It is generally recognized that non-communicable diseases, including arterial hypertension (AH), are a major public health problem in all countries, since about 70% of the causes of death in the world are associated with non-communicable diseases. However, the sudden emergence of an infectious disease - a new coronavirus infection - has changed the established understanding of diseases that pose the greatest threat to health. The coronavirus pandemic has affected all areas of people's lives, including health, economics, education and psychological aspects. It is predicted that the coronavirus pandemic will cause a new pandemic of non-communicable diseases, mostly cardiovascular (CVD). This prognosis seems very likely, given the decrease in physical activity, tense emotional environment, increased consumption of alcohol and smoking, increased stress levels during the pandemic, which are risk factors for CVD, including increased blood pressure (BP). In the context of the coronavirus pandemic, first of all, attention should be paid to patients with hypertension, given the high prevalence of this disease, as well as the fact that the components of the renin-angiotensin-aldosterone system (RAAS) are interested in the penetration of a new virus into the body. The consensus considered current data on the management of patients with hypertension in a pandemic, available as of August 2020

Thanks to the conducted studies, this pathology – acne, is considered to be an inflammatory disease with a chronic course, manifested in the form of papules, pustules and inflammatory nodes on the skin. And as the researchers' data show, the development of this disease is primarily based on violations of immune protection, which explains the multifactorial nature and various symptoms of acne. The pathological transformations of immunity are also influenced by the factors of the presence of so-called skin on the surface. conditionally pathogenic microorganisms in the form of Propionbacterium acne, parasitic and yeast-like fungi, mites of the genus Demodex, etc. All this together aggravates the course of the pathological process, leads to a malfunction in the mechanisms of formation of an adequate immune response, contributes to a change in the balance of anti-inflammatory cytokines and other inflammatory mediators, which, according to a number of researchers, leads to the chronization of the course of acne, the appearance of difficult-to-treat relapses [1.3.5.7]

The problems associated with alcohol and, in particular, with its excessive consumption, are on a par with major public health problems in the world and cause serious damage to human health, well-being and life. The problem of alcoholism is a complex, open dynamic system, dependent on and closely related to the biological and socio-economic systems. Undoubtedly, the increased negative impact of alcohol on the health of the population is due not so much to an increase in the consumption of alcoholic beverages, but to an increase in their toxicity. Studying the problem of alcohol poisoning, are few and consider it mainly from the standpoint of the clinic, pathogenesis and resuscitation. There is practically no study and analysis of the medical, socio-economic aspects of alcohol poisoning, their structure and prevalence

Antiendotoxic immunity, which is formed in inflammatory diseases of various bacterial (microbial) etiologies in children, is associated with the peculiarities of endotoxin - pyrogenic effect, activation of blood flow and intravascular thrombi formation, impaired hemodynamics, tension of the humoral joint of the immune system, impaired kidney function, weak immunogenic effect, pathogenesis of these nosological units. The identification and evaluation of antiendotoxic immunity in inflammatory diseases of bacterial etiology is of critical importance for determining the early diagnosis of these diseases, the prospect of the end of the disease. These obtained results suggest that the creation of clinical-immunological criteria to prevent the transition of diseases of bacterial etiology among children to chronic manifestations increases the educational value of antiendotoxic immunity

Clinical and biochemical monitoring of the severity of endogenous intoxication in chronic cholecystitis of the necessary component should include parameters of the antioxidant and oxidant systems, protein metabolism. One of the most informative indicators reflecting the level of endotoxicosis was the total antioxidant activity and catalase activity in erythrocytes, which make up a pool of medium-weight molecules. In the clinical and biochemical assessment of the severity of the condition of patients with cholecystitis, an important place should be occupied by calculated indices characterizing the ratio of the studied parameters in blood plasma and erythrocytes

Tactics of Treatment of Neurotrophic Corneal Ulcers

    Get PDF
Neurotrophic keratitis or trophic corneal ulcer occupies a special place in the structure of corneal pathology. Trophic corneal ulcer can be caused by both local (trauma, tumor, inflammation) and systemic processes (syndromic and endocrine pathology, consequences of damage to the central nervous system). there is often a combination of local and systemic factors . In most cases, the ulcer is accompanied by damage to the trigeminal and facial cranial nerves. Despite the treatment, trophic ulcer often ends with the development of severe keratouveitis and endophthalmitis with the loss of an eye

Purpose of work was improving treatment outcomes by developing an extramedullary minimally invasive method in fractures of the proximal portion of the humerus. Materials of the scientific work are devoted to clinical dada of 55 patients with multiple fragment fractures of the proximal portion of the humerus. The Neer (1970) classification was used to assess the severity of the injury and the condition of the bone fragments. Most patients fall into the category of surgical fracture of the humerus, of these, 8 (14.6%) were single-fragment fractures, 14 (25.3%) were double-fragmented, 15 (27.3%) were three-fragmented, and the remaining 18 (32.8%) were multi-fragmented fractures. All patients made surgery using new operative technique using an extramedullary minimally invasive method with Ilizarov apparatus and atraumatic threads in different directions through the rotator cuff muscles of the shoulder attached to the bone fragments for the ideal repositioning of the bone fragments. Results. Out of a total of 55 patients, 47 (85.4%) had a total score above 90 and were rated as good outcomes. Satisfactory results were obtained in 5 (9.2%) patients with the total score about 80 - 89. The satisfactory results were explained by the limitation of the amplitude of movement up to 10 degrees in the shoulder joint. In the remaining 3 (5.4%) patients, an unsatisfactory result was observed, the total score was less than 80, i.e., the presence of pain syndrome in the shoulder joint in these cases, limitation of range of motion to 15 degrees, and was explained by the presence of signs of osteoarthritis in the control X-ray image. After the surgery, all patients underwent shoulder rotator cuff restorative treatments for the prevention of "impingement syndrome", on the 21-25th day after the surgery, therapeutic physical exercises for the shoulder joint were started. Conclusions. Discussions on the choice of treatment for fractures of the proximal part of the humerus are growing. The proposed method of minimally invasive osteosynthesis in fractures of the proximal part of the humerus allows for less traumatic reposition of bone fragments, stable fixation of bone fragments. The proposed method of minimally invasive osteosynthesis allowed to increase the share of good and satisfactory results to 94.5%, as well as to improve the quality of life of patients
