The use of mother tongue (L1) has always been one of the most controversial issues in the field of foreign language teaching due to the changes in the foreign language teaching approaches and methods. Some strictly opposes the use of L1 in the classroom while others support the judicious use of it for various purposes. The aim of this study is to find out the perceptions of instructors and learners on using L1 in language classes and in which particular situations they prefer to use L1. Another purpose of this study is to determine whether language level of the students has an effect on the amount of L1 use in preparatory schools. In order to investigate the instructors' and the students' views, two questionnaires were applied to the participants: one prepared for students and one for instructors. The questionnaires conducted in the current study are quantitative in terms of closed items, and qualitative in terms of the open ended questions. In this respect, this study is designed as a mixed method design using both quantitative and qualitative data. 107 instructors and 170 intermediate students were sampled for this research. The findings revealed that student participants had a positive attitude towards the use of mother tongue (Turkish) in the classroom. Whereas, instructors had a negative attitude in spite of their positive comments to the open-ended question asking about their views on the use of L1. Besides, using mother tongue had different functions in the teaching and learning process such as "lowering the affective filter", "simplifying complicated concepts", etc. Therefore, the present study indicated that using mother tongue of learners had an important role in language classes

Bu düzenlemeye göre, bölge adliyemahkemelerinin kesin nitelikteki kararlarına karşı yine ilgili daireye itiraz edilmesiöngörülmüştü. 17.10.2019 tarihinde kabul edilen 7188 sayılı Kanun ile CMK’nın308/A maddesinde yapılan değişiklikle, madde günümüzdeki halini almıştır. Bu çalışmada, bölge adliye mahkemesi C.Başsavcılığının itirazı çeşitliyönleriyle etraflı bir şekilde açıklanmaya çalışılmıştırObjection of the Chief Public Prosecutor’s Offi ce of the regional court of justice, which is not written in the original version of the Code of Criminal Procedure no. 5271, was added into the Code of Criminal Procedure as clause 308/A in accordance with the Law no. 7035 adopted on 20.7.2017. According to this regulation, it was provided that objection would be made to the relevant department against fi nal decisions of the regional courts of justice. The clause reached its current version with the amendment made in clause 308/A of the Code of Criminal Procedure no. 7188 adopted on 17.10.2019. According to clause 308/A of the Code of Criminal Procedure, the department whose verdict is objected will review the objection within the shortest period of time possible, correct its verdict if it sees the objection fi t, or send the fi le to the board of presidents of criminal chambers to assess the objection. Objective: V-Y advancement flaps are pliable local flaps that can be used in nearly every part of the body with relatively minimal technical difficulty. These flaps provide the best color and quality match, especially in the facial region. With effective planning, donor site morbidity can be acceptable, even on the face. Methods: The results of 38 patients who underwent 53 V-Y advancement flap facial reconstructions were analyzed retrospectively. The defects were evaluated according to their anatomical region, etiology, postoperative complications, and final aesthetic results. Results: V-Y advancement flaps were used for defects that were either not amenable to primary repair or had a high risk of cosmetic distortion. The defects were located in the cheek (9 patients), nasolabial fold (7 patients), nose (6 patients), forehead (5 patients), preauricular area (3 patients), infraorbital region (3 patients), lower lip (1 patients), gingivobuccal sulcus (2 patients), and medial canthal region (2 patients). In 32 patients, the primary pathology was malignant tumor (22 basal cell carcinomas, 9 epidermoid carcinomas, 1 malignant eccrine poroma). Etiologies of the other patients' defects were benign tumors (n=2), traumatic wounds (n=2), and contractures (n=2). Conclusions: Despite the widespread use of V-Y flaps in surgical practice, the technique continues to demonstrate additional advantages, particularly in facial reconstruction. They are not only applicable for medium and small defects, but can be used for all defects with sufficient surrounding tissue. They enable various modifications and the elevation of multiple flaps

There are some electrotherapy studies regardingtranscutaneous electrical nerve stimulation (TENS) and interferential current therapy (IFT), but none have examined their synergistic effects. Thisinvestigation was performed to assess the effectiveness of TENS/IFT in relieving chronic low back pain (LBP). Material and Methods: In this double-blind,placebo-controlled study, a total of 123 patients were divided into four groupsusing a computer method. Group 1 received TENS, Group 2 IFT, Group 3 combined TENS/IFT, and Group 4 sham TENS/IFT five times a week for 3 weeks. All participants also received hotpack therapy. Lumbar range of motion (ROM)was assessed via an inclinometer and the modified Schober test, and pain during activity was evaluated using a Visual Analog Scale (VAS) along with patientand physician global assessments. The Rolland-Morris Disability Questionnaire(RMDQ) was administered to determine functional capacity. Results: Groups 1,2, and 3 showed significant improvement, but Group 4 showed none (p?0.001).Group 3 was superior to Group 2 with regard to the RMDQ, VAS, and global assessments (p?0.001). Conclusion: Combined TENS/IFT was more effectivethan IFT in improving functional level and decreasing pain during activity in patients with chronic LBP, however it was not superior to TENS therapy alone. The findings of this study offer insight that can be applied to future investigationsconcerning combined TENS/IFT therapy with different application frequenciesand modes or other combined therapies. We observed marked improvement infunction and pain during activity with the combination of both therapies; however, improvement was not significantly greater to that achieved with TENS therapy alone. Combined therapy was also superior to the other treatments withrespect to doctor and patient global assessments, but this was not significantlydifferent compared with that in the TENS and IFT alone groups. No improvements were found in the control (sham) group

Objective: The intrinsic connectivity networks mostly affected in Alzheimer’s disease (AD) are the default mode (DMN) and salience networks (SN). The capacity of the functional connectivity (FC) changes in these networks in differentiating subjective cognitive impairment (SCI), earliest stage of cognitive decline along the AD continuum, from objectively diagnosed mild cognitive impairment (MCI) is not yet shown. Therefore, we investigated FC changes in DMN and SN among SCI, MCI and AD dementia (ADD) groups. Methods: Resting-state fMRI data of 88 participants (21 ADD, 34 MCI, 33 SCI) were collected with 3T MRI scanner. FC of 11 regions of interest (ROI) corresponding to DMN and SN nodes were analysed using CONN-toolbox (https://web.conn-toolbox.org/). Cluster level significance threshold in the F test among the 3 groups was set at p(FWE-corr) <0.0045 (Bonferroni corrected according to number of ROIs). For ROIs with significant difference in the F test, t-tests were performed between pairs of groups, and results with p(FWE-corr) <0.017 were reported. Results: In ADD compared to both MCI and SCI, FC of posterior cingulate cortex node of DMN with temporal and occipital cortices, FC of anterior cingulate cortex (ACC) node of SN with right insula and temporal cortices and FC of right and left anterior insula nodes of SN with ACC significantly decreased. Additionally, for ADD vs MCI comparison, FC of left anterior insula node of SN with the right insula, and for ADD vs SCI comparison, FC of the right and left anterior insula nodes of SN with the supplementary motor area were reduced. Conclusion: The findings reveal that the FC changes of DMN and SN in AD continuum occur in the advanced stages of the disease with no distinctive change between SCI and MCI stages

The effect of hallux valgus surgery on quality of life

Background: Health-related quality of life (QOL) is defined as a patient’s subjective perception of his or her own health. Insufficient data exist on QOL of patients who undergo a hallux valgus (HV) operation. We used a 36-item short form survey (SF-36) to measure QOL of such patients. Our aim was to evaluate the effect of HV on QOL and to identify QOL determinants. Methods: Fifty patients who underwent surgery for HV between 2015 and 2017 were included in the study. The SF-36 questionnaire was applied to the patients before and after surgery. Patients’ medical records were examined to identify possible factors affecting QOL such as age, gender, body mass index, duration of symptoms, or smoking. Results: The mean age of the patients was 55.6 6 3.8 years, and 42 of the 50 patients were women. The mean duration of disease was 12 6 3.7 years. The surgery improved QOL scores for general health, emotional well-being, role limitations due to personal or emotional problems, physical functioning and bodily pain. However, the changes in scores for vitality and social functioning were not statistically significant. Lower postoperative QOL scores for emotional well-being and bodily pain were significantly associated with age and duration of the symptoms. Compared to the mean QOL of healthy adult Turkish population, all scores in subdimensions were lower. Conclusions: This study shows that HV in adults has a negative impact on general health, bodily pain, physical functioning, physical and emotional well-being rather than social well-being and vitality

Luteal phase support in fresh and frozen embryo transfer cycles

    Get PDF
Nearly 5 million babies have been delivered following assisted conception (IVF/ICSI) and the demand is increasing. Meticulous ovarian stimulation and well programmed luteal phase support are the landmarks of treatment success. Although the importance of luteal phase support in IVF/ICSI cycles is well established, the optimal route, dose and duration of this support is still a matter of debate. Regardless of the ovarian stimulation, parenteral and vaginal progesterone has been one of the most common routes. However, oral or subcutaneous routes are also well-investigated and reveal satisfactory clinical outcomes. It is obviously critical to choose a progesterone with adequate clinical efficacy and patient tolerability as well. Moreover, fresh and frozen embryo transfer cycles markedly different from each other in terms of physiological changes and luteal support concept should be modified accordingly. The aim of this narrative review is to provide evidence-based take home messages for the luteal phase support in either fresh or frozen embryo transfer cycles in the context of a recent scientific evidence

Objective To evaluate the short-term changes in subfoveal choroidal thickness (SFCT), ganglion cell complex (GCC) analysis, and retinal nerve fiber length (RNFL) of patients with chronic obstructive pulmonary disease (COPD) in a 3-month follow-up. Materials and methods Forty-eight patients with COPD (96 eyes) and 40 control subjects (80 eyes) were enrolled in our study. COPD patients were grouped according to disease severity as Group 1 (mild–moderate) and Group 2 (advanced). GCC, RNFL, and SFCT analysis by Cirrus SD-OCT were obtained for all eyes, in two consecutive examinations with a 3-month interval. Results SFCT in Group 2 was lower than Group 1 and control group in the initial and 3rd month examination (p < 0.001, respectively). Inferior RNFL in Group 2 were lower than control group in the initial and 3rd month examination (p = 0.002, p < 0.001, respectively) Temporal RNFL were lower in Group 2 than Group 1 in 3rd month examination (p = 0.009). Average, superior, superotemporal, inferior, and inferonasal GCC analyses of the Group 2 were lower than control group both in the initial and 3rd month examination (p = 0.001, p < 0.001, respectively) SFCT, average, and superior GCC of Group 2 were significantly reduced during the 3-month follow-up (p < 0.001). Conclusion Hypoxia is thought to be the underlying mechanism in COPD, which may influence retinal and choroidal OCT parameters. Decrease in blood flow of optic nerve head, increased vascular resistance, and reduced blood flow in choroid may affect the visual ability in these patients, which should be kept in mind during their follow-u
