    Nutritional status and severity of coronary artery disease

    Objective The aim of this study is to evaluate the association between Nutritional Risk Index (NRI), a simple tool to assess nutritional status, and coronary artery disease severity and complexity in patients undergoing coronary angiography. Methods This study is a retrospective analysis of 822 patients undergoing coronary angiography. Patients with previous revascularization were excluded. Gensini and SYNTAX scores were calculated according to the angiographic images to determine atherosclerosis severity. NRI was calculated as follows: NRI = [15.19 × serum albumin (g/dl)] + [41.7 × (body weight/ideal body weight)]. In patients ≥65 years of age, Geriatric NRI (GNRI) was used instead of NRI. GNRI was calculated as follows: GNRI = [14.89 × serum albumin (g/dl)] + [41.7 × (body weight/ideal body weight)]. Patients were then divided into three groups as previously reported: NRI < 92, NRI 92–98 and NRI > 98. Gensini and SYNTAX scores were compared between three groups. Results The mean age of study population was 61.9 ± 11.1 years. NRI 98 was measured in 212, 321 and 289 patients, respectively. There was no difference regarding to sex, BMI, smoking, hypertension and diabetes mellitus between three groups. Patients with NRI < 92 had the highest mean Gensini score than the patients with NRI 92–98 and NRI > 98 (38.0 ± 40.6 vs. 31.17 ± 42.4 vs. 25.8 ± 38.4, P = 0.005). Also patients with NRI < 92 had the highest mean SYNTAX score than the patients with NRI 92–98 and NRI > 98 (11.8 ± 12.9 vs. 9.3 ± 12.4 vs. 7.7 ± 11.8, P = 0.001). Also, Gensini score of ≥20 and high SYNTAX score of ≥33 were associated with lower NRI (P < 0.001 and P < 0.001, respectively). Conclusion In our study, nutritional status evaluated by the NRI was associated with more extensive and complex coronary atherosclerosis in patients undergoing coronary angiography

    Social stigmatization in Turkish patients with chronic hepatitis B and C

    Background and aim: Viral hepatitis is the most important cause of chronic hepatitis worldwide. Stigmatization is defined as a feeling of rejection and isolation of patients by society due to illness. There are no studies on chronic viral hepatitis in the literature in English, which has its own religious and socio-cultural structure. In our study, we aimed to investigate the presence of social stigmatism and psychosocial effects on patients with different stages of chronic viral hepatitis B and C. Methods: Forty-five patients with chronic hepatitis C and 114 patients with chronic hepatitis B were enrolled in the study. Berger’s scale was used for stigmatization, composed of 40 four-point Likert items that have four subscales: personalized stigma, disclosure, negative self-image, and public attitude. Stigma score ranges between one and four. Stigma is accepted as present if the overall score is above two. Results: Overall the mean stigma scores were 1.97 ± 0.58 and 2.14 ± 0.57 for chronic hepatitis B and C, respectively. There was stigma in 47.4% of the patients with chronic hepatitis B, and 60% of the patients with chronic hepatitis C. Being male was the risk factor on overall stigma, disclosure and public attitude in chronic hepatitis C. Living in an urban setting was the risk factor on negative self-image in chronic hepatitis C and on personalized stigma and disclosure in chronic hepatitis B

    Cost-effectiveness analysis of chronic mechanical back pain treatment modalities

    Objectives: This study aims to evaluate and compare the cost-effectiveness of inpatient physical therapy, outpatient physical therapy applications, and back school education in low back pain (LBP) treatment. Patients and methods: Between March 2011 and September 2011, a total of 105 patients (43 males, 62 females; mean age 44.8 +/- 12.7 years; range, 27 to 58 years) who were treated for chronic LBP were included in this prospective study. Of the patients, 34 received inpatient physical therapy consisting of electrotherapy, superficial-deep heat application, and basic back exercise program, 35 received the same treatment in the outpatient setting, and 36 received back school education. Each patient was evaluated five times during the study using pain intensity Visual Analog Scale (VAS), spinal mobility measurements (inclinometer), general evaluation (VAS), functional disability measurement (Roland-Morris Disability Questionnaire [RMDQ]), and quality of life (Short Form-36 [SF-36]). All expenses and costs for treatment of LBP were estimated. Results: At the end of six months of follow-up, all study groups had a statistically significant improvement in all variables. Improvements in spinal mobility, RMDQ, and SF-36 scores were significantly higher in the inpatient physical therapy group (p<0.001). Direct and total costs were the highest in the inpatient treatment group and were the lowest in the back school education group (p<0.001). No statistically significant differences were observed in terms of additional cost of one-unit improvement in these variables among the study groups. Conclusion: Our study results suggest that all three treatment modalities are effective in chronic LBP. In patients with LBP-related impairment in functional status and quality of life, inpatient physical therapy and rehabilitation should be preferred

    Evaluation of epicardial adipose tissue and carotid intima-media thickness as a marker of atherosclerosis in patients with inflammatory bowel disease

    Background and aim: this study aimed to compare carotid intima media (CIMT) and epicardial adipose tissue (EAT) measurements, which are considered as markers for the detection of early atherosclerosis in healthy controls and inflammatory bowel disease (IBD) cases. Methods: a total of 60 IBD patients (25 Crohn’s disease and 35 ulcerative colitis) and 60 healthy patients (as a control group) were included in the study. The measurements of CIMT and EAT were performed using echocardiography and ultrasonography, respectively. Statistical analysis was used to determine the relationship between the parameters. Results: the thickness of bilateral (right and left) CIMT and EAT were significantly higher in IBD than in the control group (p < 0.05). There was a positive correlation between EAT and bilateral (right and left) CIMT in IBD patients (p < 0.05)

    Galectin-3, IL-1A, IL-6, and EGF levels in corneal epithelium of patients with recurrent corneal erosion syndrome

    Purpose: To determine the galectin-3 (Gal3), interleukin-1 (IL-1), interleukin-6 (IL-6), and epidermal growth factor (EGF) levels in corneal epithelium of patients with recurrent corneal erosion (RCE) syndrome and compare them with healthy controls. Methods: In this prospective interventional case control study, 32 eyes of 32 patients with RCE syndrome who had corneal epithelial erosions and 28 eyes of 28 healthy participants scheduled for photorefractive keratectomy (control group) were included. Exclusion criteria included corneal dystrophies, ectasia, dry eye, previous ocular surgery or topical medications, and systemic diseases. Epithelial samples were obtained during epithelial debridement in the study group and mechanical epithelial keratectomy in the control group. Galectin-3 levels were studied by the chemiluminescent microparticle immunoassay method. IL-1, IL-6, and EGF levels were determined using corresponding ELISA kits. Results: The median Gal3 levels were 132.25 ng/mL in the study group and 106.50 ng/mL in the control group. The median IL-1 and IL-6 levels were 6.24 pg/mL and 10.16 pg/mL, respectively, in the study group which were higher than that in the control group. The median EGF level in the study group was lower than that the control group with 1.30 pg/mL versus 2.67 pg/mL. In the control group, there was a significant positive correlation between EGF and IL-6 (r = 0.554; P = 0.040). A similar correlation was not observed in patients with RCE (r = -0.071; P = 0.794). Conclusions: The lack of increased EGF expression and the imbalance between growth factors, adhesion molecules, and interleukins may be the reason for the impaired wound healing response in RCE syndrome

    Ekonomik, siyasi ve askeri olarak önemli bir stratejik bölgede bulunan Kıbrıs Adası, Doğu Akdeniz'de gerçekleştirilen hidrokarbon kaynaklarının keşfi ile daha çok önem kazanmıştır. Doğu Akdeniz'de enerji kaynaklarının keşfi ile Avrupa Birliği(AB), Amerika Birleşik Devletleri (ABD), Rusya, Çin, İngiltere, Fransa, gibi bölge dışı ülkelerin de ilgisi artmıştır. Bu çalışmada Doğu Akdeniz'de bulunan Türkiye, Yunanistan, Libya, Mısır, Güney Kıbrıs Rum Yönetimi(GKRY), Lübnan, Suriye ve İsrail gibi bölge ülkelerinin yürüttüğü enerji arama çalışmaları ve enerji kaynaklarının taşınması incelenmektedir. Ayrıca bölge ülkelerinin birbiri ile ilişkileri, oluşturdukları ittifaklar bölgesel aktörlerin faaliyetleri ve Kıbrıs'ın fiili durumu ve özellikle Kuzey Kıbrıs Türk Cumhuriyeti (KKTC) varlığı üzerinden Türkiye'ye etkileri ele alınmaktadır. Yapılan sismik araştırmalar ve sondaj çalışmalarında bölgede bulunan hidrokarbon kaynaklarının bölge ülkeleri için yeterli olabileceği ve ihraç edilebileceği anlaşılmaktadır. Türkiye üzerinden AB pazarına da doğal gazın ihraç edilebileceği sonucuna varılmıştır. İleri sürülen iddialar ve savunulan diğer görüşlerin KKTC ve Türkiye aleyhine yürütülen siyasi faaliyetler olduğu dikkate alınarak KKTC vatandaşlarının olaylara bakışı için yüz yüze anket çalışması gerçekleştirilmiştir. Araştırma; 18 yaş ve üzeri toplam 324 katılımcıyla önceden hazırlanmış soru formuna bağlı olarak yüz yüze anket tekniği ile uygulanmıştır. Araştırma örneklemi KKTC vatandaşlarının 18 yaş üzeri nüfusunun binde ikiye yakın oranını temsil etmektedir. Araştırma örneklemi KKTC'nin demografik koşullarına göre gerçekleştirilmiştir. Elde edilen veriler kontrol edildikten sonra bilgisayar ortamına aktarılmış, SPSS programı ile çözümlemeler yapılmıştır. Bu çalışma sonucunda KKTC ve Doğu Akdeniz'in jeopolitik ekseninde Doğu Akdeniz'e ilişkin, Türkiye'nin Doğu Akdeniz siyaset parametrelerinin ortaya çıkarılması hedeflenmektedir.Cyprus is a strategic region in economic, political and military aspects and it has gained more importance with the discovery of hydrocarbon resources in the Eastern Mediterranean. The discovery of energy resources in the Eastern Mediterranean increased the interest of the countries that are far from the region like EU, USA, Russia, China, England and France. This study examines the energy exploration works carried out by the regional countries like Turkey, Egypt, Turkish Republic of Northern Cyprus (TRNC), Greek Administration of Southern Cyprus, Lebanon, Syria and Israel as well as the transportation of energy resources. The study also deals with the relations of the countries of the region with each other, activities of the regional actors and the impacts on Turkey through Cyprus. As a result of the seismic and drilling works, it is understood that the hydrocarbon resources of the region are significant for the regional countries but the marine transportation of the resources to EU countries by pipelines is not economic. It is concluded that the supply of the gas to the EU market via Turkey is more economical. It is understood that the other allegations that are raised are the political activities against Cyprus and Turkey and a face to face interview study is conducted for the views of the citizens of Cyprus on the developments. The study was been implemented with the face-to-face survey technique based on the previously prepared question form with total 324 participants who were 18 years and older. The sample of the study represents one thousandth of the population of Cyprus. The study sample was carried out in accordance with the demographic conditions of the TRNC. The obtained data was controlled and then conveyed to the electronic medium where analyses were using the SPSS program. As a result of this study, it is aimed to reveal the political parameters of Turkey regarding the Eastern Mediterranean in the axis of Cyprus

    What is the optimum lithotripsy method for high density stones during mini-PNL? Laser, ballistic or combination of both

    Percutaneous nephrolithotomy (PNL) is the primary treatment option for renal stones > 20 mm in diameter. Mini-PNL gained popularity with its minimally invasive nature. The aim of this study was to compare the efficiency of ballistic and laser lithotripsy with the combined use of both techniques. Data of 312 patients underwent mini-PNL for renal stones with Hounsfield Unit > 1000 was investigated retrospectively. We identified 104 patients underwent combined ballistic and laser lithotripsy. Propensity score technique was used to create the laser and ballistic lithotripsy groups. Groups were matched on stone size, stone density, and Guy's stone score. Primary end point of the study was to compare the stone free rate (SFR), complication rates, and duration of surgery. Mean age of the population was 49.4 +/- 6.1, stone size was 24.6 +/- 6.3 mm, and stone density was 1215 +/- 89 HU. The groups were similar for age, stone size, stone density, and Guy's stone score. The SFR and the complication rates of the 3 groups were similar (p = 0.67). The duration of the surgery was shorter in the combined group (46.1 +/- 6.3 min) compared to the laser lithotripsy (54.5 +/- 6.6 min) and ballistic lithotripsy (57.2 +/- 6.9 min) groups. Both laser and ballistic lithotripsy are effective methods for stone fragmentation during mini-PNL. Combined use of both methods has the potential to improve the fragmentation rates and diminish the operative times in case of high density stones
