Visual rehabilitation with scleral contact lenses in patients with irregular astigmatism
Introduction: Scleral contact lenses (SCLs) are gas-permeable and are placed entirely on the sclera without contacting structures such as the cornea or limbus. These ECLs are designed to rehabilitate reduced vision of eyes with irregular corneas. Irregular astigmatism caused by corneal pathologies such as keratoconus, post-transplant, post-refractive surgery, or corneal degeneration produces poor visual acuity that cannot be corrected with air lenses, and the appearance of scleral contact lenses could be a therapeutic option in the treatment and optical correction of corneal pathologies. To determine the degree of improvement in visual acuity with adaptation of scleral lenses. Methodology: This was an observational, descriptive, retrospective, cross-sectional, retrospective study. Results: Forty cases of irregular astigmatism with different initial visual acuity effects were observed prior to the fitting of Scleral Lenses. The mean visual acuity (VA) prior to correction with scleral lenses observed in the 40 cases was 20/200 with correction on the Snellen scale with a minimum (worst VA) equal to "finger-count vision" (represented as cases with 20/ 400 vision for quantification purposes) and a maximum (best VA) of 20/70. Nineteen cases (47.5%) restored their VA to normal values (20/20), and 30 cases (75%) improved their VA by at least 10-fold from the baseline. Discussion: In this study, a remarkable and constant improvement in visual acuity was observed in all the patients. Some patients experienced an increase of up to 20 times their initial visual acuity, achieving, in many cases, a visual acuity of 20/20 on the Snellen scale. Analysis of the risk of diabetes mellitus type 2 in women workers at a reference hospital in Asuncion, Paraguay
Introduction: Obesity, age, sedentary lifestyle, and family history of diabetes mellitus are risk factors for type 2 diabetes mellitus (T2DM). Objective: determine the probable risk of developing T2DM in the next 10 years in working women at a reference hospital in Asunción, Paraguay. Methodology: This cross-sectional observational descriptive study was conducted with physicians, nurses, and administrative staff. Data were collected over three months using the FINDRISK test to assess diabetes risk, which consists of eight questions about age, body mass index, waist circumference, physical activity, fruit and vegetable consumption, history of blood pressure, high blood pressure and high glucose, and diabetes diagnosed in family members. Descriptive statistics in the form of frequencies and percentages, as well as analytical statistics using the Mann-Whitney and Kruskal-Wallis tests, were used to analyze the data. Results: A total of 112 women participated in the study, with more than 40% in the age range of 31–40 years. The waist circumference of 80-88 cm was the most common (38.4%). It was found that 33% of participants were overweight. Almost 60% of the participants did not perform daily physical activities. More than 38% of the first-degree relatives had a history of diabetes. Of the surveyed participants, 67.9%had some risk of developing T2DM in the next 10 years and 22 had a high or very high risk according to the FINDRISK test. Participants aged 35 years had a significantly higher risk (p=0.008). Conclusion: A high prevalence of overweight and obesity was observed, with 67.9% of the respondents showing some degree of risk for developing T2DM in the next 10 years, of which nearly 20% were categorized as high or very high risk.Introduction: Obesity, age, sedentary lifestyle, and family history of diabetes mellitus are risk factors for type 2 diabetes mellitus (T2DM). Fear of monkeypox and intention to vaccinate against monkeypox: a network analysis
Introduction: The presence of fear of an infectious disease triggers psychological factors associated with perceived behavioral control over vaccination acceptance, subjective norms of vaccination acceptance, attitudes toward vaccination acceptance, and perceived infectiousness from disease. Objective: The study evaluated how symptoms of fear of Monkeypox specifically relate to intention to be vaccinated against Monkeypox in a sample of 506 Peruvian individuals with a mean age of 27.11 years (SD = 9.77). Methods: Participants responded to the Monkeypox Fear Scale (MFS) and a single item of Intention to be vaccinated against Monkeypox. Using network analysis, central fear symptoms and associations between symptoms and intention to vaccinate were identified. Results: The results suggest that the node with the highest centrality was item 5 of the MFS. Regarding the connection between the intention to be vaccinated and the MFS items, a direct relationship was observed with item 1 and an inverse relationship with item 6. In addition, the relationship between these MFS items and intention to vaccinate is higher in men compared to women. While it would be expected that a greater presence of symptoms of fear of Monkeypox could lead people to have a greater intention to be vaccinated against the disease, the results could lead other researchers to generate evidence to explain this relationship in other Latin American countries. Características clínicas de los pacientes trasplantados renales ingresados al departamento de cuidados intensivos de adultos del Hospital de Clínicas, Paraguay.
Introduction: In December 1985, the first kidney transplant was performed at the Hospital de Clínicas. The multidisciplinary team worked in coordination to respond to the increasing needs of patients who required this therapy. The aim of this work was to characterize kidney transplant patients admitted to the Adult Intensive Care Department of the Hospital de Clínicas of the Faculty of Medical Sciences of the National University of Asunción. Methods: An observational, descriptive, retrospective cross-sectional study was made, with non-probabilistic sampling of consecutive cases. The records of all patients admitted to intensive care unit in the immediate postoperative period from January 2017 to December 2023 were used. Results: 74 kidney transplant patients were during the study period. The transplanted kidney proceeds from a cadaveric donor in 63.6%. Forty-eight were male (64,8%). The median age was 33 years. The main comorbidity of the recipient was arterial hypertension 79.7%. The prognostic score used was APACHE II, with a median of 12. The median number of days of hospitalization was 3. The 5.4% required mechanical respiratory assistance and the average number of days of ventilation was 9 (range: 2-19). Furosemide infusion was required in 48.6% of patients, vasodilators in 39,1% and 10.8% of vasopressors. Of the patients studied, 95.9% were on hemodialysis (n:71). Nine patients required dialysis sessions after transplantation. Doppler ultrasound of the transplanted kidney showed renal artery stenosis in 12.9%, with clinical repercussions in 2 patients, leading to transplantectomy in one of them. Discharge survival from intensive care was 100%. Two patients were readmitted in the mid-postoperative period with healthcare associated infections, who finally died. Conclusion: In Paraguay, kidney transplantation is a valid therapeutic option with good results and low complications for patients in the terminal stage of kidney disease.Introducción: En diciembre de 1985 se realizó el primer trasplante renal del Hospital de Clínicas. El equipo multidisciplinario trabajó coordinadamente para dar respuesta a pacientes que requerían de esta terapéutica. El objetivo de este trabajo fue caracterizar a los pacientes trasplantados renales ingresados al Departamento de Cuidados Intensivos Adultos del Hospital de Clínicas de la Facultad de Ciencias Médicas de la Universidad Nacional de Asunción. Metodología: Se realizó un estudio observacional, descriptivo, retrospectivo de corte transversal, con muestreo no probabilístico de casos consecutivos. Se utilizaron fichas médicas de pacientes trasplantados que ingresaron a terapia intensiva en post operatorio inmediato de enero 2017 a diciembre 2023. Resultados: Fueron sometidos a trasplante renal 74 pacientes. El riñón trasplantado provenía de donante cadavérico en 63,6 %. Fueron del sexo masculino 64,8 % (n:48). La mediana de edad fue de 33 años. La principal comorbilidad del receptor fue hipertensión arterial 79,7 %. El score pronóstico utilizado fue APACHE II, con mediana de 12. La mediana de días de internación fue 3. Necesitaron asistencia respiratoria mecánica 5,4% y el promedio de días de ventilación fue 9 (rango: 2-19). Requirieron infusión continúa de furosemida 48,6%, vasodilatadores 39,1 % y vasopresores en un 10,8 %. De los pacientes estudiados, 95,9 % se encontraban en hemodiálisis (n:71). Requirieron sesiones dialíticas posterior al trasplante 9 pacientes. La ecografía Doppler del riñón trasplantado mostró estenosis de la arteria renal en 12, 9%, con repercusión clínica en 2 pacientes, llegando a trasplantectomía en uno de ellos. La supervivencia al alta de la terapia intensiva fue 100 %. Reingresaron en postoperatorio mediato por infecciones asociadas a cuidados de la salud 2 pacientes, que finalmente fallecieron. Conclusión: En el Paraguay el trasplante renal, es una válida opción terapéutica con buenos resultados y bajas complicaciones para pacientes en etapa terminal de la enfermedad renal
Situación del ejercicio profesional del licenciado en instrumentación y área quirúrgica en Paraguay
Objective: to know the work environment of the professional and licensed Surgical Instrument Technician in Paraguay. Methodology: descriptive, quantitative and cross-sectional study. The sample is 167 graduates, graduates of public and private institutions. Non-probability sampling for convenience, the data collection instrument is an electronic questionnaire. Results: 74% are women, 50% have an age range between 21 and 30 years old, 79.6% graduated from a public institution, 40% say they have moderate difficulty finding their first job in the field. Access to employment, 71% was through personal contacts, 72% is dedicated to work as Instrument Technicians, 60% have a job seniority of 1 to 5 years. Among the postgraduates, 30% are university didactics certified. Discussion: Surgical Instrument Technicians meet the requirements to perform other functions in addition to instrumentation and circulating nurses in the operating room. Similarly, other studies show less participation in the administrative area, teaching, marketing and sales. 20 years have elapsed since the first Technicians graduated, being a new profession, not given the importance it deserves. It is noteworthy that the state can save economic resources by hiring Surgical Instrument Technicians, since they are experts in aseptic care and competent in minimizing the amount of time the patient spends in surgery.Introducción: en Paraguay en 1996 se crea la Escuela de Instrumentación y Área Quirúrgica, vinculada a una universidad pública, a partir del 2008 se crean otras ofertas en el sector privado. El instrumentador quirúrgico puede desempeñarse como instrumentador, circulante, coordinador general de quirófanos y coordinador de la central de esterilización. Objetivo: conocer la situación del ejercicio profesional del licenciado instrumentador quirúrgico en Paraguay. Metodología: estudio descriptivo, cuantitativo y trasversal. La muestra es de 167 licenciados, egresados de instituciones pública y privadas. Muestreo no probabilístico por conveniencia, el instrumento de recolección de datos es un cuestionario electrónico. Resultados: el 74 % son mujeres, el 50 % presenta un rango de edad de 21 a 30 años, 79,6 % egresados de una institución pública, 40 % dice tener dificultad moderada para el primer empleo. El acceso al empleo, 71 % fue por contactos personales, el 72 % se dedica a instrumentar, el 60 % tiene una antigüedad laboral de 1 a 5 años. Entre los posgrados, el 30 % posee didáctica universitaria. Discusión: instrumentador quirúrgico, reúne las exigencias para ocupar otras funciones además de instrumentar y circular en el quirófano. Igualmente, en otros estudios se evidencia menor participación en el área administrativa, docencia, mercadeo y ventas. Han trascurrido 20 años del egreso de las primeras promociones, siendo una profesión novel, poco visualizado. Es de subrayar que el estado puede ahorrar recursos económicos al contratar a instrumentadores, puesto que son expertos en los cuidados asépticos y competentes en optimizar los tiempos quirúrgicos al que se expone el paciente
Manifestaciones orales del COVID-19 en Paraguay: resultados de una encuesta en línea.
Introduction: Previous studies have reported that patients infected with the COVID-19 virus could manifest symptoms in the oral cavity. Objective: To evaluate the frequency of oral manifestations associated with COVID-19 in a segment of the Paraguayan population and determine the most prevalent ones. Methods: Descriptive cross-sectional study. An electronic survey was conducted from January to March 2022. The data were presented as frequencies and percentages and analyzed using the chi-square test. Statistical analysis was performed with R software version 4.0.3. Results: The sample consisted of 478 individuals. 79.50% were female, and 45.19% were between 25 and 34 years old. 65.48% reported having experienced at least 1 oral symptom or sign during the course of COVID-19. The loss of the sensation of bitter tastes, followed by the alteration of the taste of foods and the loss of sweetness perception, were the most common symptoms. A significantly higher proportion of oral manifestations was found in the 18-24 age range (?²; p= 0.003). Among people who developed COVID-19 in a moderate to severe form, a greater number of oral symptom manifestations were observed (?²; p= 0.044). Discussion: A high frequency of oral manifestations was identified in patients with moderate to severe cases of COVID-19, with taste disorders standing out as the most predominant. Younger individuals were the most affected.Introducción: Estudios previos han reportado que pacientes infectados con el virus del COVID-19, podrían manifestar sintomatologías a nivel de la cavidad oral. Objetivo: Evaluar la frecuencia de manifestaciones orales asociadas a COVID-19 en un segmento de la población paraguaya y determinar cuáles son las más prevalentes. Metodología: Estudio descriptivo de corte transversal. Fue realizada una encuesta electrónica de enero a marzo del 2022. Los datos fueron presentados como frecuencias y porcentajes y analizados mediante la prueba de chi-cuadrado. El análisis estadístico se realizó con el software R versión 4.0.3. Resultados: La muestra estuvo compuesta por 478 personas. El 79,50 % correspondió al sexo femenino y el 45,19 % tenía entre 25 y 34 años. El 65,48 % informó haber experimentado al menos 1 síntoma o signo oral durante el curso de COVID-19. La pérdida de la sensación de sabores amargos, seguida de la alteración del sabor de los alimentos y la pérdida de la percepción dulce, fueron los síntomas más comunes. Se encontró una proporción significativamente mayor de manifestaciones orales en el rango de 18-24 años (?²; p= 0,003). Entre las personas que desarrollaron COVID-19 de forma moderada a severa hubo mayor número de manifestaciones de síntomas orales (?²; p= 0,044). Discusión: Se identificó una alta frecuencia de manifestaciones orales en pacientes con casos de moderados a severos de COVID-19, destacándose los trastornos del gusto como los más predominantes. Los individuos más jóvenes fueron los más afectados
Conocimientos y prácticas sobre sífilis materna y congénita en adolescentes embarazadas en un Hospital General de Paraguay: estudio cualitativo
Introduction: Syphilis in pregnant women continues to be a major public health problem throughout the world, and in Paraguay, it is no exception. Congenital syphilis can cause abortions, fetal and neonatal deaths, low birth weight, prematurity, and other congenital anomalies; general knowledge of the population of childbearing age is essential. Objective: To analyze the knowledge and practices about maternal syphilis and congenital syphilis in pregnant adolescents in a General Hospital in Paraguay. Methodology: A qualitative, descriptive phenomenological study was carried out. A simple interview was used to collect data, which was recorded with prior authorization from the participant. The conclusion and recommendation will be delivered to the institution and to the pregnant adolescents. Results: Ten adolescents participated, mostly 19 years of age, five know that syphilis is transmitted through sexual relations, however, they do not know how congenital syphilis is transmitted. Five of the adolescent’s mention having started sexual relations at the age of 16. Two adolescents at 15 years old, two at 14 years old and one at 17 years old, nine of the pregnant adolescent’s mention attending their prenatal check-ups periodically. Two have not yet been tested for VDRL. Discussion: It is concluded in terms of knowledge that only five out of ten adolescents know about syphilis; however, they do not know about congenital syphilis. Regarding preventive practices, 10 attend their prenatal check-ups periodically.Introducción: La sífilis en embarazadas, sigue siendo un gran problema de salud pública en todo el mundo, y en Paraguay, no es la excepción. La sífilis congénita puede provocar abortos, muertes fetales y neonatales, peso bajo al nacer, prematuridad y otras anomalías congénitas, el conocimiento general de la población en edad fértil es fundamental. Objetivo: Analizar los conocimientos y prácticas sobre sífilis materna y sífilis congénita en adolescentes embarazadas en un Hospital General de Paraguay. Metodología: Se realizó un estudio cualitativo, fenomenológico descriptivo. Para la recolección de datos se utilizó la entrevista, simple que fue grabada con previa autorización del participante, la conclusión y recomendación serán entregados a la institución y a las adolescentes embarazadas. Resultados: Participaron diez adolescentes, en su mayoría de 19 años de edad, cinco conocen que la sífilis se transmite a través de las relaciones sexuales, sin embargo, desconocen acerca de cómo se transmite la sífilis congénita. Cinco de las adolescentes menciona haber iniciado las relaciones sexuales a los 16 años. Dos adolescentes a los 15 años, dos a los 14 años y una a los 17 años, nueve de las adolescentes embarazadas mencionan acudir a sus controles prenatales de forma periódica. Dos aún no se han realizado el test de VDRL. Discusión: Se concluye en cuanto al conocimiento, que solo cinco de diez adolescentes conocen acerca de la sífilis, sin embargo, no conocen sobre la sífilis congénita. En cuanto a las prácticas preventivas, 10 acuden de forma periódica a sus controles prenatales.
Salud en la Frontera: Desafíos y Oportunidades para el Bienestar de las Comunidades Fronterizas
Border health presents a series of challenges and opportunities for border communities’ well-being. These geographic areas, where political and territorial boundaries are intertwined, face unique health issues that require innovative and collaborative approaches. One of the main challenges is limited access to quality health care services. Border communities often lack adequate health infrastructure, including hospitals and clinics, which makes it difficult to access essential services. In addition, language and cultural barriers can further hinder communication between healthcare providers and patients, thereby limiting the quality of care. Despite these challenges, there are opportunities to improve border health. Cross-border collaboration can be a powerful tool for addressing common health problems. Neighboring countries can share resources, knowledge, and best practices to strengthen their health systems on both sides of their borders. This includes the promotion of joint education and training of health professionals, exchange of epidemiological information, and harmonization of health policies. In addition, border communities can take advantage of cultural diversity and traditional knowledge to improve healthcare.La salud en la frontera presenta una serie de desafíos y oportunidades para el bienestar de las comunidades fronterizas. Estas zonas geográficas, donde los límites políticos y territoriales se entrelazan, enfrentan problemas de salud únicos que requieren enfoques innovadores y colaborativos. Uno de los desafíos principales es el acceso limitado a servicios de atención médica de calidad. Las comunidades fronterizas a menudo carecen de infraestructuras de salud adecuadas, incluyendo hospitales y clínicas, lo que dificulta el acceso a servicios esenciales. Además, las barreras lingüísticas y culturales pueden dificultar aún más la comunicación entre los proveedores de salud y los pacientes, lo que limita la calidad de la atención. A pesar de estos desafíos, también existen oportunidades para mejorar la salud en la frontera. La colaboración transfronteriza puede ser una herramienta poderosa para abordar los problemas de salud comunes. Los países vecinos pueden compartir recursos, conocimientos y mejores prácticas para fortalecer los sistemas de salud en ambos lados de la frontera. Esto incluye la promoción de la formación y capacitación conjunta de profesionales de la salud, el intercambio de información epidemiológica y la armonización de políticas de salud. Además, las comunidades fronterizas pueden aprovechar su diversidad cultural y su conocimiento tradicional para mejorar la atención médica
Funcionalidad de la mano luego de una carpectomía proximal: primer caso en el servicio de kinesiología de la Universidad Nacional de Asunción
The wrist is the anatomical segment that allows the hand to be brought closer to the mouth. Fractures and dislocations of the carpal bones are rare, accounting for between 5-7% of fractures of the wrist and hand, and occur through high-risk trauma. energy thus generating a significant physical and functional disability for the patient. Proximal row carpectomy aims to preserve the mobility of the wrist and reduce its impairment for activities of daily living. We present a 41-year-old male patient with a sawmill profession who was referred to physiotherapy rehabilitation after surgery due to high-energy trauma to the dominant hand through a traffic accident. The evolution was favorable, after 8 weeks of physiotherapy rehabilitation and following an intervention protocol mediated by therapeutic and rehabilitation exercises, the patient was able to perform voluntary activities with the hand related to mobility and grip strength.La muñeca es el segmento anatómico que permite aproximar la mano a la boca, las fracturas luxaciones de los huesos del carpo son poco frecuentes, corresponden entre el 5-7% de las fracturas de la muñeca y la mano, se presentan mediante traumatismos de alta energía generando así una discapacidad física y funcional importante para el paciente. La carpectomía de la hilera proximal tiene como objetivo preservar la movilidad de la muñeca y reducir su deterioro para las actividades de la vida diaria. Se presenta un paciente de sexo masculino de 41 años de edad de profesión aserradero que fue derivado a rehabilitación fisioterapéutica post intervención quirúrgica por traumatismo de alta energía sobre la mano dominante a través un accidente de tránsito. La evolución fue favorable, a las 8 semanas de rehabilitación fisioterapéutica y siguiendo un protocolo de intervención mediado por ejercicios terapéuticos y de rehabilitación, el paciente logró realizar actividades voluntarias con la mano relacionadas a la movilidad y la fuerza de prensión.
 
Thyroid cancer in patients undergoing thyroidectomy total in the Otorhinolaryngology Department of the Hospital de Clínicas during the period 2018-2022
Introduction: Thyroid cancer, considered one of the most frequent malignant tumors of the endocrine system, accounts for 80%–90% of cases. The most common histological type observed in most studies is papillary cancer. Objective: To determine the frequency of thyroid cancer in patients undergoing total thyroidectomy at the Otorhinolaryngology Department of the Hospital de Clínicas between the period 2018-2022. Methodology: This retrospective study included 260 patients treated during the study period. Data were obtained from the patients\u27 medical records, and sociodemographic and clinical variables related to thyroid cancer were analyzed. Data were analyzed with the SPSS v25 program, using descriptive statistics. Results: The frequency of thyroid cancer was 48.1%. The mean age of the cancer patients was 42.10±13.10 years. A higher frequency was observed in females 34.2%, n=89) and among those aged 25-44 years 20.0%, n=52). The most frequent were papillary (36.9%), follicular (2.7%), medullary (1.5%), and anaplastic (1.2%) types.
