    Resistant Plane Warts Treated by 40% Salicylic Acid and 16% Lactic Acid Combination on the Face

    Human papillomaviruses (HPV) cause warts, which are the benign epithelial proliferation of the mucous membrane and skin. HPV-3 and HPV-10 commonly cause plane warts. Multiple treatment options must be used as no specific treatment option is available. We have a lot of limitations for the face as treatment can cause permanent scarring on the face. We report a case of 41 years immunocompetent male presenting with many plane warts on the face who was successfully treated with topical application of 40% Salicylic acid cream and 16% lactic acid cream made up of 44% of bees wax on lesions after a failure of many other treatment options, a) Topical zinc oxide twice per day for 3 months, b) Topical 5% salicylic acid dispensing cream made in 95% bees wax, c) Topical 5% fluorouracil cream, d) Topical imiquimod cream, e) Topical retinoids cream only at night for one month, f) Oral Isotretinoin 20mg twice per day for 2 months, and 3 sessions of cryotherapy. The patient's last treatment was oral isotretinoin 20 mg twice per day for the last 2 months. In general, doctors avoid using more than 5% salicylic acid, but after a detailed discussion with the patient, we decided to try this combination and succeeded

    Ewing sarcoma also known as peripheral primitive neuroectodermal tumors (ES/PPNETs) usually produce in the chest wall, extraosseous soft tissues, and long or flat bones, however only rarely in solid organs. Although they can develop in any part of the body, development in the adrenal glands is very uncommon. We present a case of Ewing sarcoma (PNET) of the adrenal gland and the extraordinary diagnostic and therapeutic challenges faced during its diagnosis.     Family Needs of Patients Admitted in Intensive Care Units, as Perceived by Family Members and Registered Nurses in Tertiary Care Hospitals of Karachi, Pakistan

    Admission into the Intensive Care Units of the patient is a crucial time not only for the patient but also for the family. The family is faced with various challenges. Healthcare professionals are required to understand their needs. The objective of the study was to identify and compare the family needs of patients admitted to intensive care units as perceived by family members and registered nurses. An analytical cross-sectional study was conducted at Ruth Pfau Civil Hospital and Dow University Hospital Karachi from December 2020 to February 2021. An adopted self-administered questionnaire of the Critical Care Family Need Inventory (CCFNI) was utilized for data collection. The tool consisted of 45 items of family needs in the form of a Likert scale having options rated from 01 to 04. The tool was divided into five subscales of Comfort, Information, Assurance, Support and Proximity. Frequencies and percentages were calculated for the categorical variables. Means and standard deviations were calculated for measuring the responses of the Likert scales. An Independent t-test was applied to compare the means of subscales. A total of 162 participants were included in the study. The total sample comprised 30 nurses and 132 family members. The most important need identified by the family members was to talk to the doctor every day (3.60±0.604) while the staff nurses identified to have a religious leader visit (3.90±0.305). The findings of the current study demonstrate that gap exists between the doctor and family members. The highest score for the dimension of comfort indicated that critical units lack the essential facilities

    Genetic Evaluation of Spontaneous Miscarriages and Couples through Conventional and Modern Diagnostic Tools

    ABSTRACT: Background: Genetic causes of miscarriages have been extensively reported and studied in almost half of the first-trimester pregnancy losses, for the reason genetic investigations of the product of conceptions (POCs) are crucial to help identify possible genetic etiology. This information is helpful for the devastated parents as it provides them with more acceptance of the loss and helps them recognize possible recurrence risks. Objective: The aim of this study is two folds; the first is to corroborate an alternative molecular test, QF-PCR which is rapid, more economical, and has less failure rate, and the second is to explore potential parental chromosomal abnormalities causing recurrent spontaneous miscarriages. Methodology: A total of seventy-six products of conceptions (POCs) were obtained from the Gynecology department of Civil Hospital, Karachi. Further blood samples from forty-five couples were also collected in addition to ninety-three maternal blood samples. All of these samples were subjected to amplify short tandem repeat (STR) for common chromosomal aneuploidies (13, 18, and 21). Real-time QF-PCR was performed on 76 POCs on Sansure Biotech Inc, Changsha, China, using primers D13S631 and D13S634 for chromosome 13, D18S386 and D18S535 for chromosome 18, and D21S1411 and D21S1414 for chromosome 21. Parental blood samples were examined by both QF-PCR and karyotyping for cross-checking with their POC. Chromosome 16 was used as an internal control for the amplification of each reaction. Statistical analysis was done by using SPSS version 24. Results: QF-PCR analysis revealed chromosomal aneuploidies among twenty-nine POCs with the highest number of trisomy 13 followed by trisomy 21 and then trisomy 18 while all the parental samples were normal. Nine cases of chromosomal abnormalities were ruled out through karyotyping from parental blood samples further, a Robertsonian translocation was observed through conventional cytogenetics and interestingly the corresponding POC was found to have trisomy 21. Conclusion: This study further confirmed QF-PCR as a competent technique to be included in the first line of prenatal diagnostic methods. As QF-PCR was found to be less laborious, has lesser turnaround time, is more economical, and has a low failure rate than karyotyping
