    Spontaneous age-related changes of attention in unmedicated boys with attention deficit hyperactivity disorder

    Objective: Neuropsychological, neuroanatomical, and electrophysiological studies have reported a steady increase in the different attention types until the age of 10 years. Moreover, differences between healthy control (HC) boys and those with attention deficit hyperactivity disorder (ADHD) become nonsignificant in late childhood. This cross-sectional study aimed to perform a comparative analysis of attentional processing in boys with ADHD and HC in the 6:00-10:11 years age range. Methods: Age-related changes in attentional processing were compared between Caucasian Turkic boys (72-131 months of age) with ADHD (n ¼ 144) and HC (n ¼ 112). Selective, focused, and inhibitory attention were measured using the Stroop Test (5 scores); sustained attention was measured using the Cancellation Test (3 scores); and attention span was measured using the Visual Aural Digit Span Test-Revised (6 scores). Results: At the age of 6 years, the ADHD group had a significantly lower performance for all attention types. By the age of 10 years, there were no significant between-group differences. However, the component structure of the neuropsychological test scores in the ADHD group differed from that in the HC group and previous studies. Conclusions: Attentional processing in boys with ADHD changes within the age-range of 6:00-10:00 years where it finally becomes similar to that in HC boys. This delayed maturation is consistent with the maturational lag model of ADHD. However, there was a betweengroup difference in the component structure of attentional processing, which is consistent with the maturational deviance model of ADHD

    Diagnostic value of lung ultrasonography in children with COVID-19

    Background: Lung ultrasound (LUS) has been successfully used in the diagnosis of different pulmonary diseases. Present study design to determine the diagnostic value of LUS in the evaluation of children with novel coronavirus disease 2019 (COVID-19). Methods and objectives: Prospective multicenter study, 40 children with confirmed COVID-19 were included. LUS was performed to all patients at admission. The chest X-ray and computed tomography (CT) were performed according to the decision of the primary physicians. LUS results were compared with chest X-ray and CT findings and diagnostic performance was determined. Results: Of the 40 children median (range) was 10.5 (0.4-17.8) years. Chest X-ray and LUS were performed on all and chest CT was performed on 28 (70%) patients at the time of diagnosis. Sixteen (40%) patients had no apparent chest CT abnormalities suggestive of COVID-19, whereas 12 (30%) had abnormalities. LUS confirmed the diagnosis of pulmonary involvement in 10 of 12 patients with positive CT findings. LUS demonstrated normal lung patterns among 15 of 16 patients who had normal CT features. The sensitivity and the area under the receiver operating characteristics (ROC) curve (area under the ROC curve) identified by the chest X-ray and LUS tests were compared and statistically significantly different (McNemar's test: p = .016 and p = .001 respectively) detected. Chest X-ray displayed false-negative results for pulmonary involvement in 75% whereas for LUS it was 16.7%. Conclusions: LUS might be a useful tool in the diagnostic steps of children with COVID-19. A reduction in chest CT assessments may be possible when LUS is used in the initial diagnostic steps for these children

    Comparison of the impacts of different multimodalities on incidental L2 vocabulary learning

    Multimodality of input in incidental L2 vocabulary learning has recently been a topic of interest among language acquisition researchers, yet the results have been somewhat contradictory. This study seeks to compare the impacts of two different multimodalities on incidental L2 vocabulary learning, namely, reading-plus-watching (experimental group I) vs. reading-plus-listening (experimental group II), as compared to the reading only condition, which is included as a control measure. Experimental group I watched and read the transcriptions of four news texts with electronic glosses for the target words, while experimental group II read and listened to the same news texts again with electronic glosses for the same 20 target words. Next, the two experimental groups swapped roles with a new set of four news texts glossed for another group of 20 target words. The control group only read the same eight news texts without glosses. The results suggest that reading-plus-listening can be a more conducive multimodal presentation for incidental vocabulary learning as compared to reading-plus-watching. The results also challenge the validity of some principles of the Cognitive Theory of Multimedia Learning in incidental L2 vocabulary learning, while providing supporting evidence for some other principles

    Apendektomi yapılan hastaların kültür ve antibiyotik duyarlılıkları, antibiyotik seçimleri ve klinik duruma etkileri

    Giriş: İntraabdominal infeksiyonlar genellikle antibiyotik tedavisinin ampirik olarak başlandığı infeksiyonlardır. Artan antibiyotik direnci ampirik antibiyotik seçiminde önemlidir. Doğru antibiyotik seçiminde toplumumuzdaki mevcut direnç durumunun bilinmesi gereklidir. Güncel rehberlerde sadece antibiyotik tedavisi ile tedavi edilmesi önerilen apandisit olguları bulunmaktadır. Bu durum ampirik antibiyotik tedavi uygunluğunun önemini daha da arttırmaktadır. Bu çalışmada hastanemizde apendektomi yapılan hasta grubunda ameliyat materyalinde üreyen etken bakteriler ve direnç durumları araştırılmıştır. Sonuçlar, başlanılan ampirik antibiyotik tedavi seçimleri ve hastaların klinik durumları ile karşılaştırılmıştır. Materyal ve Metod: Hastanemizde Haziran 2015-Haziran 2017 tarihleri arasında apandisit nedeniyle opere edilen hastalardan operasyon sırasında doku örneği kültürleri alınabilmiş olanlar çalışmamıza dahil edildi. Hastaların başlanmış olan antibiyotik tedavileri, demografik özellikleri kayıt altına alındı. Operasyon sırasında alınan doku örnekleri mikrobiyoloji laboratuvarımız tarafından aerobik kültür ekimi yapılarak incelendi. Üreyen bakterilerin antibiyotik duyarlılık testleri EUCAST standartlarına uygun olarak disk difüzyon yöntemi ile çalışıldı. Vitek 2 (BioMerieux, Fransa) otomatize identifikasyon sistemi kullanılarak bakteri identifikasyonu yapıldı. Bulgular: Toplam 90 hasta çalışmamıza dahil edilmiştir. Bu hastaların 54 (%60.0)’ü erkekti. Ortalama yaş 29.5 ± 9.08 (16-51) idi. Alınmış olan kültürlerden 31 (%34.4)’inde üreme olmamıştır. Üreme olan doku kültürlerinde en sık üreyen mikroorganizma (51/90, %69.9) Escherichia coli idi. Genişlemiş spektrumlu beta-laktamaz (GSBL) üreten gram-negatif bakteri izole edilme oranı 17 (%18.9) idi. On iki hastada kültürde birden fazla bakteri üremiştir. Ampirik antibiyotik tedavisi olarak 20 (%22.2) hastaya sefazolin ve metronidazol kombinasyon tedavisi, 65 (%72.2) hastaya ise seftriakson ve metronidazol kombinasyon tedavisi başlanmıştır. En sık başlanılan antibiyotik olan seftriaksona duyarlılık oranı %63.4 olarak saptanmıştır. Sonuç: Hastalardan alınan doku kültürlerinin üçte birinde bakteri üremesi saptanmadı. GSBL pozitif gram-negatif bakteri üreme oranı %18.9 idi. Ampirik olarak başlanan antibiyotik tedavilerinin hiçbiri GSBL pozitif bakterilerde etkili değildi. Bu çalışmada hastalarımızın tamamında apendektomi uygulandığından etken bakterilerdeki direnç oranları, ampirik tedavi seçimleri GSBL pozitif bakteri üreyen hastalara uygun olmasa da tedavi başarısızlığı olarak klinik pratiğe yansımadı. Ancak yaygın intraabdominal infeksiyonu olan hastalarda ve sadece antibiyotik tedavisi ile tedavi edilen apandisit hastalarında uygunsuz ampirik antibiyotik seçimleri klinik tedavi başarısızlığına sebep olabilir

    Granulomatous skin disorders are a heterogeneous group of disorders characterized by granuloma formation on histopathology. They may be triggered by various stimuli, including infectious and non-infectious stimuli (foreign bodies, malignancy, metabolites, and chemicals), of different etiological origins. Although pathophysiological mechanisms are still poorly understood, infectious granuloma formation may occur if the patient’s immune system could not eliminate an agent. Clinically, localized or disseminated infectious granuloma formation can be related to the infectious agent’s pathogenicity or the patient’s immunity. Typical infectious agents causing infectious granulomatous reactions are mycobacteria, fungal infections, or parasites, such as leishmaniasis.     A new synthetic approach for pyrazolo[1,5-a]pyrazine-4(5H)-one derivatives and their antiproliferative effects on lung adenocarcinoma cell line

    Starting from the 3,5-dimethyl pyrazole ring and acetophenone derivatives, five different N-propargylated C-3 substitutedpyrazoles were obtained. These derivatives were reacted with different amine derivatives using Cs2CO3in methanol and11 different pyrazolo [1,5-a] pyrazine-4(5H)-one derivatives were obtained, which are not found in the literature. The cytotoxiceffects of these derivatives in the A549 cell line were investigated. The 160 µM concentration of two derivatives wasfound to increase cell death rate to 50%, and two derivatives increased cell death rate by up to 40%. The structure–activityrelationship (SAR) study revealed an amide group with a long alkyl chain and benzene ring with a p-CF3 group could beimportant for efficiency.     Reproductive outcomes of patients with T-Shaped and septate uterus following metroplasty: an observational study

    Introduction: Uterine malformations are common and may contribute to infertility and adverse pregnancy outcomes. After an accurate diagnosis, correcting the abnormal uterine morphology is the main goal to optimize reproductive outcomes. The principal objective of this study was to assess the impact of metroplasty for T-shaped (U1a) and septate uteri (U2) on live birth rates in infertile patients. Methods: This was a prospective observational study of infertile women with either U1a or U2 uterine anomaly. Patients with unexplained infertility and repeated (IVF/ICSI) failure were included. Hysteroscopic metroplasty was performed by a single experienced surgeon. Fertility outcomes of all cases were evaluated prospectively evaluated. The main outcome parameter was a live birth rate either achieved spontaneously or with assisted conception. Results: A total of 48 patients were included in U1a group and bilateral longitudinal uterine-lateral wall incision was carried out. A total of 63 patients were included in the U2 group and septum incision was carried out, 60 out of these 63 patients with U2 uterine anomaly required further lateral wall incision during septoplasty. During the first 12 months following surgery, nearly half of the patients in both groups achieved spontaneous pregnancy; 45% in the U1a group and 39% in the U2 group delivered at term. Conclusions: Hysteroscopic metroplasty offers promising reproductive outcomes in the presence of U1a and U2 uterine anomalies for those with unexplained infertility and repeated IVF/ICSI failures. In addition, uterine septum cases should be carefully evaluated intra-operatively to detect and repair concurrent lateral uterine wall anomalies

    A prognostic marker in idiopathic sudden sensorineural hearing loss serum calprotectin

    Objectives. Calprotectin, a protein released by neutrophils, has been used in many studies as a biomarker showing the presence of inflammation. In this study, it was aimed to investigate the relationship between serum calprotectin level and response to the treatment of idiopathic sudden sensorineural hearing loss (ISSHL). Methods. The present study is a prospective, cross-sectional historical cohort study.The study group consisted of 44 patients with ISSHL, and the control group consisted of 41 healthy volunteers without ear pathology. At the same time, patients in the study group were divided into three groups according to the response to ISSHL treatment (recovered, partially recovered, unrecovered). The relationship between the groups was statistically evaluated in terms of serum calprotectin levels. Results. The mean serum calprotectin value was 75.67 +/- 19.48 ng/mL in the study group and 50.24 +/- 29.14 ng/mL in the control group (P=0.001). Serum calprotectin value according to the severity of hearing loss in the mild, moderate and severe was 66.20 +/- 8.82, 70.35 +/- 16.77, and 91.23 +/- 1.9.73 ng/mL, respectively. Serum calprotectin value in the severe group was significantly higher compared to the moderate and mild groups (P=0.004, P=0.001, respectively). Serum calprotectin value according to the treatment response in the recovered, partially recovered and unrecovered groups was 63.36 +/- 11.54, 80.17 +/- 12.06, and 85.33 +/- 22.33 ng/mL, respectively. Serum calprotectin value in the recovered group was significantly lower compared to the partially recovered and unrecovered groups (P=0.002, P= 0.001, respectively). Conclusion. Serum calprotectin value informs the clinician about both the severity of hearing loss and the response to treatment. Hence, serum calprotectin can be used as an important biomarker in ISSHL patients for the determination of the prognosis of disease
