    Descriptive epidemiology of gastric cancer in Costa Rica from 1990 to 2022 and projection of deaths to 2050: a population-based study

    Agencia Costarricense de Investigaciones Biomédicas/[]/ACIB-FUNIN/Costa RicaBackground In Costa Rica, gastric cancer has been the leading cause of cancer death (both sexes) since the 1980s. The objective of this study was to analyze the incidence, mortality, and geographic distribution of stomach cancer in Costa Rica for the period 1990–2022, and to make projections of deaths up to 2050. Methods Incidence and mortality data were from the Costa Rican National Cancer Registry, the Costa Rican National Institute of Statistics and Census, and the Central American Population Research Center. Age-standardized incidence and mortality rates, annual percent changes (APC), and years of potential life lost (YPLL) were calculated. Crude mortality rates were estimated at the canton level. Mortality projections were made using Poisson models. Findings The standardized rates have been decreasing in all ages over 40 years old, in men and in women (in incidence, APC = −4.4% [−4.6, −4.3] in men, APC = −3.6% [−3.8, −3.3] in women; in mortality APC = −3.5% [−3.7, −3.4] in men, APC = −3.0% [−3.3, −2.8] in women). YPLL rates increased with age, but the age group with the highest number of YPLL was between 50 and 69 years. High mortality rates were observed in the mountainous areas east and south of the capital region, and one canton in Guanacaste. Most models projected increases in number of deaths of 50%–200% by the year 2050. Interpretation Despite declining mortality rates, the number of gastric cancer deaths is projected to rise, underscoring the need for targeted public health strategies-particularly programs for Helicobacter pylori detection and eradication, along with dietary and lifestyle interventions.     Este trabajo de investigación aplicada tiene como objetivo desarrollar una estrategia para el mejoramiento de la gestión y la competitividad empresarial en pequeñas y medianas empresas (pymes) del sector odontológico ubicadas en Tres Ríos, Cartago, mediante la integración de tecnologías de información y comunicación (TIC). Para ello se aplicó una metodología de enfoque mixto, con recolección de datos cualitativos y cuantitativos, a partir de una encuesta estructurada aplicada a clínicas odontológicas de la zona. Los hallazgos evidencian un bajo nivel de madurez digital en aspectos clave como la automatización de procesos, la ciberseguridad, el mercadeo digital y el uso de plataformas de ventas. Las principales barreras identificadas incluyen la falta de infraestructura tecnológica, ausencia de planificación estratégica digital, desconocimiento de herramientas digitales y limitaciones presupuestarias. A partir del diagnóstico, se diseñó un plan estratégico segmentado según niveles de madurez digital (básico, intermedio y avanzado), el cual ofrece acciones concretas y adaptables, que las pymes pueden implementar progresivamente. Este plan busca optimizar procesos de mercadeo y ventas, mejorar la experiencia del cliente, fortalecer la presencia digital y aumentar la competitividad en un entorno empresarial cada vez más digitalizado.This final applied research project seeks to develop a strategy to improve management and organizational competitiveness in small and medium-sized enterprises, within the dental services sector of SMEs in Tres Ríos, Cartago, Costa Rica, through the integration of information and communication technologies (ICT). A mixed-method approach was administered, combining qualitative and quantitative data collected through structured surveys applied to dental clinics in the area. The findings reveal a low level of digital maturity in key areas such as process automation, cybersecurity, digital marketing, and the use of online sales platforms. The main barriers identified include the lack of technological infrastructure, absence of digital strategic planning, limited knowledge of digital tools, and financial constraints. Based on the diagnosis, a strategic plan was developed categorized by digital maturity levels (basic, intermediate, and advanced). This plan proposes concrete, adaptable actions that SMEs can implement progressively.     Sea-citizens: assessing the participation of citizen scientists in marine biology research. Citizen science (CS), in marine environments, provides stakeholders with valuable data for conservation and integrated coastal management. To overview components of marine biology CS projects, research topics and citizens participation— we review peer-reviewed articles published from 2009 to August 2023. A total of 445 scientific papers were analyzed. This review identified an increase in the publication through the years, in regions and countries who have a favorable context— including financial, institutional, and social involvement—to support such programs. 75 % of the studies lasted a maximum of eight years, with groups that range between 1 to 19 843 citizens, supported by academics and digital platforms. The human capital makes possible to conduct projects predominantly related to spatiotemporal patterns of biological groups; however, other research topics were addressed. The access and attractiveness of the ecosystems may have influenced the surveyed ecosystems, since coastal areas and reefs were the most extensively studied. Citizen scientists predominantly conducted fieldwork, yet other roles were less frequent, but still relevant. In all cases, authors pointed to the relevance of CS research in conservation outcomes. CS initiatives should be seen as a complement to traditional surveys, not a replacement. Marine CS programs present perks that exceed the cost reduction of data collection, it is a way of repaying the historical debt that science has with local communities while encouraging a framework of social participation in environmental issues. Citizen science: pathway for coral reef restoration in Ocotal Bay, Costa-Rican north pacific. Coral reefs are facing a worldwide crisis. To promote public participation in restoration efforts, we established a citizen science program in Ocotal Bay, North Pacific of Costa Rica— aimed to restore and monitor the degraded reef. This study evaluates the effectiveness of a citizen science approach, using the biological parameters and volunteers’ social insight as indicators. Artificial structures filled with Pocillopora spp. fragments were placed. Volunteers and academics monitored the substrate composition and fish community. Surveys were conducted to profile the volunteers’ social values. In two years, 63 activities were carried out. Volunteers reported seven substrates, with significant differences in the substrate composition, compared to academic evaluations. They recorded 12 out of 20 fish species documented in the restoration area, which showed changes due to the active restoration. Protocols refreshment and sustained participation seem to be influencing the monitoring efficiency. Despite the bleaching event caused by the increase in the sea surface temperature, healthy coral cover almost duplicated its area throughout the study. Volunteers are motivated by idealistic incentives and cultural ecosystem services from coral reefs. Ensuring long-term sustainability requires addressing other values. Volunteers' knowledge was positively correlated with marine sports experience and commitment to the project, suggesting that skilled volunteers are more likely to properly apply protocols. The concepts of coral restoration and citizen science are well understood, but their knowledge is restricted to their experience. Engaging communities in coral reef conservation efforts is necessary to contribute with resilient reefs that are able to overcome current and future conditions.Ciudadanos del mar: evaluación de la participación de científicos ciudadanos en la investigación de biología marina. La ciencia ciudadana (CC), en ambientes marinos, provee a diversos actores con datos valiosos para la conservación y la gestión costera integrada. Para revisar los componentes de los proyectos de CC en biología marina, los temas de investigación y la participación ciudadana, revisamos artículos científicos publicados entre el 2009 y agosto del 2023. Se analizaron un total de 445 documentos de literatura científica. Se identificó un aumento en la publicación a través de los años, en regiones y países que tienen un contexto favorable, incluyendo soporte financiero, institucional y social, para apoyar tales programas. El 75 % de los estudios duraron un máximo de ocho años, con grupos que varían entre 1 y 19 843 ciudadanos, apoyado por académicos y plataformas digitales. El capital humano hace posible realizar proyectos principalmente relacionados con patrones espaciotemporales de grupos biológicos; sin embargo, otros temas fueron estudiados. El acceso y atractivo de los ecosistemas puede estar influenciado los ambientes estudiados, ya que las zonas costeras y arrecifes fueron los sitios más estudiados. Los científicos ciudadanos principalmente realizaron trabajo de campo; sin embargo otras funciones fueron menos frecuentes, pero relevantes. En todos los casos, los autores destacaron la relevancia de la investigación de CC en resultados de conservación. Las iniciativas de CC deberían considerarse como un complemento de metodologías tradicionales, no un reemplazo. Los programas de CC marina ofrecen ventajas que sobrepasan la reducción de costos para la recolección de datos, es una forma de saldar la deuda histórica que la ciencia tiene con las comunidades locales, al mismo tiempo que se fomenta una plataforma de participación ciudadana en temáticas ambientales. Ciencia ciudadana: una ruta para la restauración de arrecifes coralinos en Bahía Ocotal, Pacífico Norte de Costa Rica. Los arrecifes coralinos están afrontando una crisis mundial. Para promover la participación pública en los esfuerzos de restauración, establecimos un programa de CC en Bahía Ocotal, en el Pacífico Norte de Costa Rica, con el objetivo de restaurar y monitorear el arrecife degradado. Este estudio evalúa la efectividad de un acercamiento de CC, usando parámetros biológicos y el entendimiento social como indicadores. Estructuras artificiales llenas de fragmentos de Pocillopora spp. fueron colocadas. Los voluntarios y académicos monitorearon la composición del sustrato y la comunidad de peces. Encuestas fueron llevadas a cabo para perfilar los valores sociales de los voluntarios. En dos años se llevaron a cabo 63 actividades. Los voluntarios reportaron siete sustratos, con diferencias significativas en la composición del sustrato, comparándolo con las evaluaciones hechas por académicos. Reportaron 12 de 20 de las especies de peces de peces documentadas para la zona de restauración, la cual mostró cambios debido a la restauración activa. El repaso de protocolos y la participación continua parecen estar influenciando la eficiencia del monitoreo. A pesar del blanqueamiento causado por el aumento de la temperatura superficial del océano, la cobertura coralina sana duplicó su área a lo largo del estudio. Los voluntarios están motivados por incentivos idealistas y los servicios ecosistémicos culturales que proveen los arrecifes de coral. Garantizar la sostenibilidad a largo plazo requiere abordar otros aspectos. El conocimiento estuvo positivamente correlacionado con la experiencia en deportes marinos y el compromiso con el proyecto, sugiriendo que la habilidad de los voluntarios hace que sea más probable que se lleven a cabo los protocolos de forma correcta. Los conceptos de restauración coralina y ciencia ciudadana son comprendidos, pero está restringido a sus experiencias. En primer lugar, examino la relación entre la protagonista y su jefe como una dinámica heredada de los inicios del capitalismo. En segundo lugar, explico cómo el filme evidencia que las demandas del capital influyen en la definición de un cuerpo saludable, el cual es determinado por elementos como la juventud, la belleza y la productividad.     Environmental conditions and carbonate chemistry variability influencing coral reef composition along the Pacific coast of Costa Rica

    Coral reef development is influenced by a wide variety of factors, including temperature, salinity, nutrient concentrations, and carbonate chemistry. Studies focusing on physicochemical drivers of coral reef distribution and composition in the Eastern Tropical Pacific (ETP) are scarce, and carbonate chemistry and nutrient data for this region are limited. This study measured coral reef composition and physicochemical parameters along the Pacific coast of Costa Rica, over a one-year period at three locations: Santa Elena and Matapalo in the north, and Parque Nacional Marino Ballena in the south. Our results show high seasonal and spatial variability of physicochemical conditions with significant differences mainly explained by inorganic nutrient concentrations, with driving processes also having a strong influence on the variability of carbonate chemistry parameters. Coastal upwelling is the main driver of the seasonal variability in Santa Elena. Comparison of seasonal dissimilarity within locations confirms the presence of a geographical gradient, with stronger influence of the upwelling in Santa Elena relative to Matapalo, where several parameters displayed a lower seasonality and a carbonate system that supports reef development throughout the year. Conversely, in Marino Ballena the river discharges during rainy season exerted a strong control on the seasonal variability. The integrated analysis of coral reef composition and physicochemical parameters suggests that in addition to inorganic nutrients carbonate chemistry also plays a key role in coral distribution. Analyzing the spatial distribution of the main reef builders provides insights into the species-specific tolerance to varying conditions. Pavona clavus is widely distributed in both the northern and southern locations, suggesting that this massive coral is very tolerant to the high variability of physicochemical conditions. The dominant corals in the north (Pavona gigantea and Pocillopora spp.) are highly tolerant to nutrient-enriched cold waters with low aragonite saturation, while one of the main reef-builders in southern locations (Porites cf. lobata) cope better with low salinity, low aragonite saturation and low light intensity caused by river discharges.     Microsatellite development for Theridion evexum (Araneae: Theridiidae) using low-coverage genome sequencing and the MiMi script

    Habitat fragmentation and unplanned urban expansion increasingly threatens biodiversity, yet the genetic impacts on arthropods, and particularly spiders, remains understudied due to the lack of suitable molecular makers. Here, we used low-coverage genome sequencing and a bioinformatics pipeline to develop polymorphic microsatellite markers for Theridion evexum, a tropical spider with urban and natural populations. To increase the yield of DNA extracted from small spiders like T. evexum, we also optimized a Cetyltrimethylammonium bromide (CTAB) DNA extraction protocol. We sequenced eight individuals at 4X using paired-end sequencing on an Illumina Novaseq 6000. Reads were cleaned and processed using the Multi-individual Microsatellite Identification (MiMi) python pipeline. MiMi produced a total of 3999 putative microsatellites. After filtering for polymorphic loci with an allelic richness greater than three and present in at least 5 of the 8 sequenced individuals, 34 final markers were identified. An experimental validation of 13 of these 34 markers showed that 10 loci were polymorphic with at least three detectable alleles, one locus was monomorphic, and two loci did not produce PCR products. These markers will allow a better assessment of the effects of urban fragmentation and isolation across populations of this spider species.     Prácticas de salud sexual en hombres con HIV-SIDA, que se autodenominan gay: una revisión integrativa

    Este proyecto aborda la situación de la población gay portadores de HIV/SIDA en Costa Rica, desde una perspectiva de salud sexual y el rol del profesional en enfermería obstétrica. La estigmatización social, junto con la desinformación y el miedo al juicio, ha limitado el acceso de esta población a servicios de salud sexual y reproductiva. Esta situación se ve agravada por prácticas sexuales de riesgo, en parte influenciadas por el uso de drogas y alcohol, y por la falta de estrategias efectivas de educación sexual. El trabajo subraya la importancia del profesional de enfermería en obstetricia como agente clave en la promoción del autocuidado, la autoestima y una salud sexual integral. Se hace énfasis en la necesidad de brindar atención libre de prejuicios, culturalmente coherente y centrada en una visión biopsicosocial de la sexualidad.This project addresses the situation of the gay population living with HIV/SIDA in Costa Rica from a sexual health perspective and the role of obstetric nursing. Social stigma, along with disinformation and fear of judgment, has limited this population's access to sexual and reproductive health services. This situation is exacerbated by risky sexual practices, partly influenced by drug and alcohol use, and by the lack of effective sexual education strategies. The work highlights the importance of obstetric nursing professionals as key agents in promoting self-care, self-esteem, and comprehensive sexual health. Emphasis is placed on the need to provide care that is free of prejudice, culturally consistent, and centered on a biopsychosocial perspective of sexuality. Our purpose is to analyze the available scientific evidence on sexual health practices in men with HIV-SIDA who identify as gay. Methodology: An integrative review was conducted through four databases: Scielo, Dialnet, LILACS, ScienceDirect. The selected studies were critically evaluated and synthesized using the constant comparative method: data reduction, data visualization, data comparison, drawing conclusions. Of these, 10 publications are included, which correspond to cross-sectional studies, systematic reviews, cohort studies, and ethnographic and experimental studies. These publications were categorized as follows: population, risky sexual practices, and health care.     Genetic diversity of Mycoplasma bovis isolated from bovine respiratory disease and bovine mastitis in Chile

    Mycoplasma bovis, a major bacterial pathogen for cattle, is responsible for diseases such as pneumonia, mastitis, otitis, and arthritis, leading to substantial economic losses and animal welfare concerns. Despite its wide global distribution, there is limited information in South America. M. bovis has been reported as a mastitis agent in Chile, but its genetic diversity is poorly understood. Therefore, this study aimed to determine the genetic diversity of M. bovis isolates from Chilean dairy cattle (from bovine respiratory disease and mastitis cases) in the last two decades and evaluate their genetic relatedness with strains isolated in different countries using a whole genome sequencing approach. The M. bovis population in Chile was found to be highly homogeneous, with MLST and phylogenomic analysis identifying ST60 as the dominant clone, representing most of the isolates (97.8 %), while just one isolate was typed as ST12 (2.2 %). Phylogenomic analysis revealed close genetic relatedness among most Chilean isolates, showing a close genetic relationship with North American strains, forming a tight clade with Canadian ST60 strains, while the single Chilean ST12 isolate clustered with North American, Israeli, and European strains and clustered with the type strain (PG45) of this species. Moreover, the pangenome analysis confirms that M. bovis has an open pangenome, with a large range of accessory genes that remain largely unexplored and may hold key insights into its genome plasticity, thereby opening future research. The findings of this study provide the first insights into the Chilean population structure of M. bovis, contributing to the global epidemiology of this pathogen with a focus on South America.     La presente investigación tiene como objetivo analizar la gestión del clima organizacional desde las dimensiones de la comunicación y la cooperación, y su incidencia directa en la satisfacción profesional, en la Escuela Isabel Brown Brown, ubicada en el circuito 02 de la Dirección Regional de Liberia. La iniciativa partió la comprensión del entorno escolar no solo es un espacio de transmisión de conocimientos, sino también un escenario de relaciones humanas que requieren atención, gestión y fortalecimiento continuo. En este sentido, se logró como la gestión del clima organizacional no debe limitarse a aspectos administrativos, sino que debe integrar elementos humanos que permitan a los actores educativos afrontar de manera asertiva los retos cotidianos, enfrentar una comunidad educativa exigente en el desarrollo de competencias emocionales y sociales. Bajo esta investigación se implementó, un enfoque cualitativo, con un estudio de casos, con el desarrollo de características descriptivas para comprender el entorno natural de los fenómenos que participan en este contexto. Por su parte, los objetivos se planteó la orientación en una propuesta resultante de este trabajo, con actividades concreta para atender las necesidades emergentes de la comunidad escolar, orientada a mejorar el clima organizacional desde la gestión de la comunicación y la cooperación, como medios esenciales para elevar el nivel de satisfacción profesional y, por ende, la calidad del servicio educativo. En definitiva, esta indagación representó una contribución al mejoramiento de los procesos de gestión institucional en el ámbito educativo, ofreciendo un enfoque integral que promueve el trabajo colaborativo, sana convivencia, valores humanos y transformación profesional. Asimismo, por su gran adaptabilidad en su estructura, permite una planificación constante a las necesidades que demanda el contexto institucional.This research aims to analyze organizational climate management from the perspective of communication and cooperation, and its direct impact on job satisfaction, at the Isabel Brown Brown School, located in Circuit 02 of the Liberia Regional Directorate. The initiative is based on an understanding that the school environment is not only a space for the transmission of knowledge, but also a setting for human relations that require attention, management, and strengthening. In this sense, it was established that organizational climate management should not be limited to administrative aspects but should integrate human elements that allow educational stakeholders to assertively confront daily challenges and face a demanding educational community in the development of emotional and social competencies. This research implemented a qualitative approach, using a case study, with the development of descriptive characteristics to understand the natural environment of the phenomena involved in this context. The objectives established, in turn, guided the resulting proposal, with concrete activities to address the emerging needs of the school community. This proposal is aimed at improving the organizational climate through communication and cooperation management, as essential means of raising the level of professional satisfaction and, consequently, the quality of the educational service. Ultimately, this research represents a contribution to strengthening institutional management processes in the educational field, offering a comprehensive approach that promotes collaborative work, healthy coexistence, human values, and professional transformation.
