    Rare Event of Intraoral Kinking of Endotracheal Tube Upon Prone Positioning in a Patient Undergoing Posterior Fossa Surgery

    Prone position is commonly employed for many surgical procedures including neurosurgeries, spine surgeries, urological procedures, and in refractory ARDS patients. An endotracheal tube is a conduit to provide a secured airway to the patient. But in case of any equipment failure, it can cause catastrophic events. Intra-oral kinking is a rarer event. It can pose a major troubleshooting standstill owing to the inaccessible view of the kinked portion and the rarity of its occurrence to cross the clinician’s mind in the emergency scenario. We thus report this as the first such case of intraoral kinking against the soft palate in the prone position

    The fundamental need of human beings is safe water, but urban development is generating more polluted water creating the scarcity for drinkable water. Wastewater plants are used reduce the pollutants to less than maximum permissible limits to prevent the threat to the environment and human health. Henceforth, maintaining the waste water plant’s water quality standards before permitting into the water cycle. Subsequently, statistical prediction and evaluation are critical for determining the performance and improvement of wastewater treatment plants. The impact of multiple intrinsic and extrinsic factors on the characteristics of the Vidyaranyapuram Wastewater Treatment Plant (WWTP) in Mysuru was investigated in this report. Hence in the present study we have analyzed the in-fluent and treated effluent parameters from two years (January 2018 -December 2019), the plant\u27s output was assessed using descriptive and statistical analysis of the quantity and quality data from both raw waste waters in-fluent and treated effluent. On analysis of parameters of wastewater treatment plant on monthly basis from January 2018- December 2019, the inflow rate was found to be reliant on precipitation rate and varied according to the season. The inflow temperature was found to be high in March and April than compared to other seasons of the study period. PH of the in-fluent was low whereas after treatment, the pH was reduced. The dependable parameters mainly Biological Oxygen Demand (BOD), Chemical Oxygen Demand (COD), Total Soluble Solids (TSS), levels were well treated and optimized to the normal effluent eligible levels. BOD, COD, and TSS removal efficiencies is estimated with fair precision using multiple regression analysis (R2 = 0.919, 0.847, and 0.977, respectively).The relationship between the residuals and the model predicted values of Biological Oxygen Demand removal (BODr), Chemical Oxygen Demand removal (CODr), and Total Suspended Solids removal (TSSr) was also used to assess the model\u27s fit to the results

    Assessment of knowledge and intake of artificial sweeteners among type II diabetic individuals and perspectives of registered dieticians in India

    Background: The beneficial effects of artificial sweeteners (AS) on the prevention of non-communicable diseases have been noticed with various ambiguity outcomes. The knowledge of AS among consumers and healthcare providers can facilitate in making informed decisions about its usage. Therefore, this study, aimed to assess the knowledge, attitude and practice and usage of AS among type II diabetic patients and the conception of registered dieticians on usage of AS, as a sugar substitute. Study design: The study was a cross-sectional, descriptive study. Methods: The data on AS consumption, discomforts, and health consequences of chronic consumption from type-II diabetics (n = 51) and the opinion of dieticians (n = 53) on AS recommendations were gathered using a pretested questionnaire online through email via docs.google.com. The association or correlation between AS consumption, discomforts, and awareness was analysed by chi-square and Spearman correlation tests. Results: The results indicated that study subjects consumed AS in the form of tabletop sweeteners (86.3%), sweets (35.3%), and beverages (31.4%). Saccharin (29.5%), sucralose (27.3%), and aspartame (27.3%) were the major tabletop sweeteners consumed. 77% of subjects consumed AS from 1-3 years, while others from 4-5 years. 59% of participants consumed AS without consultation, and only 5.9% expressed discomfort after consumption. However, a significant association was noticed between discomfort and beverage consumption. 58.8% were unaware of the health consequences, and KAP analysis showed that knowledge was negatively associated with practice and attitude was positively associated with it. The survey among dieticians revealed that 73.6% were not recommending AS, mainly due to less reliable information, particularly regarding safety issues, and ambiguity on the use of AS in dietetic practice. While others consumed AS over natural sugars in glycemic control, weight management, and lifestyle factors. Conclusion: Majority of diabetic patients did not know the side effects of chronic AS consumption, while dieticians had divergent views on AS due to insufficient research regarding the safety and side effects of chronic AS consumption. Therefore, more precise studies are required to understand the consequences of chronic consumption of AS on health benefits

    Lack of independent biomarkers is very much evident in NAFLD. Early detection of NAFLD is difficult due to the absence of specific diagnostic and prognostic markers and clinical symptoms. We retrospectively collected the information of patients hospitalised with NAFLD diagnosis and metabolic syndrome during 2019-2020 using the tertiary care hospital inpatient sample database and evaluated the changes in their serum homocysteine levels. We found that 59.063% of NAFLD in the male population and 41.667% of NAFLD in the female population had increased serum homocysteine. This shows that elevated serum homocysteine can act as a potential biomarker for NAFLD

    Heart disease is one of the major non-communicable disease and leading cause of mortality in the world. According to WHO heart disease is taking about nearly 17.9 million lives of people each year. Its mortality forecasts indicate a rise in global annual deaths to 20.5 million in 2020 and as high as 24.2 million by 2030. Risk factors are one of the most powerful predictors of heart disease. The study includes modified and non- modified risk factors that contribute to the disease such as Age, Gender, Family history, Hypertension, Diabetics, Obesity, Blood Pressure, Smoking, Alcohol intake, Exercise and Heart rate. Machine learning is one of the most useful techniques that can help researchers, entrepreneurs, and individuals for extracting valuable information from sets of data. The objective of this study is to highlight the utility and application of machine learning techniques for the prediction of heart disease to facilitate experts in the healthcare domain. A total of 336 patients were examined and their personal and medical data were collected in JSS hospital. This prospective study was consisting of 55% patients are free from the heart disease and 45% have heart disease. From the result, it has been determined that males are more likely to develop the heart diseases than females and very common in elderly persons. The accuracy of the Naïve Bayes model is found to be 94%, Obesity plays a vital role in getting the disease followed by hypertension, alcohol intake, smoking, exercise and age has more impact on developing the heart disease

    A Rare Case of Deep Vein Thrombosis Secondary to Protein S Deficiency and Antithrombin III Deficiency with MTHFR (C677T) Heterozygous Mutation, and Crohn’s Disease

    Deep vein thrombosis is a condition wherein a thrombus develops in the deep veins of the body, most commonly in the legs, due to increasing tendency of the blood to coagulate. One such reason is the MTHFR gene mutation, wherein an increase in homocysteine levels causes irritation of the blood vessel endothelium, causing a disturbance in Virchow’s triad, hence triggering clot formation. [1] The MTHFR gene is responsible for instructing the body to produce an enzyme called methylene tetrahydrofolate reductase(MTHFR) [2]. MTHFR gene mutations can lead to a wide range of disorders, either as a result of the elevated levels of homocysteine in the blood, or due to folate deficiency. In about 25% of individuals globally, there is a mutation of MTHFR. The MTHFR 677C\u3eT mutation is predominantly seen in the Hispanic ethnicity, whereas the MTHFR 128A\u3eC is predominantly seen in the ethnicity groups belonging to south east Asia. In the Indian population, the MTHFR mutation prevails a low frequency of 2.9%. Diagnosis and treatment are required only in cases where secondary diseases occur as a result of MTHFR polymorphism, or if there is significant family history [4]. The treatment is along the same lines as that of another diagnosed individual without the mutation. In this case, the correlation between a genetic mutation of the MTHFR gene and Protein S and Antithrombin III deficiency, along with Crohn\u27s disease, which resulted in the development of DVT in a middle aged, male individual and its management is discussed

    ABSTRACT BACKGROUND Multiple System Atrophy is a rapidly progressive neurodegenerative disease. Symptoms tend to appear in 50s and advance rapidly over the course of 5-10 years with progressive loss of motor and autonomic function. Some of the common symptoms include frequent falls with or without tremors and b/l akinetic rigidity with involvement of the axial muscles. Postural tremor, severe stridor and cranio cervical dystonia may be seen. Alpha synnuclenopathies are generally characterized by REM behavioral disorders and severe dysautonomia years before the onset of the disease. Individuals with MSA typically do not have sustained improvement in their symptoms with levodopa. Currently, there are no treatments to delay the progressive neurodegeneration in MSA and there is no cure. However symptomatic treatment like drugs to improve orthostatic hypotension or anticholinergics for bladder control or compression stockings to overcome fainting can be adopted.[1] CASE DESCRIPTION A 60 year old male patient with known history of type 2 DM and HTN presented to JSS hospital with history of difficulty in speaking, walking and multiple episodes of falls, giddiness and involuntary movements of the hand and loss of bladder control progressing over a period of two years which was confirmed to be a case of Multiple System Atrophy (MSA-P) with progressive cerebellar degeneration, extrapyramidal and autonomic involvement. CONCLUSION The case report presents a progressive neurodegenerative condition which is a type of Atypical Parkinson with difficult diagnosis in the early stages due to its presenting complaints being very similar to Parkinson itself but progresses more rapidly
