Rothmund-Thomson syndrome: Unpacking a rare diagnosis
Rothmund-Thomson syndrome (RTS), also known as congenital poikiloderma, is a genodermatosis that appears in infancy and is characterized by poikilodermatous changes in the skin. It is a very rare and complex genetic disorder that can present with a wide range of symptoms and affect multiple systems in the body. The syndrome is inherited in an autosomal recessive manner. A 21-year-old female patient presented to our rheumatology outpatient clinic with swelling of the hands, sclerodactyly, and stiffness of the fingers with preliminary diagnosis of systemic sclerosis. She has had deformities in her feet and hands since the age of one year. Physical examination revealed poikiloderma and extremity deformities. She also had neutropenia in her bloodstream. She was diagnosed with Rothmund Thomson syndrome after genetic test results, prescribed colchicine and followed up regularly every three months. After two years of follow-up, she was diagnosed with ovarian cancer. © 2024, Amaltea Medical Publishing House. All rights reserved
Evaluation of Hematological, Biochemical Parameters and CRP and PCT Levels in Dogs with Leishmania Infantum Detected by ELISA
Aim: Leishmaniasis, a zoonotic disease caused by Leishmania spp., follows a symptomatic or asymptomatic course in dogs due to the influence of various factors. Although hematological and serum biochemical parameters have a limited role in the diagnosis of leishmaniasis, they can be used to evaluate the clinical condition of the animal, the severity of the lesions and the prognosis of the disease. Procalcitonin (PCT), as a disease biomarker, is released by monocytes, neutrophils, liver, spleen, kidneys and lungs during sepsis. Serum C-reactive protein (CRP) increases significantly and rapidly in pathological conditions. This study aimed to examine PCT and CRP levels in dogs with Leishmaniasis. Materials and Methods: The leishmaniasis group of the study consisted of 9 dogs that tested positive for the disease using the ELISA method, among 157 dogs showing leishmaniasis symptoms. The healthy group consisted of seven dogs living in the same area, which were determined to be healthy through clinical examination, had normal hemogram and routine biochemical parameters, and tested negative in the ELISA test. Results: : The results showed that RBC, Hgb, HCT, MCV and MCHC values differed significantly between groups. The positive group had higher total protein and globulin levels, while the negative group had lower albumin, A/G ratio, sodium, and chloride values, biochemical measurements revealed. These differences between the groups were statistically significant, however serum PCT and CRP results were not different between the groups. Conclusion: Further research is required before PCT and CRP can be used as a clinical biomarker in dogs with leishmaniasis
Bee Bread as a Functional Product: Phenolic Compounds, Amino Acid, Sugar, and Organic Acid Profiles
Bee bread (perga) is a natural bee product formed by the fermentation of the pollen collected by bees via lactic acid bacteria and yeasts. This study aims to determine the bioactive compounds, amino acid, sugar, and organic acid profile of bee bread samples collected from the Ardahan province of Turkiye. The highest total phenolic, total flavonoid, and DPPH values in bee bread samples were determined as 18.35 mg GAE/g, 2.82 mg QE/g, and 3.90 mg TEAC/g, respectively. Among phenolic compounds, gallic acid had the highest value at 39.97 mu/g. While all essential amino acids except tryptophan were detected in the samples, aspartic acid was the most dominant, followed by pyrroline and glutamic acid. Among sugars, fructose was seen at the highest level. Succinic acid, among organic acids, had the highest amount at 73.63 mg/g. Finally, all the data were subjected to a principal components analysis (PCA). Bee bread samples were grouped according to the analysis results of the districts they were collected from. This study provides information about the bioactive components and some chemical properties of bee bread, a natural product that has been the subject of recent research. It also contains essential data for future functional food production
Evaluation of Patients with COVID-19 Followed Up in Intensive Care Units in the Second Year of the Pandemic: A Multicenter Point Prevalence Study
OBJECTIVE: A 1-day point prevalence study was planned to obtain country data by determining the clinical characteristics, follow-up and treatment methods of coronavirus disease 2019 (COVID-19) cases that required intensive care unit (ICU) treatment in the second year of the pandemic. MATERIAL AND METHODS: All patients who were hospitalized in the ICUs due to COVID-19 between March 11, 2022, 08.00 am, and March 12, 2022, 08.00 am, were included in the study. Demographic characteristics, intensive care and laboratory data, radiological characteristics, and follow-up results of the patients were recorded. RESULTS: A total of 811 patients from 59 centers were included in the study, 59% of the cases were male, and the mean age was 74 ± 14 years. At least one comorbid disease was present in 94% of the cases, and hypertension was the most common. When ICU weight scores were examined, Acute Physiology and Chronic Health Evaluation-II: 19 (15-27) and Sequential Organ Failure Assessment: 7 (4-10) were seen. Sepsis was present in 37% (n = 298) of cases. PaO2 /FiO2 ratios of the patients were 190 the highest and 150 the lowest and 51% of the cases were followed via invasive mechanical ventilation. On the study day, 73% bilateral involvement was seen on chest x-ray, and ground-glass opacities (52%) were the most common on chest tomography. There was growth in culture in 40% (n = 318) of the cases, and the most common growth was in the tracheal aspirate (42%). CONCLUSION: The clinical course of COVID-19 is variable, and ICU follow-up was required due to advanced age, comorbidity, presence of respiratory symptoms, and widespread radiological involvement. The need for respiratory support and the presence of secondary infection are important issues to be considered in the follow-up. Despite the end of the second year of the pandemic and vaccination, the high severity of the disease as well as the need for follow-up in ICUs has shown that COVID-19 is an important health problem. © Author(s)
How Useful are Current Chatbots Regarding Urology Patient Information? Comparison of the Ten Most Popular Chatbots' Responses About Female Urinary Incontinence
This research evaluates the readability and quality of patient information material about female urinary incontinence (fUI) in ten popular artificial intelligence (AI) supported chatbots. We used the most recent versions of 10 widely-used chatbots, including OpenAI's GPT-4, Claude-3 Sonnet, Grok 1.5, Mistral Large 2, Google Palm 2, Meta's Llama 3, HuggingChat v0.8.4, Microsoft's Copilot, Gemini Advanced, and Perplexity. Prompts were created to generate texts about UI, stress type UI, urge type UI, and mix type UI. The modified Ensuring Quality Information for Patients (EQIP) technique and QUEST (Quality Evaluating Scoring Tool) were used to assess the quality, and the average of 8 well-known readability formulas, which is Average Reading Level Consensus (ARLC), were used to evaluate readability. When comparing the average scores, there were significant differences in the mean mQEIP and QUEST scores across ten chatbots (p = 0.049 and p = 0.018). Gemini received the greatest mean scores for mEQIP and QUEST, whereas Grok had the lowest values. The chatbots exhibited significant differences in mean ARLC, word count, and sentence count (p = 0.047, p = 0.001, and p = 0.001, respectively). For readability, Grok is the easiest to read, while Mistral is highly complex to understand. AI-supported chatbot technology needs to be improved in terms of readability and quality of patient information regarding female UI
David Hume’da Kendilik İnşasında Öteki’nin Yeri
Bu çalışma, Aydınlanma dönemi filozofları arasında önemli bir yere sahip olan David Hume'un kendilik inşasına dair ortaya koyduğu görüşleri incelemeyi amaçlamaktadır. Filozofun temel amaçlarından birisi, ampirist geleneğe uygun olarak, insan doğasını anlamaya ve açıklamaya çalışmaktır. Diğer bir deyişle Hume, insan doğasını gözlem ve deneyimlere dayalı bir şekilde izah etmeyi amaçlamaktadır. Bu amaç doğrultusunda düşünür, öncelikle insan zihninin temel çalışma prensiplerini ele alır. Ardından bu prensipler doğrultusunda bir varlık olarak insanın duygu durumlarına dair genel bir yaklaşım ortaya koyar. Bu yaklaşıma göre duygular, nesne ve nedene ihtiyaç duyar. Bu nesne benlik; neden ise zihnin her türlü niteliğidir. İnsan zihni, deneyimlerin bir ürünü olarak şekillenir ve bu deneyimler, insanın kendiliğini oluşturan temel taşlardır. Hume'a göre, insanın duygusal yaşamı da bu deneyimlere dayalıdır. Duygusal deneyimler, nesnelere ve nedenlere bağlıdır, yani insanın iç dünyası ve kimliği dış etkenlere bağlı olarak gelişir. Öncelikle insanın kendisine ait duygular, sevinmesi veyahut üzülmesine sebep olur. Akabinde ise insanın bunları gören bir ötekine ihtiyacı olacaktır. Bunun nedeni de insanın toplumsal bir varlık olmasıdır ve kendilik inşası da ancak öteki'nin varlığıyla anlamlı hale gelecektir. Nitekim insan, eylemlerinde sosyal çevresini göz önünde bulundurur. Diğer bir deyişle kendiliğin inşasında insanın sosyal çevresini oluşturan öteki, güçlü bir zemine sahiptir. İnsanın duygularına ötekinin duyguları eşlik etmediğinde bu duygular da etki gücünü kaybedeceklerdir. Dolayısıyla insanın öteki’lere ihtiyacı vardır. Buradan hareketle çalışmada Hume'un düşünce dünyasında duygular ve duygulara bağlı olarak inşa edilen kendilikte ötekinin yeri ve önemi incelenecektir. Özellikle sevgi ve gurur duyguları ile bunların karşıtları üzerinden konu ele alınacaktır. Zira Hume, sevgi ve gurur gibi olumlu iki duygu ve bunların karşıtları olan nefret ve kendini küçük görme duygularının, insanın kendiliğini belirlediğini ve diğer insanlarla olan ilişkisini şekillendirdiğini düşünür. Bir kişi tek başına sevgi veya nefret duygusunu hissedemez; aynı şekilde bir şeyden gururlanmak veya kendini küçük görmek de mümkün değildir. İnsanın sahip olduğu şeylerin yanı sıra, başkalarının bu şeylere yönelik düşünceleri de önemlidir. Zira Hume’a kadar olumsuzlanan gurur gibi bir duyguya olumlu bir anlam yüklenerek yeniden bir anlam inşasına girişilmesi incelemeye değer bir husustur. Şayet başkalarının düşünceleri bu denli önemliyse gurur, benliği olumlayıcı bir etkiye de sahiptir. Buradan hareketle bu çalışmamızda söz konusu bu olumlayıcı etkinin kişinin erdemli bir benlik tesis etmesindeki yerini de belirlemeye çalışacağız. Diğer bir deyişle gurura yüklenen olumsuz düşünceler bir kenara bırakılarak, bu duygunun erdemli bir ben üretmedeki yeri ve erdeme teşvik etmedeki gücü incelenecektir. Diğer yandan insanı erdemli olmaya sevk edecek duyguların başında sevgi ve nefret gelir. Nitekim insan ötekiler tarafından sevilmek ve benimsenmek ister; bu noktada duyguların da birbiriyle olan bağı açığa çıkmaktadır. Öyle ki bir tarafta gurur, erdemli olmanın aracı olmakta, diğer tarafta insan sevgi duygusu için gururdan faydalanmaktadır. İlk bakışta paradoks gibi görünen bu durumun erdem üretmesi ve bunun kendilikle olan bağı çalışmamızın temel meselesidir. Söz konusu mesele, Humecu bir yaklaşımda duyguların erdemli bir kişilik ve kendilik inşasındaki rolünü ele alarak, bunun diğer insanlarla(öteki) olan ilişkisi üzerinden irdelenecektir
Optimization of Ultrasound Treatment for Watermelon Vinegar Using Response Surface Methodology: Antidiabetic-Antihypertensive Effects, Bioactive Compounds, and Minerals
Watermelon vinegar is a traditional fermented product with antioxidant activity. This study aimed to investigate the antihypertensive and antidiabetic properties of watermelon vinegar treated through ultrasound using the RSM method. We also evaluated the antioxidant activity (CUPRAC and DPPH), bioactive content (total phenolics and total flavonoids), mineral composition, phenolic compounds, alpha-glucosidase inhibition %, ACE inhibition %, of optimized, and alpha-amylase inhibition % during 24 months of storage of optimized watermelon vinegar. Optimized antidiabetic and antihypertensive activity was achieved at 6.7 min and 69% amplitude. The optimization of gallic acid was the dominant phenolic in the optimized ultrasound-treated watermelon vinegar (UT-WV) and showed a significant decrease during the 24 months of storage. The lycopene content of the UT-WV concentrate was 8.36 mg/100 mL, 8.30 mg/100 mL, 7.66 mg/100 mL, and 7.35 mg/100 mL after 0, 6, 2, and 24 months of storage, respectively. The levels of ACE inhibitory activity, alpha-glucosidase inhibitory activity, and alpha-amylase inhibitory activity decreased significantly (p < 0.05) after 24 months of storage. K, with values of 201.03 +/- 28.31, was the main mineral in the UT-WV. Therefore, the bioactive components and the antidiabetic and antihypertensive properties of the UT-WV produced by conventional fermentation were necessary. Therefore, further experimental studies are necessary for a better understanding of the possible and potential health effects of watermelon vinegar
Evaluation of bread wheat (Triticum aestivum L.) cultivars improved before and after millennium for grain yield and quality by genotype by environment interactions using gge biplot method
Fen Bilimleri Enstitüsü, Tarla Bitkileri Ana Bilim DalıBu araştırma, Trakya Bölgesi'nde 2000 yılı öncesi ve sonrası tescilli veya üretim izinli yirmi beş ekmeklik buğday genotipi ile üç yıl süresince ve üç lokasyonda olmak üzere toplam dokuz çevrede tesadüf blokları deneme desenine göre 3 tekrarlamalı olarak yürütülmüştür. Araştırma ile tane verimi ve bitki boyu, başak uzunluğu, başakta başakçık sayısı, başakta tane sayısı, başakta tane ağırlığı, hasat indeksi gibi verim komponentleri ile bin tane ağırlığı, hektolitre ağırlığı, protein oranı, gluten oranı, gluten indeksi, sedimantasyon değeri gibi kalite parametreleri için GGE biplot yöntemiyle değerlendirilmelerin yapılması amaçlanmıştır. GGE Biplot yöntemi ile beraber, stabilite analizlerinden regresyon katsayısı, regresyon hattından sapmaların varyansı, varyasyon katsayısı, ekovalans stabilite indeksi, belirtme katsayısı, stabilite varyansı ve geometrik adaptabilite indeksi gibi parametrik stabilite ile S_i^((1-4)) ve ?NP?^((1-4) ) gibi parametrik olmayan stabilite analizlerine ilişkin değerlendirmeler de çalışmada yer almıştır. Milenyum öncesi tescilli veya üretim izinli çeşitlere karşılaştırıldığında, sonrasındakilerin daha yüksek tane verimi ile daha düşük bitki boyu ve bin tane ağırlığı ortalamalarına sahip olduğu dikkat çekmektedir. Özellikle birleştirilmiş çevre ortalamalarına göre en yüksek tane verimi Bora çeşidinde elde edilmiş, bu çeşidi sırasıyla Rumeli, Sarı Mustafa, Selimiye, Tekirdağ çeşitleri takip etmiştir. Bu çeşitlerin stabilitelerinin düşük ve çevre seçiciliğinin olduğu görülmüştür. Bora ile Rumeli çeşitlerinin iyi çevre koşullarına, Selimiye ve Tekirdağ çeşitlerinin ise kötü çevre koşullarına adaptasyonunun yüksek olduğu anlaşılmıştır. Hem üç lokasyonda ayrı ayrı hem de birleştirilmiş protein oranı sonuçlarına göre ise Enargo, Bezostaja-1, Rumeli, Murat-1, Saraybosna, Delebrad-2, Flamura-80, Esperia çeşitleri en yüksek performansı göstermiş ve bu sekiz çeşit içinde Bezostaja-1, Delebrad-2 çeşitlerin stabilitesi yüksek, diğer altı çeşidin ise stabiliteleri düşük bulunmuştur.The study was carried out with twenty-five bread wheat genotypes registered or licensed for production before and after 2000 in the Thrace Region in a total of nine environments, including three years and three locations to the randomized block trial design with 3 replications. With the research, grain yield and yield components such as plant height, spike length, number of spikelets per spike, number of grains per spike, grain weight per spike, harvest index, and quality parameters such as thousand grain weight, test weight, protein ratio, gluten ratio, gluten index, sedimentation value. It is aimed to make evaluations using the GGE biplot method. In addition, evaluations on parametric stability such as coefficient, stability variance and geometric adaptability index with the GGE Biplot method, regression coefficient, variance of deviations from the regression line, coefficient of, echovalence stability index, etc. as well as S_i^((1-4)) and ?NP?^((1-4) ) non-parametric stability analyzes were also included in the study. When compared to the registered or production licensed varieties before the millennium, it is noteworthy that the later ones have higher grain yield and lower plant height and thousand grain weight averages. Especially according to the combined environmental averages, the highest grain yield was obtained in Bora variety, followed by Rumeli, Sarı Mustafa, Selimiye, Tekirdağ varieties, respectively. It has been observed that these varieties have low stability and environmental selectivity. It has been understood that Bora and Rumeli varieties have high adaptation to good environmental conditions, while Selimiye and Tekirdağ varieties have high adaptation to bad environmental conditions. According to the protein ratio results, Enargo, Bezostaja-1, Rumeli, Murat-1, Sarajevo, Delebrad-2, Flamura-80, Esperia varieties showed the highest performance and among these eight varieties, Bezostaja-1, The stability of Delebrad-2 varieties was found to be high, while the stability of the other six varieties was found to be low
Impact of Thermosonication Treatment on Parsley Juice: Particle Swarm Algorithm (PSO), Multiple Linear Regression (MLR), and Response Surface Methodology (RSM)
Thermosonication (TS), also known as ultrasonic-assisted heat treatment, is gaining attention in liquid product processing due to its ability to improve quality parameters and can serve as an alternative to thermal treatments. The parsley juice (TS-PJ) was subjected to thermosonication treatment (frequency: 26 kHz; power: 200 W; amplitude 60, 70, 80, 90, and 100%; temperature: 40, 45, 50, 55, and 60 degrees C; time: 4, 6, 8, 10, and 12 min) and was compared with untreated control parsley juice (C-PJ) and pasteurized treated (P-PJ) (85 degrees C/2 min) parsley juice samples. The objectives of the research work were to determine the effect of thermosonication on the quality attributes such as total chlorophyll and ascorbic acid of parsley juice using particle swarm algorithm (PSO), multiple linear regression (MLR), and response surface methodology (RSM). Thermosonication enhanced the bioactive compounds of parsley juice. The results showed that 15 phenolic compounds were detected in the samples. There was a significant (p < 0.05) increase in gallic acid contents in ultrasound-treated TS-PJ. There was no significant difference in total chlorophyll and ascorbic acid content between C-PJ and TS-PJ samples. Na and K from macro minerals and Fe and Zn from micro minerals were high in PJ samples. While K contents were increased, P contents were lower in the TS-PJ sample. RSM modeling provided superior prediction compared to MLR. PSO, on the other hand, made good predictions intuitively. A Systematic Review of IoT Technology and Applications in Animals
Precision Livestock Farming (PLF) is a mechanism that manages a production system. This mechanism includes mathematical models and controllable inputs that can predict inputs with processes and results that can be monitored periodically. These parameters of PLF systems can improve resource use efficiency and reduce cultivation costs. Many situations, such as the behaviour of animals on farms, their nutrition, estrus cycles, and epidemics, can be monitored with wearable devices containing various sensors. However, real-time monitoring of the data collected by these devices is possible with Internet of Things (IoT) technology. IoT is a multi-layered network that enables sensors within the system to communicate with each other and implement certain decisions when necessary. Sensors and IoT devices extract information from the raw data they collect from the environment, which is then shared with other objects, devices, or servers via the internet. The real-time data collection, processing, and analysis provided by IoT enables improvements in the management of animal farms.
