Clinical characteristics according to sex and symptom severity in children with selective mutism: a four-center study
IntroductionClinical information regarding selective mutism (SM), a persistent and debilitating psychiatric disorder, in children is extremely limited. We aimed to examine sociodemographic characteristics and comorbid psychiatric conditions and identify clinical variables associated with sex and SM severity among children with SM.MethodsWe analyzed the medical records of 49 children who received treatment for SM in four different tertiary hospitals in Turkey between 2016 and 2021. Children's charts were reviewed to examine clinical characteristics, comorbidities, and response to treatment.ResultsThirty-one children were female, and 18 were male (female:male ratio is 1.7:1). Most children (73.5%) with SM displayed onset of SM in 3-6 years. However, most children (57.1%) were diagnosed between the ages of 7-11. The mean time from onset to diagnosis was 1.69 +/- 1.37 years. Females displayed a later onset of SM (6.42 +/- 2.40 vs. 4.89 +/- 0.96; p= 0.013) and higher comorbidity rates (71% vs. 38.9%, p= 0.039) than males. The vast majority of children received two or more psychiatric diagnoses. Children in the severe group had a longer duration of illness, higher rates of psychiatric comorbidity, speech delay, and treatment resistance.ConclusionOur study suggests that SM may have different clinical features according to sex and symptom severity of SM. More information about children with SM is needed to understand the development and maintenance of SM
Autopsy findings and scene of incident examination in deaths resulting from electrical injury
Electrocution injuries are divided into high (HVI) and low voltage injuries (LVI). The aim of this study was to examine the sociodemographic data of cases where autopsy was performed because of death originating from electric shock, together with the clinical characteristics, scene of incident, electrical entry-exit wound localizations, pathological effects of electric shock on the body, and cause of death. A total of 4210 autopsies were retrospectively reviewed, and 49 had died from electrocution. The mean age of the cases was 29.2 +/- 16.04 years and 87.8% were males and 12.2% were females, 49% of the cases were the result of HVI and 51% the result of LVI. The presence of electrical entry and/or exit wounds were significantly higher in electrocution injuries due to home accidents than work accidents (p < 0.005). There was no statistically significant difference between the deaths caused by the direct effect of electric shock and those who died with LVI and HVI. The deaths of all the cases were accidental in origin, with no suicide or homicide. Deaths due to electrocution due to home or workplace accidents can be determined by examining the crime scene, witness statements, autopsy and histopathological findings
Royal jelly arranges apoptotic and oxidative stress pathways and reduces damage to liver tissues of rats by down-regulation of Bcl-2, GSK3 and NF-?B and up-regulation of caspase and Nrf-2 protein signalling pathways
Introduction: Royal jelly (RJ) from the honey bee, Apis mellifera, is a traditional product that is widely used as a food supplement to support the medical treatment of various diseases. Material and methods: Our study continued for 8 weeks. 42 Wistar albino (8 weeks old) male rats were used in the study. The study included 6 groups; Group 1: Control group (fed with standard diet), Group 2: RJ (100 mg/kg, bw), Group 3: F-50 (50 mg/kg, bw), group 4: F-100 (100 mg/kg, bw) group 5: F-50 (50 mg/kg, bw) + RJ (100 mg/kg, bw) Group 6: F-100 (100 mg/kg, bw) + RJ (100 mg/kg, bw). Malondialdehyde (MDA), catalase (CAT) and glutathione (GSH) activities in liver tissue were determined by spectrophotometer. Liver tissue samples were examined histopathologically and various protein levels were determined by Western blotting technique. Results: RJ caused a significant decrease in MDA level, Bcl-2, GSK3 and NF-kappa B protein expression levels, whereas induced a significant increase in GSH level, CAT activities and Bax, BDNF, caspase-6, caspase-3, Nrf-2 protein expression levels. Conclusion: Our findings suggest RJ to be used as a hepatoprotective agent in the clinic to modulate the toxic effects of fluoride and other chemicals in the future.Firat University Research Projects Unit (FUBAP); [FF.19.16]This study was supported by Firat University Research Projects Unit (FUBAP) [Project No: FF.19.16]
Biological determination, molecular docking and Hirshfeld surface analysis of rhoduim(I)-N-heterocyclic carbene complex: Synthesis, crystal structure, DFT calculations, optical and non linear optical properties
The synthesis and characterization of 1-(isobutyl)-3-(2.3.5.6 tetramethylbenzyl)-5.6-dimethylbenzimidazolium chloride and its N-functionalized Rhodium(I) N-heterocyclic carbene complex have been described. The ligand 2 and the complex 3 were characterized by IR, 1H NMR and 13C NMR spectroscopy, DART-TOF mass spec-trometry and elemental analysis. The coordination mode of the complex was confirmed by single crystal XRD analysis. We performed a comparative theoretical study in order to investigate the impact of the iridium on the optoelectronic and non-linear optical properties of the complex. The M06-2X functional with the mixed basis set (6-31G(d): def2tzvp) was found the best method for optimizing the studied complex since it displayed close geometric data to the experiment. As a novelty in this work, iridium was found to be a performer for NLO ap-plications over rhodium. This Rh(I)-N-Heterocyclic carbenes (NHC) showed good catalytic performance for the hydrosilylation of acetophenone derivatives with triethylsilane at 80 C for 1 h by using 1 mol % of this complex. The cytotoxic properties of the rhodium(I)-NHC complex have been assessed against MCF7 and MDA-MB-231 cancer cells and the complex show interest activity. The in vitro antibacterial activity of this complex were also investigated against Micrococcus luteus LB 14110, Listeria monocytogenes ATCC 19117, Salmonella Typhimurium ATCC 14028, Staphylococcus aureus ATCC 6538, Pseudomonas aeruginosa and Candida albicans microorganisms indicated their significant biological potential. Additionally, the antioxidant activity of the same complex has been analyzed with DPPH center dot free radical through spectrophotometric method and the result inferred them as an antioxidant. Bilateral thigh abscess due to rectal fistula: A rare case report with silent clinical presentation
A 53-year-old male patient who underwent a rectal cancer operation nine years ago was evaluated with complaints of pain and swelling in the lateral thigh. Radiological imaging revealed abscess foci in the right thigh muscles. Surgical drainage and debridement were performed but developed recurrence. Contrast-enhanced abdominal tomography was performed due to a positive medical history and it was seen that an enterocutaneous fistula was formed from the anastomosis site of previous cancer surgery to both sides of the thigh. Bilateral thigh abscesses were drained and radical debridement was performed in the same session with surgery for the fistula. Pathological examination of the surgical specimens showed no evidence of cancer and the patient was discharged on the 14th postoperative day with complete resolution of the infection. Hip abscesses are rare, delays in the diagnosis and treatment of the underlying cause in cases of abscess secondary to abdominal or pelvic pathologies lead to an increase in mortality. Therefore, it should be kept in mind that thigh abscesses may be another side of the coin besides being seen as an isolated infection
Combining docking, molecular dynamics simulations, AD-MET pharmacokinetics properties, and MMGBSA calculations to create specialized protocols for running effective virtual screening campaigns on the autoimmune disorder and SARS-CoV-2 main protease
The development of novel medicines to treat autoimmune diseases and SARS-CoV-2 main protease (Mpro), a virus that can cause both acute and chronic illnesses, is an ongoing necessity for the global community. The primary objective of this research is to use CoMFA methods to evaluate the quantitative structure-activity relationship (QSAR) of a select group of chemicals concerning autoimmune illnesses. By performing a molecular docking analysis, we may verify previously observed tendencies and gain insight into how receptors and ligands interact. The results of the 3D QSAR models are quite satisfactory and give significant statistical results: Q_loo & BOTTOM;2 = 0.5548, Q_lto & BOTTOM;2 = 0.5278, R & BOTTOM;2 = 0.9990, F-test = 3,101.141, SDEC = 0.017 for the CoMFA FFDSEL, and Q_loo & BOTTOM;2 = 0.7033, Q_lto & BOTTOM;2 = 0.6827, Q_lmo & BOTTOM;2 = 0.6305, R & BOTTOM;2 = 0.9984, F-test = 1994.0374, SDEC = 0.0216 for CoMFA UVEPLS. The success of these two models in exceeding the external validation criteria used and adhering to the Tropsha and Glorbaikh criteria's upper and lower bounds can be noted. We report the docking simulation of the compounds as an inhibitor of the SARS-CoV-2 Mpro and an autoimmune disorder in this context. For a few chosen autoimmune disorder receptors (protein tyrosine phosphatase, nonreceptor type 22 (lymphoid) isoform 1 (PTPN22), type 1 diabetes, rheumatoid arthritis, and SARS-CoV-2 Mpro, the optimal binding characteristics of the compounds were described. According to their potential for effectiveness, the studied compounds were ranked, and those that demonstrated higher molecular docking scores than the reference drugs were suggested as potential new drug candidates for the treatment of autoimmune disease and SARS-CoV-2 Mpro. Additionally, the results of analyses of drug similarity, ADME (Absorption, Distribution, Metabolism, and Excretion), and toxicity were used to screen the best-docked compounds in which compound 4 scaled through. Finally, molecular dynamics (MD) simulation was used to verify compound 4's stability in the complex with the chosen autoimmune diseases and SARS-CoV-2 Mpro protein. This compound showed a steady trajectory and molecular characteristics with a predictable pattern of interactions. Impacto de la intervención en un Hospital de Día de Salud Mental sobre el uso posterior de recursos sanitarios y ocupacionales
This work examines the impact of intensive treatment in a Mental Health Day Hospital (HD) on the subsequent use of healthcare resources (community and hospital), as well as occupational and training ones. It starts from a longitudinal and ambispective descriptive design in which the differential use of resources by users of the Mental Health network is studied in an innovative way, before and after treatment in HD. Finally, the clinical implications and recommendations are discussed according to the results.El presente trabajo examina el impacto del tratamiento intensivo en un Hospital de Día de Salud Mental (HD-SM) sobre el uso posterior de recursos asistenciales (comunitarios y hospitalarios), así como ocupacionales y formativos. Se parte de un diseño descriptivo longitudinal y ambispectivo en el que se estudia de forma innovadora el uso diferencial de los recursos por parte de los usuarios de la red de Salud Mental, antes y después del tratamiento en HD. Carlos Castilla del Pino: resistir, explorar, comunicar...
Based on a common experience, we try to highlight some outstanding personal characteristic of Carlos Castilla del Pino that allowed him to resist the socio-cultural and professional lethargy of Francoism and develop, despite everything, a relevant professional and theoretical life. To do so, he had to resist that lethargy and gagging of the long night of stone and mortar of Francoism and, on top of that, in a provincial city harshly retaliated during and after the Spanish civil war; therefore dominated for decades by institutional and ideological fascism. Despite this, Castilla del Pino managed to develop a highly advanced, coherent and risky theoretical work, especially in psychopathology, anthropology and ideology, for whose understanding I propose a pragmatic framework. The Republican motto, “to resist is to win”, could be a good summary of his professional and theoretical life, although it is more doubtful that such statement can have the same value for his personal life.A partir de una experiencia en común, se intenta resaltar alguna característica personal descollante de Carlos Castilla del Pino que le permitió resistir la atonía socio-cultural y profesional del franquismo y desarrollar, a pesar de todo, una vida profesional y teórica relevante. Tuvo que resistir esa atonía y amordazamiento de la larga noche de piedra y argamasa del franquismo en una ciudad de provincias duramente represaliada durante la guerra y posguerra civil española, dominada por tanto durante decenios por el fascismo institucional e ideológico. A pesar de ello, Carlos Castilla logró desarrollar una obra teórica (en especial en psicopatología, antropología e ideología) sumamente avanzada, coherente y arriesgada, para cuya compresión propongo un marco pragmático. El lema republicano, resistir es vencer, podría ser un buen resumen de su vida profesional y teórica, aunque es más dudoso que tal afirmación pueda tener el mismo valor para su vida personal
