Review on the epidemiology of Bovine Rotavirus and its public health significance
A literature review was made to assess the epidemiology, public health importance, diagnostic and control methods of bovine rotavirus. Rotavirus is the genus name under the family Reoviridae which is characterized by segmented genome. The emergence of new serotypes of the virus is related to the segmented nature of the viral genome which allows reassortment during mixed infections. The rotavirus genome consists of 11 double-stranded RNA gene segments encoding 6 nonstructural (NSP1–6) and 6 structural (VP1–4, VP6–VP7) proteins. Rotavirus A is a zoonotic disease and in children less than five years old, human rotavirus is reported to be the most common cause of gastritis. In animals, rotavirus infection usually affects calves within four weeks of age, causing huge economic losses due to death, reduction in weight gain and treatment costs. Bovine rotaviruses are globally distributed and cattle strains have been classified into 12 G types and 11 P types and among them G6, G8 and G10, and P [1], P [5] and P [11] are commonly prevalent bovine strains. However, the presence of 14 G type and 17 P type serotypes from human have reported in different surveillance studies worldwide. Among these, combinations of G1P [8], G2P [4], G3P [8], G4P [8], G9P [8] and G12P [8] are the most common human strains which are responsible for majority of human Rotavirus diseases. The virus is primarily transmitted by fecal-oral route or by direct contact. The excreta from infected animals and humans, excreta contaminated food; water, pasture and air are the potential source of infection for both animal and human rotaviruses. Age, seasonal pattern, strain diversity, poor herd management and housing system, host nutritional and immunological factors are important risk factors associated with rotavirus disease occurrences. The widely used diagnostic methods for detection of rotavirus antibody in human and animals are Enzyme-Linked Immunosorbent Assay (ELISA) and immune-chromatography while the presence of the rotavirus/antigen is identified by Enzyme Immunoassay (EIA). Electron Microscopy (EM), polymerase chain reaction (PCR) and nucleic acid hybridization. Vaccination is the primary strategy to prevent and control of bovine and human rotavirus infections. High level of antibody in pregnant animals is achieved through live attenuated and inactivated vaccines when administered at the late stage of pregnancy. In human, the two currently used vaccines are the RV5 vaccine (USA) and the RV1 vaccine (Belgium) types. </p
Death prognosis factors of chronic heart failure in the elderly in Burkina Faso
Background: Heart failure is the main reason for hospitalization in the cardiology department of Yalgado Ouédraogo University Hospital. The main causes of heart failure are arterial hypertension and myocardial ischemia. Mortality is high during hospitalization and can reach 48%. The follow-up of patients after hospitalization is difficult with many lost to follow-up. The medium and long term prognosis of chronic heart failure is not known in our country.Objective: Identify death prognosis factors of heart failure in elderly subjects aged at least 60 years old. Patients and Method: We conducted a retrospective, cross-sectional study that consisted of systematic inclusion from hospitalization registers of patients of both sexes aged at least 60 years, hospitalized in the cardiology department CHU Yalgado Ouédraogo from January 1 to December 31, 2016. We assessed the type of heart failure, electrocardiographic and echocardiographic parameters, comorbidities, etiologies of heart failure as well as the intra-hospital evolution during this hospitalization until discharge. After including these patients, they were called to participate in the study (which ran from December 1 to 31, 2020) by giving informed consent. We collected follow-up data (since discharge from the first hospitalization, i.e., one consultation each month for at least 42 months) from the patients’ individual follow-up diaries (rehospitalization, improvement, therapeutic compliance). We then performed a complete clinical examination, an electrocardiogram, a cardiac echography, and a biological workup including blood count, creatinine level, liver function tests, blood glucose and blood ionogram.Results: We included 87 elderly subjects representing 53.05% of all patients received for chronic heart failure in the cardiology department in 2016.The mean age was 71.49 ± 8.46 years and the sex ratio was 2. High blood pressure was the main cardiovascular risk factor in 62.07% of cases. In-hospital mortality was 20.69%, and factors associated with it included male sex (OR= 11.60 p < 0.01), renal dysfunction on admission (OR= 1.30 p= 0.03), and cardiogenic shock developed during hospitalization (OR= 10.30 p < 0.01). Mortality at approximately 4 years was 67.47% with a mean time to death of 13.60 months. No independent factor was found to predict death after discharge. Conclusion: The prognosis of heart failure in the elderly is poor in our context. The in-hospital death prognosis facteur are renal dysfunction, cardiogenic shock, An early and adapted management of this age group will improve survival. </p
Managing vomiting in the third trimester of pregnancy during fixed prosthodontic treatment. A case report and review of the literature
Pregnancy is a specific and critical period in a woman’s life. Some pregnant women face difficulty when performing effective oral hygiene care due to pregnancy-related vomiting symptoms. A hypersensitive vomit reaction in the third trimester of pregnancy is not frequent and may prevent the dental provider from successfully completing critical clinical stages resulting in poor treatment outcomes. Once pregnant women suffer an unpleasant gag reflex experience in a dental office, they may become phobic, delaying or postponing their dental treatment.The purpose of this article is to report a case of a 32-year-old woman, primigravida in the third trimester of pregnancy (32 weeks), partially edentulous with an exaggerated vomiting reaction, focusing on successful clinical management using a simple but effective table salt technique and proper fixed prosthesis design, as also to discuss the etiology, clinical symptoms and consequences of vomiting associated with late pregnancy during the dental, especially prosthodontics , treatment in such cases.</p
East-Central Europe-World’s Coronavirus Death Center
A review of the Countries with the highest coronavirus death rates in the world was made on February 11, 2022. The world’s average death rate that day was 746. It was not surprising to see the countries with the largest number of deaths having death rates 300- 400% of the world’s average. What was alarming was that the ten countries with the world’s highest death rates had death rates 400-800% of the world’s average and most of them were located in East-Central Europe. This paper focuses on understanding the implications of this.</p
Surgery, Chemotherapy and Radiotherapy May Promote Cancer Growth Speeds and Shorten Patient Lives
Medicine fails to find predictable cures for cancer in more than a century, and we explored the feasibility of controlling cancer growth speed by using lifestyle factors. After conducting an extensive literature review, we conducted simulations for cancer growth courses to see the feasibility of controlling cancer growth speeds.We found that (1) medical treatments are often accompanied by three to four lethal factors: treatment side-effects, emotional distress, and chronic stress, reduced exercises and physical inactivity, and excessive nutrition in some cases; (2) clinical trial exaggerates treatments short-term benefits and underestimates the slow-delivering adverse side effects as a result of statistical averaging, interfering effects of personal lifestyle factors and insufficient follow-up times; (3) the benefits of medical treatments are limited by chain comparisons, where surgery may work as a negative standard relative to the best alternatives for resolving cancer; (4) the strategy of destroying the tumor or killing all cancer cells is unworkable; (5) medical treatments can turn natural cancer growth curve into approximately doubly exponential curve; (6) multiple-factor non-medical measures are potentially much more powerful than medical treatments in controlling cancer growth and metastasis speeds; and (7) cancer early diagnosis and over treatments are unwise strategies in light of discoveries. Based on huge increases in cancer growth rate constants, substantial loss of vital organ functional capacity, and severe systemic aging-like cellular damages, we concluded that medical treatments may promote cancer growth and metastasis speeds and shorten patient lives in most situations, and the claimed benefits are caused by triple biases of clinical trials. By using the same method to explore how several lifestyle factors affect cancer growth rates, we concluded that the better strategy for ending the global cancer epidemic in the future is changing caner treatment strategy from killing cancer cells to slowing down cancer growth rates by using various lifestyle factors in combination. This study in part explains why cancer can self-resolve.</p
Excision of delayed presented giant pseudoaneurysm of left subclavian artery post stab injury
Post-traumatic pseudoaneurysms involving the subclavian artery are uncommon and whenever diagnosed they should be repaired early because of the risk of complications such as thrombosis, gangrene, limb disability, and rupture may result in death if not reached the hospital early. Presenting a case report of a 20-year-old male who presented with a left subclavian artery pseudoaneurysm following a stab injury to the left axilla. He was referred to our institute, 20 days after the alleged assault. The presentation, diagnosis, and management of this injury are discussed.</p
To study the indications of cholecystectomy, types of surgery and complications of surgery in a tertiary care hospital in Nepal
Background: Cholelithiasis is a chronic recurrent disease of the hepatobiliary system and cholecystectomy is one of the commonly performed surgeries. This study intends to evaluate the demographic of patients with cholelithiasis, its various mode of presentation, surgical intervention, and its outcome.Methods: This was a quantitative retrospective cross-sectional study conducted in the College of Medical Sciences- a Superspeciality Tertiary Care Teaching Hospital in Bharatpur, Chitwan in central Nepal. All patients undergoing laparoscopic or open cholecystectomy in the department of GI and General Surgery at the College of Medical Sciences from 1st May 2017 to April 30, 2021were included in the study. Study tools were records of the patients retrieved from the medical record section.Results: A total of 355 patients data were analyzed. The mean age of the study samples was 46.43 +/- 16.47 years. Female predominance was seen at 76.18% with M: F ratio of 1:3.18. The most common presenting symptoms were pain abdomen (70.4%), bloating of the abdomen (63.9%), and fatty dyspepsia (46.8%). Acute cholecystitis was seen in 14.36%, biliary pancreatitis in 6.2%, and gallbladder perforation in 2.25% cases. Laparoscopic cholecystectomy was done in 269(83.94%), open cholecystectomy in 57(16.05%), and laparoscopic converted into open cholecystectomy 29 in (9.73%). Intra-operative complications like gallbladder perforation and controlled bleeding were seen in 10.14% and 4.23% of cases. Post-operative complications like bile leak and major bile duct injury were seen in 6.19% and 1.40% of cases. Regarding management of bile leak, conservative management was successfully done in 54.54% of cases while they were managed with ERCP in 27.27%, hepaticojejunostomy in 13.63%, and with T-tube repair in 4.5% cases. The overall mortality was 0.8%.</p
Cellular interorganelle crosstalk in health and disease states: A glimpse on nephrology-related conditions
Cellular interorganelle crosstalk in medical sciences is a new discussion of mechanisms and pathways of physiological functions and pathogenesis of diseases. The organelles (“mitochondria”, nucleus, lysosome, endoplasmic reticulum, Golgi apparatus) are members of such functional units that are needed to perform specific tasks. Mitochondria are essential metabolic organelles in cells, but they also contribute to iron and calcium homeostasis, as well as in the regulation of apoptosis, and they are increasingly recognized as key signaling platforms. In the kidney, crosstalk between mitochondria with endoplasmic reticulum is at mitochondria-associated endoplasmic reticulum membrane in regulating calcium homeostasis. Another crosstalk between these organelles is about autophagic mechanisms. Autophagy is triggered in the kidney in response to acute kidney injury and supports against kidney injury. High glucose-induced reactive oxygen species can be produced by both enzymatic and nonenzymatic pathways. The nucleotide-binding domain and leucine-rich repeat (NLR) family or nucleotide-binding and oligomerization domain (NOD)-like receptors family pyrin domain containing 3 (NLRP3) inflammasome plays a role in inducing infectious defenses via inflammatory cytokines. The NLRP3 inflammasome is activated by the mitochondria-associated endoplasmic reticulum membrane. It has a role in nephrocalcinosis-related chronic kidney disease. This review article is a summary of interorganelle crosstalk in health and disease states, especially in kidney and nephrology-related conditions. </p
In silico phytochemicals analysis as inhibitors of the SARS-COV-2 main protease
Background: The world population's full immunization with vaccines against SARS-CoV-2 is still challenging. Therefore, more research must be needed to find an active antiviral drug against the virus, including new mutated strains.Results: Therefore, this research analyzes 35 natural compounds isolated from various plants against SARS-CoV-2 main protease (Mpro) using an in silico strategy. According to the results, it was possible to identify promising molecules using a molecular docking strategy. Furthermore, the results showed that the interaction of these molecules with protease-specific residues, including (2S)-Eriodictyol 7-O-(6''-O-galloyl)-beta-D-glucopyranoside (Trp207, Ser284, and Glu288), Hypericin (Glu166, Arg188, and Thr190), Calceolarioside B (Gly143, Ser144, Cys145, Glu166, Arg188, and Gln192), Epicatechin (Ser144, His163, and Leu167) and Myricitrin (Thr190) with ΔG was -8.5, -9.6, -8.5, -9.3 and -9.3 kcal/mol, respectively. In addition, analyzing all compounds for their ADME properties shows that compounds present an excellent pharmacokinetic profile. Conclusion: In conclusion, the results of this study indicated that these major natural compounds can be considered potential inhibitors of Mpro and should be further explored in vitro and in vivo in accordance with our data.</p
Syndrome of Inappropriate Antidiuretic Hormone secretion due to hydroxyurea
Hyponatremia is the most common electrolyte disorder in clinical practice: Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) is the most common cause of hyponatremia, especially in euvolemic patients. Drugs are among the most common causes of SIADH. While our patient was using hydroxyurea due to polycythemia vera, he was diagnosed with SIADH due to hydroxyurea treatment after investigation due to resistant hyponatremia. The improvement of hyponatremia after hydroxyurea is discontinued and the development of hyponatremia after the drug is re-started supports the development of drug-induced hyponatremia. Careful follow-up is required in terms of hyponatremia in patients using hydroxyurea. </p
