    Mechanistic insights into the analgesic and anti-inflammatory effects of alcoholic extracts from Curcuma longa

    Background: Aim of the study was to scientifically validate the traditional Indian claims of Curcuma longa's (turmeric) antinociceptive (pain-relieving) and anti-inflammatory effects. Methods: The alcoholic extract of C. longa was tested in three rodent nociceptive models: acetic acid-induced writhing: examines visceral pain, formalin test: evaluates both acute and chronic neurogenic and inflammatory pain and tail immersion test to assess thermal pain. The extract's effects were compared to a control group and morphine (reference drug). Results: C. longa extract significantly reduced abdominal constrictions in the acetic acid test (59.36% inhibition). In the formalin test, the extract significantly decreased paw licking response time in both early (54.12% inhibition) and late phases (78.59% inhibition). C. longa extract significantly increased the tail flick reaction time in the immersion test, indicating pain relief. Conclusions: This study confirms the antinociceptive and anti-inflammatory activities of C. longa, providing scientific evidence for its traditional use in pain management

    Knowledge, attitude and practice of pharmacovigilance among medical students

    Background: Spontaneous voluntary adverse drug reaction (ADR) reporting is the backbone for the successful functioning of the Pharmacovigilance Programme of India. The aim of pharmacovigilance is to ensure safe and rational use of medicines, once they are released for general use in the society. Targeting the younger doctor for sensitization toward pharmacovigilance is the key to ensure practice of adverse drug reactions (ADR) reporting in clinical practice. The objectives of this study are to evaluate the knowledge, attitude, and practice (KAP) of pharmacovigilance among medical students at LLRM Medical College, Meerut, a tertiary care teaching hospital. Methods: A cross-sectional study was carried out using a pretested questionnaire. The questionnaire was designed to assess the KAP regarding pharmacovigilance. The medical students (2nd year, final year, and interns) were included in the study. Only the participants who gave their consent were included in the study. Results were calculated by proper statistical analysis. Results: The current study was conducted in a LLRM Medical College, Meerut, which included a total of 250 medical students of which 40% 2nd year, 40% final year students, and 20% interns. Most of them (83%) accepted that reporting ADR is necessary, and pharmacovigilance should be taught in detail to health-care professionals. Conclusions: This study demonstrated that knowledge of pharmacovigilance among medical students is improving gradually and pharmacovigilance and ADR reporting needs to be made compulsory, have better, interesting ways to learn and understand it, so that the students can practice it with confidence in their clinical practice

    Effect of Murdannia edulis Faden roots on serum prolactin level and mammary glands of lactating female albino rats

    Background: Murdannia edulis (Stokes) Faden (Myit-Cho in Myanmar) is a herb which has widely been used to induce or increase lactation by ethnic groups living in Tamu township of Sagaing division in Myanmar. This study aimed to explore the effect of Murdannia edulis on serum prolactin level and mammary glands of the lactating female rats. Methods: The type of the study was laboratory based experimental study. Thirty-six lactating dams were divided into six groups of 6 lactating rats in each group (n=6), namely group I (normal saline) as control, group II (5 mg/kg metoclopramide) as standard, groups III and IV (1 gm/kg and 2 gm/kg watery extract of Murdannia edulis), groups V and VI (1 gm/kg and 2 gm/kg ethanolic extract of Murdannia edulis), respectively. The extract and the drugs were orally administered from day 4 to 15 of lactation. At 16 days of lactation, the heart blood of lactating dams was collected under chloroform anaesthesia and serum prolactin level of dams was measured by using ELISA kit. The histological changes of mammary glands of the rats were also assessed. Results: The significant higher serum prolactin level were seen in ethanolic extract 2 gm/kg body weight treated group when compared to control. Histology of mammary glands in ethanolic extract treated rats showed stimulation of lobuloalveolar cell development. Conclusions: The lactogenic activity of the roots of Murdannia edulis may be due to promoting prolactin secretion and lobuloalveolar cell development of mammary glands

Here we present a case of 23 years old female patient came to hospital with chief complaints of pain radiating to left quadrant in the last 2 years and cough with sputum and nausea and admitted in the hospital for further evaluation. CT chest shows cystic mass in the left hypochondrium lateral to the stomach compressing the stomach, later did CT abdomen which clearly indicates the spleen is enlarged in size with well-defined hypodense non enhancing unilocular cystic lesion measuring 13×13×11 cm (CC×AP×TR) in the upper and mid poles

    Prevalence of self-medication practices in the Maharashtra population: a cross-sectional study

    Background: Self-medication is a significant issue in public health, with varying global prevalence. It is one of the major causes of the emergence of antibiotic resistance in India. The study's objective was to identify the pattern, cause, and adverse events of self-medication in Maharashtra's urban and rural areas. Methods: A cross-sectional survey was conducted using a pre-populated questionnaire; including information on age (18-85 years), non-prescription and prescription medication use. Age, gender, past medical and medication history, social history, and self-medication pattern, frequency, reason, source of information, and adverse events noticed as a result of self-medication are all sociodemographic questions on the questionnaire. A percentage analysis was used to examine the data. Results: Study states that out of 400 sample, 76% of the individuals self-medicate, with painkillers (49.4%), antacids (23.7%), antibiotics (15.3%), being the most often used drugs for ailments like headache, fever, bodily pain, and acidity. The top most medications taken for self-medication are paracetamol (53.1%), pantoprazole (30.43%), ondansetron (13.62%), and azithromycin (12.46%); 66.07% usage of prescription drugs as self-medication was found. The most frequent causes of self-medication were discovered to be mild illnesses, the easy accessibility of medications, and chemist recommendations, 8% of those surveyed had adverse events, and sources of information were chemists, past prescriptions, family members, and acquaintances. Conclusions: In Maharashtra, self-medication is a prevalent practice that primarily utilizes over-the-counter (OTC) drugs. As a result, consumers should be made aware of the hazards of antibiotic resistance and other pharmaceuticals

    Antimicrobial utilization pattern in indoor patients of obstetrics and gynecology ward of a tertiary care hospital

    Background: Rational prescribing of antimicrobials is fundamentally necessary to reduce incidence of antimicrobial resistance. To evaluate the rationality of antimicrobial prescribing practices in patients admitted to obstetrics and gynecology ward of a tertiary care hospital using ICMR guidelines and to check how many drugs were prescribed from NLEMI.          Methods: It was a prospective observational study. Study included newly admitted patients in obstetrics and gynecology ward receiving antimicrobial therapy. Drugs prescribed to the patients during their hospital stay were noted from case sheets in case record form. WHO core and economic indicators were used to analyze prescribing patterns. Medguideindia.com was referred for prices of drugs. Results: Average number of drugs prescribed for a patient admitted in the obstetrics and gynecology ward were 3.22 and 4.31 respectively. Percentage of drugs prescribed by their generic name were 37.11% and 39.42% in obstetrics and gynecology ward respectively. 26.85% and 34.14% of patients received drugs through parenteral route in obstetrics and gynecology wards respectively. All antimicrobials were prescribed from NLEMI. Conclusions: WHO recommends prescribing all drugs by their generic names. Average duration of antimicrobial treatment was higher than recommended average duration suggested by ICMR. ICMR recommends use of 2nd generation cephalosporins but in the present study 3rd generation cephalosporins were used. For rational use of antimicrobials and to reduce their cost it is recommended to prescribe antimicrobials by their generic names and reduce treatment duration by referring to ICMR Guidelines and NLEMI

    Analysing adverse drug reaction patterns observed during district residency program: a prospective observational study

    Background: Adverse drug reactions (ADRs) pose a substantial cost to global healthcare systems. The heterogeneous patient demographics and healthcare environments of district residency programmes (DRPs) provide special possibilities for detecting trends of ADRs. In order recognize recurring trends and related variables, this study will examine ADR incidents that occur during DRPs. Methods: A prospective observational research with forty-three patients was carried out. Standardized reporting forms were used to gather data on ADRs, and descriptive statistical techniques were used to analyse the results. We evaluated medication information, patient demographics, and ADR features to seek for patterns and potential causes. Results: Preliminary analysis revealed a diverse range of ADRs observed during DRPs, spanning various severity levels and therapeutic classes. Common ADRs included gastrointestinal disturbances, allergic reactions, and central nervous system effects. Factors such as patient age, polypharmacy, and comorbidities emerged as potential predictors of ADR occurrence. Conclusions: The panorama of ADRs seen during DRPs is clarified by this study, underscoring the significance of careful monitoring, and reporting mechanisms in these initiatives. Gaining insight into ADR trends and related variables can help in improving patient safety, simplifying drug management plans, and directing future educational initiatives for medical professionals

    Assessment of knowledge, attitude, and practice of pharmacovigilance amongst undergraduate students at a tertiary care teaching hospital: a questionnaire-based study

    Background: Pharmacovigilance ensures patient safety and rational medication use. Spontaneous reporting by healthcare professionals is crucial, but underreporting is a major limitation. Education and training are vital to improve Adverse Drug Reaction (ADR) reporting. Inculcating pharmacovigilance into undergraduate programs empowers medical students to report ADRs. Methods: This prospective questionnaire-based study focused on assessing knowledge, attitude, and practice (KAP) regarding pharmacovigilance. A total of 223 medical undergraduate students of which 156 were students in 2nd year and 67 in 3rd final years who gave their consent were included in this study. Participants completed a pre-questionnaire before undergoing a training session on pharmacovigilance related to KAP. Following the training session, a post-questionnaire was given. Pre- and post-test questionnaires were compared and analysed using an appropriate statistical test. Results: The study showed that knowledge and attitude scores among second and third-year medical students improved after the training program. Following training, the mean knowledge score for second-year students significantly increased from 5.56 to 8.6, while their mean attitude score increased from 5.9 to 7.24. Similarly, for third-final-year students, the mean knowledge score improved from 6.62 to 9.01, and the mean attitude score increased from 6.69 to 7.52. The results clearly showed that the training program had a positive impact on the knowledge and attitude of both student groups. The mean practice scores for second-year and third-final-year students were 0.76 and 1.34, respectively. Conclusions: The study showed significant improvement in knowledge and attitude after the educational intervention, indicating a positive impact of the training program

    Subclinical hypothyroidism and plasma levels of commonly prescribed anti-seizure medications

    Background: Epilepsy remains the most common neurological problem worldwide. Most commonly prescribed antiseizure medications (ASMs) like phenytoin, sodium valproate, and carbamazepine show high inter-individual variability in bioavailability due to genetic and epigenetic factors resulting in either therapeutic failure or toxicity. In our study, we aim to determine the association between subclinical hypothyroidism and the plasma levels of these commonly prescribed ASMs, leading to either therapeutic failure or adverse drug reactions. Methods: We collected demographic data, details about the antiepileptic medication, and plasma levels of ASMs (phenytoin, carbamazepine, and sodium valproate) from patients on antiseizure medications who came for routine therapeutic drug monitoring (TDM). High-performance liquid chromatography (HPLC) estimated plasma levels of the antiseizure medications as per the TDM laboratory standard operating procedure (SOP) and thyroid function test (TSH) by a standard enzyme-linked immune-sorbent assay (ELISA) kit. Results: Of the 158 patients who had TDM, 75.31% were taking sodium valproate, 15.18% were on phenytoin, and 9.49% were on carbamazepine monotherapy. In our study, we found that 12 patients (7.59%) had TSH levels between 4 to 10 mIU/l, indicating subclinical hypothyroidism (SCH). The plasma drug levels of all 12 patients who had SCH were found to be below the therapeutic range. Conclusions: This study has established a significant association between ASMs, particularly sodium valproate, and thyroid dysfunction, specifically SCH, in patients with epilepsy. Our findings suggest that prolonged ASM therapy can lead to thyroid disturbances, warranting careful monitoring of thyroid function in these patients.

    Drug prescription pattern in asthma in a tertiary care hospital in South India

    Background: Asthma, a chronic inflammatory disorder of the airways, affecting millions of individuals is a global health concern. It necessitates long-term management for optimal outcomes. impacting therapy costs and patient’s compliance. This study was done to analyse current prescribing trends in the management of asthma patients. This study examines current asthma treatment trends to enhance patient care and disease outcomes. Methods: This is an Observational cross-sectional study conducted at Government T. D. medical college, Alappuzha between October 2019 to September 2022. All patients clinically diagnosed with or a previously known case of asthma between 18-60 years of both sex attending the outpatient department of Pulmonary Medicine who gave consent for study were included. Data was collected by interviewing and from the patients OP card using a prestructured proforma. Data collected included patient details, allergy and smoking history, comorbid conditions, drugs prescribed for asthma along with dose, route and frequency of administration, concomitant medications, diagnostic tests if any. Results: Out of 200 asthma cases, 119 (59.5%) were females. The mean age was 43.95±10.62 years. 21 patients (10.5%) were smokers. 57 (28.5%) patients had an allergic predisposition. Hypertension n=48 (24%) was the most common comorbidity among the patients. A mean of 3.05±1.52 drugs per prescription. Out of 609 drugs prescribed, 450 (73.89%) drugs were from the National list of essential medicines (NLEM) and 366 (60.09%) drugs were from the WHO essential list. A combination of LABA with ICS was the most common drug given in 121 (60.5%) patients followed by Glucocorticoids 101 (50.5%) and PDE-inhibitor-Deriphylline 74 (37%) patients. Antibiotics was prescribed in 43 (21.5%) patients. 164 (82%) patients received a combination therapy. Out of the 609 drugs prescribed, 334 (54.8%) were oral followed by inhalational route 264 (43.3%). Conclusion: Study demonstrates the variability of drug utilization in asthma patients. The rising significance of PPMS (Prescription pattern monitoring studies) stems from heightened marketing of new medications, shifts in prescribing patterns and drug consumption, escalating worries regarding delayed adverse reactions, drug costs, and prescription volumes
