Records of 200 patients who received laparoscopic or open appendectomy between 2017 and 2019 were reviewed in the current study. Cumulative evidence shows the effectiveness of laparoscopic appendectomy through exhibiting less post-operative complications, shorter hospital stay, shorter time to recovery, and better quality of life than open appendectomy. Specifically, the rate of postoperative complications including surgical site infections and abdominal surgical site, incisional adhesions which are major causes of morbidity and chronic abdominal pain were significantly less in the laparoscopic patients. In addition, comparative analysis of Quality of life demonstrated the significant improvement of physical, mental, and social functioning in the laparoscopic patients. The present work focuses on the role of the surgery methods in the prognosis of long-term outcomes for patients, and illustrates the current trends in the development of minimal-contact surgical tactics. These findings are valuable in order to inform practice and empower patients about the approach to the treatment of acute appendicitis

    Moderating Effect of Mobile Technology on the Relationship Between Health Systems Governance and Service Delivery in National Referral Hospitals in Kenya

    In recent years, there is an increased attention to build formidable health systems governance to promote the highest attainable standard of health service delivery which has become a fundamental part of our human rights and of our understanding of a life in dignity. The health systems governance through the establishment of health policy has taken on increasing importance in the provision of health care services in the health institutions. The public hospitals in Kenya have weak health systems governance attributed to the existing health policy affecting quality healthcare. The current study sought to examine the moderating effect of mobile technology on the relationship between health systems governance and service delivery in National Referral Hospitals in Kenya. The study was anchored to the Contingency Leadership Theory. The study identified six categories of the target respondents, namely, 5 directors, 43 board members and 88 head of departments. The study used regression analysis to analyze the association between the variables at 0.05 level of significance. Results revealed that mobile technology positively and significantly moderated the relationship between health systems governance and service delivery in national referral hospitals in Kenya. The results support the current theories related to the study. Consequently, this study provides national referral hospitals with insights of how to improve health care service delivery through the adoption of appropriate mobile technology and health systems governance in the national referral hospitals. This could go a long way in ensuring there is improved service delivery in the national referral hospitals in Kenya. The implications of the study are that the moderating effect of mobile technology on the relationship between health systems governance and healthcare service delivery in Kenyan hospitals can lead to positive outcomes such as improved communication, data management, and patient-centered care. However, it also presents challenges that need to be carefully addressed through appropriate governance mechanisms. The successful integration of mobile technology requires a comprehensive and adaptive approach to healthcare governance in the national referral hospitals in the country

    An Herbal Tea Blend of Hibiscus sabdariffa, Zingiber officinale, and Mentha spicata: A Potent Source of Antioxidant and Anti-Obesity Properties

    Background: There is an urgent need to find safer and more sustainable solutions to tackle the rising global epidemic obesity and associated complications. The main objective of this study was to develop formulations of herbal tea blend from three plant species and to assess the antioxidant and antiobesogenic properties of the best formulation. Methods: The best formulation (FX) obtained (80% Hibiscus sabdariffa, 10% Zingiber officinale and 10% Mentha spicata) was validated by their better sensory acceptability and antioxidant properties. In vivo study using high-fat diet Wistar rats revealed that FX alleviated oxidative stress and metabolic disorders including those affecting hepatic and renal functions caused by high-fat died. Results: The administration of FX resulted in a reduction in food intake, body weight gain and metabolic efficiency index alongside lower blood content in triglycerides, total cholesterol, LDL-cholesterol, glucose, and atherogenic index when compared to the control groups. The results were comparable if not better than those obtained from the reference groups treated with a standard obesity treatment medicine, Orlistat. Conclusion: The developed herbal blend showed promising results for use as a safer product for obesity prevention and management as well as other oxidative stress-related health issues

    Harmony in Healing: Exploring the Potentials and Obstacles of Psionic Medicine in Integrative Healthcare

    Incorporating psychological considerations is an essential component in both the practice of medicine and the management of various complex situations. Psychological aspects can play a role in the etiology, diagnosis, and course of treatment. Psionic medicine is an approach to healing that combines traditional medicine with radiesthesia to address the root causes of illness without using any artificial chemicals. Psionic medicine ushers in a new era as it can potentially maintain and restore patients' health. Additionally, it significantly adds to the range of treatment options developed through biochemical medicine. Psionic medicine emphasizes applying subtle "informational" inputs that affect the patient's biofield, as opposed to chemistry and surgical intervention. Psionic medicine primarily emphasizes identifying and treating the underlying causes of disease. The "theory of mind" and heuristic methodology are at the core of this endeavor. Even though the "theory of mind" gives humans the ability to reason about their own and other people's mental states, the heuristic method is the approach to problem-solving that requires the least amount of mental effort. The heuristic method is based on empirical evidence. When using psionics, several aspects of the patient's psychic reality and pathogenic beliefs, the affected system, the experience of feeling, emotion, mood, and so on, are all investigated

    Phenomenological Approach for the Application of the FOAP Concept of the Vicious Circle of Ill-Health

    Background: The Field (F) Observation(O) for Accurate(A) Practicum(P) Concept of Vicious Circle of Ill- Health is an informative and innovative concept that focuses on how human activities within the environment reinforce each other in ways that contribute to vicious circle of ill-health. It is intended to change the perspective of community dwellers to embrace the new ideas of adapting healthy environmental practices to prevent and/or control the vicious circle of ill-health among them.Method: This study used the innovative qualitative research method that focused on the realist and meta-narrative reviews. The following databases were used as search engines: Google Scholar, OpenMD, RefSeek, BASE, Semantic Scholar, Academia, ResarchGate, and Google Search Engine.Result: The four stages of the phenomenological approach to the FOAP concept were vividly explained with stated characteristics. These four stages, when cleverly used, can make huge impacts on the lives of community dwellers. The various risk factors of vicious circle of ill-health, under the illustration of the Causal Theory, were greatly elaborated with all cited evidences to back the various claims.Conclusion: Most Liberians look healthy in an unhealthy environment; they get sick from contracting preventable diseases from the unhealthy environment; they go to hospitals or clinics for quality treatment against the preventable diseases; and they return to the same unhealthy environment that made them sick in the first place. This is the vicious circle of ill-health the FOAP Concept addresses. The FOAP Concept is a novel tool that will help national stakeholders and community dwellers successfully address the issue of unhealthy environmental practices

    Recognizing, Evaluating, and Prioritizing the Fundamental Factors Influencing the Growing Prevalence of Transgender and Non-Binary Gender Identity in the Pediatric Population

    This review observes the growing demographic of individuals identifying as transgender or non-binary, which currently represents approximately 1.3% (depending on the source) of the U.S. population. This population is shown to skew younger.Environmental xenobiotics, particularly those containing endocrine-disrupting chemicals (EDCs), are scrutinized for their role in affecting hormonal regulation crucial for gender identity differentiation. The research also highlights how exposure to pharmaceuticals with endocrine-disrupting properties may induce atypical patterns in gender identity development, suggesting that a variety of exogenous elements could impact neuroendocrine sexual differentiation.Medical disorders associated with altered androgen levels, such as congenital adrenal hyperplasia (CAH) and androgen insensitivity syndrome (AIS), are considered for their potential influence on gender identity from a biological perspective. Moreover, the role of diet and nutrition, particularly plant-based diets rich in phytoestrogens and conditions linked to obesity and metabolic syndrome, are explored for their effects on hormonal balance and, consequently, gender identity expression.This review acknowledges the significant impact of media representation and societal norms on shaping attitudes concerning gender expression and identity.The article points to psychological assessments and somatic markers as tools to explore prenatal hormone influences on gender identity, though emphasizing that they are not definitive. Furthermore, genetic and epigenetic findings offer a more in-depth understanding but lack diagnostic application. The identification and recognition of transgender and non-binary individuals continue to be self-determined processes that defy objective measurement by current medical standards

    Social Anxiety and Internet Addiction among Primary Health Care Workers at PSMMC, Riyadh

    Social Anxiety Disorder (SAD) and Internet Addiction (IA) significantly impact the quality of life and professional efficacy among Primary Health Care Workers (PCWs). Given their prevalence and the potential for negative comorbidities, including depression, this study aims to explore the occurrence and correlation of SAD and IA among PCWs in Riyadh, Saudi Arabia. Methods: This cross-sectional, questionnaire-based study assessed SAD and IA among 154 PCWs using the Social Phobia Inventory (SPIN) and Internet Addiction Test (IAT). The study was conducted over 12 months from April 2023 to March 2024 at Prince Sultan Military Medical City (PSMMC), Riyadh; it employed random sampling to ensure diverse participation. The Social Phobia Inventory (SPIN) and Internet Addiction Test (IAT) questionnaires were used to assess the prevalence of SAD and IA, respectively. Pearson’s correlation was used to determine the association between both variables. Results: The study found that 27.3% of participants exhibited some level of social anxiety, while 45% showed signs of IA. Specifically, mild to moderate internet addiction was present in 44.8% of respondents. Only 8.4% of participants fell into the severe category of social anxiety, while severe internet addiction was absent. A significant positive correlation was observed between SAD and IA (r = 0.36, P < 0.001). Additionally, higher income and certain professional roles were associated with lower instances of both conditions. Conclusion: The study highlights the prevalent but primarily mild to moderate levels of SAD and IA among PHCWs, underscoring the need for targeted interventions. While present, these issues may not severely impact PHCWs at PSMMC. The correlation between SAD and IA suggests the internet's dual role as a coping mechanism and potential risk factor, indicating areas for future research and intervention strategies

    Pattern of Hemoglobinopathies among Patients Attending PHC in Riyadh, Saudi Arabia

    Background: Hemoglobinopathies pose a significant public health burden globally, with varying prevalence rates across different references. Understanding the pattern of hemoglobinopathies among patients attending primary healthcare centers (PHCs) is essential for guiding effective prevention and management strategies. Study Aim: To determine the pattern of hemoglobinopathies among patients attending PHCs in Riyadh, Saudi Arabia. Methodology: A cross-sectional study design was employed, involving 320 participants attending PSMMC PHCs in Riyadh. Demographic data and HGB types were collected, and Chi-square tests were conducted to test associations between demographic variables and HGB types. Results: The majority of participants were aged between 18 to 30 years (40.6%), with a nearly equal distribution of females (52.8%) and males (47.2%). All participants were of Saudi nationality. The most prevalent HGB genotype was S/S (63.1%), followed by A/S (36.6%). Significant associations were found between age groups and HGB types (χ^2 = 28.233, p < 0.001), with the S/S genotype more prevalent among younger participants. Gender did not show a significant association with HGB types. Conclusion: This study provides insights into the prevalence and patterns of hemoglobinopathies among patients attending PHCs in Riyadh, Saudi Arabia. The findings underscore the need for targeted screening and intervention programs tailored to specific age groups to effectively address hemoglobinopathies within the population

    Prevalence of Chemotherapy-Induced Nausea and Vomiting: An Algerian Study

    Prevalence of chemotherapy-induced nausea and vomiting Prospective study at Kasr El Hiran hospital in Laghouat the objective was to study the prevalence of chemotherapy-induced nausea and vomiting (CINV), determine if the prescriptions in our department comply with the recommendations of expert societies, and evaluate the influence of certain patient-specific factors on the occurrence of CINV. This is a prospective observational study conducted at the day hospital of the medical oncology service at EPH Kasr El Hiran Laghouat, spanning a period of 3 months from September 21 to December 21, 2022. At the end of this study, 50 women and 18 men were interviewed, with a mean age of 49.40 years. Breast cancer was the most frequent underlying neoplastic pathology (50%). The majority of chemotherapy protocols used were classified as low emetic risk (44.11%). The prevalence of CINV was 79%, of which 51.9% occurred in the delayed phase. According to our results, none of the risk factors appear to have an influence on the prevalence of CINV, and the prescriptions are not in compliance with the recommendations of expert societies. Our results indicate that the management of CINV remains overall unsatisfactory and could be improved by providing increased accessibility to aprepitant
