El impacto de la crisis de la posguerra con Chile en la comedia costumbrista limeña: Acisclo Villarán (1841-1927), Abelardo Gamarra (1852-1924) y Manuel Moncloa y Covarrubias (1859-1911)
Esta investigación estudia el impacto de la crisis de la posguerra con Chile en el teatro costumbrista post bellum, a través de la obra teatral de tres comediógrafos: Acisclo Villarán (1841-1927), Abelardo Gamarra (1852-1924) y Manuel Moncloa y Covarrubias (1859-1911); considerando al teatro costumbrista como la expresión artística privilegiada de esta crisis post bellum. Se analizan y describen los cambios y continuidades que se produjeron desde una perspectiva comparada, principalmente con el tipo de costumbrismo que se cultivó durante el periodo ante bellum o de inicio de la guerra (década de 1870)

IMPACTO DE LA ADICCIÓN A REDES SOCIALES EN LA SALUD MENTAL DE LOS ESTUDIANTES DE MEDICINA HUMANA, EN TIEMPOS DE COVID-19: IMPACT OF ADDICTION TO SOCIAL NETWORKS ON THE MENTAL HEALTH OF HUMAN MEDICINE STUDENTS, IN TIMES OF COVID-19
Objective. To determine the association of addiction to social networks and the consequences in mental health in human medicine students of the Ricardo Palma University, year 2021. Materials and methods. Cross-sectional observational study, with the participation of 454 human medicine students from first to fifth year, chosen by stratified random sampling proportional to size. The instrument was validated, obtaining a Cronbach's Alpha (α=0.90), the questionnaires used were: Addiction to Social Networks (ARS), Depression, Anxiety, Stress Scale (DASS-21) and Pittsburgh Sleep Quality Index (PSQI). In order to evaluate the association between variables, we used crude and adjusted prevalence ratio (PR), using the Poisson regression model with the logarithmic link and with the 95% confidence interval. Results. 65.4% were women, with an average age of 21.2 years. A significant association was found between high social network addiction and mental health (PRc: 2.59; 95% CI: 1.92-3.50; p< 0.01) and, likewise, between high and medium levels of social network addiction. social and with depression, anxiety, stress (p< 0.01) and sleep quality (p<0.010), respectively. 41%, 49.1% and 9.9% of the students presented low, medium and high addiction to social networks, respectively. 57.5% had depression, 56.4% anxiety, 70.5% stress and 77.3% poor sleep quality. Conclusions. There was a strong association between ARS and mental health. Therefore, it is recommended that university students visit mental health specialists quarterly for proper identification, evaluation, and management of the problems generated by ARS.

Determinar la asociación de la adicción a redes sociales y las consecuencias en salud mental en estudiantes de medicina humana de la Universidad Ricardo Palma, año 2021. Materiales y métodos. Estudio observacional transversal, participaron 454 estudiantes de medicina humana de primero a quinto año, elegidos por muestreo aleatorio estratificado proporcional al tamaño. El instrumento fue validado, obteniendo un Alpha de Cronbach (α=0,90), los cuestionarios empleados fueron: Adicción a redes sociales (ARS), escala de depresión, ansiedad, estrés (DASS-21) y índice de la calidad del sueño de Pittsburgh (PSQI). A fin de evaluar la asociación entre variables, utilizamos razón de prevalencia (RP) crudo y ajustado, mediante el modelo de regresiones de Poisson con el enlace logarítmico y con intervalo de confianza del 95%. Resultados. El 65,4% eran mujeres, con edad promedio 21,2 años. Se evidenció asociación significativa entre adicción a redes sociales alto y salud mental (RPc: 2,59; IC95%: 1,92-3,50; p< 0,01), así mismo, los niveles alto y medio de adicción a redes sociales con síntomas depresivos, ansiedad, estrés (p< 0,01) y calidad de sueño (p<0,01), respectivamente. El 41%, 49,1% y 9,9% de los estudiantes presentaron adicción a redes sociales bajo, medio y alto respectivamente. Un 57,5% presentaron síntomas depresivos, 56,4% ansiedad, 70,5% estrés y 77,3% mala calidad de sueño. Conclusiones. Encontramos una fuerte asociación entre ARS y salud mental. Por lo cual se recomienda a los universitarios que sean evaluados por especialistas de salud mental para una correcta identificación, evaluación y manejo de los problemas de la ARS.

Transformando la educación médica del siglo XXI: el rol de la educación médica basada en competencias: Transforming medical education in the 21st century: the role of competency-based medical education
Introduction: With the changing landscape of medical education, it is crucial to critically examine existing approaches and frameworks. competency-based medical education (CBME) emerges as a promising paradigm shift, which prioritizes learner-centered, outcome-oriented development of skills, knowledge, and attitudes. Objective: To identify the essential structural and procedural adjustments needed for effective implementation of CBME. It underscores the importance of reorganizing educational institutions, adapting instructional and assessment techniques, and fostering acceptance of CBME in the classroom. Results: To effectively deliver CBME, it is imperative to restructure educational institutions to focus on competency-based curricula and personalized instruction. Also, there is a need to modify teaching and assessment strategies to facilitate continuous assessment and active learning. Conclusions: Adopting CBME principles can enable medical education to produce professionals equipped with the skills necessary to meet the dynamic demands of modern medicine.Introducción: Con el panorama cambiante de la educación médica, es crucial examinar críticamente los enfoques y marcos existentes. La educación médica basada en competencias (EMBC) surge como un prometedor cambio de paradigma, que prioriza el desarrollo de habilidades, conocimientos y actitudes explícitas centrado en el alumno y orientado a los resultados. Objetivo: Identificar los ajustes estructurales y procedimentales esenciales necesarios para una implementación eficaz de la EMBC. Se subraya la importancia de reorganizar las instituciones educativas, adaptar las técnicas de instrucción y evaluación y fomentar la adopción de la EMBC en el aula. Resultados: Para impartir con eficacia la EMBC, es imperativo reestructurar las instituciones educativas para que se centren en planes de estudios basados en competencias y en la enseñanza personalizada. Asimismo, es necesario modificar las estrategias de enseñanza y evaluación para facilitar la evaluación continua y el aprendizaje activo. Conclusiones: Adoptar los principios de EMBC puede permitir que la educación médica produzca profesionales equipados con las habilidades necesarias para satisfacer las demandas dinámicas de la medicina moderna.
Indicadores de gestión en investigación 2023 INICIB-URP: Research management indicators 2023 INICIB-URP
En respuesta a los desafíos emergentes y las demandas de la sociedad, la Universidad Ricardo Palma ha fortalecido su compromiso con la investigación como un pilar fundamental de su política institucional. A lo largo de los últimos años, el Instituto de Investigaciones en Ciencias Biomédicas (INICIB), que depende de Rectorado y se encuentra adscrito a la Facultad de Medicina Humana, ha logrado avances significativos, consolidando su contribución al desarrollo de la investigación en la universidad.
La gestión de la investigación es uno de los ejes esenciales en el Instituto de Investigaciones en Ciencias Biomédicas de la Universidad Ricardo Palma. Desde el Rectorado y Vicerrectorado de Investigación se establecen políticas claras para el desarrollo del sistema de investigación, el fortalecimiento de los grupos productivos y la disposición de incentivos y estímulos para estimular la investigación y fomentar la construcción permanente de una comunidad científica. Es una necesidad valorar si los recursos asignados en investigación impactan positivamente en la universidad, y cuantificar y evaluar la tendencia y evolución en el tiempo (1)
Cáncer de cérvico uterino en la pandemia de COVID-19 en comunidades rurales de Ayacucho: Cervical Cancer During the COVID-19 Pandemic in Rural Communities of Ayacucho | 在COVID-19大流行期间,秘鲁Ayacucho农村社区的宫颈癌
Introduction: Due to the COVID-19 pandemic, vulnerable rural women faced significant challenges.
Objective: To explore the situation of cervical cancer during the pandemic from an intercultural perspective among rural women in Ayacucho in 2021.
Methodology: Mixed-method research with a concurrent triangulation design. Sample: Healthcare providers and rural women. Purposeful and reasoned sampling. Techniques: Surveys, in-depth interviews. Data analysis: Data systematization using Excel, SPSS for quantitative data, and Atlas.ti for qualitative data. Level of validity and confidence: triangulation.Results: Information from surveys and interviews was contrasted, revealing that the problem may have alternative solutions within the knowledge of the same communities.Conclusion: Women reported lack of awareness, insufficient information in the Quechua language, and undervaluation of their traditional knowledge. Fear and contagion concerns prevented them from seeking healthcare services. Services were reduced, redirected, or discontinued, often due to budget constraints.Introducción: Debido a la pandemia de COVID-19, las mujeres rurales vulnerables enfrentaron desafíos importantes.
Objetivo: Explorar la situación del cáncer de cuello uterino durante la pandemia desde una perspectiva intercultural entre mujeres rurales de Ayacucho en 2021.
Metodología: Investigación de métodos mixtos con diseño de triangulación concurrente. Muestra: Proveedores de atención médica y mujeres rurales. Muestreo intencionado y razonado. Técnicas: Encuestas, entrevistas en profundidad. Análisis de datos: Sistematización de datos mediante Excel, SPSS para datos cuantitativos y Atlas.ti para datos cualitativos. Nivel de validez y confianza: triangulación.
Resultados: Se contrastó información proveniente de encuestas y entrevistas, revelando que el problema puede tener alternativas de solución dentro del conocimiento de las mismas comunidades.
Conclusión: Las mujeres reportaron falta de concientización, información insuficiente en idioma quechua y subvaloración de sus conocimientos tradicionales. El miedo y las preocupaciones por el contagio les impidieron buscar servicios de atención médica. Los servicios se redujeron, redirigieron o interrumpieron, a menudo debido a limitaciones presupuestarias.
 
Hiperglicemia como factor de riesgo de mortalidad hospitalaria en infarto de miocardio: Hyperglycemia as a risk factor for in-hospital mortality in myocardial infarction
Introduction: Myocardial infarction continues with high mortality rates, from 4.6% to 13.1%. There are predictive risk stratification models, such as the Grace Score, which does not include glycemia as a variable. Patients hospitalized for myocardial infarction with hyperglycemia on admission may have higher mortality, in ST elevated and non-elevated infarcts.
Objectives: The objective of this review is to identify and systematize the evidence on hyperglycemia on admission as a biomarker of mortality and heart failure in acute myocardial infarction.
Materials and Methods: The search was carried out in the MEDLINE database including the MeSH terms hyperglycemia and hospital mortality or heart failure in myocardial infarction, selecting 12 articles.
Results: Hospital mortality was calculated in 11 articles, in 9 of them a significant association was found between hyperglycemia and hospital mortality, both in the bivariate and multivariate analysis, and in 2 articles this association was not demonstrated. For in-hospital mortality, the results of 11 articles included in this review were synthesized. The frequency of occurrence of heart failure was determined in 11 articles, finding a higher frequency in 9 of them. For in-hospital mortality, the results of 11 articles included in this review were synthesized and analyzed, in 8 the analysis was performed in non-diabetics, obtaining OR: 4.15, IC 95% (2.853-6.035), in 3 for diabetics obtaining OR 2.365 IC 95% (1.778- 3,146) and in 6 for the total population finding OR 3,314 (2,910-3,774).
Conclusions: Hyperglycemia on admission is associated with increased mortality and frequency of occurrence of heart failure during hospitalization for myocardial infarction, with evidence of moderate quality.Introducción: El infarto de miocardio continúa con altas tasas de mortalidad, desde 4.6% hasta 13.1 %. Existen modelos predictivos de estratificación de riesgo, como el Score Grace, que no incluye la glicemia como variable. Los pacientes hospitalizados por infarto de miocardio con hiperglicemia al ingreso pueden tener mayor mortalidad, en infarto ST elevado y no elevado.
Objetivos: El objetivo de esta revisión es identificar y sistematizar la evidencia sobre hiperglicemia al ingreso como biomarcador de mortalidad y de insuficiencia cardiaca en infarto de miocardio.
Materiales y Métodos: La búsqueda se realizó en la base de datos MEDLINE incluyendo los términos MeSH hiperglicemia y mortalidad hospitalaria o insuficiencia cardiaca en infarto de miocardio, seleccionando 12 articulos.
Resultados: Mortalidad hospitalaria se calculó en 11 artículos, en 9 de ellos se encontró asociación significativa entre hiperglicemia y mortalidad hospitalaria, en el análisis bivariado y en el multivariado, y en 2 artículos no se demostró dicha asociación. En 11 articulos se determinó la frecuencia de aparición de insuficiencia cardiaca, encontrándose mayor frecuencia en 9 de ellos. Para mortalidad hospitalaria se sintetizaron y analizaron los resultados de 11 artículos incluidos en esta revisión, en 8 se realizó el análisis en no diabéticos, obteniendo OR: 4,15 , IC 95% (2,853-6,035), en 3 para diabéticos obteniendo OR 2,365 IC 95% (1,778-3,146) y en 6 para población total encontrando OR 3,314 (2,910-3,774).
Conclusiones: Hiperglicemia al ingreso está asociada a mayor mortalidad y frecuencia de aparición de insuficiencia cardiaca durante la hospitalización por infarto de miocardio, con evidencia de moderada calidad
Beneficios de la quimioterapia adyuvante en los resultados de supervivencia del cáncer de mama triple negativo pT1N0M0: Benefits of adjuvant chemotherapy on pT1N0M0 triple-negative breast cancer survival outcomes | 辅助化疗对 PT1N0M0 三阴性乳腺癌生存结果的益处
Introduction: Triple-negative breast cancer (TNBC) is notably an aggressive breast cancer (BC) subtype, leading to early relapse and poor prognosis. Effects of adjuvant chemotherapy among early-stage TNBC (pT1N0M0) patients remain unclear in different populations. Objectives: Our study aimed to determine the impact of adjuvant chemotherapy on overall survival (OS) and progression-free survival (PFS) within the specific subset of Peruvian pT1N0M0 TNBC patients (pT1a/b vs. pT1c). Methods: We retrospectively analyzed 2007 TNBC cases diagnosed between 2000-2014 at the Instituto Nacional de Enfermedades Neoplásicas (Lima, Peru). We included only non-metastatic TNBC cases and classified them as pT1N0M0 after surgery. TNBC patients who underwent neoadjuvant chemotherapy were excluded. We describe our population according to the tumor size from the residue disease (pT1a/b vs. pT1c). We used the Kaplan-Meier method test to determine differences in survival curves for OS and PFS. A Univariate Cox proportional hazards model was used to identify risk factors for PFS. Results: Our study cohort included 124 TNBC patients. Around 65.3% (n=81) were undergoing adjuvant chemotherapy. Notably, among pT1c patients, this treatment was more prevalent compared to pT1a/b (72.1% vs. 50.0%). Survival analysis showed no significant OS benefit from chemotherapy (HR:2.46,95%CI:0.74-8.13,p=0.13). However, a marked improvement in PFS was noted exclusively in the pT1c subgroup, with patients not treated with chemotherapy offering a prognostic risk (HR:20.10,95% CI:5.54-73.10,p<0.0001). pT1a/b patients demonstrated no benefit from chemotherapy regarding progression (HR:3.07,95% CI:0.27-34.50,p=0.34). Conclusion: Our study highlights that adjuvant chemotherapy significantly improves PFS in pT1cN0M0 TNBC patients but shows no clear benefit for smaller tumors (pT1a/bN0M0). Asma casi fatal: Reporte de un caso en emergencia del Hospital Santa Rosa: Near-Fatal Asthma: Case Report in the Emergency Department of Hospital Santa Rosa | 接近致命的哮喘:圣罗莎医院急诊科病例报告
Introduction: Asthma, classically, is defined as a chronic inflammatory disease of the airways; characterized by a history of respiratory symptoms, such as wheezing, shortness of breath, chest tightness, and cough, that vary over time and in intensity. Near-fatal asthma are situations in which asthma exacerbations can lead to cardiorespiratory arrest, orotracheal intubation and mechanical ventilation, admission to an intensive care unit (ICU); Knowing the characteristics and risk factors that predict this situation in a patient who arrives at the emergency room is very important for early action. Clinical case: We present a 23-year-old patient with a history of asthma since he was 5 years old and irregular treatment; with a stay in the ICU and on mechanical ventilation for almost fatal asthma 8 months before; the clinical signs and the arterial blood gas analysis predicted an almost fatal asthma condition, which is why it was decided to intubate and mechanically ventilate the patient, and then transfer to the ICU with a favorable evolution and discharge 9 days after admission. Conclusion: Patient who already had a previous admission for almost fatal asthma and admission to the ICU, persistence of desaturation in the face of rescue treatment for said pathology; determining factors for deciding rapid sequence orotracheal intubation and transfer to the intensive care unit; with remission of the admission clinical picture and prompt discharge of the patient.Introducción: El asma, clásicamente, se define como una enfermedad inflamatoria crónica de las vías respiratorias; caracterizado por la historia de síntomas respiratorios, como sibilancias, dificultad para respirar, opresión en el pecho y tos, que varían con el tiempo y en intensidad. El asma casi fatal son situaciones en que las exacerbaciones asmáticas pueden conducir al paro cardiorrespiratorio, intubación orotraqueal y ventilación mecánica, ingreso en una unidad de cuidados intensivos (UCI); conocer las características y los factores de riesgo que predicen dicha situación en un paciente que llega a emergencia es muy importante para la actuación temprana del mismo. Caso clínico: Presentamos un paciente de 23 años con antecedente de asma desde los 5 años y tratamiento irregular del mismo; con estancia en UCI y en ventilación mecánica por asma casi fatal 8 meses antes; la clínica y el examen de análisis de gases arteriales predijeron un cuadro de asma casi fatal por lo cual se decidió la intubación y ventilación mecánica del paciente, para luego pasar a UCI con evolución favorable de mismo y alta a los 9 días del ingreso. Conclusión: Paciente que ya tenía ingreso anterior por asma casi fatal y su ingreso a UCI, persistencia de desaturación ante el tratamiento de rescate para dicha patología; factores determinantes para decidir intubación orotraqueal de secuencia rápida y su pase a la unidad de cuidados intensivos; con remisión del cuadro clínico de ingreso y el alta pronta del paciente
