Efficacy of PCSK9 inhibitors in the treatment of heterozygous familial hypercholesterolemia: A clinical practice experience
Embargado hasta 01/01/2100Background: PCSK9 inhibitors are a treatment option for patients with familial hypercholesterolemia
not on low-density lipoprotein cholesterol goals despite the use of maximally tolerated high intensity-
statins dose.
Objective: To evaluate the efficacy of alirocumab and evolocumab in LDL-C reduction and targets
attainment in patients with heterozygous familial hypercholesterolemia in clinical practice setting.
Methods: SAFEHEARTisanopen,long-termprospectivestudyofacohortofsubjectswithmolecular
diagnosis of familial hypercholesterolemia. This study analyze subjects ≥ 20 years of age on stable
lipid-lowering therapy, who received PCSK9 inhibitors during the period 2016 to January 2020.
Results: 433 patients (mean age 55 years, 53% male, 39% with cardiovascular disease) were included
andfollowed-upforamedianof2.5years(IQR1.6–3.0).MedianLDL-ClevelpriortoPCSK9inhibitors
was 145 mg/dL (IQR 125–173). The addition of PCSK9 inhibitors (211 alirocumab, 222 evolocumab)
reduced LDL-C by 58% (IQR 41–70) p<0.001, in men and women, achieving a median LDL-C level
of 62 mg/dL (IQR 44–87) without differences between both PCSK9 inhibitors. Out of them 67% with
and 80% without cardiovascular disease reached 2016 ESC/EAS LDL-C targets, and 46% very high
risk and 50% high risk patients achieved 2019 ESC/EAS LDL-C goals. Independent predictor factors
for attainment of 2019 ESC/EAS LDL-C goals were to be male, smoking and the use of statins with
ezetimibe. Both inhibitors were well tolerated.
Conclusions:
PCSK9 inhibitors on top of maximum lipid-lowering treatment significantly reduced LDL-C levels in patients with familial hypercholesterolemia and improved the achievement of LDL-C targets

Torres regentó en Madrid de 1915 a 1930. De su estudio y análisis se pueden extraer
algunas de las claves para entender su personalidad artística y privada, ya que era un
lugar estrechamente vinculado al pintor. Para definir nuestro objetivo, partimos de la
hipótesis de que, tras la muerte del pintor, el ambiente que se vivía en su taller sirvió
para la creación de diseños y discursos museográficos destinados al Museo que se le
dedica en Córdoba en 1931. En la actualidad, el estudio madrileño de Romero de
Torres ha desaparecido, es por ello que el objetivo de esta investigación es proponer
las bases metodológicas para la recuperación de su ambiente

Sierra Morena en la encrucijada: Despoblación, crisis rural y expectativas de desarrollo sostenible
Embargado hasta 01-01-2100Aunque el término España vacía haya sido puesto en cuestión por ser desacertado (Molinero &
Alario, 2022), lo cierto es que ha servido para poner en evidencia una problemática acuciante en
el interior peninsular: el de la despoblación, la crisis rural y la falta de expectativas de desarrollo
sostenible. Y es precisamente en estas temáticas de actualidad en las que se ha puesto el foco
central en este libro, pero en este caso localizadas y contextualizadas
en un espacio geográfico de enorme significación territorial como es el de Sierra Morena, situado
entre tres comunidades autónomas y seis provincias.
El libro es resultado de un Simposio Nacional que reunió a importantes investigadores en
noviembre del año 2022, siendo, por tanto, una obra colectiva que aglutina un total de dieciséis
capítulos a través de los cuales se analizan y se discuten problemáticas de rabiosa actualidad, aun
cuando vienen arrastrándose desde hace tiempo y aun acrecentándose en las dos últimas décadas
en los ámbitos rurales profundos de Sierra Morena a un ritmo ciertamente vertiginoso y de mayor
afectación incluso que en otros muchos espacios de la España rural de interior.
Estas temáticas derivan en múltiples consecuencias negativas para el territorio estudiado y son
resultado de diferentes y complejas causas que hoy día tienen un importante impacto mediático,
pese a lo cual no han sido habitualmente abordadas en Sierra Morena, por lo que esta publicación
resulta de gran utilidad y oportunidad, más aún por dedicar la atención a un espacio geográfico
periférico, aislado y marginado tradicionalmente por las Administraciones Públicas por su escasa relevancia poblacional y su relativa lejanía de núcleos urbanos centrales y grandes ejes de comunicación y pese a su importante papel como territorio generador de recursos naturales y su
extensa y significativa red de espacios naturales protegidos.
Este libro realiza múltiples miradas hacia la realidad territorial de Sierra Morena desde
perspectivas, disciplinas y visiones diferentes, pero bien ensambladas y articuladas que hacen de
esta publicación todo un ejercicio interdisciplinar de conocimiento y reflexión sobre este espacio
geográfico como no se había abordado durante muchos años. Para ello, los profesores Alfonso
Mulero Mendigorri y Manuel Rivera Mateos, pertenecientes al Grupo de Investigación “Territorio,
Patrimonio y Sociedad” (TePS) de la Universidad de Córdoba, como coordinadores y editores de
la obra, organizaron durante el año 2022 un simposio nacional centrado en los temas antedichos
sobre Sierra Morena, que ha sido finalmente el germen de este libro, que se estructura en cuatro
partes. La primera aborda las dinámicas sociodemográficas recientes y la crisis rural de Sierra
Morena; la segunda, los espacios naturales protegidos y las iniciativas de desarrollo sostenible; la
tercera, la gestión de los recursos patrimoniales y los problemas del desarrollo territorial y, por
último, la cuarta, las actividades económicas, el empleo y el emprendimiento

Identifying prognostic gene panels in acute myeloid leukemia
Embargado hasta 01/01/2100Introduction: Acute myeloid leukemia (AML) is a heterogeneous disease currently including 12 entities defined by genetic findings with remarkable differences in prognosis and targeted therapies availability. Therefore, identification of genetic abnormalities by efficient techniques has become a necessary tool in routine clinical practice for AML patients.
Areas covered: In the present review, we will focus on our current knowledge of relevant prognosis gene mutations in AML, as recently updated by European Leukemia Net Leukemia risk classification.
Expert opinion: About 25% of newly diagnosed younger AML patients will be promptly classified as favorable prognosis by demonstrating the presence of NPM1 mutations or CBF rearrangements by qRTPCR, allowing for implementing molecular measurable residual disease-guided chemotherapy-based protocols. In fit AML patients, rapid detection of FLT3ITD is mandatory to associate midostaurin or quizartinib to treatment and assignment to intermediate prognosis. Conventional cytogenetics and FISH still have a role for detection adverse prognosis karyotypes and KMT2A, MECOM, or NUP98 gene rearrangements. Further genetic characterization is performed with NGS panels including favorable prognosis gene CEBPA bZIP and adverse prognosis genes, such as TP53 and myelodysplasia associated genes
Impact of measurable residual disease by decentralized flow cytometry: a PETHEMA real-world study in 1076 patients with acute myeloid leukemia
Embargado hasta 01/01/2100The role of decentralized assessment of measurable residual disease (MRD) for risk stratification in acute myeloid leukemia
(AML) remains largely unknown, and so it does which methodological aspects are critical to empower the evaluation of MRD
with prognostic significance, particularly if using multiparameter flow cytometry (MFC). We analyzed 1076 AML patients in
first remission after induction chemotherapy, in whom MRD was evaluated by MFC in local laboratories of 60 Hospitals
participating in the PETHEMA registry. We also conducted a survey on technical aspects of MRD testing to determine the
impact of methodological heterogeneity in the prognostic value of MFC. Our results confirmed the recommended cutoff of 0.1%
to discriminate patients with significantly different cumulative-incidence of relapse (-CIR- HR:0.71, P < 0.001) and overall
survival (HR: 0.73, P=0.001), but uncovered the limited prognostic value of MFC based MRD in multivariate and recursive
partitioning models including other clinical, genetic and treatment related factors. Virtually all aspects related with
methodological, interpretation, and reporting of MFC based MRD testing impacted in its ability to discriminate patients with
different CIR. Thus, this study demonstrated that “real-world” assessment of MRD using MFC is prognostic in patients at first
remission, and urges greater standardization for improved risk-stratification toward clinical decisions in AML
