Time to Antibiotic in Pediatric Oncology: Nursing role in saving lives
Total survival of childhood cancer in LMIC is estimated to be 20% whereas survival in HIC has exceeded to more than 80%. Pediatric patients undergoing chemotherapy treatment for cancer are at risk for severe infections due to chemotherapy-induced neutropenia, fever in neutropenic oncology patients is an emergency that requires immediate evaluation and intervention. The total Time to Antibiotic (TTA) administration in pediatric oncology patients who present with fever and neutropenia is critical to improving outcomes and decreasing morbidity and mortality. Current guidelines recommend antibiotic administration within 60 minutes of the patient's arrival at the emergency department and delay can result in complications like sepsis and death. There are very strong body of evidence in the literature supporting the Nursing Lead Project in reducing TTA in pediatric oncology patients with febrile neutropenia

The Herlyn-Werner-Wunderlich Syndrome: a Rare Case Report
Obstructed Hemivagina and Ipsilateral Renal Agenesis (OHVIRA) is an aberrated and special type of Mesonephric duct malformation and Müllerian duct anomaly. This mostly reports 5%of Müllerian duct anomalies. The majority of cases of OHVIRA are presented alongside uterine didelphys. The presence of uterus didelphys with OHVIRA are the conditions, collectively referred to as the Herlyn- Werner- Wunderlich Syndrome. Its cause is a complete failure of the Müllerian conduits to fuse. Initial symptoms are typically induced by secretions concentrating within the hemivaginal blockage. Patients are usually of reproductive age. Symptoms in patients with this syndrome are mostly complaints of severe dysmenorrhea, pain in the lower abdomen, palpable mass in the pelvis or vagina abrupt urinary retention, fever, or puking, in addition to atypical vaginal secretions, and abnormal vaginal discharge. Cases seldom reveal complaints of infertility in the majority. Here, we focus on the distinctive symptoms of HWW syndrome and the significance of imaging findings in the diagnosis of the condition in a case of a 23-year-old girl who visited our outpatient department primarily with concerns of infertility and periodic pelvic ache. She was initially diagnosed with OHVIRA syndrome through ultrasonography. The outcomes of her MRI are also discussed. The couple was counselled regarding the disease, complications, and treatment options however due to financial constraints they were lost to follow-up

Junior Doctors’ Education and Training Facilitated by Quality Improvement Project
Quality improvement projects are vital to improve and maintain the quality of care. Focused education and training is one of the most effective interventions to improve quality. We used the quality improvement cycle also called the Shewhart cycles protocol to objectively identify areas requiring focused education and training and measure the effectiveness of our education strategies through the completion of the quality improvement cycle.
This project comprised two phases. In the first phase discharge summaries of surgical patients in a single surgical ward for two weeks were reviewed. Data regarding the accuracy and appropriateness of essential components of discharge summaries were collected. The education session was arranged for doctors involved in making discharge letters.
Considerable (Accuracy / Appropriateness <75%) deficiencies were identified in 7 out of 10 components of the discharge summaries. Most of the doctors felt the educational session was beneficial. There was no component of the discharge letters with considerable deficiencies in the second phase. Significant improvement was seen in 6 out of 7 components which were identified to be deficient.
We conclude that focused education and training of healthcare staff facilitated by the implementation of the Shewhart cycle is an effective way of quality improvement.
A Comprehensive Review on Treatment Modalities of Malignant Melanoma
Melanoma is an aggressive tumor that exhibits an increased occurrence and poor prognosis if metastasized. Early diagnosis and effective management are the mainstays for melanoma management. Effective management includes the selection of an effective treatment plan, supportive care, and training of patients for early diagnosis; various diagnostic options are available i.e. biopsy assessment, imaging procedures, and different biomarkers like liquid biopsy, various proteins, and polymorphism. As for the biomarkers, assessment of circulating tumor cells, microRNAs (miRNAs), and cell-free DNAs and RNAs are effectively used in the diagnosis of melanoma cases; the dysfunction of these molecules gives rise to the pathogenesis of melanoma. Many treatment modalities could be used potentially for melanoma management i.e. radiation therapy, surgery, systemic therapy, immunotherapy, and antibodies therapy. Proper care and training for melanoma patients contribute to enhance monitoring of the cases, in response to different treatment modalities. In this review, we have summarized details about malignant melanoma, keeping in view its epidemiology, classification, diagnosis, and various treatment modalities
Burden of Depression and its Correlates among Patients of Skin Diseases: a Snap-shot from Dermatology Outpatient Clinic in Karachi, Pakistan
Background: Skin problems are widespread health issues that account for a significant portion of the burden of disease worldwide and because of this der-matologists frequently face mood disorders.
Objective: The present study has been planned to determine the prevalence and correlates of depression among skin diseases patients presenting in a derma-tology clinics in Karachi, Pakistan.
Methods: This cross-sectional study was performed at dermatology clinic at Noor-e-Zia Consultant Clinics during April to August, 2022. Depression screening was performed using Patient Health Questionnaire of 9 items (PHQ-9). The cut-off for positive depression screening was 10 and above.
Results: Total 385 patients were studied with a median age of 38 (IQR=27-48) years and majority of them were females (n=256, 66.5%). Patients presented with a median disease duration of one month with range of 1-24 months. 188 (48.8%) had the PHQ-9 score ≥10. On multivariable logistic regression, lower education, occupation of student, patients living in joint family system, current smokers, presence of comorbid, severe diseases and diagnosis of urticarial and folliculitis were associated with higher odds of depression.
Conclusion: The depression burden is higher in dermatology clinic with sig-nificantly higher depression among patient of lower education, students, current smokers, patients with comorbid, severe diseases, and diagnoses of urticaria and folliculitis
Making Pakistani Primary Care More Resilient
It is established through evidence that regions with functioning and well organised primary care systems have better health outcomes. One of the components of primary care is disease prevention which is associated with reduced mortality, morbidity and overall better improved health. Countries with higher proportion of primary care physicians (PCP) also called family physicians (FPs) or general practitioners (GPs) to population have reduced all-cause mortality ratios across various age group
