Objective: To determine the clinical and epidemiological characteristics of patients with noninfectious uveitis at a university hospital in Paraguay. Methodology: An observational, descriptive, cross-sectional, retrospective study was performed. Consecutive patients who attended the Ophthalmology Service of the Hospital de Clínicas from January 2020 to October 2021 and who were diagnosed with non-infectious uveitis were included. The variables studied were sex, age, origin, reason for consultation, anatomical classification, clinical course, systemic and ocular associations, and bilaterality. Results: A total of 78 medical records of patients meeting the inclusion and exclusion criteria were analyzed. Of the patients, 66.7% were female and 42 (53.8%) were from the Central Department. The anterior anatomical location of uveitis was the most frequent (53.9%), followed in decreasing order by panuveitis, posterior uveitis, and intermediate uveitis. Regarding anterior uveitis, 24 patients (57.1%) had unilateral uveitis, 40 (95.2%) were non-granulomatous, 19 (45.2%) presented an acute clinical course, the most frequent reason for consultation was "red and painful eye" with 15 patients (35.7%), the most frequent systemic association was HLA- B27+ in 16 patients (38.1%). As for panuveitis, 13 patients (50%) were associated with Vogt-Koyanagi-Harada syndrome, and of the latter, two patients were related to rheumatoid arthritis. Conclusion: The clinical and epidemiological patterns observed in this study were largely consistent with those reported in the literature. Notably, the most frequent associations differed from those reported in the literature: HLA-B27+ for anterior uveitis, rheumatoid arthritis for posterior uveitis, and a significant percentage of Vogt-Koyanagi-Harada syndrome among panuveitis cases. Objective: To determine the clinical and epidemiological characteristics of patients with noninfectious uveitis at a university hospital in Paraguay. Introduction: Hematological parameters provide the medical team with useful prognostic markers in the clinical evolution of COVID-19 disease. The objective was to analyze the relationship between the unfavorable evolution of hematological values and the death of patients hospitalized with COVID-19. Methodology: The study is observational, correlational, and longitudinal. A review of the clinical records of patients over 18 years of age hospitalized between January and March 2021 was carried out. The results of the hematological parameters at admission, at 48 hours, at 8 days and at discharge were recorded in an Excel spreadsheet. The data were analyzed with Epi Info 7.1 (CDC, Atlanta) according to descriptive statistics. Results: Of 144 patients, 68 (47.2%) deaths were observed, with a predominance of males (63.2%) and those over 60 years of age (47.1%). The factors associated with mortality were hypertension (52.9% vs 31.6%; p=0.009), diabetes (50% vs 26.3%; p=0.003) and obesity (41.2% vs 25 %; p=0.039). The unfavorable hematological parameters associated with mortality occurred after 8 days of hospitalization with leukocytosis (64.1% vs 33.3%; p=0.003), lymphopenia (96.8% vs 79%; p=0.008), neutrophilia (98.5% vs 87.7%; p=0.029) and a high neutrophil/lymphocyte ratio (NLR) (96.9% vs 75.4%; p=0.001). Discussion: The hematological parameters that were associated with mortality were leukocytosis with neutrophilia, lymphopenia, and elevated NLR. These parameters could have prognostic value in follow-up to contribute to the management of these patients. Introduction: Frailty and depression risk are common in older adults undergoing chronic obstructive pulmonary disease (COPD) treatment. However, little is known about this association in people with COPD residing in low- and middle-income countries. Objective: The study aimed to establish the relationship between frailty and depression among ambulatory adults over 60 years with COPD in Santa Marta, Colombia. Methodology: A cross-sectional study was designed in which consecutive patients from the pulmonology outpatient clinic were invited to participate. Frailty was quantified with the FiND (Frail Non-Disabled [FiND] Screening Tool) [Cronbach\u27s alpha 0.65] and depression with the Primary Care Screening Questionnaire for Depression (PSQD) [Cronbach\u27s alpha 0.73]. Results: Three-hundred forty-nine patients with COPD between 60 and 98 years participated (M=75.6, SD=8.4), 61.9% were males, and 19.8% presented a C or D combined evaluation. Two hundred eighty-six patients (76.8%) presented frailty with and without mobility disability, and 31.2% presented depression. The relationship of frailty with depression remained significant, even after adjusting for confounding variables (OR=2.80, 95%CI 1.42-5.51). Conclusions: Frailty and depression are significantly associated after adjusting for confounding variables. More studies are welcome.Introduction: Frailty and depression risk are common in older adults undergoing chronic obstructive pulmonary disease (COPD) treatment. Introduction: Paraguay assumed the challenge of achieving universal coverage through networks based on Primary Health Care with Family Health Units (USF) at the first level of care. One challenge is comprehensive care for non-communicable diseases, the main cause of mortality in the country. Objective: to analyze the capacity of the USF to care for people with high blood pressure and diabetes in the national health system. The design was non-experimental, quantitative, cross-sectional, descriptive with an analytical component. Methods: It included 761 USF from 12 health regions grouped into 4 territorial axes. The WHO SARA evaluation method was adapted with 75 variables, applying a questionnaire to health professionals between November and December 2022. Availability and preparation indices were calculated as well as an index that integrates them. The continuous measurement of these indices was categorized into 3 groups: sufficient >0.75 to 1; intermediate 0.5 to 0.75 and low <0.5. Results: Only in 38.0% of the USF the availability index was sufficient, in 31.5% for the readiness index and in 31.1% for the SARA DM/HTA integrating index. The performance was significantly associated with the territorial axis, but not with the area or with the coverage of the indigenous population. Discussion: the USF presented limitations for the care of people with diabetes and hypertension in these regions of the country.

    No full text
The importance of the study of retinopathy in public health is because almost 3% of global blindness is attributed to diabetic retinopathy, according to data provided by the Pan American Health Organization (PAHO). At the national level, from the National Eye Health Program of the Ministry of Public Health and Social Welfare (MSP and BS), patients diagnosed with Diabetes Mellitus are urged to attend check-ups at ophthalmological clinics. Currently, in Paraguay, we are in the planning stage for further development of the new Rapid Survey of Preventable Blindness (RAAB) and the Tool for Analysis of the Ocular Health Situation (ECSAT) within the Paraguayan National Vision Commission (CONAVIP) under the advice of PAHO. We hope to understand the current state of eye health in our country and thereby determine new objectives for the next National Eye Health Plan, which dates to 2011. Together with CONAVIP, the Paraguayan Society of Ophthalmology (SPO), and PAHO, they are developing a program for the introduction of ophthalmology at the primary care level, since the sense of sight is not isolated from the rest of the body., based on multiple systemic conditions, such as diabetes and hypertension, the most prevalent pathologies that can lead to retinopathies and a probable cause of blindness. The pandemic has left us with invaluable experience in the practice of teleophthalmology, both in its potential and limitations. The most disruptive aspect is related to Artificial Intelligence applied in ophthalmology, especially in the detection and diagnosis of Diabetic Retinopathy in children and young adults, making it more expensive for children and young adults to have a young population with visual disabilities. There are many challenges in Paraguay regarding the scope of detection and treatment of diabetic retinopathy, but through strategies focused on the aforementioned points, we will be able to change the situation in our territory, towards better visual health and quality of life. for our patients.
