Preoperative Diagnosis of an Esophageal Duplication Cyst by Endoscopic Ultrasound Examination
A 78-year-old woman was referred to our hospital for close examination of an extramural submucosal tumor in the gastroesophageal region, suspected based on an imaging test performed for a chief complaint of epicardial pain while eating. Contrast-enhanced computed tomography revealed a 3 cm sized mass with well-defined margins and a homogeneous interior near the gastroesophageal junction. Endoscopic ultrasonography (EUS) revealed a large (28 mm) unilocular cystic lesion with a heterogeneous hypoechoic internal structure. The cyst wall was layered with a hypoechoic layer that appeared to be muscular and continuous with the external longitudinal muscle of the esophagus. Based on the EUS findings, an esophageal duplication cyst was diagnosed. Cystectomy was performed because the patient was symptomatic. Pathological examination revealed that the specimen was covered with columnar and pseudostratified ciliated epithelium without atypia and that the cyst wall comprised two layers of smooth muscle. No cartilaginous tissue was present, which is consistent with esophageal duplication cysts. Exploring suramin's antiviral effects: targeting Lymphocytic Choriomeningitis virus cell entry and replication/transcription processes
The Arenavirus family comprises of multiple pathogenic human viruses. Currently, there are no licensed vaccines or antiviral agents against arenavirus infections; therefore, there is a pressing need to develop prophylaxis and/or countermeasures. Suramin is an approved antiparasitic drug that also blocks the propagation of multiple viruses. In this study, the antiviral effect of suramin against lymphocytic choriomeningitis virus (LCMV), a prototype of the arenavirus, was examined. Our results showed that suramin targeted at least two replication steps of LCMV, an entry step and a replication step, but not the particle production step. Our in silico study revealed that the target of suramin in replication could be an RNA-dependent RNA polymerase (L), and this observation was consistent with previous reports from other viral families, including coronaviruses and filoviruses. These results imply that suramin has less potential to emerge as a suramin-resistant LCMV due to its dual viral targets.Virology, 609, art. no. 110557; 2025journal articl
Craniocaudal spread patterns of retropharyngeal lymph node metastasis in patients with hypopharyngeal carcinoma
Background: The wide adoption of intensity-modulated radiotherapy (RT) renders an extended neck position unnecessary. In this study, the appropriate craniocaudal border of the retropharyngeal lymph node (RPLN) was assessed based on diagnostic and RT images in patients with hypopharyngeal carcinoma (HPC) for recommendation of appropriate target volume for elective nodal irradiation (ENI). Methods: Two board-certified radiation oncologists evaluated the craniocaudal spread of RPLN metastases (RPLNMs) and the position of the hyoid bone using magnetic resonance images (diagnostic position) and RT-planning computed tomography simulator images (RT position with neck extension). Results: Of the 154 patients in whom 308 sides were assessed, 19 (12.3%) were diagnosed with RPLNMs in 24 (7.8%) sides. The cranial border of the RPLNMs was found above the hard palate on 3 (1.0%) and 1 (0.3%) side, between the hard palate and cranial edge of the C1 body on 10 (3.2%) and 13 (4.2%) sides, and at the C1 body on 11 (3.6%) and 10 (3.2%) sides in the diagnostic and RT positions, respectively. The caudal border of the RPLNMs was observed between the hard palate and caudal edge of the C2 body on 22 (7.1%) and 22 (7.1%) sides, at the C2/3 disk level on 1 (0.3%) and 1 (0.3%) side, and at the C3 body level on 1 (0.3%) and 1 (0.3%) side in the diagnostic and RT positions, respectively. In the 19 patients with RPLNMs, the most common level of the caudal edge of the hyoid bone body was at the C4 body in 10 (52.6%) and C3 body in 8 (42.1%) patients in the diagnostic and RT positions, respectively. Conclusions: RPLNMs above the hard palate and below the C2/3 disk level extremely rarely develop in patients with HPC. The cranial edge level of the hyoid bone varied significantly among treatment positions. Regardless of the treatment position, the cranial and caudal borders of the RPLN for ENI should be defined as the hard palate and cranial edge of C3, respectively.Radiation Oncology, 20(1), art. no. 88; 2025journal articl

Photoredox‐Catalyzed α‐Arylation of Bromotrifluoroacetone with Electron‐Rich (Hetero)Arenes
Photoredox-catalyzed radical addition of bromotrifluoroacetone and (hetero)arenes to carbonyl compounds has been established under mild conditions. The proposed method enables the synthesis of valuable α-aryl trifluoromethyl ketone compounds, including biologically relevant arene derivatives, with good yields and regioselectivities. RUNX1 expression is regulated by a super-enhancer and is a therapeutic target in adult T-cell leukemia/lymphoma
Nagasaki University (長崎大学)博士(医学)Super-enhancers (SEs) play an important role in regulating tumor-specific gene expression. JQ1, a Bromodomain-containing protein 4 (BRD4) inhibitor, exerts antitumor effects by disrupting SE-mediated regulation of gene expression. We investigated the anti-adult T-cell leukemia/lymphoma (ATL) effects of JQ1. JQ1 induced apoptosis and inhibited ATL cell proliferation. JQ1 suppressed RUNX1expression through the disruption of SE-mediated gene regulation. In the previous reports, it was shown that IC50s of AI-10-104 and Ro5-3335, RUNX1 inhibitors were 1-10 µM for lymphoblastic leukemia cell lines carrying RUNX1 mutations. In the present study, we demonstrated that IC50s of AI-10-104 and Ro5-3335 were also 1-10 µM or lower for ATL cell lines. Simultaneously, AI-10-104 suppressed MYC proto-oncogene (c-MYC) expression. RUNX1 is a potential therapeutic target for ATL that promotes c-MYC expression. Improved survival among elderly patients with aggressive adult T-cell leukemia/lymphoma: Impact of mogamulizumab-containing chemotherapy
Nagasaki University (長崎大学)博士(医学)Due to the poor prognosis of adult T-cell leukemia/lymphoma (ATL), new treatments are urgently needed, especially for elderly patients with aggressive ATL. The anti-CCR4 antibody drug mogamulizumab (MOG) has been approved for the treatment of untreated ATL. To analyze the impact of MOG on elderly patients, we conducted a retrospective analysis of patients aged 70 years and older with aggressive ATL diagnosed at our institution between 2015 and 2021. Among 32 patients, including those who received best supportive care, the median survival time (MST) and 2-year overall survival (OS) rate were 14.6 months (range, 0.0–83.7), and 34.7% [95% confidence interval (CI), 18.2–51.9], respectively, which were better than outcomes in our previous study. The MST and 2-year OS for patients treated with MOG-containing chemotherapy were 18.1 months (range, 4.0–83.7) and 45.0% (95%CI, 23.1–64.7), respectively, demonstrating clear improvement. Adverse events observed with MOG-containing treatment, such as myelosuppression and skin rash, were similar to those reported previously. Univariate analysis identified comorbidity as a predictor of poor outcomes, but not intensity of MOG-containing treatment, suggesting a different mechanism of action than that of classical chemotherapy. Potential effect of topical antibiotics administration in the oral cavity on the reduced number of bacteria entering the lower respiratory tract after oral cancer surgery
Nagasaki University (長崎大学)博士(歯学)Background/purpose: One of the causes of ventilator-associated pneumonia (VAP) is aspiration of oropharyngeal fluid containing pathogenic microorganisms into the lower respiratory tract. In this study, we aimed to investigate whether antibiotic ointment applied to the oral cavity can reduce the number of bacteria in the fluid on the cuff of a tracheal cannula. Materials and methods: Tetracycline ointment was applied intraorally once to a patient under endotracheal intubation by postoperative tracheostomy for oral cancer. The tetracycline concentrations in the oropharyngeal fluid and fluid on the cuff of tracheal cannula were determined by bioassay, and the total viable bacterial count was determined by delayed real-time polymerase chain reaction developed by the authors from before to 6 h after application. Results: A total of seven patients were enrolled. Very high antibiotic concentrations were maintained, ranging from 481 μg/ml to 2060 μg/ml in oropharyngeal fluid and from 267 μg/ml to 858 μg/ml in fluid on the cuff from 1 h to 6 h after application. Compared to the pre-application results, the inhibition rates of viable bacteria were 80.0–97.7% for oropharyngeal fluid and 47.6%–91.9% for fluid on the cuff at 1–6 h after application, indicating that antibiotic ointment can inhibit bacteria entering the lower respiratory tract for a long period of time in intubated patients.
