Healthcare Professionals’ Awareness Regarding Preparing Pediatric Patients for Pain and Distress during Invasive or Stressful Procedures
目的 子どもと関わる医療者がプレパレーションをどの程度認識しているのか,子どもの主体的な参加のためにどのような援助をしているのか,子どもの主体的な参加を阻害しているものをどのように捉えているかについて職種による差異を明らかにすることが本研究の目的である.
方法 A県内の小児科の診療科目をもつ施設の放射線技師,検査技師,医師,看護師を対象とし,先行研究をもとに独自に作成した自記式質問紙調査を実施した,SPSS ver.26を使用して分析を行った.
結果及び考察 医療者のプレパレーションの認知では,医師,看護師は9割以上が知っており,放射線技師,検査技師は1~2割であったが,検査や処置への保護者同席の推奨については,放射線技師は同席を勧める割合が多く医師,看護師は少なかった.保護者の同席は,子どもの主体的な参加のために重要であるが,その必要性について医療者の認識が低いことが課題となる.プレパレーションの実施については,看護師以外の職種の「終了後の心理的ケア」の実施得点が低く,プレパレーションは単に説明や準備をすることではなく,検査や処置の過程を通して行われる支援であることを理解する必要があると考えられた.「苦痛軽減」では,医師の実施得点が他職種より低く,外用局所麻酔剤の使用やディストラクションによる苦痛緩和が必要と考えられた.Purpose To reveal the recognition level of medical staff working with preschool-aged children on the preparation procedure. Further, to observe the kinds of support they offer and the differences in what they recognize as obstructive factors for the patients’ positive participation depending on their occupation.
Method The questionnaire survey created based on pre-existing research was administered to radiological technicians, electroencephalogram technicians, doctors, and nurses from hospitals with a pediatric department. The result was analyzed using SPSS version 26.
Results and Discussion Over 90% of the doctors and nurses were aware of the preparation procedure. Simultaneously, approximately 10% to 20% of the radiological and electroencephalogram technicians were aware of the preparation procedure. However, electroencephalogram technicians often encouraged parents’ company through
tests and procedures while the doctors and nurses did less. The presence of the parents is especially significant for the patients’ positive participation, and the staff must be fully aware of this element. Radiological technicians, electroencephalogram technicians, and doctors scored low on exercising “after mental care.” The staff also need to be aware of the preparation procedure not only as a simple explanation and preparation into the course, but also as holistic support offered through each process of patients’ experience. The doctors scored low on their support in “pain reduction” compared to other occupations, and the application of lidocaine-propitocaine cream (EMLA) and other destructions were needed
Difficulties in Heart Failure Palliative Care for Nurses Working in Cardiovascular Wards -Focusing on Years of Clinical Experience-
目的 本研究の目的は,看護師の経験年数と心不全緩和ケアにおける困難の関連要因を明らかにすることである.
対象 日本循環器学会認定の循環器専門医研修施設のうち看護部長の承諾を得た234施設の循環器病棟に勤務する看護師1,926名を対象とした.
方法 無記名自記式の質問紙調査を行った.質問紙の内容は,属性,心不全の緩和ケアの実践での困難と対処,および自身の考えとした.循環器病棟での看護師の経験年数を従属変数,心不全の緩和ケアの実践での困難と対処を独立変数として重回帰分析を行った.
結果と考察 循環器病棟での経験年数との関連要因として,[医師に相談する]が正の要因であり,[病棟師長または先輩看護師(チームリーダー)に相談する],および[疼痛]が負の要因であった.心不全緩和ケアが必要な患者に対して看護師は,患者と家族の希望だけでなく,治療方針や医師の個別性に応じて対応することが求められる.一方で,看護師経験が短い看護師は看護実践が限られており,ケアに苦慮していることが明らかとなった.Purpose This study aimed to clarify the difficulty of palliative care for heart failure by focusing on nursingexperiences among nurses working in cardiovascular wards.
Subjects The study encompassed 4,889 nurses employed in the cardiac care units of 234 facilities,all of whichhad obtained approval from director nurses within cardiovascular specialist training facilities certified by theJapanese Circulation Society.Of which 1,926 nurses were participated in this study.
Methods This study was an anonymous self-administered questionnaire-based survey.The questionnaire coveredparticipant demographics,challenges encountered,heart failure palliative care coping strategies,and personal
beliefs.
Results and Discussion Ninety-one-point-two percent of the subjects were women.The average number of nursing experiences was 11.5 years,of which 5.3 years experienced in the cardiovascular ward.Regarding difficulties in practicing palliative care for heart failure,nurses with less than three years of experience found it difficult to perform multidisciplinary team medical care (cooperation).To deal with difficulties in heart failure palliative care,nurses with less than three years of experience should“ consult the head nurse or senior nurse( teamleader)”,and nurses with more than three years of experience have practiced “consult with a doctor”.In heartfailure palliative care,it has become clear that nurses with less than three years of experience are required to receive support from senior nurses,and nurses with more than three years of experience are required to be able to consult with doctors and other multidisciplinary professionals

The Process of Latent Nurses Attending Outplacement Workshops and Returning to Work
P(論文)背景 臨床現場のおいては, 少子高齢化により看護職員の不足が深刻化している.目的 潜在看護師が再就職支援講習会を受講するに至ったきっかけ, 受講して得られたもの, 講習会に望むもの, 再就職への考えを明らかにすることを目的とした.方法 再就職支援講習会に参加した看護職7名に半構造化面接を行った.結果 再就業支援講習会に参加する看護職は, 結婚や出産, 育児などの理由で離職し, 離職期間が5年以上であり, 非常勤での復帰を希望している. 講習会の受講は, 復帰に躊躇するという逆効果もあるが, 現場の勘を取り戻すことや刺激を受けることで復帰への効果がある.結論 看護職においては女性が多いということから, 女性のライフイベント面からの影響を検討し, 潜在看護師が医療現場に復帰するために, 家庭生活との両立に向けた支援がより一層求められる.Background The aging society and declining birth rate is causing a worsening of a shortage of nursing staff in the clinical workplace.Objective The objective was to clarify what triggered latent nurses to attend outplacement workshops, what they gained from attending these workshops, what they expected from the workshops, and their own opinions with regards to re-employment .Method Semi-structured interviews were held with seven nurses who participated in a lecture offering support for returning to work.Results Nurses participating in lectures offering support for returning to work have left employment due to marriage, childbirth, childrearing, etc., and have been out of the workplace for five years or more, but tend to be looking to return to work on a part-time basis. Participation in a lecture can have the negative effect of making them more hesitant about returning to work, but can also enable them to recapture their instincts regarding the workplace, and provide a stimulus that is ultimately effective in their returning to work. Conclusion Since the nursing profession is made up of a majority of women, it is therefore necessary to consider the impact of life events in the lives of women, and to provide a further level of support to assist women to achieve a work-life-balance
Administrators of Nursing Institutions about Their Opinions for Their Workers to Go on to Graduate School Education:As Intended for Administrators of Nursing Institutions in Shiga Prefecture
P(論文)背景 近年, 看護系大学院の増加に伴い社会人大学院生が増加している. しかし, 職場の理解が得られず, キャリアを中断して学業に専念する学生や進学そのものを断念する者も多い. 目的 本学が大学院を開設する場合には, 他大学と同様に社会人が入学生の多くを占めると推測されるため, 看護職の大学院進学に関する看護管理者の認識について調査を行なった. 方法 滋賀県内の病院及び実習関連施設等の管理者144名を対象に質問紙調査を実施した. 結果 70名からの回答 (回収率48.61%) が得られた. 大学院修了看護職に期待する管理者は51.42%, 所属施設に大学院卒看護職が必要とする管理者は38.57%であったが, 両者ともわからないと回答した管理者は42.85%であった. また, 進学制度がある施設は25.71%であった. 結論 本調査対象である看護管理者が大学院修了看護職に期待する内容, 必要とする理由は, 研究や教育, 管理的役割, 看護実践の質的向上など先行研究と同様の結果であったが, 大学院修了看護職にどのように活動してもらうかイメージをもてていない管理者も多いことが明らかになった.Background Recently, the number of the graduate students of nursing who also work for full time jobs has been increasing as many new graduate schools of nursing have been established. However, many of them have been had to give up pursuing either their career or education, due to the lack of the understanding from their work places.Object When establishing a graduate school of nursing in our university, it is expected that many of the students will be working full time as it is seen in many other schools. Thus, we conducted a survey to the administrators of nursing institutions asking about their opinions for their workers to go on to graduate school education.Method Surveyed on 144 administrators of nursing institutions in Shiga prefecture, including the hospitals and institutions where our students train in, through questionnaires.Result 70 cases were collected (response rate: 48.61%). 51.42% have positive opinions forward the workers who have finished the master's course of the field, 38.57% said they need them at their work places, and 42.85% said they do not know. However, only 25.71% of the institutions provided the system of supporting the workers for their further education and their main concern seems to be how the organizations and the workers can make mutual understanding for their career.Conclusion The survey showed that some of the administrators have positive opinions toward the workers who have finished the master's degree course of nursing and that they feel the need for them at their work places as it was shown in previous studies. However, it has also been found that many of them do not have a clear picture of how those workers can be actively participated at their work places
