    Manajemen Beberapa Fibroid Rahim Pada Kehamilan

    ABSTRACT Uterine Fibroids is a benign tumors originating from the smooth muscle of the uterus. In general, uterine myomas are asymptomatic and are found incidentally during ultrasound examination of pregnancy. We report that a 39-year-old woman with G2P1001 who admitted she was 40 weeks pregnant came with complaints of intermittent abdominal pain since 04.00 in the morning, which was getting worse 2 hours before being admitted to the hospital. Complaints are accompanied by the discharge of blood mucus through the vagina. The patient was married 2 times and the distance between the first child and the second was 19 years. The patient's vital signs were within normal limits. On physical examination, there was a palpable mass with a smooth surface that seemed to stick to the uterus. Obstetrical examination revealed: uterine fundal height: 32cm, HIS + adequate, fetal heart rate 140 beats/minute, performance; head position. When a vaginal touch was performed, it was found that there was an opening of 1 cm with 25% thinning, and the membranes were still intact. The patient was diagnosed with G2P10001  40-41 weeks of gestation + single alive Intra uterine + old secondary primary + uterine fibroids. The patient underwent a cesarean section, and the outcome of the baby and mother was good. Intraoperative findings found that 2 uterine myomas were measuring more than 5 cm located in the subserosa and intramural so the patient underwent a supravaginal hysterectomy. The age of the patient above 35 years is one of the considerations for tubectomy.Fibroid Rahim adalah tumor jinak yang berasal dari otot polos rahim. Pada umumnya mioma uteri tidak menunjukkan gejala dan ditemukan secara kebetulan pada pemeriksaan USG kehamilan. Kami melaporkan bahwa seorang wanita 39 tahun dengan G2P1001 yang mengaku hamil 40 minggu datang dengan keluhan nyeri perut intermiten sejak pukul 04.00 pagi, yang semakin memburuk 2 jam sebelum masuk rumah sakit. Keluhan disertai keluarnya lendir darah melalui vagina. Pasien menikah 2 kali dan jarak antara anak pertama dan kedua adalah 19 tahun. Tanda-tanda vital pasien dalam batas normal. Pada pemeriksaan fisik, teraba massa dengan permukaan halus yang tampak menempel pada uterus. Pemeriksaan obstetri didapatkan: tinggi fundus uteri: 32cm, HIS+ adekuat, denyut jantung janin 140 denyut/menit, kinerja; posisi kepala. Saat dilakukan vaginal touch didapatkan adanya bukaan sebesar 1 cm dengan penipisan 25%, selaput ketuban masih utuh. Pasien didiagnosis dengan G2P10001 usia kehamilan 40-41 minggu + intra uterin hidup tunggal + primer sekunder lama + fibroid uteri. Pasien menjalani operasi caesar, hasil bayi dan ibu baik. Temuan intraoperatif ditemukan 2 mioma uteri berukuran lebih dari 5 cm yang terletak di subserosa dan intramural sehingga pasien menjalani histerektomi supravaginal. Usia pasien di atas 35 tahun merupakan salah satu pertimbangan untuk tubektom

    In Sanggau Regency there are still dental and oral health problems, including Tayan Hilir District. Initial data were obtained through questionnaires on 42 children at MTs Negeri 03 Tayan about knowledge of brushing teeth, food consumption, and dental examinations. The results obtained, the average knowledge is good (54.8%) and moderate (45.2%). This study aims to describe and analyze the effect of the incidence of dental caries (DMF-T), dental and oral hygiene status (OHI-S) on the tooth brushing behavior of adolescent girls based living areas on the river coast or highlands in Tayan Hilir District, Sanggau Regency. The tool used is a cross-sectional questionnaire, Spearmand Kendall's Correlate test, and Regression, with univariate, bivariate, and multivariate analysis. The results of the study are that DMF-T, OHI-S are influenced by tooth brushing behavior by 57% with siq value. 0.000<0.05. Living areas on the coast of rivers or highlands affect DMF-T, OHI-S by 36.8%, age affects DMF-T, OHI-S by 10.7% and cariogenic food types affect DMF-T, OHI-S by 70 %. Overall, DMF-T, OHI-S, is influenced by tooth brushing behavior of adolescent girls based on living area on the river coast or highlands by 22.6% with siq value. 0.05≤0.05. The conclusion is that DMF-T, OHI-S are strongly influenced by tooth brushing behavior based on where they live on the river coast or highlands.Pada Kabupaten Sanggau masih terdapat masalah kesehatan gigi dan mulut, termasuk Kecamatan Tayan Hilir. Data awal diperoleh melalui kuesioner pada 42 anak MTs Negeri 03 Tayan tentang pengetahuan menggosok gigi, konsumsi makanan, dan pemeriksaan gigi. Hasil yang diperoleh, rata-rata berpengetahuan baik (54,8%) dan sedang (45,2%). Penelitian ini bertujuan untuk mendeskripsikan dan menganalisis pengaruh kejadian karies gigi (DMF-T), status kebersihan gigi dan mulut (OHI-S) terhadap perilaku menyikat gigi remaja putri berdasarkan daerah tinggal di pesisir sungai atau dataran tinggi di Kecamatan Tayan Hilir, Kabupaten Sanggau. Alat yang digunakan adalah kuesioner dengan cara cross sectional, uji Correlate Spearmand Kendall’s dan Regresi, dengan analisa univariat, bivariat, multivariat. Hasil penelitian yaitu DMF-T, OHI-S, dipengaruhi oleh perilaku menyikat gigi sebesar 57% dengan nilai siq. 0,000<0,05. Daerah tinggal di pesisir sungai atau dataran tinggi mempengaruhi DMF-T, OHI-S, sebesar 36,8%, umur mempengaruhi DMF-T, OHI-S, sebesar 10,7% dan jenis makanan kariogenik mempengaruhi DMF-T, OHI-S sebesar 70%. Secara keseluruhan DMF-T, OHI-S, dipengaruhi oleh perilaku menyikat gigi remaja putri berdasarkan daerah tinggal di pesisir sungai atau dataran tinggi sebesar 22,6% dengan nilai siq. 0,05 0,05. Kesimpulannya adalah DMF-T, OHI-S sangat dipengaruhi oleh perilaku menggosok gigi berdasarkan tempat tinggal di pesisir sungai atau dataran tinggi

    Breadfruit Flour, Food to Lower Blood Sugar Levels in Mice Model Diabetes Mellitus

    Consumption of foods with high amylose content (>25%) as well as a low glycemic index (<55) is able to improve the insulin sensitivity of people with diabetes mellitus. Breadfruit has in about 23-60 which means that breadfruit is safe to consume. The purpose of this research is to find out the levels of reduced sugar, amylose, and blood sugar levels mice after being fed breadfruit flour. This research used a Complete Randomized Design using 5 treatments namely normal control group (KN), negative control (K-), positive control (K+), STZ + breadfruit flour dose 5 g (P1) and STZ + breadfruit flour dose 10 g (P2). Breadfruit is taken and then processed into breadfruit flour, after which testing of reduced sugar content and amylose breadfruit flour. Before treatment, all mice were measured for initial glucose levels. After that in groups K (-), K (+), P1, and P2 injected STZ on days 1, 2, and 3. After 3 days, the blood glucose level (post-STZ) was measured again, then compared to the blood glucose level on the first day before being given STZ. If there is an increase in blood glucose levels of mice that is to ±128 mg / dL then mencit is considered to be diabetic. Then in group K (+) drug given Metformin, P1 and P2 were fed breadfruit flour with a predetermined dose for 7 days. The results showed that the sugar content of reduced breadfruit was cut by 3.26%. While the level of amylose in breadfruit flour is 19.3%. Breadfruit flour can also lower the sugar content of mice

    Layout and Design of Dental Clinics During the Covid19 Pandemic

    The Coronavirus Disease (COVID-19) pandemic has occurred since March 2020. Dentists and dental therapists have a very high risk of transmitting the SARS-CoV-2 virus. Dentists and dental and oral therapists are not spared from this virus because transmission can be through the release of aerosols or droplets containing the virus or direct contact with mucous membranes, oral fluids, and instruments and surfaces contaminated with the virus. For this reason, an appropriate layout and design approach is needed to avoid these aerosols or droplets. Objective: to get an overview of the Layout and Design of a Dental Clinic that is responsive to the spread of the virus during the Covid-19 Pandemic at a Private Dental Clinic in the Jakarta Region 2022.Methods: This research is a descriptive study with a sample of 30 private dental clinics in the South Jakarta area using random sampling technique. Results: The results showed that private dental clinics in the South Jakarta area which had complete availability of complementary equipment 10 clinics (33.3%), 17 clinics (56.7%) were incomplete, 3 clinics (10%) were incomplete, compliance with the location and design as well as the percentage of complementary tools that are very suitable, appropriate, and not suitable are the same, namely (26.7%) and (20%) are not very suitable, the air flow is in accordance with the PDGI guidelines, namely (36.7%), (3 ,3%) is not suitable, and (60%) is not suitable. Conclusion: Most of the private dental clinics in the South Jakarta area have, the layout of the complementary equipment according to the PDGI guidelines, the criteria are very suitable, still less than 1/3 of the number of respondents and 1/5 of the number of respondents who are not very suitable, complementary tools are incomplete and some large has inappropriate airflow.Pandemi Coronavirus Disease (COVID-19) di Indonesia terjadi  sejak Februari/Maret 2020. Dokter gigi dan terapis gigi memiliki risiko yang sangat tinggi terhadap penularan virus SARS-CoV-2. Dokter gigi dan terapis gigi dan mulut tidak luput dari sasaran virus ini karena penularannya dapat melalui pelepasan aerosol atau droplet yang mengandung virus atau kontak langsung dengan membran mukosa, cairan mulut, dan instrumen dan permukaan yang terkontaminasi virus. Untuk itulah diperlukan pendekatan tata letak dan desain yang tepat untuk menghindari aerosol atau droplet tersebut.Tujuan: untuk mendapatkan gambaran Tata Letak dan Disain  Klinik Gigi yang tanggap terhadap penyebaran virus pada masa Pandemi Covid-19 di Klinik Gigi Swasta Wilayah Jakarta 2022. Metode: penelitian ini merupakan penelitian deskriptif dengan sampel 30 klinik gigi swasta di wilayah Jakarta Selatan dengan pengambilan Teknik sampel random sampling Hasil: hasil penelitian menunjukkan bahwa klinik gigi swasta di wilayah Jakarta Selatan yang memiliki ketersediaan alat pelengkap lengkap 10 klinik (33,3%), 17 klinik (56,7%) kurang lengkap, 3 klinik (10%) tidak lengkap, kesesuaian tata letak dan desain serta alat pelengkap perentase kriteria sangat sesuai, sesuai, dan tidak sesuai ternilai sama yaitu (26,7%) dan (20%) sangat tidak sesuai, aliran udara sesuai dengan panduan PDGI yaitu (36,7%), (3,3%) kurang sesuai, dan (60%) tidak sesuai. Kesimpulan: Sebagian besar klinik gigi swasta di wilayah Jakarta Selatan memiliki, tata letak alat pelengkap yang sesuai dengan panduan PDGI kriteria sangat sesuai masih kurang dari 1/3 jumlah responden dan 1/5 jumlah responden sangat tidak sesuai, alat pelengkap yang kurang lengkap dan sebagian besar memiliki aliran udara yang tidak sesuai

    ACCURACY OF CANCER ANTIGEN 125 AND ALBUMIN BEFORE OPERATION TO PREDICT THE OPERATION OUTCOME IN PATIENTS WITH ADVANCED STAGE EPITHELIAL TYPE OVARIAN CANCER

    Ovarian cancer is a malignancy that grows and develops in the ovaries with a high mortality rate. Primary debulking surgery (P.D.S.) is diagnostic and therapeutic and has become the standard option. CA-125 levels were increasing with the worsening disease conditions in ovarian cancer. In addition, preoperative albumin levels were strongly associated with worse ovarian cancer outcomes. This study assessed the prediction of CA-125 markers and albumin levels on P.D.S. outcomes. This research is a diagnostic test conducted at Prof. Dr I.G.N.G. Ngoerah Hospital. The study sample consisted of 50 women aged 18 and over who underwent P.D.S. from 2018 to 2020 with P.A. results of advanced stage (III-IV) epithelial-type ovarian cancer. CA-125 and albumin levels were assessed before surgery and compared with the findings of P.D.S., which were grouped into suboptimal debulking and optimal debulking. There were no significant differences in the characteristics of the two groups. The accuracy of CA-125 levels on the outcome of P.D.S. surgery with a sensitivity of 75.6%, specificity of 55.6%, PPV of 88.6%, N.P.V. of 35.3% and accuracy of 72%. While the accuracy of albumin levels on the outcome of P.D.S. surgery with a sensitivity of 55.6%, specificity of 51.2%, PPV of 20%, N.P.V. of 84% and accuracy of 52%. CA-125 levels were higher in the suboptimal group but not statistically significant, but the sensitivity (70%) and specificity (80%) were still high, so they still have diagnostic value for ovarian cancer. The relationship between albumin levels and ovarian cancer is multifactorial, so it cannot be used to predict surgical outcome

    THE DEVELOPMENT OF CLINICAL RISK MANAGEMENT IMPLEMENTATION MODEL IN IMPROVING QUALITY AND PATIENT SAFETY

    Clinical risk management plays an important role for hospitals in promoting patient safety and improving service quality in order to reduce or prevent risk related to patient care. The purpose of this study was to develop a model for implementing clinical risk management in improving quality and patient safety in class C private hospital in West Jakarta. This research using Participatory Action Research method. Participants in this study amounted to 20 people divide into 8 clinical units in the hospital. Data were collected using focus group discussions (FGD), using observation sheets, and quantitative assessment using questionnaires. Qualitative data analysis was performed by content analysis and quantitative data analysis using Wilcoxon test. The results of this study are clinical risk register model as a form of clinical risk management implementation, 8 clinical units in the hospital had clinical risk registers and the result of Wilcoxon test showed that there were significant differences in the level of participant knowledge about clinical risk management before and after the implementation (Asymp. Sig. value of 0.000 < α 0.05)

    COMPARISON OF IN VIVO CYANIDE EXAMINATION WITH PICRIC ACID AND PRUSIAN BLUE METHODS

    This study aims to determine the comparison of invivo cyanide examination by comparing the Pyric Acid method with the Blue Prusian method in the stomachs of experimental animals. It is an imental expre research. Samples of 40 white Wistar rats weighed between 100-200 g. Then the 40 rats were given lethal dose cyanide orally and divided into 2 groups: 1 group will be detected cyanide by picric acid method and 1 other group using blue prusian method. In such groups are taken gastric tissue. Consecutive examination time intervals on days 1 and 7. The results of cyanide examination with the Picric Acid method were obtained on day 1 of positive gastric tissue cyanide 94.7%, while with the Blue Prusian method on day 1 examination positive cyanide 85%. The results of cyanide examination with the PikraAcid t method on day 7 found that the gastric tissue was positive for cyanide 36.8%, while with the Blue Prusian method 35%. Both methods showed high sensitivity to cyanide examination in the gastric postmortem (P<0.05)

There were 8 articles selected and received Grade A. 8 articles were obtained research conducted in developing countries. There are 7 factors that affect the success of exclusive breastfeeding for working mothers, namely knowledge, educational status, breastfeeding facilities, employment status, husband support, family support, and health worker support

    Therapeutic Transactions And Informed Concent And Liability Agreement Between Doctors And Patient

    In the therapeutic transaction, the relationship between patient and health service providers tends to bring conflict. Many factors caused that conflict, e.g., lack of patient health understanding, inequality of position between the patient and the provider before the law, and the imperfection of service quality handed over by the Provider. Viewed from a legal standpoint, therapeutic transactions have far-reaching consequences of a reciprocal nature between the patient and the doctor. The execution of informed consent is considered merely a formality to be signed. Juridically, the doctrine of informed consent contains the obligations imposed on doctors and the rights of patients to be fulfilled by doctors. Among the rights of the patient in the doctrine of informed consent are: the right to obtain information about his or her illness and the action to be taken against him; the right to obtain answers to questions he or she asks; the right to choose alternatives; and the right to refuse to initiate action. The right to safety, the right to be informed, the Right to Choice, and the right to Be Heard are also basic consumer rights that doctors must fulfill. These rights are guaranteed by the Consumer Protection Act. The process of informed consent is a manifestation of the preserved equality of relations between doctors and patients who are respectful and communicative; both have rights and duties to be respected, and together they determine the best course of action for patients in order to achieve the desired purpose of medical service. An informed consent is also deemed valid if: (a) the patient has been given explanation or information; (b) the patient or his authorized representative is competent to give a decision or consent; and (c) the consent must be given voluntarily. The process of obtaining informed consent is not running as it should

    Chemical Analysis of Ethanolic Extract of Ripe Fruit, N-Hexane Fraction of Half-Ripe Fruit, and Water Fraction of Half-Ripe Fruit of Sesoot (Garcinia Picrorrhiza Miq.) with Liquid Chromatography-Mass Spectrometry (Lc-Ms)

    Garcinia is a plant belonging to the family of Guttiferae or Cluciaceae and has more than 180 famous species. Most of Garcinia species contains many prenelated xanthones, biflavonoids, and benzophenones. Using Liquid Chromatography - Mass Spectrometry (LC-MS), the development of electrospray ionization (ESI) could be detected and could be applied to a wide range of molecular biology such as xanthones, biflavonoids, and benzophenones and their derivatives. This method with a high speed of scanning at once could be measured a high degree of multiplexing and many components can be measured with a single analysis. These results were separated and analyzed using literature software of Dictionary of Natural Product 2006. The ethanolic extract of ripe fruit (IC), the n-hexane fraction of half ripe (IIB), and the water fraction of half ripe fruit (IVB) of sesoot (G. picrorrhiza Miq.) showed that the major compounds of them were xanthones, benzophenones with their derivatives,  furanone and pyranone which were proven as cytotoxic to the brine shrimp nauplii using Brine Shrimp Lethality Test (BSLT) and to the doxorubicin-resistant breast cancer cell line of MCF-7, the doxorubicin-sensitive breast cancer cell line of T47D
