    Embryological basis of polydactyly in 56 years old male patient

    Polydactyly is a common congenital hand deformity. The limb buds are the precursor structures of the limbs. Their formation begins in the 4th week, with the activation of mesenchymal cells in the somatic layer of lateral plate mesoderm. As mesenchyme cells enter the limb region, they secrete factors that induce the overlying ectoderm to form a structure called the apical ectodermal ridge (AER). Polydactyly is the congenital deformities which is seen in 1000 births. To observe and study the various type of polydactyly in relation to the preaxial polydactyly and postaxial polydactyly with other classification. So, thumb polydactyly is a interphangeal synostosis and is classified by the Wassel type for the thumb duplication. Here we report a male patient aged 56 years who presented with h/o fever for routine treatment to medicine OPD, here we observed 6 fingers on both hands attached to thumb with no independent movement at interphangeal joint with no other gross congenital anomaly. No other family history with similar anomaly

    : Tomato yellow leaf curl virus (TYLCV) is a major devastating viral disease, majorly affecting the tomato production globally. The disease is majorly transmitted by the Whitefly. The Begomovirus (TYLCV) having a six major protein coding genes, among them the C1/AC1 is evidently associated with viral replication. Owing to immense role of C1/AC1 gene, the present study is an initial effort to elucidate the factors shaping the codon usage bias and evolutionary pattern of TYLCV-C1/AC1 gene in five major Asian countries. Based on publically available nucleotide sequence data the Codon usage pattern, Evolutionary and Phylogeographic reconstruction was carried out. The study revealed the presence of significant variation between the codon bias indices in all the selected regions. Implying that the codon usage pattern indices (eNC, CAI, RCDI, GRAVY, Aromo) are seriously affected by selection and mutational pressure, taking a supremacy in shaping the codon usage bias of viral gene. Further, the tMRCA age was 1853, 1939, 1855, 1944, 1828 for China, India, Iran, Oman and South Korea, respectively for TYLCV-C1/AC1 gene. The integrated analysis of Codon usage bias, Evolutionary rate and Phylogeography analysis in viruses signifies the positive role of selection and mutational pressure among the selected regions for TYLCV (C1/AC1) gene

    Biofeedback: Can it be used as an Assessment Tool?

    Biofeedback forms an integral part of Complementary and Alternate Medicine (CAM). It acts as a self-regulation technique through which individuals voluntarily learn to control what they believe are involuntary body processes. It records physiological signals using sensors and converts them into meaningful visual and auditory cues that provide feedback about physiological responses through a computer screen. It has been widely used as an intervention tool since the time of its development. The utility and effectiveness of biofeedback are not only restricted to illness but also to enhancing health and well-being. The biofeedback mechanism relies on two primary principles: Psychophysiological Mechanism and Operant Conditioning Mechanism. Applying the same mechanisms, biofeedback can also be used as an assessment tool. It may be used in research studies to assess the efficacy of a particular intervention at various data points and also be used in clinical practice to assess the improvement in the patient, which in turn will be a self-reinforcement for the patient. Thus, research in biofeedback as an assessment tool besides an effective intervention measure is warranted in both clinical studies and pure theoretical research

    Robinson’s cytological grading of breast carcinoma on fine needle aspirates and its correlation with Modified Bloom-Richardson histopathological grading for Breast carcinoma

    Introduction:Grading of breast carcinoma on fine needle aspiration cytology provides useful information about prognosis & also beneficial for selecting patients for neoadjuvant chemotherapy. Aim: To grade the breast carcinoma on FNAC using Robinson grading system and to assess the concordance of cytological grading (CG) with histological grading (HG) using Elston‑Ellis modification of Scarff‑Bloom‑Richardson grading system. Material method:This prospective study was done in Department of pathology, B.J medical college & Hospital Ahemdabad from Augustl 2016 to october 2018. A total of 200 cases of cytologically confirmed breast carcinoma were included in this study and correlated with histopathological findings. Robinson’s cytological grading was done on cytology smears and modified Bloom-Richardson grading was done on histological sections. Result:In the present study, majority (110) were Grade II tumors (55%) followed by 53 Grade I tumors (26.5%) and 37 Grade III tumors (18.5%) by cytological grading.where as majority (107 cases) of the tumors (53.5%) were Grade II, followed by 43 (21.5%) Grade I tumors and 50 (25%) Grade I tumors by Histological grading. The result showed overall 78% concordance of CG with HG, with grade II showing highest degree of concordance (86.36%), which is comparable to previous studies. Conclusion: Cytologic grading method is Simple,non–invasive and comparable with histologic grading system.It provides information about aggressiveness of tumor and is also useful parameter while selecting neo adjuvant chemotherapy in patients of breast carcinoma

    Hollow Bulb Definitive Obturation of a Post-Covid Mucormycosis Maxillectomy Defect – A Case Report.

    The novel Coronavirus, also known as SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2), causes COVID-19.In the wake of COVID-19, incidences of mucormycosis were rising alarmingly, especially rhino-orbital-cerebral mucormycosis.Most cases required surgical intervention followed by the fabrication of obturators.A hollow obturator is the most practical, convenient, and cost-effective solution for prosthodontic rehabilitation. A patient with pain overthe maxillary region (left) reported to the department of Prosthodontics and Crown & Bridge. His past medical history revealed that he tested and recovered from SARS-CoV-2. Clinical, radiographic, and smear tests confirmed rhino-cerebral mucormycosis with post covid pneumonia. Surgical left partial maxillectomy performed 4 months back. His intra-oral examination revealed a large maxillary defect extending from the left-sided alveolar bone, along with a missing base of the maxilla and palatal bone.The case report presents a hollow bulb definitive obturator fabrication for the treatment of acquired maxillary defects after COVID-19-caused mucormycosis

    Contact tracing for COVID-19 in a healthcare institution: Our experience and lessons learned

    During the initial phases of the COVID-19 pandemic contact tracing was used to control spread of the disease. It played a key role in health care institute which continued to work even during lockdown. In this piece of work, we share the lessons learnt from the contact tracing activity done in the health care institution during April to July 2020. The training needs of persons involved in contact tracing, the follow of activities, use of technology, methods to fill the missing gaps were the key lessons learnt. Its documentation supports in setting up contact tracing activity for any emerging infectious disease outbreaks in future

    Modified electroconvulsive therapy as maintenance treatment

    Depression is the most common psychiatric condition that significantly affects the quality of life of individual affected with it and it can present with various signs and symptoms. It usually presents with persistent and pervasive sadness of mood, anhedonia, sleep disturbance, reduced appetite, reduced concentration, multiple somatic symptoms, and suicidal thoughts. In this case report a 61-year-old gentleman presented with sadness of mood, loss of interest, sleep disturbance, multiple body aches, and abdominal pain which varied from 2 to 6 weeks. He had failed to respond to multiple classes of antidepressants, mood stabilizers, and antipsychotics. However, he would respond well to modified electroconvulsive therapy and he maintained well for 8-12 months before he had another episode. Since the patient responded well to modified electroconvulsive therapy and to reduce relapse maintenance-electroconvulsive therapy was considered
