    Bu çalışmamızda Edirne ili merkezindeki aile sağlığı merkezlerine başvuran erişkin hastaların gıda takviyesi kullanımı durumları ve hastaların bu konudaki bilgi-bilinç düzeyi'nin araştırılması hedeflenmiştir. Çalışmamıza 403 gönüllü hasta katılmıştır. Katılımcılardan sosyodemografik özellikler ve gıda takviyeleri konusunda bilgi, tutum ve davranışlarının tanımlanmasının talep edildiği anket formu yardımıyla veriler toplanmıştır. Verilerin analizinde tanımlayıcı istatistikler ve gruplar arasında fark bulunup bulunmadığı yerine göre Ki-kare testi kullanılarak karşılaştırılmıştır. İstatistiksel anlamlılık p<0,05 olarak kabul edilmiştir. Çalışmaya katılan 403 erişkin bireyden %41,7'si gıda takviyesi kullandığını belirtmiştir. En sık tercih edilen takviyeler multivitamin, D vitamini, balık yağı ve C vitamini olmuştur. Takviye kullanımı, boşanmış/dul bireylerde, komorbid hastalığı olanlarda ve düzenli sağlık kontrolü yaptıranlarda daha yaygındır. Katılımcılar doğal besinlerin yeterli olmadığını düşünmekte, takviyelerin kronik hastalıkları önleyebileceğine inanmakta ancak kanser gibi hastalıklarda etkili olduğunu daha az düşünmektedir. Takviyelerin hekim önerisiyle kullanılmasını ve laboratuvar testleriyle takip edilmesini savunan katılımcı oranı yüksektir. İnternetten satın alma ve ambalaj bilgileri konularında güven düşüktür. Vitamin ve minerallere dair bilgi düzeyi değişkendir. A ve C vitamini iyi bilinmekte; D vitamini, B grubu vitaminler, çinko, magnezyum, K ve E vitamini hakkında ise bilgi düzeyi düşüktür. Selenyum, krom, glutatyon, koenzim Q10 ve glukozamin gibi maddelere yönelik farkındalık çok düşüktür. Probiyotikler toplumda en çok bilinen doğal desteklerden biridir; omega yağ asitleri ve propolis ise daha az tanınmaktadır. Sonuç olarak, özellikle birinci basamak sağlık hizmetlerinde görev yapan aile hekimlerinin, bireyleri gıda takviyeleri konusunda doğru şekilde yönlendirmesi, gerekli durumlarda laboratuvar değerlendirmeleriyle rehberlik etmesi ve sağlık okuryazarlığını artırıcı bilgilendirme yapması büyük önem taşımaktadır. Gıda takviyesi kullanımının bireysel ihtiyaçlara göre, bilimsel temele dayalı ve güvenli bir şekilde sürdürülebilmesi için toplum düzeyinde aile hekimliğini merkez alan planlı bilgilendirme ve danışmanlık uygulamalarına ihtiyaç duyulmaktadır.     In this study, we investigate the variation of binding energy and self-polarization of an impurity in double square quantum wells made of three different materials, depending on the well width, laser field, and impurity position, under effective mass approximation. The subband energies are obtained by the finite difference method, and the variational method calculates the impurity energies. The binding energy and self-polarization under the effect of a laser field in a double square quantum well made of three different materials are examined in detail for the first time in this study. In double-quantum wells, it has been observed that material selection plays an important role in the calculation of binding energy and self-polarization. Studying the binding energy and self-polarization in a double square quantum well with three different materials under the effect of a laser field will enhance understanding of low-dimensional structure properties

    A randomized controlled trial: Mobile app vs videoconference telerehabilitation for rotator cuff tendinopathy

    Introduction The aim of this study was to compare the effectiveness of two methods of telerehabilitation (TR) delivery: mobile health TR (mHealth-TR) and video conference TR (VC-TR) in improving outcomes for patients with RC tendinopathy. Methods Eighty-five participants diagnosed with RC tendinopathy were randomized into synchronous (VC-TR) and asynchronous (mHealth-TR) groups. Both groups received an identical 8-week exercise programme delivered through their assigned platform. The programme included scapular mobilization, range-of-motion, strengthening, and stretching exercises. The primary outcomes were pain level (Visual Analogue Scale [VAS]), disability (Disabilities of the Arm, Shoulder and Hand [DASH] score), quality of life (Short Form-36 Health Survey [SF-36] score), and shoulder mobility (universal goniometer). The assessments were carried out at baseline, after the treatment (week 8) and at a follow-up of 16 weeks. Results The mean participant age was 51.8 years (SD 9.24), with 27% (n = 23) male. No significant between-group differences were observed for pain (VAS) or shoulder mobility (P > .05 for both). However, a significant group-by-time interaction effect was found for disability, measured by the DASH score (F(1,83) = 10.56, P = .001), and quality of life, measured by the SF-36 overall score (excluding physical role functioning, emotional role functioning, and social function) (Vitality/Energy: F(1,83) = 7.34, P = .006; Pain: F(1,83) = 4.78, P = .034; General Health: F(1,83) = 4.82, P = .032). Post-hoc analysis indicated significant improvements in disability and quality of life scores in the synchronous VC-TR group compared to the asynchronous mHealth-TR group. Specifically, DASH scores in the synchronous group decreased by 9.41 points (95% CI: 3.46 to 15.36, P = .002) from baseline to after treatment, and by 9.34 points (95% CI: 3.48 to 15.20, P = .002) by the 16th week. For quality of life, the VC-TR group showed significant improvements in the Vitality/Energy, Pain, and General Health domains from baseline to follow-up, with mean differences of 6.41, 11.68, and 10.83, respectively (all P < .05). Discussion This study suggests that patients with RC tendinopathy may experience greater improvements in pain management, disability, and overall quality of life through synchronous VC-TR compared to asynchronous mHealth-TR

    Evaluation of nursing students' ethical decision-making biases and attitudes toward artificial intelligence in nursing education

    Aim: This study examines nursing students' attitudes toward artificial intelligence (AI) and their association with biases in ethical decision-making processes. Background: AI technologies are central to healthcare, particularly clinical decision support systems and simulations. While AI accelerates decision-making processes, it also brings ethical responsibilities. Design: This is a descriptive cross-sectional study. Method: 265 nursing students were selected through stratified sampling from two universities in Turkey. Data were collected via an online survey using a demographic information form, the AI Attitude Scale (GAAIS) and the Ethical Decision-Making Bias Scale (EDBS). Results: Most participants were female (n = 223), with an average age of 20.45 (SD 1.67) years. The results of the GAAIS revealed that students generally had a positive attitude (3.38 SD 0.47). However, 36.2 % expressed distrust toward AI students who trusted AI more successfully to solve ethical issues and used AI tools more effectively. The average score on the EDBS was 2.48 SD 0.41. Additionally, students who encountered ethical decisions more frequently (2.30 SD 0.32) showed lower bias levels than those who experienced them less often (2.50SD0.44). Positive attitudes toward AI were positively associated with students' confidence in ethical decision-making (p < 0.05). Distrust in AI and difficulty accessing accurate information were identified as significant barriers. Conclusions: Attitudes toward AI significantly influence students' biases in ethical decision-making processes. The nursing curriculum should include AI ethics, critical thinking and decision-making skills.     1979 yılında Ohri Gölü, 1980 yılında Ohri şehri UNESCO tarafından dünya kültür mirası listesine alınmıştır. Bu durum Ohri Gölü'nün ve Ohri şehrinin ne kadar önemli bir yere sahip olduğunu göstermektedir. Ohri, 1385 yılında Osmanlı hâkimiyetine girmesinin ardından Rumeli Eyaleti'ne tabi sancak merkezi konumuna getirilmiştir. Çalışmada, Balkanlarda stratejik açıdan önemli sancaklardan biri olan Ohri sancağının merkez kazasının XVI. yüzyıl sonlarından XVIII. yüzyıl başlarına kadarki idarî, demografik ve iktisadî yapısının tespit edilmesi amaçlanmıştır. Çalışma, giriş bölümü hariç dört bölümden oluşmakta olup giriş bölümünde çalışmanın sınırları ve kaynakları tetkik edilerek Ohri'nin kısa bir tarihçesine yer verilmiştir. Birinci bölümde Ohri kazasının dönem içeresindeki coğrafi önemi ve idari yapısı ortaya konulmuştur. Devamında kazada görevli olan başta sancakbeyi olmak üzere alaybeyileri, çeribaşıları, subaşıları ve diğer kale görevlilerinden oluşan ehl-i örf ile ehl-i şer' mensubu kadıların kimler olduğu tespit edilmeye çalışılmıştır. İkinci bölümde Ohri şehrinin fizikî yapısı kısmında kale, saray, çarşı ve pazarlar ile derphane incelenmiştir. Diğer kısımda ise şehrin fizikî yapının şekillenmesinde vakıfların rolü ile sancaktaki vakıf ve vakıf eserleri açıklanmıştır. Üçüncü bölümde çalışılan dönem içerisinde Ohri kazasının nüfus yapısı incelenmiştir. Ohri kazasının iktisadî yapısının ele alındığı son bölümde ise Ohri kazasındaki zirai üretim, balıkçılık, hayvancılık ve kükürt maden işletmeciliği incelenmiştir. Ayrıca kazadaki avârız, cizye, nüzül, sürsat vb. gibi vergi gelirlerine de yer verilmiştir.     Evaluation of the impact level of coronavirus and occupational balance in geriatric individuals with and without a history of COVID-19

    This study aims to compare the impact of coronavirus and occupational balance levels while exploring the relationship between these two factors in geriatric individuals, both with and without a history of Coronavirus disease (COVID-19). A total of 102 geriatric individuals (51 with a history of COVID-19, 51 without a history of COVID-19) between the ages of 65-75 with a mean age of 68.60±2.51 participated in the study. The levels of impact of geriatric individuals to coronavirus were evaluated with the Coronavirus Impact Scale and their occupational balance was evaluated with the Occupational Balance Questionnaire (OBQ). A statistically significant difference was found between the groups in terms of the Coronavirus Impact Scale and OBQ results (p<0.05). It was found that geriatric individuals with a history of COVID-19 were more negatively affected by the coronavirus outbreak and had worse occupational balance than geriatric individuals without a history of COVID-19. In the study, a statistically significant relationship was found between the coronavirus impact scale and occupational balance results in geriatric individuals in both groups (p<0.05). It was concluded that the occupational balance of geriatric individuals deteriorated with the increase in the level of being affected by the coronavirus outbreak in both groups. To reduce the effects of pandemics and diseases, plans should be made to ensure the continuity of activity routines to support the healthy aging process of geriatric individuals. In collaboration with healthcare professionals, central and local governments, associations, foundations, and non-governmental organizations, community-based rehabilitation programs aimed at enhancing activity spaces for geriatric individuals should be implemented

    Protective effects of N-acetylcysteine against titanium dioxide nanoparticles-induced kidney damage in rats

    The objective of this study was to evaluate the potential protective effect of N-acetylcysteine (NAC) against kidney damage induced by titanium dioxide nanoparticles (TiO2NP) through biochemical, histological, and immunohistochemical analyses. Forty rats were randomly divided into four groups of 10 animals each. Saline was administered intragastrically to control group for 14 days. In NAC group, 150 mg/kg NAC was injected intraperitoneally for 21 days. In TiO2NP group, TiO2NP at a dose of 50 mg/kg/day, dissolved in saline, was administered intragastrically for 14 days. TiO2NP + NAC group received 50 mg/kg/day TiO2NP for 14 days and 150 mg/kg NAC for 21 days, starting 7 days before TiO2NP administration. At the end of experiment, rats were anesthetized, serum samples were collected for biochemical analysis, and kidney tissue was removed for histological and immunohistochemical analyses. There was no significant change in body weight, kidney weight, or serum urea-creatinine levels between the groups. TiO2NP caused a significant increase in vacuolization and brush border loss scores in tubular cells, as well as scores for congestion and leukocyte infiltration. However, NAC supplementation significantly ameliorated these impairments. Additionally, TiO2NP significantly increased NF-kB, TNF-alpha, and caspase-3 immunoreactivities, as well as the number of PCNA-positive and TUNEL-positive cells. NAC treatment decreased all immunoreactivities and TUNEL-positive cells, but did not change the number of PCNA-positive cells after TiO2NP exposure.     Omitting Axillary Dissection in Triple-Negative and HER2-Overexpressed Breast Cancers With Positive Sentinel Lymph Nodes During Upfront Surgery: SENATURK-OTHELLO Study

    The present study included patients with clinically node-negative TNBC or HER2-positive breast cancer who were treated with SLNB alone with pN +-disease. Almost all patients (97.5%) received nodal irradiation. Of 118 patients, the majority ( n = 98, 83.1%) had 1 metastatic lymph node in SLNB. The lymph node metastases were macrometastasis in 50% ( n = 59), micrometastasis in 36.4% ( n = 43), and isolated tumor cells in 13.6% ( n = 16). At a median follow-up of 53 months (34-73), the locoregional recurrence rate was 2.5% ( n = 3) without any axillary recurrence, and the systemic recurrence rate was 11.9% ( n = 14). Patients with HER2+ or TNBC early breast cancer with low volume axillary metastases treated with upfront SLNB-alone have shown an excellent local control with nodal irradiation. Purpose: Randomized trials including ACOSOG Z0011 and SENOMAC mostly included patients with hormone receptor-positive breast cancer, but a limited number of patients with aggressive tumor biology. Therefore, we assessed the oncological safety of omitting axillary dissection in patients with SLN-positive HER2-positive or triple-negative breast cancer at upfront surgery. Materials and Methods: This retrospective cohort study included patients with clinically node-negative HER2-positive and triple-negative breast cancer who had sentinel lymph node biopsy (SLNB) alone with pN+ disease. Almost all patients (97.5%) received nodal irradiation. Results: Between 2015 and 2020, 118 patients with HER2-positive ( n = 79, 67%) and triple-negative ( n = 39, 33%) tumors were included in the study from 8 centers. Of those, 94.9% were cT1-2 and 72% underwent breast-conserving surgery. Most patients ( n = 98, 83.1%) had 1 metastatic sentinel lymph node. Among those with involved sentinel lymph nodes, 59 (50%) had macrometastasis, 43 (36.4%) had micrometastasis and 16 (13.6%) had isolated tumor cells. After a median follow-up of 53 months, the locoregional recurrence rate was 2.5% without any axillary recurrence, and systemic recurrence rate was 11.9%. Factors associated with worse disease-free survival were having a cT2-3 stage and a triple-negative subtype disease. Having triple-negative tumor was the only significant factor associated with worse disease-specific survival. Conclusion: Patients with cN0 HER2-positive and triple-negative breast cancer with low-volume axillary metastases treated with upfront SLNB-alone showed excellent local control with nodal irradiation

    The impact of artificial intelligence on orthodontics: a systematic review of applications and implications

    INTRODUCTION: Artificial intelligence (AI) has emerged as a revolutionary technology with diverse applications across various domains, orthodontics included. This review article explores the widespread applications of AI in orthodontics and discusses the advantages and potential implications of AI in this specialized domain. The use ofAI algorithms and machine learning techniques has shown promise in enhancing orthodontic diagnosis, automating cephalometric tracing, estimating growth and development, assessing facial proportions, aiding in orthodontic treatment planning, guiding decisions on orthognathic surgery, and facilitating image segmentation and landmark identification. By leveraging AI technologies, orthodontists can improve efficiency, accuracy, and decision-making in patient care. EVIDENCE ACQUISITION: To assess the available literature on the subject, selective database searches were done up until September 2023. With the assistance of a health-sciences librarian, a search strategy that utilized all discovered key words and free-standing terms was developed. Embase, Scopus, PubMed, and Web of Science were the databases used. EVIDENCE SYNTHESIS: Following athorough analysis of the complete texts of those papers, 30 studies were deemed suitable for inclusion in the review. CONCLUSIONS: The review highlights the need for standardized assessment methods and the integration of AI into orthodontic research to further enhance clinical examinations and uncover new insights. Future directions may involve developing cloud-based platforms for data integration and sharing, leveraging large datasets for improved predictive results, and expanding AI applications in orthodontic research. Overall, AI holds significant potential in transforming orthodontic practice and improving patient outcomes
