    Clinical features and outcome of prosthetic pulmonary valve infective endocarditis

    Background: Infective endocarditis is a serious complication in patients with prostheses in the right ventricle outflow tract. The aim of our study is to assess clinical features in these patients.Methods: We have analyzed all cases of prosthetic pulmonary valve infective endocarditis (PPVIE) and compared them to the rest of the patients with IE. Since the year 2000, 375 cases of IE have been diagnosed in our center and nine patients had a PPVIE.: 3 patients with Melody percutaneous prostheses, 5 with a homograft (3 after a Rastelli procedure, 2 a Ross procedure), and 1 patient with a heterograft (Contegra conduit).Results: Mean age of PPVIE patients was 31±15 years old and 89% were male. A presumed portal of entry could only be identified in 4 of the 9 patients with PPVIE. The most common was a dental origin (3 patients). A causative microorganism was detected in all but one PPVIE patient: 3 S.viridans, 2 S.aureus, 2 coagulase-negative staphylococci, and 1 enterococcus. Most patients suffered complications during the in-hospital phase: 7 out of 9 PPVIE patients (78 vs.75% of the rest of IE patients), the most common congestive heart failure (5 patients), and persistent sepsis (4 patients). Six patients were operated on during hospitalization (67 vs. 55%) with a mean time from admission to surgery of 15 days. In-hospital mortality was similar (22 vs. 26%).Conclusions: Prevalence of PPVIE is low, with a clinical outcome, surgery rate, and survival similar to the rest of patients with IE.</p

    Introduction: Pharyngeal airway space has become an important criterion for diagnosis and treatment planning and this has led to a paradigm shift towards soft tissue evaluation for orthodontic diagnosis and treatment planning, The aims and objectives of the study were to evaluate and compare upper pharyngeal airway space in Cleft Lip &amp; Cleft Palate patients who underwent orthodontic treatment under different treatment modalities like Camouflage treatment &amp; Maxillary Advancement.Methods: This is a retrospective cephalometric study in which 20 Pre-treatment Cephalograms and 20 Post-treatment Cephalograms of Cleft Lip and Palate patients were taken. Orthodontic treatment involving Camouflage therapy and Maxillary Advancement were studied in each respective group. The Cephalogram was traced on Matt acetate paper and the pharyngeal airway space was measured using the McNamara analysis method.Results: Statistically significant difference was observed in the Pre-treatment and Post-treatment mean values in both groups, where the Post-treatment mean was higher than the Pre-treatment means in both groups. A highly significant mean was noted in the Cleft Surgical group as compared to the Camouflage (xiii) group, where, the post-treatment upper airway mean was higher in the Cleft Surgical group than the Cleft Camouflage group.Conclusion: There is a significant increase in upper pharyngeal airway space in patients with CLCP treated with Maxillary Advancement and Camouflage modality. The significance is comparatively more in the Maxillary Advancement group than in the Camouflage group.</p

    Since “living well with dementia” is a concept that varies from person to person, the care provided is expected to be customized to the individual needs of each person with dementia. Support is expected to involve shared decision-making between people with dementia and their caregivers, and a comprehensive geriatric assessment within the framework of the International Classification of Functioning, Disability, and Health is expected to be conducted to help these persons with dementia realize their goals. Additionally, the Plan-Do-Check-Act cycle is expected to be regularly implemented to verify the effectiveness of the intervention. Although the disability of the individual with dementia increases as the disease progresses, it is important to support them to live positively by reducing their difficulties in daily living.</p

    Selenium (Se) is a ubiquitous element akin to sulfur (S) existing in the Earth’s crust in various organic and inorganic forms. Selenium concentration varies greatly depending on the geographic area. Consequently, the content of selenium in food products is also variable. It is known that low Se is associated with an increased incidence of cancer and heart diseases. Therefore, it is advisable to supplement your diet with this element albeit in a proper form. Although blood increased concentrations of Se can be achieved with various pharmacological preparations only one chemical form (sodium selenite) can offer true protection. Sodium selenite, but not selenate, can oxidize thiol groups in the virus protein disulfide isomerase rendering it unable to penetrate the healthy cell membrane. In this way, selenite inhibits the entrance of viruses into the healthy cells and abolishes their infectivity. Therefore, this simple chemical compound can potentially be used in the recent battle against the coronavirus epidemic.</p

These treatment options are not specific to the cause of the disease and therefore result in a large number of side effects including nausea, fatigue, hair loss, etc. The use of oligonucleotides may be a more efficient and safe method of cancer therapy. This mini-review will cover how oligonucleotides have been used to treat diseases in the past as well as different methods for delivery into the body and finally the applications of oligonucleotides in pancreatic cancer therapy.</p

They can also handle temperature regulations since the phase change process occurs at a constant temperature. The implementation of PCMs is one of the potential passive cooling strategies which has received increasing attention in recent years.     Psychopathologic evidence in the Italian “Trap Music” population

    Background and aims: In the last decade, a musical strand has emerged in the Italian national scene that has international roots since the 1990s of the last century: “Trap Music” and younger generations are increasingly fascinated by this genre, for various reasons. The present research hypothesizes the existence of a link between the choice of preference of this musical genre and the psychopathological profile of those who choose their first preference, hypothesizing that such individuals have on average a higher level of dysfunctional traits typical of cluster B (borderline, narcissistic, histrionic and antisocial), according to the PICI model and compared to the population.&nbsp;Materials and methods: Clinical interview, and administration of the battery of psychometric tests. The population sample was selected based on previous clinical contacts and voluntary participation through recruitment in major social networks (Facebook, Instagram, Twitter, TikTok), a total of 4,368 participants, divided into three age groups (18-25, 26-37, 38-46) and two groups (the first “clinical” and the second “control”). SPSS, Anova test (with Bonferroni).Results and discussion: On average, the users selected in the clinical group population sample presented 81% of cases with a psychopathological personality profile (PICI-2) with at least 5 dysfunctional traits afferent to cluster B (bipolar, borderline, histrionic, narcissistic, antisocial, and psychopathic) and at least 4 dysfunctional traits afferent to cluster C (paranoid, delusional, schizophrenic spectrum, dissociative), according to the PICI model, compared to 23.1% of the cases in the control group, which, however, shows traits more oriented toward neurotic tendencies (anxious, phobic-avoidant, obsessive, somatic). The investigation of dysfunctional sexual behaviors then showed, in the clinical group, the marked presence of the clinical condition of the users, with an average of 96.8% compared to 24% in the control group; in particular, the presence of a tendency toward pedophilic (under 13 years old) and pederastic (13-17 years old) paraphilia is noted for the average value between only the markings of the second and third clinical groups equal to 54.3% (with an overall phenomenon slightly more inclined toward the male group).&nbsp;Conclusion: It is concluded, therefore, that the starting hypothesis can be confirmed, as the hypothesized link between the primary preference choice of “Trap Music” and the psychopathological profile afferent to the dysfunctional traits of Cluster B (borderline, narcissistic, histrionic, antisocial and psychopathic), according to the PICI model and compared with the control group (CG) population, which has significantly lower pathological values (57.9% - 72.8%) than the clinical group (CG), appears credible and non-random.</p

    Development of a management guide wheel for cervical cancer

    We designed and developed a management guide wheel for cervical cancer to facilitate a standard approach and improve the quality of care in managing patients diagnosed with different stages of cervical cancer. Each step of the wheel considers the patient’s current medical condition, FIGO stage, and the possible treatment modalities for cervical cancer in her case. We reviewed existing international guidelines on the management of cervical cancer and compared their respective recommendations. This guide wheel is based on recommendations by the following organizations: the National Comprehensive Cancer Network (NCCN), the Society of Gynecologic Oncology (SGO), the European Society of Gynaecological Oncology (ESGO), the European Society for Medical Oncology (ESMO), the American Society of Clinical Oncology (ASCO), the British Gynaecological Cancer Society (BGCS) and the International Federation of Gynecology and Obstetrics (FIGO).&nbsp;</p
