    Diagnóstico diferencial entre doença celíaca e sensibilidade ao glúten não-celíaca: uma revisão

    Gluten consumption has been associated with a spectrum of diseases called “gluten-related disorders”. This review highlights the epidemiological, pathophysiological and diagnostic features of celiac disease and non-celiac gluten sensitivity. Celiac disease results from a complex interaction between genetic factors (specific HLA alelles) and environmental factors (gluten intake); diagnosis relies, in most cases, on serological tests and intestinal biopsy. Non-celiac gluten sensitivity is a non-allergic, nonautoimmune disease of increasing importance. Diagnosis is based on exclusion of associated diseases and confirming that the patient’s symptoms are linked with gluten consumption. While both may have similar and overlapping clinical manifestations, a systematic assessment enables effective differential diagnosis

    Adequação de medicamentos prescritos em pacientes em uso de sonda enteral em um hospital público no sul do Brasil

    Patients receiving enteral nutritional therapy are fed via enteral feeding tubes, which are also routes of medical administration. Nutrients and drugs interact with each other and with the gastrointestinal tract; care is needed to ensure that this therapy is properly performed in order to avoid adverse effects related to enteral nutritional therapy. The professionals involved in prescribing, dispensing and administrating drugs by feeding tubes should have the technical ability and skills that ensure the adequacy of the drug treatment in patients who use this type of therapy. There are few clinical protocols that refer to drug use in solid pharmaceutical form via enteral catheter. This study aimed to verify the inadequacy of the prescription of the solid pharmaceutical form through enteral feeding tube in patients hospitalized in a clinical unit in a public hospital in the south of Brazil. This is an observational, cross-sectional study whose data were analyzed from medical prescriptions of patients using enteral tubes. Among the drugs with pharmaceutical form of oral use prescribed via enteral feeding tubes, 94% (604) were prescribed in a solid dosage form, of which 42% (253) were inadequately prescribed. The high prevalence of inadequately prescribed drugs for tube administration demonstrates the importance of the pharmaceutical activity among the multidisciplinary team who assists patients with enteral nutritional therapy

    Maternal expectations about breastfeeding: a prospective observational study of pre-partum and post-partum evaluation

    Introduction: Breastfeeding is one of the main experiences that occur in the relationship between mother and baby. As stated in the Child Health Manual, breastfeeding is much more than nurturing the child, it is a process that involves deep interaction between mother and child. The view or psychological reading of the pregnant mother about the act of breastfeeding her baby and the socio-cultural image of the mother/woman who breastfeeds impact the sphere of the future personality of the newborn. Objectives: to map the set of intervening factors that interfere in the breastfeeding process, taking into account the expectations conceived by the pregnant woman before breastfeeding, in addition to the analysis of the positive and negative experiences of nursing mothers during breastfeeding in the postpartum period. Methods: an exploratory and descriptive cohort study, using qualitative and quantitative variables resulting from a questionnaire applied to 35 pregnant women during pregnant women and subsequently interviewed with them in postpartum situations. Results: Regarding the age group, the highest concentration of age was between 26 and 33 years old, but there was the participation of women in the extremes of reproductive life; the association between the time of pregnancy and the search for content/guidance on breastfeeding, most participants were between the 20th and 30th week of the pregnancy period, when they sought information on the subject; to the level of education and information on when breastfeeding should be started, among the 19 pregnant women who had higher education, 17 said it should be started during the baby's first hour of life and 02 said it should be after the first hour; among the 15 participants who had completed high school, 14 responded that it should be in the first hour and only 01 responded after the first hour, and 01 participants who reported having completed elementary school, responded that breastfeeding should be started after the first hour of life. of the baby; when asked if they were breastfed and if they had breastfed previously, 04 pregnant women answered that they had not received breast milk and of these, 03 had already breastfed children before the current pregnancy, among the 31 participants who had been breastfed, 27 said they had not yet breastfed and 04 informed that who had already lived the experience with other children. When approached in the puerperium, the most frequent findings were: 14 reported having received professional help; 13 intended to breastfeed exclusively until the 6th month; 15 had already started formula milk; 17 intended to continue breastfeeding after the baby was six months old and 09 were unable to latch on. Conclusion: Breastfeeding is shown to be one of the biggest factors of doubt and insecurity for pregnant women who enter the puerperium in search of humanized and comprehensive care. Thus, the participation of the health professional becomes essential for the success of breastfeeding and the effective dissemination of concrete and assertive knowledge, both prepartum and postpartum

    Impactos da microbiota intestinal na saúde do lactente e da criança em curto e longo prazo

    Background: There is sufficient evidence to support the importance of programming in the perinatal period and early life in the prevention or development of disease throughout life. Methods: Using as main references the lectures in ESPGHAN 2016 we made a literature review in the databases PubMed, Scielo and Google Scholar, the main objective is to show the importance of the impacts of the intestinal microbiota in the health of infants and children in the short and long term. Findings: It is documented from a fetal epigenetic imprinting by bacterial translocation during pregnancy, which provides the offspring one microbiome intrauterine pioneer. The presence of maternal components of the microflora of the fetus in utero is an important fetal immune system development supply leading to the induction of oral tolerance and respiratory allergens, but may also result in an unfavorable epigenetic factor in maternal obesity. Conclusions: It is clear and evident the importante of pediatricians in monitoring the development of the intestinal microbiota of children from intrauterine life. There are several windows of opportunity for the child’s programming intervention to minimize the impact on health in the short and long term

    Transplante de microbiota fecal no tratamento da infecção por Clostridium difficile recorrente—uma revisão

    Clostridium difficile infection (CDI) has become one of the major complications associated with the use of systemic antibiotics. Antimicrobials disrupt the balance of the colonic intestinal flora, predisposing the individual to infections by opportunistic strains, such as an CDI, that can characteristically cause a course of diarrhea and colitis. The standard treatment for these cases is the use of metronidazole or vancomycin for a period of 10 to 14 days. However, it is estimated that this therapy will fail in ∼ 5 to 35% of the patients, who will present a recurrence of CDI. The treatment options for recurrent CDI remain limited, but with promising therapies being constantly researched. The use of stools obtained from a donor to perform fecal microbiota transplantation (FMT) has been gaining space in several health centers, with reported success rates of 80 to 100%. However, there still is a limited experience in this subject, with the need for studies that allow the creation of a specific protocol for FMT

    Mudanças no estilo de vida após primeira consulta em ambulatório de obesidade infantojuvenil

    Objective: To identify the main recommendations for the agreed changes in lifestyle and the adherence of overweight children in the time interval between the first and second outpatient visits. Method: Retrospective cohort. We analyzed the data from the visits of children and adolescents to the outpatient obesity clinic from January 1st 2008 to November 30th 2011. The children were free of genetic disease and did not use controlled medications. The data were collected directly from the standardized forms used in the visits, conducted by a multidisciplinary team. In the first visit, goals related to lifestyle, diet, and physical activity were proposed. When the patients reported at the second visit that they had achieved fully or partly an agreed goal, it was considered as “adherence.” The analysis was performed with PASW Statistics for Windows, Version 18.0 (SPSS Inc, Chicago, IL, USA). The project was approved by the Research and Ethics Committee. Results: A total of 305 subjects, with a mean age of 10 ± 3.8 years were analyzed, 51.1% of whom were female; of the total number of subjects, 261 (85,6%) attended the first and second visits. A total of 401 challenges were negotiated regarding changes in eating habits (adherence of 86.8%), 264 regarding physical activity (adherence of 70.2%), and 218 regarding changes in the lifestyle (adherence of 72.4%). Conclusions: Changes in eating habits were the most recommended and had greater adherence

    The New Face of the Brazilian Journal of Nutrology

    Dear authors and readers, The Brazilian Journal of Nutrology is facing changes on the editorial board to accomplish the international demands for higher indexations of the journal. This is a challenge for the new editorial board that will follow the new trends in science publications preserving the peer review process and keeping up with the high quality of the published manuscripts. It was undoubtedly a great challenge to contribute to the most important scientific journal of Nutrology in South America. Several actions have been planned to speed up the article review process of our journal and make it more attractive to authors. With the constant progress of science and of its outreach channels, the Brazilian Journal of Nutrology needs to be prepared to keep up with these changes and innovations. In this editorial, we would like to thank our peers and colleagues, who have manifested sincere and total support to our work. We are pleased to present Professor Moacir Fernandes de Godoy, who will assume the role of Editor-in-Chief of the Brazilian Journal of Nutrology. In his professional activities, Godoy interacted with over 300 employees in the coauthorship of scientific papers. He also works in medicine as cardiologist and hemodynamicist, received his medical training at the Universidade Federal de São Paulo (Unifesp), state of São Paulo, Brazil, and is lecturer-professor at the Faculdade de Medicina de São José do Rio Preto (FAMERP, in the Portuguese acronym) in São José do Rio Preto, state of São Paulo, Brazil, in the Department of Cardiology and Cardiovascular Surgery. Afterwards, he occupied a prominent position of Deputy Head and Former Deputy Director of Education at FAMERP. He has been serving the scientific community as a professional since 1971. He has experience in medicine, focusing on cardiology, acting on the following subjects: heart rate variability, nonlinear dynamics, cardiovascular surgery, atherosclerosis, oxidative stress, myocardial revascularization, heart failure, and congenital heart disease. We are sure that the future of our Brazilian Journal of Nutrology is brilliant and will continue to merge with the history of Brazilian Nutrology. However, some things never change, and the “good science” and the relevance of the articles continue to depend on traditional scientific aspects, such as the changes in the clinical practice and the generation of new knowledge on the scientific mechanisms, efficacy, and safety of dietary interventions in the context of disease prevention and health benefits. It was a great honor and privilege to have served as the Editor-in-chief of this publication. Finally, but most important, we would like to thank the herculean job done by the previous editors with the editorial board, who worked very hard to keep our journal running well and smoothly up to date

    Children with Feeding Difficulties Present Changes in the Development of Feeding Skills: A Study with a Control Group

    Introduction: Feeding skills (FS) are important to child development, as the delay in their presence could suggest feeding difficulties (FD) symptoms. The aim of the present study was to compare the development of three types of FS (autonomy to eat, posture at meals, and adequate use of cutleries) among children with FD and without FD. Methods: This was a case-control retrospective observational study. The sample consisted of 316 children from case and control groups in accordance with the presence (or not) of FD. The control group was recruited by convenience (verbal approaching at the emergency care unit), and the case group was based on the medical records collected at the reference center. A same-structured questionnaire was used for both groups. Results: Children with FD (63.2%) used baby-bottles in an inadequate way with a higher frequency after 24 months of age. Inadequate posture at meals was observed with higher frequency in children with FD (78.1%). Children without FD (89.1%) had more autonomy to eat. In children > 18 months old, this frequency was higher (90.6%). Conclusion: Children with FD showed changes on the development of FS. Inadequate posture at meals was associated with a 36-fold higher risk of having FD. Not eating alone after the age of 18 months was associated with a 6-fold higher risk of having FD, while not using baby bottles was associated with a lower risk (52%) of FD complaints.     Caffeine ingestion by students at the Brazil faculty of medicine: a prospective observational cross-sectional study

    Introduction: In higher education, the consumption of stimulant substances such as coffee is increasing intensely due to the high demand of concentration and disposition of individuals. This occurs mainly among medical students. Objective: It was to quantify and analyze caffeine intake and its effects on medical students at the Faculty of Medicine of Catanduva, Centro Universitário Padre Albino (UNIFIPA). Methods: This study followed a prospective observational cross-sectional model, following the rules of clinical research of the STROBE. Data collection took place from June 2019, through an objective questionnaire and self-application, which was answered by students from the first to the sixth year of FAMECA who were interested in answering and contributing to the research, which had as objective to quantify and analyze the use of caffeine, in addition to its motivations and side effects on the physiology of these students. This study was analyzed and approved by the Research Ethics Committee according to a substantiated opinion number 5,347,594, and obtaining the Informed Consent Form. For data analysis, a common descriptive analysis was performed, obtaining the values of total N, and statistical percentage (%) for all predictors. The One-Way test (ANOVA) was applied, adopting the α level lower than 0.05, with a statistically significant difference for the 95% CI. Results and Conclusion: The results of this study revealed that a very large number of university students ingest caffeine. Most of the students started consumption in the pre-college course and in college, with the aim of compensating for sleep, because they like the taste of energy drinks and improve academic performance, due to the intense routine due to the high workload requires greater attention, in order to to meet the demand for studies. Most students consume moderately caffeine, which is not harmful to health. However, a minority ingest excessive amounts of substances that contain caffeine and side effects are noted. Among these, the ones that stood out were stomach problems, anxiety and tachycardia. Thus, even representing the minority, it is of fundamental importance to carry out more studies on this topic. With this, it will be possible to provide more information to the population so that it is able to assess the impacts of the use of stimulants in clinical practice, in order to minimize possible adverse effects. Finally, the population will be able to use caffeine properly, according to the recommended dose, avoiding major health problems
