    Severe Lice Infestation in Patient of Atopic Dermatitis

    A 13-year-old female came to us with the complaint of severe itching in her scalp, low-grade fever, loss of weight, and enlarged occipital lymph node. The patient was having very severe infestations of lice in her scalp. The patient was very anxious and irritated because of her itching. We excluded all the options for sexual, mental, and physical abuse by having proper examination and history taking in the absence of any guardian after taking permission from guardians. The patient was having lice and nits only on her scalp, other parts of body hairs were spared from the lice infestations. The patient was maintaining proper hygiene and belongs to an upper socioeconomic family. Along with this patient's mother informed us about her diagnosis of atopic dermatitis. The patient has been diagnosed with atopic dermatitis since she was at the age of 3. Lice infestation is a very common problem in low socio-economic societies and generally, it is considered the person who has the lice infestations must be unhygienic, in reality, it has been seen very often in very neat and clean people. Very severe lice infestations depend on many factors not only in neglected, homeless, or non-hygienic people, it depends on sebum production, many skin diseases e.g. hyperhidrosis, etc

    Post-Infectious Glomerulonephritis in Elderly Patients

    Post-Infectious Glomerulonephritis (PIGN) is a disease of childhood. It is an important cause of acute kidney injury in childhood. It is now frequently identified in elderly people with compromised immune status and risk factors like malignancy and diabetes mellitus (DM). We have reported two cases of patients (age: 61 years and 80 years) which presented to us with unexplained renal failure. Both patients required renal replacement therapy (RRT) in the form of hemodialysis for their renal failure. Auto-immune workup for both patients was negative. Renal biopsy was performed and a diagnosis of PIGN was established on the basis of histopathological characteristics. We treated both cases with intravenous antibiotics. Renal parameters of both patients started improving and both achieved full renal recovery on long-term follow-up of 18 months. PIGN can be seen in the adult population and should not be considered solely a childhood disease

    Episcleritis Followed by COVID-19 Pneumonia after COVID-19 Vaccination

    A 56-year-old man presented at the emergency department with bilateral eye pain, itching and watery discharge for 10 days, fever for seven days, and cough for one day. He had been vaccinated against COVID-19 and had developed ocular symptoms two days after receiving the second dose of the vaccine Sinopharm. Investigations revealed that the nasal PCR was positive for COVID-19. We managed him on the lines of episcleritis and critical COVID-19 pneumonia. The patient later deteriorated and was intubated. He died on the sixth day of his hospitalization as a result of a protracted disease course and ST-segment elevation myocardial infarction (STEMI). We report the development of episcleritis and critical COVID-19 infection after COVID-19 vaccination to emphasize the consideration of antibody-dependent enhancement after vaccination

    Influence of Prognostic Group Classification of Advanced Male Germ Cell Tumor on Treatment Outcomes

    Objective: To investigate the influence of IGCCCG prognostic group classification and histology on treatment outcomes in our local Male Germ Cell Tumors population. Design: It was an observational study. Subjects: Fifty patients diagnosed with advanced male germ cell cancer completing first line treatment, between 2011- 2014. Intervention(s): Patients with proper follow-up record of 5 years post-treatment were included. Patients were classified into good, intermediate and poor prognostic groups according to IGCCCG criteria. Main Outcome Measure(s): The outcomes of all three prognostic groups were measured including response to first line treatment according to MSKCC criteria and five years OS. Survival rates were calculated using Kaplan and Meier method. The level of significance was set at P < 0.05. Results: Mean age of patients was 30.6 years + 9.49 years. Major primary site of involvement was right testicle i.e.56.0%. Complete response was observed in 23 (46.0%) patients. The patients classified into good prognostic group (n=29) had significantly superior (p=0.002) five years OS (86.2%, n= 25). Additionally, CR was higher in seminoma i.e. 12 (63.15%) while it was limited to 11(35.48%) in NSGCT; however, inferior response rate in NSGCT did not translate into statistical significance in 5 years OS. Conclusion: The IGCCCG prognostic groups are an effective tool for predicting treatment outcomes in terms of 5 y Overall survival in our local population and histology plays a secondary role in this regard
