    Effectiveness of Etanercept Biosimilar Initiating for Etanercept-Naive Patients, Using Ultrasound, Clinical, and Biomarker Assessments in Outcomes of Real-World Therapy (ENPORT-NGSK Study): An Interventional, Multicenter, Open-Label, Single-Arm Clinical Trial

    Background/Objectives: This study aimed to investigate the effectiveness of etanercept biosimilar 1 under real-world clinical conditions in patients with rheumatoid arthritis (RA), using not only clinical evaluation but also musculoskeletal ultrasound (MSUS). Methods: This multicenter, interventional, open-label, single-arm clinical trial conducted a 24-week follow-up. Patients with RA with moderate to high disease activity received weekly subcutaneous injections of etanercept biosimilar 1 at 50 mg/dose for 24 weeks. The effectiveness was evaluated with clinical indices and MSUS. Results: Twenty-three patients were evaluated during the study period. The primary endpoint involves a change in the Global OMERACT-EULAR Synovitis Score by MSUS in bilateral second–fifth metacarpophalangeal joints from baseline, demonstrating median (IQR) values of 0 (−4, 1), including 4 (1, 9.8) and 2 (0, 5) at baseline and 24 weeks, respectively. The clinical endpoints exhibited a good treatment response, with 15 (68%) and 18 (86%) patients achieving low disease activity or remission at 12 weeks and 24 weeks, respectively. Additionally, MSUS scores improved at both 12 and 24 weeks compared to baseline. The patients who achieved power doppler remission (total power doppler score = 0) at 24 weeks demonstrated a shorter disease duration and no previous use of biological disease-modifying antirheumatic drugs compared to those with no power doppler remission. Conclusions: Etanercept biosimilar 1 exhibited significant improvements not only in clinical indices but also in MSUS assessment, indicating its effectiveness at the structural level.Journal of Clinical Medicine, 14(5), art. no. 1775; 2025journal articl

    In recent years, Hong Kong has seen a significant rise in migration, driven largely by political upheavals such as the 2014 Umbrella Movement, the 2019-2020 protests, and the implementation of the Hong Kong National Security Law. As a result, many Hong Kong residents have chosen to relocate abroad, with Japan emerging as a popular destination. This paper explores the factors behind this migration trend, with a particular focus on the experiences of Hong Kong migrants in Japan. It examines the life of Mary, a female entrepreneur who opened a restaurant in Hokkaido, to illustrate the broader socio-political dynamics. The paper highlights several key findings: first, political instability has been a primary driver of migration; second, limited Japanese language skills have posed challenges to social integration for migrants; third, the migration of women independently reflects a shift toward greater independence and self-reliance among a new generation of Hong Kong women; fourth, migration plans often involve a strategic division of labor within families, balancing economic rationality with risk management; and finally, the cultural clash between Hong Kongers and their new environment has, in many cases, strengthened their sense of identity.     Investigation on stress distribution and prestress loss model of prestressed anchor cables considering corrosion-induced debonding

    Corrosion can significantly impact the safety and stability of the entire structure by reducing the service life and load-bearing capacity of anchors. This study provides an in-depth examination of the effects of corrosion on prestressed anchor cables, covering the effects on the anchor cables themselves and the bond interface. The force characteristics and load transfer mechanisms within the anchorage structure were explored through a detailed analysis of the three key components: the anchor cable, the grout, and the surrounding rock. The distribution functions of axial force and interfacial shear stress considering the debonding of the anchor-grout interface were derived, and the prestressed corrosion damage model was further developed. Taking the anchoring project on the slopes in Nagasaki as an example, the stress distribution of anchor cables under different surrounding rock conditions was analyzed in depth. The results showed that the relative deformation of the grout and the surrounding rock decreases when the elasticity modulus of the surrounding rock increases, resulting in a reduced axial force in anchor cables and an increased interface shear stress. Thresholds exist for the effect of the total anchor length and radius on prestressing stability. When designing anchor structures in corrosive environments, there is no need to choose excessive anchor length or anchor radius to achieve better cost-effectiveness. In practical underground engineering, the force in anchor cables is transferred to the surrounding rock through the anchoring section, where the length of the anchorage section has a more direct impact on prestress transfer and stability.Rock Mechanics Bulletin, 4(2), art. no. 100189; 2025journal articl

    Real-world effectiveness and lung abnormalities associated with amikacin liposome inhalation suspension

    Background: The guideline recommends the addition of amikacin liposome inhalation suspension (ALIS) for patients with refractory Mycobacterium avium complex pulmonary disease (MAC-PD). However, factors influencing culture conversion and managing ALIS-related adverse events, particularly ALIS-related lung abnormalities (ALIS-RLAs) remain insufficient. Methods: This was a two-center retrospective cohort study involving 87 patients with refractory MAC-PD who received ALIS from August 2021 to September 2023. Refractory MAC-PD was defined as failure to achieve sputum culture conversion after six months or more of guideline-based therapy. On the basis of data obtained from medical records, adverse events were assessed in 76 patients, whereas treatment outcomes were evaluated in 49 patients. Results: The sputum culture conversion rate was 28.6 %. Prognostic factors associated with a negative impact included a body mass index below 18.5 kg/m2 (adjusted odds ratio [aOR], 0.07, 95 % confidence interval [CI], 0.02–0.35) and the presence of cavities (aOR 0.02, 95 % CI 0.002–0.18). Patients achieved culture conversion had a shorter interval from MAC-PD treatment initiation to ALIS administration. The most frequent adverse event was dysphonia, which occurred in 53.9 %. ALIS-RLAs were observed in 82.1 %: 53 patients (79.1 %) had multiple nodular patterns, 18 patients (26.9 %) had organizing pneumonia patterns, one patient (1.5 %) had a diffuse alveolar pattern, and another (1.5 %) had a hypersensitivity pneumonitis pattern. Among patients with ALIS-RLAs, 92.7 % were asymptomatic, and 85.5 % were able to continue ALIS. Conclusion: Initiating ALIS before significant progression is important. Although ALIS-RLAs were observed in 82.1 % of patients, discontinuation of ALIS appeared unnecessary in asymptomatic cases.Respiratory Medicine, 248, art. no. 108408; 2025journal articl

    Comparison of the Use of Immune Checkpoint Inhibitor/Tyrosine Kinase Inhibitor and Tyrosine Kinase Inhibitor Alone for Patients With Low Risk Metastatic Renal Cell Carcinoma

    Background/Aim: Currently, a combination of an immune checkpoint inhibitor (ICI) and a tyrosine kinase inhibitor (TKI) is the most widely used first-line treatment for metastatic renal cell carcinoma (mRCC). However, patients in the IMDC favourable risk group have been reported to have a relatively good prognosis, even when they undergo TKI monotherapy. The aim of this study was to evaluate whether ICI/TKI combination therapy or TKI monotherapy is more effective in patients with favourable IMDC risk. Patients and Methods: We retrospectively reviewed 11 patients with favourable IMDC risk who underwent ICI/TKI combination therapy and 12 who underwent TKI monotherapy as first-line treatment for mRCC at our institution between April 2008 and September 2024 and compared their characteristics and treatment outcomes. The endpoints were overall survival (OS), progression-free survival (PFS) and treatment response, which was assessed using the overall response rate (ORR) and disease control rate (DCR). The safety of the regimens was evaluated using the incidences of grade ≥3 adverse events (AEs) and treatment discontinuation. Results: There was a significant difference between the groups regarding the duration of follow-up (24.4 months for the ICI/TKI group vs. 65.9 months for the TKI group, p=0.01), but no other differences were noted in the characteristics of the patients. The PFS and OS of the groups did not significantly differ following initial treatment. The ORR did not significantly differ, but tended to be better in the ICI/TKI group. The incidence of grade ≥3 AEs and the discontinuation of treatment owing to AEs did not significantly differ but tended to be higher in the TKI group. Conclusion: Based on its superiority with respect to ORR and AE at our Institution, ICI/TKI therapy should be considered whenever possible, even in patients in the IMDC favourable risk group.In Vivo, 39(6), pp.3445-3452; 2025journal articl

    Anti–early antigen Epstein-Barr virus titer and atherosclerosis in relation to vascular endothelial growth factor (VEGF) polymorphism rs3025039 among older Japanese individuals

    Background: Epstein-Barr (EB) virus infection stimulates the production of vascular endothelial growth factor (VEGF), which contributes to the progression of angiogenesis. Angiogenesis plays an important role in the development of atherosclerosis. Since serum anti-early antigen EB virus IgG (EBV EA-IgG) titer is a sign of active EB virus infection, EBV EA-IgG titer could be associated with atherosclerosis. The number of minor (T) alleles in VEGF polymorphism rs3025039 has been reported to be inversely associated with serum VEGF concentration, suggesting that rs3025039 might have a strong influence on the association between EBV EA-IgG titer and atherosclerosis. By focusing on the role of VEGF in the development of atherosclerosis, this study aimed to investigate the association between active EB virus infection and atherosclerosis. Methods: A cross-sectional study of 2,661 older Japanese individuals aged 60–89 years who participated in annual health check-ups during 2017–2019 was conducted. Logistic regression was used to evaluate the association between EBV EA-IgG titer and atherosclerosis in relation to rs3025039 genotype. The influence of rs3025039 (T) allele carrier status on the association between EBV EA-IgG titer and atherosclerosis was also evaluated by using logistic regression. Results: Among rs3025039 CC-homozygotes, with the lowest EBV EA-IgG titer tertile as the reference, the multivariable odds ratio (95% confidence interval) was 1.11 (0.82, 1.50) for the medium tertile and 1.07 (0.78, 1.47) for the high tertile. Among rs3025039 (T) allele carriers, the corresponding values were 1.44 (0.88, 2.36) and 1.88 (1.15, 3.05), respectively. There was a significant interaction between rs3025039 (T) allele carrier status and the association between EBV EA-IgG titer and atherosclerosis (adjusted p = 0.0497). Conclusion: EBV EA-IgG titer was significantly positively associated with atherosclerosis only among participants who are genetically less likely to have progressive angiogenesis. An angiogenesis-related genetic factor was revealed as a determinant of the association between EBV EA-IgG titer and atherosclerosis.     An analysis of the desire to make radiation measurements and to dialogue with experts among the residents of Tomioka town, Fukushima Prefecture: about the implementation of the co-expertise process

    Nagasaki University (長崎大学)博士(医学)The article presents a contribution concerning the desire to make radiation measurements and to dialogue with experts among the residents of Tomioka town close to the Fukushima Daiichi Nuclear Power Plant in Japan at the time of the lifting of the relocation order in 2017. Relying on data from a questionnaire conducted at the same time with approximately 8000 residents aged 20 years or older who lived in Tomioka before the accident, a logistic regression analysis was performed to identify retrospectively the factors having driven these desires. The results shed some light on the motivation of affected people to engage in the co-expertise process implemented in Tomioka by experts from Nagasaki University and local authorities. In essence there are two significant results: (i) the people who have the desire to make radiation measurements are those who are reluctant to eat foods produced in the town and who already had an experience of radiation measurement, (ii) the people who have the desire to dialogue with experts about radiation exposure and health effects are above 60 years old and are reluctant to eat foods produced in the town and to drink tap water.
