    Navigating Thyroid Crises: A Nationwide Analysis of the Weekend Effect and Mortality Trends in Myxedema Coma (2016-2020)

    Our objective was to utilize the National Inpatient Sample (NIS) database for analyzing the outcomes of myxedema coma based on the day of admission and explore five-year mortality trends. This retrospective cohort study examined in-patient mortality and secondary outcomes of patients with myxedema coma from 2016 to 2020 using the NIS database. Patients were selected using International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) codes and divided into weekday and weekend admission groups. From 174,776,205 discharges, 5095 patients were included. Mean age of admitted patients was higher on weekdays (67.1 year) than weekends (66.5), with females constituting 69.4% and 67.4% of admissions, respectively. Weekend admissions had higher odds of inpatient mortality compared to weekdays [adjusted odds ratio (aOR): 1.9, p = 0.01]. The overall mortality rate for myxedema coma rose from 6.8% in 2016 to 13.4% in 2020 (p-value = 0.01). No significant difference in the length of stay, hospitalization cost, and charges, blood transfusion, acute kidney injury requiring dialysis, acute respiratory failure requiring intubation and parenteral nutrition was noted between weekday and weekend admissions. Further studies are needed to identify factors contributing to this disparity and to confirm the findings of increasing mortality related to myxedema

    The increasing prevalence of cannabis use, with an estimated 219 million users globally, underscores the need to examine its potential health impacts. This review focuses on the arrhythmogenic properties of cannabis, particularly considering its active component, tetrahydrocannabinol, and its interactions with the endocannabinoid system. Epidemiological data and multiple studies indicate a significant association between cannabis use and various arrhythmias, particularly atrial fibrillation. The risk is notably higher among younger users and males. Additionally, case reports have linked cannabis use to other arrhythmias such as ventricular tachycardia and ventricular fibrillation, especially in individuals with underlying cardiac abnormalities. This review also discusses the arrhythmogenic potential of synthetic cannabinoids, which are more potent than natural tetrahydrocannabinol. Despite some studies suggesting no significant difference in arrhythmia burden between cannabis users and nonusers, the preponderance of evidence supports a correlation between cannabis use and increased arrhythmia risk. Given the rising tetrahydrocannabinol content in cannabis products and the limited data on the long-term cardiovascular effects, this review underscores the need for large-scale prospective studies. Until more comprehensive data are available, it is advisable for patients with channelopathies, structural heart disease, or prior myocardial infarction to avoid cannabis use

    Beyond the Diagnosis: A Deep Dive Into the End Stage Liver Disease Experience From the Patient Perspective

    Introduction: End-stage liver disease (ESLD) presents a multifaceted challenge that encompasses not only physical but also emotional, psychological, and social dimensions. This study aims to explore the experiences of ESLD patients within the United States healthcare system. Methods: Utilizing a convenience sampling methodology, 15 ESLD patients from a tertiary care hospital in the USA participated in semi-structured interviews between April 2023 and January 2024. Data analysis was conducted using MAXQDA 2023, employing a phenomenological approach to identify common themes. Results: The study identified six primary themes: the significance of communication style in diagnosis delivery, the crucial role of family and social support, varied understanding and preferences for palliative care, diverse attitudes towards advanced care planning, preferences for coordinated healthcare experiences, and the emotional and psychological impact of ESLD. Conclusion: Our study underscores the complexity of ESLD patient care beyond medical treatment, highlighting the importance of clear communication, empathetic care, and the integration of family and palliative care services

    Maternal and fetal outcomes of autoimmune hepatitis in pregnancy: A United States hospitalized patient study

    Background: Autoimmune hepatitis (AIH) is a chronic inflammatory disease primarily affecting the liver, with a higher prevalence among women of reproductive age. The latest nationwide statistics regarding its impact on maternal and fetal outcomes during pregnancy are lacking. Aims: To analyze the real impact of AIH on maternal and fetal outcomes in hospitalized delivery patients, and provide theoretical guidance for comprehensive clinical management. Methods: A retrospective analysis was conducted using data from the 2016-2020 National Inpatient Sample database in the United States. Multivariate logistic regression analysis was used to assess the influence of AIH on maternal and fetal outcomes during pregnancy. Results: A total of 17,825,445 hospitalized delivery patients were included, among which 1,185 had AIH. After adjusting for potential confounding factors, compared to hospitalized delivery patients without AIH, the AIH group exhibited significantly higher rates of adverse maternal and fetal outcomes, including hypertension complications of pregnancy (AOR 1.68, 95 % CI 1.09-2.58), preterm birth (AOR 2.89, 95 % CI 1.91-4.38), fetal growth restriction (AOR 2.21, 95 % CI 1.34-3.64), and fetal death (AOR 4.13, 95 % CI 1.33-12.83). AIH showed no association with cesarean section or large fetus. In the group of delivery in patients with AIH, patients who develop hypertensive disorders have a higher probability of concomitant diabetes mellitus (OR 6.85, 95 % CI 2.19-21.45), hypertension (OR 4.64, 95 % CI 1.68-12.82), and obesity (OR 3.06, 95 % CI 1.26-7.42). Additionally, AIH patients incurred higher total costs and longer hospital stays during the delivery hospitalization. Conclusion: Patients with AIH face an increased risk of hypertensive disorders of pregnancy, preterm birth, fetal growth restriction, and fetal death during delivery. It is crucial to enhance awareness of these potential occurrence risks

    Aortic Valvular Papillary Fibroelastoma Presenting as Transient Ischemic Attacks

    We present an aortic valve Papillary Fibroelastoma diagnosed on a transesophageal echo (TEE) in 2020 in a patient who had been presenting with recurrent CNS symptoms since 2013 without any known cause. She had clean imaging including transthoracic echo (TTE) and the Fibroelastoma was discovered only on a TEE in 2020. We hope to highlight the elusive nature of the lesion as an obscure cause of CNS symptomatology
