Indicadores bibliométricos de Medicina Clínica y Social
Dear readers and authors, Medicina Clínica y Social started its academic life in 2017, and over the years has achieved several milestones in terms of indexing to high-impact national, regional, and global databases. Starting with DOAJ in 2020, SciELO Paraguay in 2021, Redalyc in 2022 with a rating of excellent, culminating with its entry to SCOPUS in 2023, a historic milestone as the first Paraguayan journal to enter this prestigious global database and after one year of its entry we already have a quartile within SCIMAGO, being the only Q4 journal in Paraguay to date (1).
Let us analyze what SCIMAGO presented during the 2019-2023 period of the journal. First, it is in position 59 in Latin America in the category of medicine (miscellaneous) with an SJR of 0.12, an H-index of 3, 78 articles, and 18 citations (0.18 citations per document) (2). This presents us with a challenge in terms of considerably improving the citations that we can receive in SCOPUS, since, if we make a broader analysis in Google Scholar, we find another scenario where there is a greater number of citations at a global level.
Before proceeding with the overall bibliometric analysis, I will present the definition of SJR. SJR is a size-independent prestige indicator that ranks journals based on their "average prestige per article.” It is based on the idea that "not all citations are equal”. SJR is a measure of the scientific influence of journals, which considers both the number of citations a journal receives and the importance or prestige of the journals from which these citations originate. Measures the scientific influence of a journal\u27s average article and expresses how central a journal\u27s average article is to global scientific discussions (1).
For this reason, we can find more articles and citations outside of SCIMAGO by looking at the raw data in SCOPUS. After joining SCOPUS, 168 articles were reported that received 92 citations in total, with an average of 18.4 citations per year and 0.55 citations per article, assuming an H-index of 4. This also changes if we consider the articles cited according to Google Scholar (which collects citations from more resources and is not limited to scientific journal articles).
Considering Google Scholar, there are 222 articles published from 2017 to 2024, with a total of 937 citations (896 in the last five years) with an H-index of 17 (16 in the last five years) (3). Efforts should focus on improving international collaboration, thus improving the impact in impact journals to improve the quartile and quality of our editorial efforts.Estimados lectores y autores, Medicina Clínica y Social inició su vida académica en el año 2017, y a lo largo de estos años ha logrado varios hitos en cuanto a indexación a bases de datos nacionales, regionales y globales de alto impacto. Iniciando con DOAJ en 2020, SciELO Paraguay en 2021, Redalyc en 2022 con calificación de excelente, culminando con su ingreso a SCOPUS en 2023, un hito histórico al ser la primera revista paraguaya en ingresar a esta prestigiosa base de datos global y luego de un año de su ingreso ya contamos con un cuartil dentro de SCIMAGO, siendo la única revista Q4 en Paraguay hasta la fecha (1).
Analicemos lo que presentó SCIMAGO durante el periodo 2019-2023 de la revista. En primer lugar, se encuentra en la posición 59 a nivel latinoamericano en la categoría de medicina (miscelánea) con un SJR de 0,12, un índice H de 3, 78 artículos y 18 citas (0,18 citas por documento) (2). Esto nos presenta un reto en términos de mejorar considerablemente las citas que podemos recibir en SCOPUS, ya que, si hacemos un análisis más amplio en Google Académico, nos encontramos con otro escenario donde hay un mayor número de citas a nivel global.
Antes de proceder con el análisis bibliométrico general, presentaré la definición de SJR. SJR es un indicador de prestigio independiente del tamaño que clasifica a las revistas en función de su “prestigio promedio por artículo”. Se basa en la idea de que “no todas las citas son iguales”. SJR es una medida de la influencia científica de las revistas, que considera tanto el número de citas que recibe una revista como la importancia o prestigio de las revistas de donde provienen dichas citas. Mide la influencia científica del artículo promedio de una revista y expresa cuán central es el artículo promedio de una revista para las discusiones científicas globales (1).
Por esta razón, podemos encontrar más artículos y citas fuera de SCIMAGO mirando los datos brutos en SCOPUS. Después de unirse a SCOPUS, se reportaron 168 artículos que recibieron 92 citas en total, con un promedio de 18,4 citas por año y 0,55 citas por artículo, asumiendo un índice H de 4. Esto también cambia si consideramos los artículos citados según Google Académico (que recopila citas de más recursos y no se limita a artículos de revistas científicas).
Considerando Google Académico, hay 222 artículos publicados desde 2017 hasta 2024, con un total de 937 citas (896 en los últimos cinco años) con un índice H de 17 (16 en los últimos cinco años) (3). Tiempo de diagnóstico de enfermedad en pacientes con diabetes e hipertensión arterial con síntomas de depresión en una muestra de pobladores peruanos
Introduction: The prevalence of AHT is between 11.9 % and 49 %, and that of DM2 is 10.5 %. The prevalence of depressive disorders in patients with diabetes ranges from 10 to 15%. Objective: To determine the association between the time of illness in patients with hypertension and DM2 with depressive symptoms. Methodology: This was a cross-sectional analytical study. The secondary base analyzed was the Demographic and Health Survey of Peru (ENDES) for 2019, 2020, and 2021. The Patient Health Questionnaire-9 (PHQ-9) was used. The chi-square test was used for each possible factor associated with depression, and the associations of interest were evaluated using Poisson distribution. Results: At the time of the study, 21.59% of patients had symptoms of depression. Of these patients, 51.40 % were female. A total of 16.58 % of the participants were older than 60 years of age. Hypertension was present in 5.9% of the patients, while DM2 was present in 2.09%. In the bivariate analysis, all variables were associated with the presence of depressive symptoms, except for daily smoking (p=0.275). Discussion: The results showed an association between the time of illness since the diagnosis of HT and DM2 and depressive symptoms. In the case of both pathologies, having less than one year since diagnosis increased the probability of depressive symptoms by 51% and 44%, respectively.Introducción: La prevalencia de HTA se encuentra entre 11,9 % y 49 %, la prevalencia de DM2 es de 10,5 %. La prevalencia de trastornos depresivos en diabéticos va en el rango de 10 % a 15 %. Objetivo: determinar la asociación entre el tiempo de enfermedad de los pacientes con HTA y DM2 con los síntomas de depresión. Metodología: Estudio analítico transversal. La base secundaria analizada fue la Encuesta Demográfica y de Salud del Perú (ENDES) de los años 2019, 2020 y 2021. Se utilizó el Cuestionario de Salud del Paciente-9 (PHQ-9). Se utilizó la prueba del chi cuadrado para cada posible factor asociado a la depresión y las asociaciones de interés se evaluaron con una distribución de Poisson. Resultados: El 21,59 % tenían síntomas de depresión al momento del estudio. El 51,40 % eran del sexo femenino. El 16,58 % presentaba mayor a 60 años. La HTA estuvo presente en el 5.9%, mientras que la DM2 en el 2,09 %. En el análisis bivariado, todas las variables se asociaron con la presencia de síntomas depresivos, excepto la variable que fuma a diario (p=0.275). Discusión: Los resultados señalan una asociación entre el tiempo de enfermedad desde el diagnóstico de HTA y DM2 con los síntomas depresivos. En el caso de ambas patologías el tener menos de un año de diagnóstico aumentaban en 51 % y 44 % respectivamente la probabilidad de presentar síntomas depresivos
Cobertura de vacunación contra COVID-19 e influenza en personal de salud y administrativo en Paraguay
Introduction: Worldwide, vaccination coverage against COVID-19, as well as against influenza, is low both in the general population and in health professionals, despite the fact that the vaccine is free and mandatory for health personnel. Objective: To describe the COVID -19 and influenza vaccination coverage in health and administrative personnel of a reference hospital of the Ministry of Public Health and Social Welfare in the period 2021-2022. Methods: Cross-sectional descriptive observational study. Charts of the health and administrative personnel of the vaccination center of the Itauguá National Hospital of the COVID-19 and influenza vaccination campaign in the period 2021-2022 were reviewed. Results: Of the 3,586 personnel, 999 (27.9%) were medical personnel, 1,494 (41.7%) nursing personnel, 366 (10.2%) other category of health personnel, and 727 (20.3%) administrative personnel. Overall, 86.5% of the employees received at least the two doses that constitute the primary schedule and 73% the booster dose; 2.1% of the staff did not receive any dose of the anti COVID-19 vaccine, which was higher in the administrative staff (4.8%). Influenza vaccination coverage was 20% in 2021 and 25% in 2022. Discussion: Even though the vaccination coverage of anti-COVID-19 was comparable to other countries, vaccination anti-influenza was very low. It is urgent to implement strategies aimed at increasing the perception of risk and acceptability of mandatory vaccines for health personnel.Introducción: A nivel mundial, la cobertura de vacunación contra el COVID-19, así como contra la influenza es baja tanto en la población general como en los profesionales de la salud a pesar de que la vacuna es gratuita y obligatoria en el personal sanitario. Objetivo: Describir la cobertura de vacunación contra el COVID -19, y la influenza en personal de salud y administrativo de un hospital de referencia del Ministerio de Salud Pública y Bienestar Social en el periodo 2021-2022. Metodología: Estudio observacional descriptivo de corte trasverso. Se hizo la revisión de los registros del personal sanitario y administrativo del centro vacunatorio del Hospital Nacional de Itauguá de la campaña vacunal contra el COVID-19 y de anti-influenza en el periodo 2021- 2022. Resultados: De los 3.586 funcionarios, 999 (27,9 %) eran médicos, 1494 (41,7 %) personal de enfermería, 366 (10,2 %) otra categoría de personal sanitario, y 727 (20,3 %) personal administrativo. En forma global, el 86,5 % de los funcionarios recibió por lo menos las dos dosis que constituyen el esquema primario y el 73 % la dosis de refuerzo. El 2,1 % del personal no recibió ninguna dosis de vacuna anti covid-19, la cifra fue mayor en el personal administrativo (4,8 %). La cobertura de vacunación contra la influenza fue de 20 % en el 2021 y 25 % en el 2022. Discusión: Si bien cobertura de vacunación anti-COVID-19 fue comparable a otros países, la vacunación contra la influenza fue muy baja. Es urgente implementar estrategias dirigidas a aumentar la percepción de riesgo y aceptabilidad de las vacunas obligatorias para el personal sanitario
Características epidemiológicas de la mortalidad por enfermedades infecciosas intestinales (diarreas), Paraguay (2015 – 2019)
Introduction: Infectious intestinal diseases (diarrhea) are a major cause of morbidity and mortality worldwide. In 2015, it constituted the ninth leading cause of death for all ages. Objective: To describe the epidemiological characteristics of mortality due to infectious intestinal diseases (diarrhea) in Paraguay from 2015 to 2019. Methods: Descriptive, ecological, cross-sectional, retrospective, and non-probabilistic sampling of consecutive cases was performed. ICD-10 deaths included cholera (A01), typhoid and paratyphoid fever (A01), shigellosis (A03), other bacterial intestinal infections (A04), other bacterial food poisoning (A05), amebiasis (A06), other intestinal diseases due to protozoa (A07), intestinal infections due to viruses (A08), and diarrhea and gastroenteritis of presumed infectious origin (A09). Open access data were obtained from the WEB page of the General Directorate of Strategic Information in Health of the MSP and BS of all the Departments of the country. Crude and adjusted rates (per 100,000 inhabitants) were calculated. Excel and EPI INFO 7.0 were used. Results: 495 deaths were registered; the highest prevalence was in 2019 (adjusted rate of 1.83) and the lowest in 2017 (1.36). The months with the highest mortality rates were January and July (9.9%). 51.72% were women (256), 30.91% were over 80 years old and 28.28% (140) were under 5 years old, average age 50.8, single 56.77% (281) and without any type of education 41.82% (207). A total of 80.61% (399) of the respondents resided in urban areas. The highest rates were registered in Boquerón (33.3) and Amambay (15.2). Diarrhea and gastroenteritis of infectious origin accounted for 95.56% of the cases (473). Discussion: There was a high percentage of mortality from infectious diarrhea. Extremes of life were the most vulnerable populations.Introducción: Las enfermedades infecciosas intestinales (diarrea) son una causa importante de morbilidad y mortalidad a nivel mundial. En 2015, constituyeron la novena causa principal de muerte para todas las edades. Objetivo: describir las características epidemiológicas de la mortalidad por enfermedades infecciosas intestinales (diarreas) en Paraguay durante los años 2015 al 2019. Metodología: Estudio descriptivo, ecológico, transversal, retrospectivo, muestreo no probabilístico de casos consecutivos. Se consideraron los óbitos del CIE – 10, que incluyen al cólera (A01), fiebres tifoidea y paratifoidea (A01), shigelosis (A03), otras infecciones intestinales bacterianas (A04), otras intoxicaciones alimentarias bacterianas (A05), amebiasis (A06), otras enfermedades intestinales debidas a protozoarios (A07), infecciones intestinales debidas a virus (A08) y diarrea y gastroenteritis de presunto origen infeccioso (A09). Se utilizaron datos de acceso abierto de la página WEB de la Dirección General de Información Estratégica en Salud del MSP y BS de todos los Departamentos del país. Se calcularon tasas crudas y ajustadas (por 100.000 habitantes). Se utilizaron Excel y EPI INFO 7.0. Resultados: Se registraron 495 muertes, la mayor prevalencia fue en 2019 (tasa ajustada de 1,83) y menor en 2017 (1,36). Los meses con mayor mortalidad fueron enero y julio (9,9%). El 51,72% fueron mujeres (256), 30,91% mayores de 80 años y 28,28% (140) menores de 5 años, edad promedio 50,8, solteros 56,77% (281) y sin ningún tipo de educación 41,82% (207). El 80,61% (399) residía en área urbana. Las mayores tasas se registraron en Boquerón (33,3) y Amambay (15,2). El 95,56% (473) fueron diarreas y gastroenteritis de origen infeccioso. Discusión: Se registra alto porcentaje de mortalidad de diarreas de origen infeccioso. Los extremos de la vida constituyen la población más vulnerables
Efectividad de la terapia empírica con Cefotaxima y Ceftriaxona en servicios médico-quirúrgicos del hospital “Dr. Benigno Sanchez” de Quillacollo
Introduction. Inappropriate use of antibiotics contributes to the increase in bacterial resistance; resistant strains are very difficult to treat with available antibiotics. Objetive. To evaluate the effectiveness of empirical pharmacological treatment with Cefotaxime and Ceftriaxone in clinical and surgical services of the “Dr. Benigno Sánchez” from Quillacollo, during the 2022 administration. Methodology. It is an observational study of analytical type, case-control subtype, prospective longitudinal. The population was 1395 patients, obtaining a sample of 884 patients, subdividing cases n=741 and controls n=126 into groups. Results. In the surgical service, the frequency among men and women was 45 % and 55 % respectively; 73 % of the total did not present underlying diseases, 90 % of the total had a hospitalization of less than one week with 95 % improvement without significant difference between cases and controls. In the clinical service, 60 % were women, the rest men, 50 % of the total did not present underlying diseases, 78 % of the total had a hospitalization of less than a week with 85 % improvement without significant difference between cases and controls. Discussion. The sociodemographic characteristics, the present study identified that in the surgical service the average age was 43 years, of these 45 % were men and 55 % women, however in the clinical service, the average age was 55 years, 60 % women the rest men. The present study identified that, in the surgical service, 95 % of the cases have improvement, without there being a relationship between the therapy used or not. In the clinical service, an 85 % improvement was seen with no difference between the groups.There is no significant difference between the groups, in medical surgical services, with respect to improvement.Introducción. El uso inadecuado de antibióticos contribuye al aumento de la resistencia bacteriana, las cepas resistentes son muy difíciles de tratar con los antibióticos disponibles. Objetivo. Evaluar la efectividad del tratamiento farmacológico empírico con Cefotaxima y Ceftriaxona en servicios clínicos y quirúrgicos del hospital “Dr. Benigno Sánchez” de Quillacollo, durante la gestión 2022. Metodología. Es un estudio observacional de tipo analítico, subtipo casos y controles, longitudinal prospectivo. La población fue pacientes 1395, obteniendo una muestra de 884 pacientes subdividiendo en grupos casos n=741 y controles n=126. Resultados. En el servicio quirúrgico la frecuencia entre varones y mujeres fue de 45 % y 55 % respectivamente; 73 % del total no presentó enfermedades de base, el 90 % del total tuvo una hospitalización de menos de una semana con 95 % de mejoría sin diferencia significativa entre casos y controles. En el servicio de clínico, 60 % fueron mujeres el resto varones, 50 % del total no presentó enfermedades de base, el 78 % del total tuvo una hospitalización de menos de una semana con 85 % de mejoría sin diferencia significativa entre casos y controles. Discusión. Las características sociodemográficas, el presente estudio identificó que en el servicio quirúrgico a la edad media fue 43 años, de estos 45 % fueron varones y 55 % mujeres, en cambio en el servicio clínico, la edad media fue de 55 años, 60 % mujeres el resto varones. El presente estudio identificó que, en el servicio quirúrgico, el 95 % de los casos cuentan con mejoría, sin que exista una relación entre la terapia empleada o no. En el servicio clínico se apreció un 85 % de mejoría sin que exista diferencia entre los grupos.No existe una diferencia significativa entre los grupos, en los servicios medico quirúrgicos, con respecto a la mejoría
Efectos de una intervención educativa sobre la integridad científica en estudiantes universitarios
Introduction: Universities have an essential role in shaping scientific integrity in their students. Despite its importance, few interventions have been studied to improve it. Objective: To evaluate the effects of an educational intervention on scientific integrity in university students in the southern region of Peru. Methodology: An educational intervention was applied in three phases (pretest, intervention, and post-test). Robust comparison tests were applied for dependent samples, as well as AKP effect size measures. Mixed ANOVA tests were applied. Pairwise comparisons were used using Bonferroni\u27s adjusted p. method. Results: A total of 402 students (63.93% female) participated. The comparison of dependent samples between the post-test and pretest measures found statistically significant differences in the knowledge dimension (tyuen = 3.93; gl = 240; p < .001; AKP = 0.22 [95% CI: 0.12 – 0.33]) and in the practical dimension (tyuen = 4.88; gl = 240; p < .001; AKP = 0.30 [95% CI: 0.20 – 0.42]) and region (2,392) = 4.16, p < .001, ?2 = 0.008. Discussion: The intervention was effective according to the post-test evaluation, with some regional differences.Introducción: Las universidades tienen un rol importante en la formación de la integridad científica en sus estudiantes. A pesar de su importancia, se han estudiado pocas intervenciones que la mejoren. Objetivo: Evaluar los efectos de una intervención educativa sobre la integridad científica en estudiantes universitarios de la región sur del Perú. Metodología: Se aplicó una intervención educativa en tres fases (pretest, intervención y postest). Se aplicaron pruebas de comparación robustas para muestras dependientes, así como también medidas de tamaño del efecto AKP. Se aplicaron pruebas ANOVA mixtos. Se utilizaron las comparaciones por pares siguiendo el método de la p. ajustada de Bonferroni. Resultados: Participaron 402 estudiantes (63.93% de sexo femenino). La comparación de muestras dependientes entre las medidas postest y pretest encontró diferencias estadísticamente significativas en la dimensión conocimientos (tyuen = 3.93; gl = 240; p < .001; AKP = 0.22 [IC 95%: 0.12 – 0.33]) y en la dimensión prácticas (tyuen = 4.88; gl = 240; p < .001; AKP = 0.30 [IC 95%: 0.20 – 0.42]) y la región (2,392) = 4.16, p < .001, ?2 = 0.008. Discusión: La intervención fue efectiva según la evaluación en el postest con algunas diferencias según regiones

Caracterización clínico-epidemiológica y agentes etiológicos de las infecciones respiratorias agudas graves en adultos en Paraguay
Introduction: Sentinel Surveillance of Severe Acute Respiratory Infections (SARI) includes the monitoring and characterization of hospitalized cases. Objective: To describe the clinical-epidemiological characteristics and etiological agents of adults with SARI in a sentinel hospital in Paraguay. Methodology: Observational, descriptive, cross-sectional study, with retrospective data collection, of adult patients with SARI hospitalized at the National Hospital of Itauguá between June 2022 and August 2023. The information was taken from the database of the National System of Epidemiological surveillance. Results: The data of 1598 patients with SARI were analyzed, 820 men (51.3%), 875 (54.8%) aged 60 or older, 1054 (66.0%) from the Central department. 79.2% (n=1265) of patients had some risk factor or comorbidity, 764 with hypertension (47.8%). 38.5% (n=632) were admitted to the ICU and mortality was 34.7% (n=554). SARSCoV-2 was confirmed in 13.8% (n=353), rhinovirus (1.7%), influenza A (1.5%), influenza B (0.6%), parainfluenza 3 (0.6%), RSV (0.2%), bocavirus (0.4%) and Influenza AH1N1 (0.6%). There was a significantly higher frequency of deaths in those aged 60 years and over (40.7%), with some risk factor (35.5% vs 25.9%), and in those confirmed with covid-19 (38.2% % vs 30.5%). Conclusion: SARSCoV-2 continues to be the main etiological agent of SARI in a sentinel hospital in Paraguay. Continued knowledge of the epidemiology and viral agents involved is crucial to developing effective prevention and treatment strategies.Introducción: La Vigilancia Centinela de las Infecciones Respiratorias Agudas Graves (IRAG) incluye el monitoreo y caracterización de casos hospitalizados. Objetivo: Describir las características clínico-epidemiológicas y agentes etiológicos de los adultos con IRAG en un hospital centinela en Paraguay. Metodología: Estudio observacional, descriptivo, de corte transversal, con recolección de datos retrospectivos de los pacientes adultos con IRAG hospitalizados en el Hospital Nacional de Itauguá entre junio de 2022 a agosto de 2023. La información se tomó de la base de datos del Sistema Nacional de Vigilancia Epidemiológica. Resultados: Se analizaron los datos de 1598 pacientes con IRAG, 820 varones (51,3%), 875 (54,8%) con 60 o más años de edad, 1054 (66,0%) del departamento Central. El 79.2% (n=1265) de los pacientes presentaba algún factor de riesgo de gravedad o comorbilidad, 764 (47,8%) con hipertensión arterial. El 38.5% (n=632) fue admitido a UCI y la mortalidad fue de 34,7% (n=554). En el 13,8% (n=353) se confirmó SARSCoV-2, rhinovirus (1,7%), influenza A (1,5%), influenza B (0,6%), parainfluenza 3 (0,6%), VSR (0,2%), bocavirus (0,4%) e influenza AH1N1 (0,6%). Hubo significativamente mayor frecuencia de fallecidos en los de 60 años y más (40,7%), con algún factor de riesgo de gravedad (35,5% vs 25,9%) y en los confirmados con COVID-19 (38,2% vs 30,5%). Conclusión: SARSCoV-2 sigue siendo el agente etiológico principal de las IRAG. El conocimiento de la epidemiología y los agentes virales es crucial para desarrollar estrategias de prevención y terapéuticas efectivas
Comparative Study of Staphylococcus epidermidis Virulence Profiles in an animal Model of Endophthalmitis
Introduction: Gram-positive staphylococci are responsible for over 90% of cases of postoperative infectious endophthalmitis, with Staphylococcus epidermidis accounting for approximately 70% of isolated microorganisms. Objective: To evaluate the relationship between S. epidermidis virulence and severity of endophthalmitis in an animal model. Methodology: New Zealand albino rabbits were divided into two groups and administered 100 colony-forming units of S. epidermidis strains. In the virulent group, four rabbits received a multi-antibiotic resistant, mecA, ica, and atlE gene carrying S. epidermidis strain, from a patient´s conjuntival microbiota and three a biofilm-forming S. epidermidis ATCC 35984. In the non-virulent group, five rabbits were inoculated with a strain sensitive to all tested antibiotics and lacking mecA, ica, and atlE genes, also from a patient and three rabbits received the non-producer biofilm S. epidermidis ATCC 29122. Clinical and ultrasound examinations were conducted every three hours until endophthalmitis symptoms appeared, followed by daily clinical assessments. Histological evaluations were performed 15 days post-inoculation. Results: The less virulent group displayed milder inflammation and reduced intraocular damage in comparison to the more virulent group based on clinical and ultrasound observations. Nevertheless, histopathological analysis revealed similar inflammation in all groups, 15 days post-inoculation. Discussion: Less virulent S. epidermidis strains induced less severe inflammation as observed through clinical and ultrasound assessments. However, long-term histopathological assessments showed effects comparable to those seen with the more virulent strain.Introduction: Gram-positive staphylococci are responsible for over 90% of cases of postoperative infectious endophthalmitis, with Staphylococcus epidermidis accounting for approximately 70% of isolated microorganisms. Objective: To evaluate the relationship between S. epidermidis virulence and severity of endophthalmitis in an animal model. Methodology: New Zealand albino rabbits were divided into two groups and administered 100 colony-forming units of S. epidermidis strains. In the virulent group, four rabbits received a multi-antibiotic resistant, mecA, ica, and atlE gene carrying S. epidermidis strain, from a patient´s conjuntival microbiota and three a biofilm-forming S. epidermidis ATCC 35984. In the non-virulent group, five rabbits were inoculated with a strain sensitive to all tested antibiotics and lacking mecA, ica, and atlE genes, also from a patient and three rabbits received the non-producer biofilm S. epidermidis ATCC 29122. Clinical and ultrasound examinations were conducted every three hours until endophthalmitis symptoms appeared, followed by daily clinical assessments. Histological evaluations were performed 15 days post-inoculation. Results: The less virulent group displayed milder inflammation and reduced intraocular damage in comparison to the more virulent group based on clinical and ultrasound observations. Nevertheless, histopathological analysis revealed similar inflammation in all groups, 15 days post-inoculation. Discussion: Less virulent S. epidermidis strains induced less severe inflammation as observed through clinical and ultrasound assessments. Efectividad de bioceramicos en el mantenimiento de la vitalidad pulpar en recubrimiento pulpar directo en denticion primaria y permanente: revisión sistemática con metaanálisis
Introduction: Bioactive materials have been developed for direct pulp capping in order to preserve the dental pulp and keep the tooth longer in the oral cavity. Objective: The objective of this review was to evaluate the effectiveness of the use of bioceramics as direct pulp capping material to maintain pulp vitality in primary and permanent teeth with reversible pulpitis. Methods: A systematic review with meta-analysis was performed. Success in treating vitality maintenance, postoperative pain, and discoloration was studied. An electronic search was carried out in the databases: MEDLINE, EMBASE, COCHRANE, SCOPUS, LILACS, BBO. Data were extracted and analyzed with Rev Man. Results: Randomized clinical trials in which direct pulp capping with tricalcium silicates was performed compared with other capping materials were included. Comparing three bioceramics, no significant differences were found in the maintenance of pulp vitality. Regarding discoloration, statistically significant differences were observed in favor of Biodentine (p <0.001). Discussion: the clinical relevance of these findings is debatable due to their small overall magnitude and the high risk of bias of the included studies.Introducción: Se han desarrollado materiales bioactivos para el recubrimiento pulpar directo con el objetivo de preservar la pulpa dentaria y mantener el diente por mayor tiempo en la cavidad oral. Objetivo: El objetivo de esta revisión fue evaluar la efectividad del uso de bioceramicos como material de recubrimiento directo pulpar para mantener la vitalidad pulpar en dientes primarios y permanentes con pulpitis reversible. Metodología: Se realizó una revisión sistemática con meta análisis. Se estudió el éxito en el tratamiento del mantenimiento de la vitalidad, el dolor postoperatorio y la decoloración. Se realizó una búsqueda Electrónica en las bases de datos: MEDLINE, EMBASE, COCHRANE, SCOPUS, LILACS, BBO. Se extrajeron los datos y se analizaron con Rev Man. Resultados: Se incluyeron ensayos clínicos aleatorizados en la cual se realizaron recubrimiento pulpar directo con silicatos tricalcicos comparado con otros materiales de recubrimiento. Comparando tres biocerámicos no se encontraron diferencias significativas en el mantenimiento de la vitalidad pulpar. En cuanto a la decoloración se observaron diferencias estadísticamente significativas a favor de Biodentine (p <0,001). Discusión: la relevancia clínica de estos hallazgos es discutible debido a su pequeña magnitud general y al alto riesgo de sesgo de los estudios incluidos
